THE SKELETAL MATURITY OF CERVICAL VERTEBRAE OF CHILDREN WITH NORMAL OCCLUSION AND SKELETAL CLASS III MALOCCLUSION (정상교합자와 골격성 III급 부정교합자의 경추골성숙도에 관한 연구)

  • This study was to evaluate and compare differences of the cervical vertebral skeletal maturity of normal occlusion and skeletal Class III malocclusion. Normal occlusion (172 girls) and skeletal Class III malocclusion(191 girls) were classified according to diagnosis stone model and lateral cephalogram of Korean girls aging from 8 to 12 years. The concavity of inferior border, vertico-horizontal ratio of cervical vertebrae were observed and measured according to age. Differences of the cervical vertebral skeletal maturity were evaluated. The results were as follows : 1. The concavity of inferior border of the 2nd to 6th vertebrae of normal occlusion and skeletal Class III had uniformly increased with age. 2. The vertico-horizontal ratio of the 3rd to 6th vertebrae of girls with normal occlusion and skeletal Class III had uniformly increased with age. 3. There was no significant difference in cervical vertebral skeletal maturity between normal occlusion and skeletal Class III malocclusion in the concavity of inferior border of the 2nd to 6th vertebrae and in the vertico-horizontal ratio of the 3rd to 6th vertebrae. The results in the study indicate that there is no significant difference of cervical vertebral skeletal maturity between girls with normal occlusion and skeletal Class III malocclusion.

CHARACTERISTICS OF CRANIOFACIAL STRUCTURES IN SEVERE ADULT CLASS III MALOCCLUSION (심한 골격성 III급 부정교합을 가진 성인 두개안면골격의 특성)

  • The purpose of this study is to define the characteristics of the skeleton and soft tissues of severe adult class III malocclusion. The materials selected for this study were lateral cephalograms of 112 adult class III malocclusion patients with ANB difference below -2 degrees. and the mean age was 22.9 years old. The normal control sampler consisted of lateral cephalograms of 50 adults in normal occlusion and the mean age was 22.1 years old. The Horizontal reference line was FH line and the vertical reference line was nasion perpendicular to FH line. The skeletal and soft tissue characteristics of Class III malocclusion are as follows : 1. In the skeletal profile evaluated by vertical reference line (Nasion perpendicular to FH), the forehead and maxilla was similar to normal, but the mandible was protruded significantly. 2. The soft tissue profile is concave. The thickness of soft tissue covering forehead area and nose is within normal range. but the upper lip is thicker and the nasolabial angle is smaller than normal. The lower lip and inferior labial sulcus is thinner than normal. The degree of eversion of lower lip is lesser than normal. 3. The cranial base of class III malocclusion is shorter and saddle angle is smaller than normal. 4. The location of midface evaluated in relations to cranial base is within normal range but, the length of midface is shorter than normal when compared from the deep portion of the facial skeleton. 5. The location of maxilla in reference to cranial base is within normal range but the length of maxilla was shorter in class III malocclusion. 6. The mandible was protruded, ramus height and body length, gonial angle were greater than normal, and the chin angle was smaller. 7. Upper incisor was proclined, lower incisor was retroclined.

  • 같은 급의 부정교합이라도 악골의 수직 수평관계, 악골과 치아의 크기에 따라서 여러 유형으로 분류된다. 여러 유형에 따라 치료방법에도 일률적일 수 없고 다양하게 된다. 진단과 치료계획 수립 시에 종종 어려움을 겪게 되는 경우가 있다. 진단의 어려움을 겪는 경우는 발치할 것인지 여부, 어떤 치아를 발치할 것인지, 악궁과 치아크기의 부조화를 어떻게 해소시킬 것인지 들의 결정을 내릴 때이다. 이런 경우 정확한 두부방사선분석 석고모형 분석, 성장과 발육분석에 의하여 판단하며 무엇보다 술자의 능력에 맞게 최선의 방법을 선택하는 것이 중요하다. 부정교합의 유형에 따라 다양한 발치 등 방법을 보고하고자 한다.

Trends in Malocclusion Patients of Yeungnam University Hospital (영남대학교병원 치과에 내원한 부정교합환자의 특성)

  • Background: This study was done to better understand patterns, trends and local distribution patterns of malocclusion so that we can provide adequate information to patients, to help make the appropriate diagnosis and therapeutic plans, and to assess the future directions of malocclusion treatment. Materials and Methods: Malocclusion patterns, distribution and trends of visiting patients were examined in 993 malocclusion patients who had been evaluated and diagnosed at the Department of Dentistry, Yeungnam University Hospital over a 10-year period from 1995 to 2004. Results: The number of visiting patients per year showed an increasing trend and the visit rate was 1.28 time-higher in females(56.1%) than in males(43.9%). 1) Age distribution showed that the 7-12 year-old group was the largest (36.7%). Geographic distribution showed the majority of patients were from the Dalseo district(28.2%). Angle's malocclusion classification revealed that class III was the largest(38.4%). Crowding was the chief complaint in the highest percentage of patients(33.9%). The therapeutic method used was the fixed appliance in 61% of cases and a combination with extraction in 30.8%. Conclusions: The availability of dental services can accommodate orthodontic needs adequately as well as obtain reliable quantitative information regarding the characteristics of orthodontic patients.

