Unusual Semimembranosus Muscle Metastasis from Hepatocellular Carcinoma (간세포암의 흔치 않은 반막양근 전이)

  • Hepatocellular carcinoma is one of the most common cancers worldwide. Extrahepatic metastasis commonly occur in the lung, lymph, nodes, bone, and adrenal glands. On the other hand, a metastasis of hepatocellular carcinoma to the skeletal muscle is rare. A 42-year-old woman presented for evaluation of a palpable mass with tenderness in her right thigh area. She has been diagnosed with hepatocellular carcinoma and pulmonary metastasis seven years ago and has received treatment. We performed incisional biopsy with suspicion of hepatocellular carcinoma metastasis from imaging studies and blood test results. The patient was finally diagnosed with metastasis of hepatocellular carcinoma in the semimembranosus muscle and treated by extensive resection. We report this case with a review of the relevant literature.

Interpretation of screening for congenital adrenal hyperplasia in preterm infants (미숙아에서의 선천 부신 과형성에 대한 선별검사의 해석)

  • Purpose : This study was undertaken to identify factors that influence 17-OHP levels in preterm infants and to suggest a reasonable follow-up schedule of screening for congenital adrenal hyperplasia (CAH) in preterm infants. Methods : The 17-OHP concentrations in filter paper blood spots of 427 preterm infants were obtained. The effects of gestational age (GA), systemic diseases, and antenatal dexamethasone on screening and follow-up 17-OHP values were investigated. Results : The screening 17-OHP values were markedly variable (range: 0.1-143.3 ng/mL). The screening 17-OHP levels were negatively correlated with GA (r=-0.535, P<0.01). In infants with GA<32 weeks, the screening 17-OHP levels were significantly higher in sick infants or infant with hypotension than in healthy infants. The screening values of prenatal dexamethasone-treated infants had a tendency to be low. In infants with initial 17-OHP values ${\geq}20ng/mL$, the intervals until rescreening 17-OHP <10 ng/mL or serum 17-OHP <20 ng/mL were negatively correlated with GA (r=-0.541, P<0.01) and were prolonged in infants with bronchopulmonary dysplasia (P<0.01). None of the preterm infants were confirmatively diagnosed with CAH. Conclusion : The 17-OHP values of preterm infants were influenced by GA, prenatal dexamethasone, and postnatal diseases. Because the 17-OHP values of preterm infants were markedly variable, a follow-up schedule should be developed considering both 17-OHP values and clinical status.

A Change of Adrenal Androgen and Cortisol in Kawasaki Disease (가와사끼병에서 부신 안드로겐과 코티솔의 변화)

  • Purpose : Endocrine and immune systems are connected and interdependent. Adrenal glands play an important role in this network and control the balance between serum levels of dehydroepiandrosterone sulfate(DHEAS) and cortisol. These steroids have an antagonistic effect on the T cell progression into Th1 and Th2 cells and on the induction of correlated interleukins. Therefore we evaluated the role of adrenal androgen and cortisol as immune modulators in Kawasaki disease( KD) with changes of T cell immunity. Methods : From April to August in 2001, we examined serum DHEAS and 24 hour urine free cortisol(F) before administration of immunoglobulin and steroids by radioimmunoassay in 14 KD patients. It's clinical severity was determined by Harada score and coronary lesion. Results : The age of the patient group ranged from 4 months to 4 years; its average age was 2.3 years. Three patients(21.4%) were below 1 year, 2(14.3%) between 1 and 2 years, 5(35.7%) between 2 and 3 years, 4(28.6%) between 3 and 4 years of age. Male to female ratio was 1:1.3. DHEAS was significantly decreased in patients($11.1{\pm}6.0{\mu}g/dL$) more than controls($81.6{\pm}13.3{\mu}g/dL$)(P<0.05). Twenty-four hour urine free cortisol was significantly increased in patients($36.9{\pm}21.9{\mu}g/dL$) more than controls($13.6{\pm}5.5{\mu}g/dL$)(P<0.05). Ratio of DHEAS/F was decreased remarkably in patients($0.33{\pm}0.20$) more than controls($6.65{\pm}2.56$)(P=0.016). There was no difference between ratio of DHEAS/F and Harada score, but its ratio was very low in patients with coronary aneurysm. Conclusion : These data demonstrate that there are changes of DHEAS and cortisol in acute stage of KD and the dis-equilibrium between two steroids may be relevant in the T cell immune response induction of Kawasaki disease. These changes support the use of DHEAS/F ratio as one of the predictive factors of coronary arteries complication.

