The Prevalence of Malocclusion and Related Self-Esteem of Adolescents in Yangsan (양산시 청소년의 부정교합 유병률 및 관련 자존감 조사)

  • For the purpose of evaluating the prevalence of malocclusion and self-esteem in adolescents, 1,380 middle-school students in the Yangsan area were surveyed by oral examination and questionnaires and the obtained results were as follows: The distribution of 1st molar occlusion by Angle's classification was 69.0, 19.4, 10.6% for Class I, II and III respectively. In the horizontal relationship, the prevalence of normal overjet and crossbite was 86.9% and 5.6% respectively whereas larger and extremely larger overjet was found in 6.6% and 0.8%. In the vertical relationship of anterior teeth, normal, deep overbite and openbite was shown in 94.1%, 4.7%, and 1.2%. For the midline discrepancy, the distribution of groups with 0~1 mm, 2~4 mm and over 5 mm was 98.2%, 1.4%, and 0.4%. Crowding only in the maxilla was found in 9.6%, while that only in the mandible and in both arches was 14.1% and 24.1% respectively. Spacing only in the maxilla was seen in 3.0%, while that only in the mandible and in both arches was 2.4% and 1.7% respectively. Significant difference in self-esteem was revealed in female and malocclusion groups of crossbite and openbite(p < 0.05).

Relationship between the Malocclusion and Caries Prevalence of Adolescents in Iksan (익산시 청소년의 부정교합과 치아우식 유병률 사이의 연관성)

  • The purpose of the present study was to evaluate the relationship between the malocclusion and dental caries in adolescents with permanent dentition. The subjects of the study were 385 adolescents aged 12 to 15 years. Dental Aesthetic Index (DAI) and DMFT index were recorded as clinical indicators of malocclusion and dental caries. The mean Dental Aesthetic Index (DAI) score of the subjects was 12.42 and the mean DMFT index of the subjects was 2.89. Four students (1.04%) were included in the group of orthodontic treatment mandatory, which signifies the handicapping malocclusion. Moreover, the result of gender-specific analysis of the DAI components observed that the prevalence of midline diastema and mandibular overjet were significantly higher among boys (p < 0.05). In 7 components (missing teeth, incisal segment crowding, maxillary anterior irregularity, mandibular anterior irregularity, mandibular overjet, anterior openbite, antero-posterior molar relationship) among the 10 DAI components, abnormal groups showed significantly higher DMFT index than normal groups (p < 0.05). The subjects in the group of definite to handicapping malocclusion (DAI ${\geq}$ 26) showed significantly higher DMFT index than the subjects in the group of minor or no malocclusion (p < 0.01). In addition, the DAI score had significant positive linear correlation with the DMFT index (r = 0.584, p < 0.01). Consequently, the current findings suggested a positive relationship between the malocclusion and caries prevalence. And several specific types of malocclusion were supposed to be significantly correlated with dental caries.

An Epidemiological Study on the Dental Treatment Needs of Adolescents in Yangsan (양산시 거주 청소년의 치과치료 필요도에 대한 조사연구)

  • To evaluate the oral health status and to assess the resultant dental treatment needs in adolescents, 2,062 adolescents aged 14-17 years attending middle and high schools in Yangsan were surveyed by clinical examination and questionnaires. The obtained results were as follows. In the dental caries examination based on WHO criteria, the treatment needs of 66.7% of the subjects were determined. Assessment of dental erosion by the VEDE system indicated the treatment needs in 27.8% of the subjects, while MIH examination based on EAPD criteria indicated the treatment needs in 14.7%. Assessment of malocclusion using the occlusal index showed good occlusion in 67.8%, no need for treatment in 19.7%, slight need for treatment in 6.8%, definite need for treatment in 4.5%, and worst occlusion in 1.1% of the subjects, showing 12.5% of average treatment need. The prevalence and treatment need for periodontal disease was found to be 71.2% by CPITN assessments. The assessment of temporomandibular disorders by Helkimo's anamnestic index showed no symptoms in 67.1%, mild symptoms in 13.4%, and severe symptoms in 19.5% of the examinees. The overall treatment need of TMD was 20.6%. Periodontal diseases were ranked the highest in treatment need, followed by dental caries, dental erosion, MIH, TMD, and malocclusion in order. The results of this study indicated the overall status of adolescents'oral health and dental treatment needs. These might hopefully provide fundamental data and contribute to establishing promotional projects for adolescent oral health in Korea.

