Effects of Functional Electrical Stimulation of Rectus Abdominis on Respiratory Capabilities in Children with Spastic Cerebral Palsy (복직근의 기능적 전기자극이 경직형 뇌성마비 아동의 호흡 능력에 미치는 효과)

  • 본 연구는 복직근의 기능적 전기자극이 경직형 뇌성마비 아동의 1회 호흡용적과 폐활량 등의 호흡능력과 복직근의 근육활동에 미치는 영향을 알아보기 위해서 대구 시내 OO특수학교에 재학 중인 14세 이상 16세 미만의 경직형 사지 뇌성마비 아동 6명(남자 3, 여자 3)을 대상으로 하여 복직근에 1회 20분. 주 3회씩 총 6주간 기능적 전기자극을 적용하였다. 기능적 전기자극의 적용 6주 전과, 직전에 두 번의 사전 검사와 적용 후 사후 검사를 실시하였다. MP30(Biopack, USA)을 이용하여 폐활량과 예측치 폐활량 백분율, 1회 호흡용적을 검사하였으며 Table면전극 근전도(Noraxon, USA)를 이용하여 복직근의 근육 활동량을 측정하였다. 분석결과는 다음과 같다. 1. 복직근의 기능적 전기자극 적용 6주 전과 직전의 폐활량은 유의한 차이가 없었으나, 적용 후의 폐활량은 적용 전에 비해 유의한 증가를 나타내었다(p<0.05). 2. 복직근의 기능적 전기자극 적용 6주 전과 직전의 예측치 폐활량 백분율은 유의한 차이가 없었으나, 적용 후의 예측치 폐활량 백분율은 적용 전에 비해 유의한 증가를 나타내었다(p<0.05). 3. 복직근의 기능적 전기자극 적용 6주 전과 직전의 1회 호흡용적은 유의한 차이가 없었으나, 적용 후의 1회 호흡용적은 적용 전에 비해 유의한 증가를 나타내었다(p<0.05). 4. 복직근의 기능적 전기자극 적용 6주 전과 직전의 복직근의 활동 값은 유의한 차이가 없었으나 적용 후의 복직근의 활동 값은 적용 전에 비해 유의한 증가를 나타내었다(p<0.05). 이상의 결과를 종합해 볼 때 6주간의 복직근의 기능적 전기자극이 경직형 뇌성마비 아동에 있어서 복직근의 근력을 강화시킴으로써 폐활량과 1회 호흡 용적 등의 호흡능력을 향상시킴을 확인할 수 있었다.

  • We experienced a case of 63 years old male patient who had synchronous rectus abdominis intramuscular schwannoma and chest wall lipoma. Schwannoma is rare benign tumor which derived from nerve sheath and mainly peripheral nerve of flexor part. The authors report rare synchronous schwannoma and lipoma development.

The effect of postural abdominal breathing exercise on posture muscle activity and respiratory function in adults in their 20s (자세에 따른 복식호흡 운동이 20대 성인의 자세근육 활성도 및 호흡 기능에 미치는 영향)

  • This research is to find out which position is more effective on exercise. This study compared paricipants in their twenties on sitting position & standing position doing abdominal breathing of their respiration & abdominal muscle activation for 4 weeks. In sitting position, FEV1 was the biggest after breathing exercise. In standing position, the gap was biggest between the FEV1 & forced expiratory flow before and after the exercise. In standing and supine position, Lt. rectus abdominis before exercise and Rt. rectus abdominis after exercise muscle activities were the biggiest. Both posture exercise of repeated measure of analysis result, before and after the exercise of the rectus abdomonis muscle and external oblique muscle were not difference in both muscle activity(p>.05). Left Musculus obliquus externus abdominis before and after physical exercise, it's showed a meaningful difference(p<0.05). Before and after exercise, saw a meaningful difference the efforts of FEV1/FVC(p<0.05). The left external oblique abdominis muscle showed the most muscle activity in the supine position before/after exercising, the efforts expiratory showed the lowest muscle activity.The results from the repeated measures ANOVA regarding to before/after the volume and muscle activity, it showed significant difference between at the right rectus abdominis muscle(p<.05). This study finally gathered data about the muscle activity according to the position. This could be used to find in what position the abdominal breath exercise is the post effective & a new treatment for the patients who has worsened respiration function.

Effect on Activation of Abdominal Local Muscles During Modified Bridge Exercise in Healthy Individuals (변형된 교각운동이 복부 국소근육의 활성화에 미치는 효과)

  • The purpose of this study was to investigate abdominal local muscle activity during modified bridge exercise. 17 subjects participated in this study. Abdominal muscles measured. External oblique abdominis (EO), internal oblique abdominis (IO), transvers abdominis (TrA), and rectus abdominis (RA) during general bridge exercise and modified bridge exercise, respectively. Electromyogram (EMG) and real-time ultrasound were used to verify alteration of muscles. Activation of RA and EO muscles of non-dominant foot was significantly difference in general bridge exercise group, not modifiedl bridge exercise group. In the modified bridge exercise group, thickness of IO and TrA muscle of non-dominant foot was significantly difference in modified bridge exercise group than general bridge exercise group. Therefore, modified bridge exercise may be apply as more effective exercise for local muscle activity than global muscle.

