Purpose: This study was done to investigate nurses' knowledge of, and compliance with the multidrug-resistant organism (MDRO) infection control guidelines. Methods: A survey questionnaire was developed based on the institutional and national guidelines and was administered to a convenience sample of 306 nurses in a university hospital. Results: The mean score for knowledge was 33.87 (percentage of correct answers: 82.61%). The percentages of correct answers for basic concepts, route of transmission, hand washing/protective devices and environment management were 74.27%, 94.29%, 92.90% and 75.54% respectively. The mean compliance score was 4.15 (range: 1-5). The compliance scores for education, communication, contact precaution, disinfection, surveillance culture, and hand washing were 3.29, 4.05, 4.20, 4.50, 4.40 and 4.48 respectively. Nurses indicated "lack of time (30.06%)", "lack of means (10.78%)" and "lack of knowledge (9.48%)" as reasons for noncompliance. Conclusion: While most educational programs have focused on hand washing or use of protective devices to prevent transmission of MDRO in acute care settings, hospital nurses' knowledge of the basic concepts of MDRO and environmental management has remained insufficient. Nurses are relatively non-compliant to the guidelines in the areas of education (staff, patient, family) and communication. Comprehensive educational programs are needed to decrease hospital infection rates and to improve the health of patients.
Purpose: In this study influenza A (H1N1) infection control performance and stress in nurses were surveyed, to identify factors influencing stress for the nurses, and to provide basic materials for promoting infectious disease control by nurses. Method: The participants were 447 nurses who had worked at isolation clinics and/or isolation rooms in hub hospitals in the Honam region during the period of the outbreak of 2009 pandemic influenza A (H1N1) virus. Data were collected in February 2010 using self-report questionnaires. Results: The performance of participants from clinics was high for 'helping patients wearing masks', but low for 'maintaining distance over 1 meter among waiting patients'. The performance of participants from hospital rooms was high for 'putting a surgical mask on a patient going out of the room', but low for 'keeping patients' family out of the room'. The participants' stress was higher in those younger and less experienced nurses, those working at a hospital with 500-999 beds, those working at both isolation clinics and rooms, and those working at a clinic longer. Conclusion: In order to lower nurses' stress from working at isolation clinics/rooms for infectious diseases, we may need to deploy experienced nurses and limit their working days to five or less.
Purpose: This study was conducted to assess the factors that affect the confidence of nurses at small and medium-sized hospitals in the control of infections caused by multidrug-resistant organisms and to establish basic data for the interventional strategy to improve the nurses' execution of the control of infection caused by multidrug-resistant organisms. Methods: A total of 154 nurses working at four small and medium-sized hospitals situated in City C were selected as participants of this study. Data were collected using a structured questionnaire and analyzed using the IBM SPSS 21.0 software. Results: Confidence in controlling infections caused by multdrug-resistant organisms had a positive correlation with the level of knowledge of this control and the health beliefs of the nurses. Factors that affect nurses' confidence in controlling infections caused by multidrug-resistant organisms included perceived benefits (β=.431, p<.001) and knowledge (β=.354, p<.001) with an explanatory power of 36.4%. Conclusion: It was found that the nurses have higher confidence in executing their duties with higher perceived benefits of and knowledge about the control of infections caused by multidrug-resistant organisms. As such, it is deemed necessary to establish strategies to elevate the advantages of and knowledge about infection prevention activities at the time of training/education on controlling infections caused by multidrug-resistant organisms.
목적: 국가 결핵 관리 프로그램을 통해 국내 결핵 발병률과 사망률은 감소하고 있지만, 결핵은 여전히 중요한 감염병이다. 저자들은 국내 소아청소년을 대상으로 한 결핵 관련 논문의 동향과 연구 분야를 분석하여 향후 연구가 필요한 분야를 확인하기 위해 연구를 시행하였다. 방법: 국내외 6개의 데이터베이스에서 'tuberculo*' 또는 'TB', 'child*' 또는 'adolescen*' 또는 'neonat*' 또는 'infant*' 또는 'pediatric*' 그리고 'korea*'를 포함하여 제목과 초록을 검색하였다. 국내 소아와 청소년을 대상으로 한 결핵 관련 문헌의 논문 종류, 발행 연도, 연구 영역, 투고된 학술지명, 연구 대상을 분석했다. 결과: 검색된 257편의 문헌 중 120편의 논문이 분석에 포함되었다. 이 중 82편(68.3%)은 원저였고, 증례 33편(27.5%), 종설 4편(3.3%), 지침 1편(0.8%)이었다. 원저의 연구 영역을 분류했을 때, 가장 흔한 주제는 임상적 특징(36.6%)이었고 진단(29.3%), 접촉자 조사(9.8%), 역학(6.1%), 치료(4.9%), 백신(3.6%), 잠복결핵감염(3.6%), 합병증(3.6%) 그리고 결핵에 대한 인식(2.4%)에 대한 연구가 뒤를 이었다. 1962년에 출판된 논문을 시작으로 1960년대 4편, 1970년대 10편, 1980년대 11편, 1990년대 22편, 2000년대 26편, 2010년 이후 47편의 문헌이 발표되었다. 결론: 국내 소아를 대상으로 한 결핵 관련 연구는 지난 50년 간 증가해 왔지만, 연구 영역이 임상적 기술과 진단에 치우쳐 있었다. 치료나 백신과 같은 또 다른 영역에 대한 연구가 다양하게 수행되어야 할 필요가 있겠다.