A Study on the Image Registration Algorithms for the Accurate Application of Multimodality Image in Radiation Treatment Planning (방사선치료 계획시 다중영상 활용의 정확도 향상을 위한 영상정합 알고리즘 분석)

  • There have been many studies on the application of the reciprocal advantages of multimodality image to define accurate target volume in the Process of radiation treatment planning. For the proper use of the multimodality images, the registration works between different modality images should be performed in advance. In this study, we selected chamfer matching method and mutual information method as most popular methods in recent image registration studies considering the registration accuracy and clinical practicality. And the two registration methods were analyzed to deduce the optimal registration method according to the characteristics of images. Lung phantom of which multimodality images could be acquired was fabricated and CT, MRI and SPECT images of the phantom were used in this study. We developed the registration program which can perform the two registration methods properly and analyzed the registration results which were produced by the developed program in many different images' conditions. Although the overall accuracy of the registration in both chamfer matching method and mutual information method was acceptable, the registration errors in SPECT images which had lower resolution and in degraded images of which data were removed in some part were increased when chamfer matching method was applied. Especially in the case of degraded reference image, chamfer matching methods produce relatively large errors compared with mutual information method. Mutual information method can be estimated as more robust registration method than chamfer matching method in this study because it did not need the prerequisite works, the extraction of accurate contour points, and it produced more accurate registration results consistently regardless of the images' characteristics. The analysis of the registration methods in this study can be expected to provide useful information to the utilization of multimodality images in delineating target volume for radiation treatment planning and in many other clinical applications.

  • Localization of isoenzyme of glutathione transferase Pi class was compared in different human tissues by immunohistochemical analysis. Strong enrich-ment of GST-Pi in the epithelial tissues was observed in the granular layer of skin, nipple and esophagus which are vulnerable to exogenous chemicals and in the duct epithelium such as pancreatic, biliary, salibvary, renal tubules as well as in the steroid biosynthesis organs such as theca and granulosa of ovary, leydig cell of testis and zona reticularis of adrenal glands.

  • Background: In malignancies, detection of metastatic foci is of value in making therapeutic plans for treatment of disease and prevention of life-threatening complications. Common sites for metastasis of bronchogenic cancer include lymph nodes, liver, brain, adrenals and bone. Skull, vertebrae, ribs and long bones are common sites for bone metastasis. But in epidermoid carcinoma, the incidence of bone metastasis is relatively low and especially to the distal phalangeal bone is rare. Methods: We experienced a case of epidermoid carcinoma with the first distant metastasis to the fifth distal phalangeal bone, right toe. Results: The initial stage in the diagnosis of epidermoid carcinoma was T4N3MO. During the third round of anticancer chemotherapy, we recognized the distant metastasis to the fifth distal phalangeal bone for the first time. Localized abnormal findings were noted by bone X-ray and scanning. By a histopathologic examination of the amputated toe, we confirmed the metastasis of epidrmoid bronchogenic carcinoma. Conclusion: If localized abnormal finding is discovered at an unusual site for metastasis, we recommend physicians to consider the possibility of metastasis even though it is very low.

Anti-stress Effect by the Treatment of Vitamin C (Vitamin C 투여에 의한 항스트레스 효과)

  • The anti-stress effect by the treatment of vitamin C was investigated in this study. The treatment of ascorbic acid in the presence of $Cu^{2+}$ion induced strong time- and dose-dependent degradation of hitamine, and also the addition of histamine accelerated time-dependent decomposition of ascorbic acid in vitro. The treatment of ascorbic acid in $ODS^{od}/_{od}$rats, which cannot synthesize ascorbic acid, significantly decreased the urinary histamine. The protreatment of ascorbic acid, dexamethasone and promethazine inhibited the lethal effect induced by immobilization stress, but that of dimethylsulfoxide did not. The addition of ascorbic and to a culture of spleen cells of $ODS^{od}/_{od}$rats significantly increased the Con A-dependent T lymphocyte proliferation.