The Effects of Sling and Vibrator Application of Knee Push-Up Plus Motion on Trunk Muscle Activities in Healthy Subjects (무릎 푸시업 플러스 동작의 슬링과 진동기 적용이 정상인의 체간 근육 근활성도 변화에 미치는 영향)

  • The purpose of this study was to investigate the effects of sling and vibrator application of knee push-up plus motion on trunk muscle activities. Ten healthy adult males(age, 23.00±0.45 years; height, 176.60±1.64 cm; body mass, 67.50±1.22 kg; and BMI, 21.65±0.34 kg/㎡) were participated in this study as subjects. Three types' knee push-up plus motions were performed(basic knee push-up plus motion, BKPP; knee push-up plus motion with sling, KPPS; knee push-up plus motion with vibration, KPPV). We measured the right side's trunk muscle activities of the upper trapezius(UT), pectoralis major(PM), serratus anterior(SA), rectus abdominis(RA), and external oblique(EO). The research findings were as follows. UT, PM, SA, RA, and EO muscle activities were greatest during KPPV(p<.001). These results are expected to serve as reference materials for knee push-up plus motion applications in training programs for trunk muscle strengthening.

  • 본 연구는 교각운동시 불안정한 지지면과 안정적인 지지면을 제공하여 체간 및 하지의 근 활성도의 변화를 알아보고자 하였다. 이 연구의 대상자는 20대 남, 여 14명이었다. 실험방법은 일반적 교각자세와 다이나믹 에어쿠션을 이용한 교각자세를 실시한 후에 무선 8채널 표면근전도 검사기를 이용하여 체간부의 척추기립근, 복직근, 대퇴의 대퇴직근, 외측 슬괵근, 하퇴부의 전경골근, 비복근 외측두에서 근활성 정도를 측정하였다. 체간부의 척추 기립근, 대퇴이두근, 가자미근은 불안정한 지지면에서 근활성도가 높게 나타났다(p<.05). 따라서, 교각운동에서 불안정한 지지면의 제공은 신체의 배측에 위치한 근육의 활성화를 나타내어, 교각운동시 지지면의 안정성의 변화만으로도근육활성도의 변화가 가능함을 알 수 있었다.

Free Flap Transplantation to the Injured Foot (족부 손상에 시행한 유리조직 이식술)

  • The aims of free tissue transplantation to the injured foot are to cover the exposed blood vessels, nerves, muscles, tendons and bones, to clear up infection, to lessen the morbidity, to shorten the hospitalization, and to prepare for further surgical procedures when no local or transpositioning flaps are available. Authors have carried out free flap transplantation in 13 cases of crushing injury, osteomyelitis and electrical burn in the foot at Chonbuk National University Hospital from June 1992 through May 1996. The results were as follows : 1. 9 cases of 13 (69%) were sustained from the traffic accident. 2. The dorsalis pedis free flap transplantation has been performed most frequently in 5 cases (38.5%), followed gracilis muscle flap in 4 cases(30.7%), rectus abdominis muscle flap in 2 cases(15.4%), latissimus dorsi muscle flap and upper arm flap in 1 each. 3. 6 muscle flaps were covered with split thickness skin graft 20 days after microsurgical anastomosis. 4. All of 13 cases were survived after microsurgical procedure and showed excellent coverage in the foot.

Verification of the Difference in Effectiveness of Static Plank Exercise by Motion -Focusing on EMG Analysis- (정적 플랭크 운동의 동작 별 효과성 차이 검증 -근전도 분석을 중심으로-)

  • The purpose of this research was to investigate the comparison of core muscle activities according to different performance in static Plank exercise. Ten "J" University's males(age, 23.20±0.65 years; height, 174.54±1.51 cm; body mass, 70.00±2.24 kg; and BMI, 22.94±0.51 kg/m2) completed this study as the subjects. Four type's static Plank motions were performed(full Plank, FP; elbow Plank, EP; side Plank, SP; reverse Plank, RP). For the EMG analysis, we measured the core muscle activities of right side on the rectus abdominis(RA), external oblique(EO), latissimus dorsi(LD), and erector spinae(ES). This research's results were as follows. LD and ES muscle activities were greatest during RP(p<.001). RA and EO muscle activities were greatest during EP(p<.001). Therefore, this results are anticipated to serve as basic data for static Plank performance applications in effective exercise programs.
