  • Malperfusion of major organs which frequently accompanies acute aortic dissection is one of the major causes of death. Malperfusion does not only develop before surgery, but also during or after surgery in various manifestations according to the aortic branches involved. Expeditious diagnostic and therapeutic measures based on high degree of clinical suspicion are mandatory for successful treatment. The authors report four cases of acute aortic dissection accompanied by malperfusion of various organs that were successfully treated.

  • Elderly suicide rates tend to be high in Korea. Suicide by the elderly is no longer a personal problem; consequently, further research on risk and regional factors is necessary. Disease mapping in epidemiology estimates spatial patterns for disease risk over a geographical region. In this study, we use a simultaneous conditional autoregressive model for spatial correlations between neighboring areas to estimate standard mortality ratios and mapping. The method is illustrated with cause of death data from 2006 and 2010 to analyze regional patterns of elderly suicide in Korea. By considering spatial correlations, the Bayesian spatial models, mean educational attainment and percentage of the elderly who live alone was the significant regional characteristic for elderly suicide. Gibbs sampling and grid method are used for computation.

  • Community-acquired pneumonia (CAP) is a major risk factor of mortality in chronic obstructive pulmonary disease (COPD). Streptococcus pneumoniae (colloquially known as pneumococcus) is one of important pathogens of CAP in patients with COPD. Preventive interventions for pneumonia include pneumococcal and influenza vaccinations. A prospective, cohort study has been performed to investigate the protective effects of pneumococcal and influenza vaccinations on the severity of community-acquired pneumonia requring hospital admission in patients with COPD. Seven university-affiliated hospitals in Korea have participated in the study. The aim of this study was to construct an online registration system for the multi-institutional researchers that facilitates efficient collection and management of COPD patient data. This study has presented three basic strategies-accurate data input, convenient data completion, and real-time data management-to supplement the demerits of existing offline clinical study. The proposed online registration system has already been applied to a multi-institutional clinical study and was acknowledged for its high performance.

(Reichhardt, 1995). • This study was conducted to analyze the results of inhalation anesthesia performed at the equine hospital of Korea Racing Authority (KRA) and to find out the influencing factors on mortality during and/or after inhalation anesthesia. Among 585 cases of anesthesia, orthopedic surgery (410) was performed the most frequently, followed by colic surgery (85) and upper airway surgery (45). Twenty out of 585 horses were either euthanized or died during and/or after anesthesia. Among those twenty horses, fourteen horses received colic surgery, three received orthopedic surgery, and three others received upper airway surgery. The major causes of mortality were rupture of intestine in colic surgery and airway obstruction during recovery in upper airway surgery. Myopathy, refracture, laminitis were the causes of mortality in orthopedic surgery. Consequently, the horses that received colic and upper airway surgeries showed significantly high mortality rate rather than horses that received orthopedic surgery (p < 0.01). According to the results, horses that received colic surgery showed the highest mortality rate from euthanasia due to poor and grave prognosis. To reduce the perioperative mortality of horses, it is recommended to perform perioperative intensive care for colic surgery and careful monitoring for upper airway surgery during recovery.

  • From January 1985 to September 1990, 7 patients with carcinoma of the extrahepatic biliary system received external radiaiton therapy combined with hyperthermia. Of the 3 patients with extrahepatic bile duct cancer, two were primary cholangiocarcinoma and one was metastatic peripancreatic carcinoma. Of the 4 patients with carcinoma of the gallbladder, two were locor-egionally advanced and unresectable carcinoma and the remaining two were local-regional recurrence after cholecystectomy. They were all pathologicallly proven adenocarcinoma. The radiation dose received ranged from 3000 cGy/2weeks to 5040 cGy/7 weeks. The hyperthermia was done once or twice a week and 4 to 12 sessions in total. The tumor response was confirmed by T-tube cholangiography, percutaneous transhepatic cholangiography and CT scan. 6 out of 7 ($86\%$) showed partial regression of the tumor. The median survival time was 7 months (range $4\~11$ months).6 out of 7 patients were dead: one died of septicemia, 4 of primary disease, one of distant metastases. Only one out of 7 patients is still alive but new metastatic lesion was found. There was not any treatment related deaths. There was also no evidence of treatment related problems with liver, stomach and duodenum, although the observation period was short.

  • Korea has entered the ranks of advanced countries in terms of economic size and technological competitiveness. However, its industrial accident fatality rate remains among the lowest in OECD countries, and recent incidents such as various building collapses have resulted in numerous deaths of workers or citizens, reminiscent of accidents in developing countries. According to the 2022 Industrial Accident Status Analysis by the Ministry of Employment and Labor, out of the 874 fatalities in work-related accidents in 2022 across all industries, 402 were in the construction industry, accounting for approximately 46% of all fatalities. In particular, the construction industry's fatality rate stands at 1.61, significantly higher than the overall industry fatality rate of 0.43, indicating its severity. Construction ranks highest in terms of fatality rates, with mining at 12.18 and fishing at 1.80. When categorizing construction projects into private and public, private projects show significantly higher figures in terms of contracts, contract amounts, accident numbers, and fatalities compared to public projects. However, unlike public agencies, many private clients lack adequate safety and health activities and lack established safety and health systems. This study aims to raise awareness among private clients about the need to establish safety and health systems and enhance safety and health activities, and to discuss the direction of future development of advanced safety and health practices among private clients.