  • 본 연구는 청소년 스트레스 호르몬 변화를 보기위한 연구로 컬러 컨설팅 환경개선에 따른 스트레스 호르몬 변화를 연구하였다. 청소년 스트레스 연구들에 따르면, 아동이나 청소년도 성인만큼 스트레스를 경험하고 있고 청소년들의 스트레스는 정서적 부적응과 신체적 건강에도 영향을 미친다. 스트레스가 지속되면 부신피질에서 스테로이드 호르몬인 코티솔도 분비하는 근거를 통해 환경변화로 생긴 코티솔 분비를 측정하였다. 코티솔 분비를 통한 스트레스 반응 정도 측정은 침해성을 최소화하기 위한 간편한 방법인 타액검사로 실시하였으며 비 침습적인 방법으로 특별한 장비나 도움이 없이 수집할 수 있었고 부작용이 적은 효율적 검사였다. 연구 결과 전체 학생(46명)의 코티솔은 환경개선 사전($0.121{\pm}0.087{\mu}g/dL$)에 비해 사후($0.096{\pm}0.058{\mu}g/dL$)에 감소하였고(t=1.718, p=.093) 중고등학생(36명)의 코디솔은 사전($0.137{\pm}0.092{\mu}g/dL$)에 비해 사후($0.104{\pm}0.062{\mu}g/dL$)에 감소하였으며, 유의수준 10%에서 통계적으로도 유의미한 차이를 보였다(t=1.750, p=.089). 따라서 환경 개선 전에 비해 환경 개선 후 청소년들의 스트레스가 감소되었음을 알 수 있다. 하지만 초등학생(10명)의 코티솔은 사전($0.064{\pm}0.023{\mu}g/dL$)에 비해 사후($0.065{\pm}0.016{\mu}g/dL$)에 변화를 보이지 않아, 유의수준 5%에서 통계적으로 유의미한 변화는 나타나지 않았다(t=-0.121, p=0.906). 따라서 후두엽이 발달하는 중학생 시기는 환경 변화에 민감하며 색채 자극에 크게 영향을 미치는 시기인 만큼 본 연구에서 환경전후로 검사한 코티솔 분비의 측정값도 감소함을 볼 수 있었다. 하지만 본 연구는 환경 변화에 코티솔 분비량에 유의미한 변화를 나타내지 않은 초등학생들은 스트레스 호르몬 변화에는 차이가 없었지만 환경변화의 자극이 없었다고 볼 수는 없다.

Atypical Cushing's Syndrome Associated with Sex Steroids Excess in a Dog (개에서 발생한 성호르몬 과다 분비와 관련된 비정형 부신피질기능 항진증)

  • A 10-year-old, intact male, toy poodle was presented with abdominal distension, truncal alopecia, hepatomegaly, and sustained elevation of alkaline phosphatase. Vacuolar hepatopathy and glycogen deposition in hepatocytes were confirmed by liver biopsy and ultrasound-guided fine-needle aspiration with periodic acid-Schiff (PAS) staining of mass lesion respectively. Cortisol and some sex hormones associated with adrenal gland were analyzed at IDEXX Reference Laboratories before and 1 hour after ACTH stimulation. The results of analysis confirmed elevation of some sex hormones including androstenedione, progesterone and 17 hydroxyprogesterone, not cortisol concentration, before and 1 hour after ACTH stimulation. The dog was diagnosed as atypical form of hyperadrenocorticism associated with sex steroids excess. The treatment was initiated with trilostane (0.5 mg/kg, PO, q12hr) that is an adrenal steroid synthesis inhibitor. Trilostane was administered for 8 weeks and the clinical sign including truncal alopecia was improved.

General Activity and Stress Response of Rats Following Removal of the Mamillary Bodies (유두체를 떼어버린 흰쥐의 일반활동 및 스트레스에 대한 반응)

  • For the purpose of assessing the influence of the mamillary bodies on the general activity and stress response, mamillary body·damaged rats(mamillary body group), operated control rats, and normal control animals were prepared and 2 experiments were carried out. The mamillary bodies. were damaged electrolytically by passing 0.3 ma d.c. current through stereotaxically implanted electrodes. The operated control group received the same treatment except passage of current. In the 1st experiment, each subject belonging to the mamillary body group .(9 rats) or the operated control group (13 animals) was put individually into an activity cage across which an infrared beam traversed. The number of beam interruptions by the movement of the subject in the cage was. recorded hourly for 48 hours and was regarded as an index of general activity. In the 2nd experiment, each of the mamillary body group (52 animals), the operated control group (45 rats), and the normal control group (37 animals) was further divided into 4 subgroups and the adrenal ascorbic acid content of one of the 4 subgroups was measured before, another subgroup immediately after, a third subgroup 2 hours after, and the remaining subgroup 4 hours after termination of a cold stress ($-10^{\circ}C$ for 1 hour). Following results were obtained: 1. Though the total activities in 48 hours of the mamillary body group and the operated control group were not significantly different, the activities of the malnillary body group for a few hours. at the beginning of the measurement and also in the evenings of both the 1st and the 2nd experimental days were significantly greater than those of the operated control group. 2. Judged by the significant reduction in adrenal ascorbic acid content, the stress response of the mamillary body group was as marked as that of the operated control or the normal control group-immediately after termination of cold exposure. Recovery from the stress was accelerated significantly in the mamillary body group and insignificantly in the operated control group compared with that of the normal control group, the intergroup difference of the former two groups being insignificant. It was inferred from the above results that the mamillary bodies may exert an inconspicuous inhibitory influence upon the central mechanism of general activity, and that the mamillaryies bodies may not be the main seat of the stress mechanism, although more works are desirable to confirm the results.