Oral Features in a Child with Noonan Syndrome : A Case Report (누난 증후군 환자의 구강 내 특징 : 증례 보고)

  • Noonan syndrome is characterized by distinctive facial features, short stature, and congenital heart disease. It is a congenital genetic disorder with a prevalence of between 1/1,000 and 1/2,500 in both genders. An 11-year-old boy with Noonan syndrome visited the hospital with an ectopically positioned tooth. A pulmonary stenosis was diagnosed and his growth and development were delayed. In many cases of this diseases there is obvious hemostasis, which he was not experiencing. His facial appearance showed characteristic features of Noonan syndrome. The patient showed a dental class II relationship, labioversion of the upper anterior teeth, and a shallow overbite. Radiographic examination revealed that the upper right canine was ectopically positioned, which led to root resorption of the upper right lateral incisor. A lateral cephalometric radiograph revealed a craniofacial pattern that was within normal limits. Surgical opening and button attachment on the impacted upper right canine were performed and traction was applied on the impacted tooth using a removable appliance. This patient was mildly affected by Noonan syndrome and showed some dental problems. However, few studies have reported the oral characteristics of Noonan syndrome despite its high incidence. Thus, this case report describes the oral features and management of Noonan syndrome.

Epidemiologic Survey on Failed Eruption of Mandibular First Molar (매복된 하악 제1대구치의 매복 양상과 역학적 조사)

  • The purpose of the present retrospective study was to analyze gender, age, awareness, etiology, and treatment choice in patients with failed eruption of the mandibular first molar. Radiographic material and dental records from 67 patients with 74 mandibular first molars with failed eruption treated during the years 2001-2014 were evaluated. As results, the prevalence of mandibular first molar with failed eruption was higher in males than females, and the average awareness age was 9.19 years old. The most common etiological cause was unknown, and the next most common causes were abnormal eruption pathway and odontogenic tumor in order. In this study, it is suggested that more dental education to children about the time of eruption of permanent tooth is required, and the importance of regular dental check-ups is emphasized during mixed dentition.

A study on the prevalence of the idiopathic osteosclerosis in Korean malocclusion patients (한국인 부정교합자의 악골에 발생한 특발성 골경화증의 유병률에 관한 연구)

  • Purpose : This retrospective study was performed to investigate the prevalence of the idiopathic osteosclerosis (IO) in Korean malocclusion patients according to age, sex, and the Angle's classification of malocclusion. Materials and Methods : This study consisted of 2,001 randomly selected patients from the Department of Orthodontics at the Gangneung-Wonju National University Dental Hospital, Korea. The prevalence of IO in Korean malocclusion patients was recorded using their panoramic radiographs, and the following parameters were surveyed; age, sex, and the Angle's classification of malocclusion. The chi-square test was analyzed to determine the statistical significance of differences in the prevalence of IO between age, sex, and the Angle's classification of malocclusion. Results : The prevalence of IO in the jaws was 6.7% in a total of 2,001 examined orthodontic patients. The majority of IO was found in the mandible (96.58%). The 30-39 age group showed the highest prevalence of IO (9.60%). There was a higher prevalence in females (6.89%) than in males (6.45%). The prevalence of IO in Angle Class I group (7.07%) was the most frequent, followed by Angle Class II group (6.72%), and Angle Class III group (6.40%). However, there was no statistical significance in sex and Angle's classification of malocclusion. Conclusion : The prevalence of IO in malocclusion patients showed the differences between various age groups and most of them were found in the mandibular posterior area. However, sex and the type of malocclusion are not to be considered as a contributing factor of IO.
