Status of Infectious Disease Inpatients at Long-Term Care Hospitals in Korea (국내 요양병원의 감염병 입원환자 실태 분석)

  • This study investigated the status of infectious disease inpatients at long-term care hospitals in Korea. A descriptive study was conducted on patients with 14 infectious diseases at 798 long-term care hospitals during 2016-2017. The number of infected patients, total admission days, and total medical expenses were higher in 2017 than in 2016. The most common infectious diseases were enterocolitis due to Clostridium difficile, influenza, and scabies. The number of hospitals with patients who had enterocolitis due to C. difficile and resistance to carbapenem was higher in 2017 than in 2016. Hospitals with 150-299 beds had higher numbers of infectious disease patients than those with under 150 or over 300 beds. Therefore, intensive efforts are needed to control the most common diseases at long-term care hospitals, such as enterocolitis due to C. difficile, influenza, and scabies. It is recommended to apply relevant guidelines related to infection control management as well as implement educational programs. It will also be necessary to develop applicable infection monitoring standards and support the facilities and health workforce of long-term care hospitals under 300 beds through an effective infection surveillance system.

Recognition of Skin Infection and Infection Management Practice on Caregivers in Geriatric Hospital (노인요양병원 요양보호사의 피부감염에 대한 인식 및 감염관리 수행)

  • Purpose: This study was to investigate the recognition of skin infection (RSI) and management practice (MP) on caregivers in geriatric hospital. Methods: The subjects were 209 caregivers who work at geriatric hospital in G city and J do. Data were analyzed with ${\chi}^2$-test, t-test. and ANOVA using SPSS 18.0 program. Results: Prevalence rate of caregivers' skin diseases was 76.6%. Diagnosis of Skin disease was contact dermatitis 42.5%, scabies 26.9%, and skin xerosis 25.0%. The Mean RSI score was 3.81 and MP was 4.12. In addition, MP was significantly different by number of bed hospitals (F=4.63, p=.011) and number of caring patients (F=2.67, p=.049). Conclusion: This study will be a useful on continuing education for caregivers and basis for the guidance of medical infection control standards development.

Emergency Medical Technology Students' Awareness and Performance of Standard Precautions in Hospital Infection Control (응급구조과 학생의 병원감염 표준주의에 대한 인지도와 수행도)

  • This study was to provide the basic data to raise the efficient practices that can reduce the Infection Prevention and Exposure by understanding the awareness, Performance degree of the Administrative Standard Guideline of Nosocomial Infection Control for Emergency Medical Technician students who are exposed to hospital infections while conducting clinical practice. After the study, the performance degree was lower than the awareness of the Performance degree of the Administrative Standard Guideline of Nosocomial Infection Control. And awareness and performance degree are have significant positive correlation, significant differences statistically(r=0.325, p=0.000). In the future, to improve the Performance degree of Nosocomial Infection Control for Emergency Medical Technician students, the continuing education programs and the curriculums should be performed in the practice agencies.

The Study on Organization, Infection Controller, Patient Infection Control of Dental Clinic in Certain Areas (일부지역 치과의 기관, 감염관리자, 환자의 감염관리에 관한 연구)

  • The purpose of this study was to investigate the performance of dental infection control. This survey was performed on 158 of the dental hygienist in certain areas. The research was performed using a self-reported questionnaire and interview method from June to July, 2014. The collected data was analyzed by PASW Statistics ver. 18.0. Guideline of infection management, infection controller, regular infection control training, Sterilization of the handpiece/per head resulted 57~74% for dental hospital. It was higher than 13~20% for dental clinics (p<0.05). Infection control guidelines and vaccination recognition of dental were more performed in dental hospital. Patient's hand hygiene performed, tooth brushing before treatment, cross infection educational experience was less than 20%, research cooperation of medical history was over 90% (p<0.05). Performing rate of the dental hospital workers were investigated higher in protective personal devices, infection control of treatment room. A correlation analysis about institutional support, infection control of dental hygienists, patients with infection control cooperation showed a positive correlation statistically significant. Infection control can be enhanced when the medical staff, the patient, the organization combined cognitive and practice. Dental hygienist is required to recognize and practice the infection control guidelines through continuing education.

