Oncologic Results and Functional Assessment of Limb Salvage Surgery in Primary Bone Tumors Around the Shoulder Girdle (견관절 주위 원발성 골 종양에서 사지 구제술의 종양학적 결과 및 기능적 평가)

  • Purpose: The purpose of this study was to assess the oncologic results and functional outcomes of limb salvage surgery performed in patients of primary bone tumors of the shoulder girdle. Materials and Methods: Twenty-nine patients who underwent limb sparing resection for shoulder girdle neoplasm between 1982 and 2001 were analyzed. Follow up periods averaged 7 years and 1 month. Mean age of the patients was 35 (11~71) years. There were 14 males and 15 females. Primary malignant bone tumors of shoulder girdle (proximal humerus 21, scapula 3, both 1) were 23 cases; osteosarcomas 7, chondrosarcoma 14, parosteal osteosarcoma 1, hemangioendothelioma 1, and giant cell tumor of proximal humerus were 6 cases. Limb salvage surgery was performed by curettage and cementing in 7 patients, by cement molding arthroplasty in 10 patients, and by tumor prosthesis in 7 patients, by other method such as resection only, bone graft, arthrodesis in 5 patients. The Musculoskeletal Tumor Society functional rating system was used to assess functional outcomes. Results: One osteosarcoma and 2 chondrosarcoma patients died, and the survival of the salvaged limb was 88.6% at the final follow-up. There were 6 local recurrences, 2 lung metastases, 2 local recurrences and lung metastases. The functional outcome was 80%. There was statistically significant difference of functional results among the patients treated by cement filling (86%), cement molding arthroplasty and IM nailing (71%), and tumor prosthesis (83%). (p=0.034) There were three complications including 1 radial nerve palsy and 1 axillary nerve palsy, and 1 wound infection. Dislodgement of vascularized fibular graft in one patient was treated by internal fixation. Conclusion: Limb salvage surgery seems to be useful method to treat bone tumors of the shoulder girdle.

A Case Report of a Patient with Guillain-Barre Syndrome Complaining of Quadriplegia and Anorexia Improved by Korean Medicine Treatment (사지마비와 식욕부진을 호소하는 길랑바레 증후군 환자의 한방치료 증례보고 1례)

  • Objective: This case study reports improvement in a case of Guillain-Barre Syndrome by Korean medicine treatment in a patient with quadriplegia and anorexia. Methods: A 79-year-old woman diagnosed with Guillain-Barre Syndrome was treated with acupuncture, electroacupuncture, Bojungikgi-tang-gami, cupping, moxibustion, and rehabilitation. Her clinical symptoms were measured with the Manual Muscle Test (MMT), Modified Barthel Index (MBI), Functional Independence Measurement (FIM), and the EuroQol five-dimensional descriptive system (EQ-5D-5L). Results: After treatment, improvements in the MMT, MBI, FIM, and EQ-5D-5L were observed and her anorexia decreased. Conclusion: Korean medicine treatment appeared to be effective in the treatment of a patient with Guillain-Barre Syndrome with quadriplegia and anorexia.

A Case Report of Upper Extremities Rehablitation of 2 Cervical Spinal Cord Injured Patients by Functional Electrical Stimulation with Korean Medical Interventions (경추부 척수손상 사지마비 환자의 한방치료와 기능적 전기 자극요법을 통한 상지 재활 치험 2례)

  • Two patients, diagnosed as cervical spinal cord injury, were hospitalized and treated by Korean medical interventions with Functional Electrical Stimulation (FES). The range of motion of the shoulder joint, manual muscle test were adopted to measure the resulting motor recovery after treatment. And the Korean version of Barthel index (K-MBI) was adopted as a method of measuring the improvement of quality of life after treatment. The range of motions and muscle strengths of the shoulder joints were improved. The activities of daily living were slightly improved in case 2, but showed no improvement in case 1. FES was effective for upper extremities rehabilitation of spinal cord injured patients. Further studies are needed to set up an Korean medical protocol for spinal cord injury rehabilitation.

  • Objective: This study is a report on a case of a Guillain-Barre syndrome patient with quadriplegia and paresthesia, whose condition was improved by Korean medicine treatment. Methods: A 71-year-old woman diagnosed with Guillain-Barre Syndrome was treated by acupuncture, electroacupuncture, Sambi-tang-gami, cupping, moxibustion, and rehabilitation. Clinical symptoms were measured with the Manual Muscle Test (MMT), the Numeric Rating Scale (NRS), the Modified Barthel Index (MBI), and the Functional Independence Measurement (FIM). Result: After treatment, clinical symptoms showed improvement. Conclusion: Korean medicine treatment could be effective in the treatment of Guillain-Barre syndrome patients with quadriplegia and paresthesia.

