The Effects of Virtual Reality-based Exercise on Energy Expenditure during Gait in Chronic Stroke Patients (가상현실을 이용한 재활운동이 뇌졸중 환자의 보행시 에너지 소모에 미치는 효과)

  • This study was aimed at determining the effect of virtual reality(VR) based exercise programs on energy expenditure during gait in chronic stroke patients. Thirty-two stroke patients were participated for this study. They underwent stroke for more than 6 months and were assigned to a VR-exercise group (n=16) or a control group (n=16). The VR-exercise group executed a rehabilitation exercise three times a week during 6 weeks. The VR-exercise was performed by PS2 for one hour. Control group maintained their usual life without application of exercise. Energy expenditure index(EEI) during gait was calculated from heart rate and gait velocity. After the completion of the VR-exercise, the EEI was decreased significantly (p<0.05). These results showed that the rehabilitation exercise using a virtual reality is effective in the improvement of energy efficiency during gait in chronic stroke patients.

  • In this study, the patterns and characteristics according to gait cycle were analyzed using to EMG signals during walking, and analyzed in the time domain and frequency domain. The experiments was performed divide to level-ground walking and stair walking, and gait cycle was analysis by stance and swing. In the sagittal plane by using the tilt sensor measures the angle of the lower leg, and EMG was measured from the quadriceps and biceps femoris. The tilt of the lower leg was showed the biggest tilt at HS, and showed lowest value at TO. All in walking according to the gait cycle IEMG showed a specific pattern, and is expected useful to determine the gait cycle and kind. In the frequency domain analysis was using STFT on able to frequency analysis according to time, and using the tilt sensor was identify gait cycle. We analyzed also spectrum of the results of the STFT in all gait types, and recognized that stance had broad bandwidth than that of swing. Through this study, it was confirmed the possibility of judgment and analysis of the gait cycle using EMG and the tilt in the sagittal plane of the lower leg. When used it, can improve the quality of life of amputation patients

  • 본 연구는 기능성 및 일반적 캐쥬얼화간에 빠른보행과 편안한 보행 시 족저 압력에 미치는 영향을 확인하여 보고자 한다. 남성 물리치료사 8명을 대상으로 하루 간격을 두고 동일한 대상에게 일반적 캐쥬얼화와 기능성 캐쥬얼화를 착용하고 10m 편안한 보행과 빠른 보행 동안 족저 악력을 각각 측정하였다. 전체, 1번째 발가락, 2-3번째 발가락, 4-5번째 발가락, 안쪽 중족골, 바깥쪽 중족골, 안쪽 뒷꿈치, 바깥쪽 뒷꿈치로 나누어 분석하여 4 보행 조건(일반적 캐쥬얼화-편안한 보행, 기능성 캐쥬얼화-편안한 보행, 일반적 캐쥬얼화-빠른 보행, 기능성 캐쥬얼화-빠른 보행) 간에 비교하였다. 비교 분석 결과 유의하지 않지만 빠른 보행이 편안한 보행 보다 안쪽 발가락과 뒷꿈치 압력이 증가하는 것으로 볼 때, 기능성 캐주얼화가 일반적 캐쥬얼화보다 보행의 움직임성을 증가시키는 것을 확인할 수 있었다.

Effect of Backward Walking Exercise on ROM, VAS score and Proprioception in Anterior Cruciate Ligament Reconstruction Patients (전방십자인대 재건술 환자의 후방보행 재활운동이 관절의 가동범위 및 통증점수, 고유수용성감각에 미치는 영향)

  • This study examined the effects of 4-weeks of backward walking exercise on ROM, VAS (Visual Analogue Scale) score and proprioception in anterior cruciate reconstruction patients. Fourteen subjects were randomly assigned to either a backward walking group (BWG, n=7) or a control group (CON, n=7) and then participated in rehabilitation program for 4 weeks. The ROM of the flexion significantly decreased in both groups (BWG, p<0.001 vs CON, p<0.01), while ROM of extension significantly decreased only in the BWG (p<.05) post exercise, but no significant difference was observed between groups. Additionally, VAS score decreased significantly in both groups (P<0.001) and BWG was lower than CON post exercise (p<0.001). Finally, proprioception decreased significantly in both groups (BWG, p<0.001 vs CON, p<0.05), but did not differ significantly between groups. These results suggest that the 4 week backward walking rehabilitation program improves ROM, VAS score, and proprioception and has positive effects on functional recovery of anterior cruciate ligament reconstruction patients.