Factors related to infection management performance of health workers at Long-Term Care Hospitals in Korea: systemic review and meta analysis (국내 요양병원 종사자의 감염관리 수행도 관련요인 연구: 체계적 고찰 및 메타분석)

  • The current study is a meta analysis study to identify the factors related to infection management performance and the effect size according to the factors. Data collection was included from the data of KMBASE, RISS, KISS, DBpia, National Library of Korea, Pubmed, and EMBASE. R3.5.1 was used for the data analysis. A total of 22 factors were identified. The meta analysis showed the effect size as follows: gender(.16), age(.30), education(.32), beds(.28), needs of education in infection management(.44), participating in education of infection management(.25), recognition/knowledge in infection management(.70). Based on the findings, evidence based programs need to be provided to improve the infection management performance of health workers at Long-Term Care Hospitals.

Outbreak of Hepatitis A caused by Groundwater Contamination in a Long-term Psychiatric Hospital in Korea (장기 요양 정신병원에서 지하수 오염에 의하여 발생한 A형간염 집단발병)

  • 목적: 2017년 지하수를 식수로 사용하고 있는 한 장기요양 정신병원(H병원)에서 A형간염 환자가 집단 발병하여 이에 대한 역학조사를 실시하고 조치 결과를 기술하고자 하였다. 방법: 노출기간 동안 H병원의 근로자 및 재원 환자 234명을 대상으로 사례군 조사 디자인으로 역학조사를 실시하였고, IgM, IgG 혈청검사 및 A형간염 바이러스(HAV)에 대한 PCR검사를 시행하였다. 또한 오염원으로 의심되는 지하수, 병원에서 제공되는 식품 및 인근 저수지의 물에서 HAV 검사를 실시하였고, 검출된 HAV는 유전형 검사를 진행하였다. 결과: H병원 환자 및 직원 234명 중 IgG 양성인 168명을 제외한 66명 중 19명이 최종적으로HAV 감염자로 확인되어 감수성자 중 발병률은 28.8%로 나타났다. 환자, 지하수, 식품(석박지) 및 저수지에서 동일 유전형의 HAV가 검출되어 지하수 오염에 의한 집단발병으로 결론 내렸으나, 최초 오염원은 확인하지 못하였다. 유행 종결 선언 이후 지하수에 대한 관리로 염소소독과 UV 조사를 하였음에도 불구하고 6개월 동안 지속적으로 HAV가 검출되어 새로운 관정을 개발하여 상황을 종결하였다. 결론: 본 연구에서 지하수를 식수로 사용하는 장기요양 정신병원에서 지하수 오염에 의한 19명의 HAV 집단발병을 조사하였다. HAV 항체가 없는 대상자 중에서 HAV의 높은 발병률을 확인하였다. 지하수 수질검사에서 바이러스 검사는 포함되어 있지 않기 때문에 지하수가 HAV에 오염시 HAV 집단발병 가능성이 높고 상당기간 지속적으로 검출되기 때문에 지하수에 대한 관리지침에 바이러스 검출을 위한 방안을 추가하고 관련 법을 정비할 필요가 있다.

Knowledge and Education Needs Related to Multidrug Resistant Organisms Infection Control among Small and Medium Size Hospital (중소병원 간호사의 다제내성균 감염관리 지식 및 교육요구도)

  • This study was conducted to aid in the development of an infection control educational program for nurses, by assessing the knowledge and educational demand of nurses for MDRO infection control. Totally, 115 nurses participated in the study. Data were collected from November 15-30, 2019, using structured questionnaires. Descriptive statistics, t-test, and ANOVA were applied for analyzing the data. Experience of caring for MDRO patients was reported by 86.1% nurses, whereas 67.8% nurses had received training on MDROs. The average score for knowledge on MDROs was 25.51 out of 27 points, with minimal correct answers given for the query on level of disinfection for medical equipment used by patients, criteria for the preemptive precaution, patient management in the cohort, and timing for removing personal protective equipment. The educational demand was highest for assignment to the precaution, criteria for screening examination, and management of outbreak. Also, educational needs differed with respect to the general educational characteristics and position of the individual. We propose the need to differentiate the educational status according to the career when developing the MDROs program, and the necessity to execute education of MDROs for new nurses and career nurses.

A Study on the Practice for Infection Prevention of Dental Clinic Worker (치과의료기관 종사자의 감염예방 실천에 관한 연구)

  • This survey was conducted between January 15, 2014 and February 25, 2014 to investigate the practice for infection prevention among dental hygienists. Data were obtained from 294 dental clinic worker who worked in dental hospital (clinics) of Gwangju and Chonnam. At the conclusion of this investigation, according to the conclusion of this investigation, the necessity of infection control education for dental hygienists and washing their gowns after caring infected patients affected the practice for infection prevention. They also well practiced the infection prevention working in dental hospital. To prevent infection in dental clinic, it is necessary to provide the continuing education programs for dental infection control to dental clinic worker.