  • Oral self-injurious behavior (SIB) can be defined as the intentional, direct injuring of oral tissue, most often done without conscious suicidal intent and most commonly associated with tongue or lip biting. Chronic biting of oral mucosa is an innocuous self inflicted injury, commonly seen in children suffering from developmental and psychological problems. The cases presented in this report discuss oral SIB due to epilepsy, quadriplegia and their treatments. This report documents a successful self-injurious behavior treatment of epilepsy patients within a short time by applying a removable intraoral device. Clinicians should notice the possibility of oral SIB in various disorders. Moreover, different treatment should be performed according to the causative disorders and symptoms.

  • A 45-year-old male received cervical continuous epidural block for posterior neck pain radiating to right upper extremity secondary to cervical herniated nucleus pulposus. Three days after epidural catheterization, fever, radicular pain and weakness of both upper extremities were developed. On admission, his temperature was $38.3^{\circ}C$ and showed progressive weakness and numbness in both upper and lower extremities. Cervical epidural abscess was suspected; MRI showed an epidural abscess from C4 to C7 level. Within 24 hours of admission, surgical decompression and drainage was effected. Culture of pus obtained at the lesion yielded Staphylococcus aureus. He was treated with intravenous antibiotics for 7 weeks resulting marked improvement of neurologic signs and symptoms.

  • Objective : This is the clinical report about the Wei symptom patient diagnosed as Hypokalemic periodic Paralysis. Methods and Results : Three Wei symtom patients diagnosed Hypokalemic Periodic Paralysisat at Cheonan oriental medical hospital were treated with Oriental-Western therapies. Conclusion : Hypokalemic Periodic Paralysis is regarded as wei symtom in Oriental Medicine. The treatment of Hypokalemic Periodic Paralysis regarded as wei symtom of Oriental Medicine had favorable effect. Parallel treatment showed favorable effect on other symptoms caused by Hypokalemic Periodic Paralysis such as nausea, chest discomfort, leg pain besides main symptoms such as weakness and numbness.

A Case Report of Complex Korean Medical Treatment for Cervical Spinal Cord Injury and Neurogenic Bladder (한의복합치료 후 호전된 경부척수손상 환자의 사지마비 및 신경인성 방광: 증례보고)

  • A 57-year-old male patient was diagnosed as Incomplete cervical spinal cord injury and Neurogenic bladder after falling accident and suffered from tetraplegia and urinary retention. The patient was hospitalized and treated with Complex Korean Medical treatment such as acupuncture, electro-acupuncture, bee venom pharmacopuncture and herbal medication for 13 weeks. International Standard for Neurological Classification of Spinal Cord Injury (ISNCSCI) was adopted to evaluate the functional recovery and Korean version of Modified Barthel Index (K-MBI) was adopted to evaluate the recovery of activities of daily living (ADL). After treatment, Total scores of ISNCSCI and K-MBI were improved. Also patient's micturition reflex was recovered and symptoms of neurogenic bladder were improved. This result shows that Korean Medical treatment may be an effective treatment option for spinal cord injury and neurogenic bladder patients. Further clinical studies are needed to clarify the effect of Korean Medical treatment on spinal cord injury.

  • Bickerstaff's brainstem encephalitis (BBE) is a rare disease diagnosed by specific clinical features such as 'progressive, relatively symmetric external ophthalmoplegia and ataxia by 4 weeks' and 'disturbance of consciousness or hyperreflexia' after the exclusion of other diseases involving the brain stem. Anti-ganglioside antibodies (GM, GD and GQ) in the serum or cerebrospinal fluid (CSF) are sometimes informative for the diagnosis of BBE because of the rarity of positive findings in other diagnositic methods: brain magnetic resonance imaging (MRI), routine CSF examination, motor nerve conduction study, and needle electromyography. We report a rare case of childhood BBE with elevated anti-GM1 antibodies in the serum, who had specific clinical symptoms such as a cranial polyneuropathy presenting as ophthalmoplegia, dysarthria, dysphagia, and facial weakness; progressive motor weakness; altered mental status; and ataxia. However, the brain MRI, routine CSF examination, nerve conduction studies, electromyography, somatosensory evoked potentials, and brainstem auditory evoked potentials were normal. BBE was suspected and the patient was successfully treated with intravenous immunoglobulins.

A Case Report of Quadriplegia Patients with Decubitus Ulcer (사지부전마비를 동반한 뇌경색 환자의 욕창 치험 1례)

  • Objectives The purpose of this report is to represent decubitus ulcer(pressure sore) improved by carbon arc and oriental medicine. Methods Patients with decubitus ulcer were treated with carbon arc, acupuncture, herbal medicine and simple dressing. Laser was irradiated at ulcer site once a day for 5 hour, using cored carbon number 3000,3002. Simple dressing with wound cleansing was performed to prevent reinfection. Herbal medicine and acupuncture treatment were applied to improve patients' general condition. Ulcer sizes and aspects were carefully checked by one week. Results Sizes of the decubitus ulcer decreased. Stage of ulcer was improved from stage 3 to stage 2. These results show that carbon arc may play a role in treating the decubitus ulcer with oriental medicine.
