  • Purpose: The purpose of the present study was to examine the causal relationships among hospital nursing organizational characteristics (organizational climate, workload), individual characteristics (experience, education) and outcome variables (job satisfaction, job stress, task performance) by constructing and testing a conceptual framework. Method: Five large general hospitals located in Seoul were selected to participated. The total sample of 245 registered nurses represents a response rate of 94 percent. Data for this study was collected from January to February in 2006 by questionnaire. Path analyses with LISREL program were used to test the fit of the proposed model to the data and to examine the causal relationships among variables. Result: Both the proposed model and the modified model fit the data excellently. The model revealed relatively high explanatory power of work stress (40%), job satisfaction (46%) and task performance (27%) by predicted variables. In predicting work stress, job satisfaction and task performance, the finding of this study clearly demonstrate organizational climate might be the most important variable. Conclusion: Based on the findings of the study, it was suggested that desirable organizational climate was needed to increase the nurses' mental and physical health as well as qualified task performance.

Method: The subjects of this study were 215 nurses who had worked in one university hospital in P city. For this study the following study tools were used: the Staggers Nursing Computer Experience Questionnaire and self-leadership scale. Data was analyzed using mean, standard deviation, t-test, ANOVA, Scheffe and Pearson correlation coefficient. Results: The mean score of each item in informatics skill domain and informatics knowledge domain $2.93{\pm}.49$ and $3.40{\pm}.50$. The average score of self-leadership was $3.39{\pm}.53$. 'Self-leadership' was positively related with 'nursing informatics competency'. Conclusion: This study proved that there were significant positive correlation between nursing informatics competency and self-leadership. These results suggested that it would be necessary to develop regular educational program which promotes the nurses' self-leadership as well as nursing informatics competency in schools. And practical education program for nurses must be planned continuously for informatics competency in hospitals.

  • Purpose: The aim of this study was to investigate whether job satisfaction in clinical nurses was dependent on work intensity and whether physical discomfort mediated the relationships between these variables. Methods: Structural equation modeling was used with a sample of 253 clinical nurses from four general hospitals. In the model, absolute work intensity, relative work intensity, and flexibility were considered as exogenous variables and physical discomfort as a mediating variable. Data were collected using self-report measures such as the Labor Intensity Questionnaire, the Rating of Perceived Exertion, and the Index of Job Satisfaction. Results: The results of the structural equation modeling found that the higher scores on absolute and relative work intensity were positively associated with physical discomfort but only relative work intensity was significantly related to job satisfaction. Physical discomfort mediated the relationships between absolute work intensity and job satisfaction and between relative work intensity and job satisfaction. Among three kinds of work intensity, only relative work intensity had direct and indirect effects on job satisfaction. Conclusion: The findings suggest that increase in relative work intensity might play an important role in decreasing job satisfaction in clinical nurses and a reasonable reward system considering relative work intensity could be necessary.

  • Purpose: Purposes of this study were to identify non-value-added nursing activities (NVANAs) and to estimate the amount of time spent on NVANAs among total nursing activities. Methods: Non-value-added nursing activities were identified though the use of a focus group. The nurses in the focus group were nurses working in a tertiary hospital. They discussed possible activities that could be NVANAs. Based on the focus group discussion, a pilot study was done to examine the actual occurrence of NVANAs in clinical settings. Results: The focus group discussion showed that NVANAs occurred in ten categories of nursing performances including communication with physicians, communication with other departments, medications, equipment/supplies, nursing records, tests, admission, discharge, and transfer. Direct nursing activities accounted for 35.5% of total nursing activities while indirect nursing activities accounted for 64.5%. Of indirect nursing activities, 16% were NVANAs. Most NVANAs were related to communication and equipment/supplies. Conclusions: To improve the quality and efficiency of nursing activities, it is necessary to identify NVANAs and their causes. Results of this study suggest that improvement in the work process and nursing unit structure, support for equipment/supplies, and effective communication are needed to reduce NVANAs in tertiary hospitals in Korea.

  • Purpose: To examine the changes in nurse staffing in hospitals after implementing the policy of differentiating inpatient nursing fees by staffing grades. Method: The study sample included 43 tertiary hospitals, 185 general hospitals, and 282 non-general hospitals that were operating in both 1999 and 2008. Nurse staffing grade was categorized from Grade 1 (highest) to 6 (lowest) in 1999 or Grade 7 in 2008, based on the nurse-to-bed ratio. Results: Tertiary hospitals at Grade 3 and Grade 4 accounted for 49% and 35%, respectively, in 2008, whereas 63% were Grade 6 in 1999. General hospitals at Grade 6 decreased from 87% to 48%. In non-general hospitals, little change was found in the staffing distribution, in that 92% still remained in Grade 6 or 7 in 2008. Forty tertiary hospitals (93%) and 45% of general hospitals improved their staffing grades, while only 7% of non-general hospitals did. Greater likelihood of improvement in staffing grades was found in general hospitals located in metropolitan areas or having 250 or more beds. Conclusion: Elaboration of the financial incentive system is needed to increase the policy impact on staffing improvement.