The Effects of Tabata Exercise using Health Care Application on Balance, Gait and Activities of Daily Living in Patients with Stroke (건강관리 애플리케이션을 통한 타바타 운동이 뇌졸중 환자의 균형, 보행 및 일상생활 활동에 미치는 영향)

  • The purpose of this study was examined the effect of Tabata exercise using health care application on balance, gait and activities of daily living in patients with stroke. Twenty patients with stroke were participated patients with stroke in this study. Nineteen participants were randomly allocated to ten in experimental and nine in control groups. The both groups received the rehabilitation therapy during for 4 weeks. The experimental group performed Tabata exercise based on health care application, supervised under caregivers, during 30 min/day. The control group performed self-exercise supervised under caregivers, during 30 min/day. Both groups exercised for 4 weeks, 3 times a week, 30 min/day. Tabata exercise was trained by an physical and occupational therapists. The outcome measures were the BBS(: Berg Balance Scale), TUG(: Timed Up and Go, K-MBI(: Korean Modified Barthel Index). In results, the experimental groups showed a significant improvements after intervention in all assessments(p<.05), The control groups showed a significant improvements after intervention in BBS(p<.05) The experimental group showed a greater significant improvements than control group in TUG, BBS, K-MBI(p<.05). We suggest that Tabata exercise using health care application can have a effects of the improvements of balance, gait and activities of daily living than only self-exercise in patients with stroke.

Case Study of Gait Training Using Rhythmic Auditory Stimulation(RAS) for a Pediatric Patient with Cerebellar Astrocytomas (리듬청각자극(RAS)을 사용한 소뇌 별아교세포종(CA) 환아의 보행훈련 사례 연구)

  • This single case study is to examine the gait parameter changes of a 12-year old patient with Cerebellar Astrocytomas using RAS in gait training program. Kinematic and temporospatial changes were analyzed using VICON 370 Motion Analysis System. A total of nine RAS gait training sessions was provided and each training program took 30 minutes. Gait analysis revealed that the patient showed improvement in cadence, velocity, stride length, and step length and improved the range of joint movements by showing gait patterns similar to normal distribution from a pathological pattern. This study showed possibilities to apply the RAS technique to the various population including clients with cerebellum damaged; however more further research should be done in this area.

Biomechanical Research on Forward Gait with Backward Mechanism (후진 보법을 이용한 전방향 보행의 생체역학적 연구)

  • The purpose of this study was to investigate possibility of a forward gait with backward mechanism(dance gait) as rehabilitation and/or walking exercise by means of biomechanical variables. Thirteen professional women dancers(age, $21.1{\pm}1.3yrs$; height, $159.3{\pm}7.2cm$; body mass, $45.1{\pm}8.4kg$)participated in this study. We found that speed, stride length and double limb support time of a dance gait were more greater than backward gait, but stride width of dance gait less than a backward gait. Maximum RoMs, moments and powers of the lower limb joints on a dance gait were more frequent than a backward dance. These results were judged to be sufficient by the possibility of dance gait as rehabilitation and walking exercise.

  • The aims of this study are to investigate the walking intention detection of a rollator based on Infraed (IR) sensor measuring knee joint anterior displacement and leg muscle activities. We used Active Walker attached IR sensor to measure the knee joint anterior displacement and EMG signal of leg muscles(rectus femoris, biceps femoris, tibialis anterior, gastrocnemius) were taken by Delsys bagnli-8ch. Subjects were eight healthy males(age $23.7{\pm}0.5years$, height $175.4{\pm}2.3cm$, weight $70.6{\pm}5.6kg$) and they were involved in experiments which had been proceeded 30 minutes a week, during 3 weeks. This system indicates that the knee joint anterior displacement had the distinction increases according to the gait slope and velocity. We showed the increase of the femoral muscle activities along the anterior tilt and the increase of the crural muscle activities along the posterior tilt.

  • We developed a walking-assistance robot for walking rehabilitation and assessed the kinematic characteristics of a prototype. The walking-assistance robot is composed of hip, knee, and ankle joints, and each joint is driven by a motor with a decelerator. The equations of angular displacement while walking were derived by theoretically analyzing human locomotion, and the calculated angular displacements were then applied to the robot controller. The output angular displacement of each joint was measured and compared with its input angular displacement in walking experiments on a treadmill at various walking speeds and strides. The differences between the input and output angular displacements are 5.22% for the hip and 2.97% for the knee joints, and it has been confirmed that the walking-assistance robot works well.

The Effects of Rehabilitation Exercise Using a Home Video Game (PS2) on Gait Ability of Chronic Stroke Patients (가정용 게임기를 이용한 재활운동이 뇌졸중 환자의 보행 능력에 미치는 효과)

  • The purpose of this study was investigate the effect of rehabilitation exercise using a home video game(PS2, Sony) on gait ability in the chronic stroke patients. The subjects of this study were 32 patients who underwent stroke for more than 6 months and were assigned to a game-based exercise group (n=16) or a control group (n=16). The Game-based exercise group executed a rehabilitation exercise three times a week during 6 weeks, at the rehabilitation unit. The exercise was performed by PS2 for one hour. Control group maintained their usual life without application of exercise. Outcome measures included gait ability. After the completion of the game-based exercise, 10m walking velocity was improved significantly (p<0.05). Six minute walking distance was increased significantly (p<0.05). These results showed that the rehabilitation exercise using a home video game is effective in the improvement of gait ability in chronic stroke patients. This study suggest that interesting, motivating game-based rehabilitation exercise and effective in recovery of function exercise for chronic stroke patients should be necessary to develop and apply.