  • If patients complain about sudden ill-fitting occlusal sensation(acute malocclusion), a dentist has to consider many possible causes about that. Acute malocclusion is characterized its sudden onset and mainly can be caused by teeth and its related structure problem. But we always keep on mind that acute malocclusion also can be caused by temporomandibular disorders In this 3 cases of acute malocclusion, evaluating by patient history, clinical and radiographic findings, we diagnosed them as 1. Lateral pterygoid muscle spasm, 2.. Retrodiscitis, 3. Osteoarthritis. Through this non-odontogenic acute malocclusion cases, we emphasize the key of diagnosis is on comprehensive and careful patient history taking and clinical examination.

  • Class III malocclusion usually becomes manifested at a very early age, most typically evidenced clinically by the appearance of either an edge-to-edge incisor relationship or an anterior crossbite. Anterior crossbite, by it-self, retards growth of maxilla, and accelerates growth of mandible. So, treatment should be started as early as the patient cooperates, removing any factors or forces that inhibit growth and development in the same physiologic maxillary displacement direction. The facial mask is effective in most developing Class III patients, because the appliance system affects virtually all areas contributing to a Class III malocclusion. Thus, the facial mask can be applied to most developing Class If cases regardless of the specific etiology. In these cases, the results were followed. Anterior crossbite was corrected by anterior movement of maxilla and downward backward rotation of mandible and simultaneously, lower facial height was increased. So, it can be concluded that the facial mask is effective in treating growing patients with a deficient maxilla.

ROENTGENOCEPHALOMETRIC STUDY ON FACIAL HEIGHT AND OCCLUSAL PLANE INCLINATION IN CLASS II MALOCCLUSION GROUP (성인 II 급 환자의 안면 수직고경및 교합평면의 특징에 관한 두부방사선학적 연구)

  • This study was investigated to assess the difference of facial height and occlusal plane inclination between normal occlusion group and class II malocclusion group. The subjects consisted of 50 normal occlusion (male 25, female 25) and 50 class II(male 25, female 25) malocclusion patients. All subjects are adult. lateral cephalogram was taken with standard method traced, and digitized for each subjects. The computerized statiscal analysis was carried out with SPSS program. The results were as follows 1. In class II malocclusion group, variables significant different from normal occlusion group were as follows ; SN-FOP, FH-BOP, MP-BOP, AB-BOP, AB-FOP, Facial plane-BOP, FP-FOP 2. In class II malocclusion group, the posterior facial height -especially posterior lower facial height-was significantly smaller than normal occlusion group.(P<0.05) 3. In class II malocclusion group, the angles between occlusal plane and upper and lower incisor, the angle between upper molar and bisected occlusal plane were significantly larger than those of normal occlusion group. (P<0.05) 4. L1 to Mandibular plane (mm) was a unique factor of occlusal plane position that showed significant difference in class II malocclusion group. 5. The correlation between overbite and occlusal plane inclination existed in class II malocclusion group, but the correlation didn't exist in normal occlusion group.

Association between Ectopic Eruption of the Maxillary First Permanent Molar and Skeletal Malocclusion (상악 제1대구치의 이소맹출과 골격성 부정교합의 연관성)

  • This study assessed the association between ectopic eruption of the maxillary first permanent molar and skeletal malocclusion in 5- to 10-year-old children. As subjects, 786 children who attended the Wonkwang University Dental Hospital for orthodontic diagnosis were included. Children with unerupted first permanent molars or fully erupted second permanent molars were excluded. The study group demonstrated ectopic eruption of the maxillary first permanent molar, while the control group did not. Cephalometric radiographs taken between January 2003 and August 2015 were analyzed. Skeletal class III malocclusion was detected in 57.0% of the study group, which differed significantly from that in the control group (p<0.05). The SNA, ANB angles, and A to N-perpendicular distance were significantly smaller, whereas the A-B plane angle and APDI were significantly greater in the study group than in the control group (p<0.05). The SNB and mandibular plane angles were not significantly different between the groups. Thus, maxillary undergrowth is a risk factor for ectopic eruption of the maxillary first permanent molar.

Perception Survey of Virtual Reality Simulation with Malocclusion Models (부정교합 모델을 적용한 가상현실 시뮬레이션에 대한 인식조사)

  • The purpose of this study was to develop prototype of virtual reality(VR) simulation with malocclusion models and evaluate its applicability. Task abilities, task completion time, and a satisfaction survey were compared between dentist trainees and dental students. Participants were instructed to observe virtual malocclusion models and then performed three tasks to diagnose the type of malocclusion, determine clinical findings and develop treatment plans. Their satisfaction with the simulation experience were evaluated using a questionnaire containing five questions. Task abilities of trainees related to clinical features and treatment plans were significantly higher than that of students(p < 0.01). In both groups, the task completion time for the second case was significantly reduced compared to that for the first case(p < 0.01). The satisfaction survey showed high scores and positive responses for this simulation in both groups. If the prototype of VR simulation is continuously advanced, it will be applicable for orthodontic education in pediatric dentistry.