일본에서 '팀 의료'는 임상병리사가 당뇨병팀, 영양지원팀, 감염관리팀, 진료지원팀 등 다양한 종류의 팀에서 활발하게 활동하고 있다. 전반적으로 최근 의료환경이 지속적으로 변화함에 따라 임상병리사는 채혈, 검체 채취, 검사, 심전도 등을 수행할 뿐만 아니라 의사, 간호사와 협력하여 '진료보조나 지원'에 적극적으로 참여할 것이 요구되고 있다. 따라서 국내 병동이나 응급실에 상주하는 임상병리사는 임상검사실과 연결고리 역할을 수행함으로써 의료의 질 향상과 의료안전 확보에 더 잘 기여할 수 있을 것이고, 의사와 간호사는 업무부담이 경감되고 보다 나은 환자관리를 위해 헌신할 수 있을 것으로 기대된다.
Purpose: This study was designed to identify recognition and the performance level for nosocomial infections amongst student nurses and to provide data for preparing on efficient policy and control program for nosocomial infections. Method: 191 senior nursing student participated in this research from 5 nursing college which allowed data collection, in the city of Seoul. The questionnaire was composed of 73 items(likert scale) about eight areas; aseptic technique, disinfection, precaution, hand washing, urinary tract infection, respiratory infection, catheter related infection, and self care about nosocomial infections. Results: The mean score of recognition and performance level for management of nosocomial infection were 4.29 and 3.41 respectively. The mean score of the recognition level was significantly lower than the performance level in the eight areas. The mean score of both recognition and performance were highest in the area of disinfection. However, the mean score of recognition was lowest in the area of aseptic technique and the mean score of performance was lowest in the area of catheter related infections. The correlation of recognition and performance level was statistically significant. Conclusions: These research findings should be useful in promoting an intensive and continuous educational program on nosocomial infection for nursing students and to establish an efficient policy for preventing nosocomial infections.
Purpose: The purpose of this study was to evaluate the theory of planned behavior (TPB) in the structural relationship of nursing students' performance related to nosocomial infection control (NIC). Method: Data was collected by using a questionnaire completed by 238 nursing students of three nursing colleges in Suwon, Sokcho. Results: The mean score of performance related to NIC was 3.86. The highest mean score of performance related to NIC was 4.18 (${\pm}0.91$) for 'hand washing' and the lowest mean score was 3.56 (${\pm}1.08$) for 'respiratory system'. In prediction of the intention of nursing students' on NIC, the attitude, the subjective norm, and the perceived behavior control(PBC) of TPB resulted in statistically significant influencing factors (p<0.050). These three variables explained 47.6% of the total variance of the intention of nursing students' on NIC. In predicting the performance related to NIC, the PBC resulted in the direct and main influencing factor of nursing students' performance related to NIC (p<0.010). Intention was not a significant determinant. These two variables explained 13.2% of total variance of the performance related to NIC. Conclusion: This study shows the TPB model's applicability in explaining performance related to NIC of nursing students and highlights the importance of PBC for strategies to enhance performance related to NIC in nursing students.
목적: 이 연구는 2019학년도 역학조사관 입문교육 과정에 참여한 29명의 수습과정생에게 참여형 자기주도 학습 역학조사관 연수 프로그램(FETP)의 효과와 만족도 등 역량 변화를 분석해 그 결과를 향후 과정 개발의 참고 자료로 활용하고자 하였다. 방법: 교육 프로그램의 만족도와 교육 후 모듈에 대한 역량 변화를 평가하는 연구가 수행되었다. 만족도와 역량의 차이 비교는 크루스칼 왈리스 검정(Kruskal-Wallis test)를 실시하였고, 역량의 차이는 윌콕슨 부호순위검정(Wilcoxon signed rank test)에 의해 이루어 졌다. 결과: 2019년 FETP에 참여한 역학조사관 중 여성은 48.3% 였으며, 40세 미만은 9.4% 였다. 역학조사관 입문교육과정 모듈(역학조사, 보건통계 및 정보통계, 감염병 국가 체계, 감염병 질환 감시 체계, 진단 및 실험실 검사, 생물 안전 및 관리, 주요 감염성질환 관리와 조사, 커뮤니케이션, 협동과 리더십, 일반과정)별 만족도는 실무적 도움, 전문성, 기능, 태도 등에서 4점(5점 만점)을 초과하였고, 전체 4.2±0.21(5점 만점)점으로 높은 수준이였다, 모듈의 교육훈련 전후 평균 점수는 2.25±0.91, 3.68±0.63점 등으로 유의한 향상이 있었으며, 모든 모듈 및 하위 주제들도 유의한 향상이 있었다(p<0.001). 그 중에서 현장역학조사 경험이 가장 높은 변화가 있었고, 표본 수집과 실무가 가장 낮은 역량 변화가 있었다. 결론: 2019년 진행된 입문교육 과정은 수료 후 학생들의 역량은 개선되었고, 만족도는 높은 편이었다. 참여형 자기주도학습의 촉진은 역량을 향상시킬 뿐만 아니라 보건 종사자들의 자신감을 높일 수 있었다.
의료시장의 개방과 더불어 의료기관의 경쟁력확보를 위한 핵심전략으로 환자관리의 중요성이 강조되고 있다. 이 연구는 치과위생사의 업무능력향상과 전문성을 높이기 위해 수행업무인식과 교육요구주제를 조사 분석하여 Dental Hygienist Coordination System을 개발하는데 기초자료로 활용하고자 수행하였다. 비례표본추출법을 이용하여 광주광역시 85개 치과의원에 근무하는 치과위생사 156명을 대상으로 직접 방문하여 설문법을 이용하여 조사 분석한 바, 다음과 같은 결론을 얻었다. 1. 코디네이터교육경험 유무에 따른 교육과정별 우선순위 요구과목으로 서비스분야에서는 코디네이터이론과 실무(유무:상담 및 교육실무), 코디네이터를 위한 기초(유무:의학용어), 코디네이터를 위한 원무관리(유무:건강보험), 시설 및 환경관리(유:환경관리, 무:감염관리)이고, 매니저분야에서는 의료서비스마케팅(유무:의료환경과 고객특성, 의료서비스마케팅), 조직인사관리(유무:병원조직), 회계 및 재무관리(유무:병원회계), 건강보험실무(유:진료비청구관리, 무:진료비산정)으로 조사되었다(p>0.05). 2. 코디네이터교육경험 유무에 따른 교육요구과목 차이를 분석한 결과, 코디네이터이론과 실무 0.14, 건강보험실무와 의료서비스마케팅 0.13이고, 인사관리 - 0.23, 코디네이터를 위한 의료기초 -0.19로 조사되었다(p>0.05). 3. 코디네이터교육경험 유무에 따른 교육요구주제는 경험자군과 비경험자군 모두 상담 및 교육실무로 나타났으며, 교육경험자군과 비교육경험자군의 차이를 보면 고객접점관리 0.25, 고객응대기법 0.24로 조사되었다(p>0.05). 이상과 같은 결론에서 우선순위가 높은 교육과목인 코디네이터 이론과 실무, 건강보험 실무에서 진료비 청구관리를 중심으로 체계적인 교육과정을 포함한 Dental Hygienist Coordination System(D.H.C.S)개발에 필요한 근거자료로 활용될 수 있을 것으로 사료된다.
본 연구는 건강신념모형을 적용하여 임상에서 근무하는 간호사의 손씻기 수행도를 분석하기 위한 서술적 조사연구이다. 본 연구의 대상은 G 광역시에 위치한 일개 병원에 근무하는 임상 간호사 162명이다. 자료수집은 2017년 9월 1일부터 2017년 10월 31일까지였다. 수집된 자료는 SPSS (SPSS Version 20.0, IBM, US)를 이용하여 빈도분석, 백분율, 일원분산분석, t-검정, pearson 상관계수분석 및 다중회귀분석으로 통계처리 하였다. 일반적 특성에 따른 손씻기에 대한 수행도는 모든 영역에서 평균은 3.1점으로 높았으나 통계적으로 유의한 차이는 보이지 않았다. 일반적 특성에 따른 손씻기에 대한 건강신념은 연령에서는 지각된 유익성 영역에서 유의한 차이를 보였으며(p<0.05), 근무연수에서는 지각된 유익성, 지각된 장애성 영역에서 유의한 차이를 보였다(p<0.05). 다섯 개의 하위영역간의 상관관계를 분석한 결과는 지각된 유익성과 지각된 장애성은 부의 상관관계를 나타냈으며, 수행을 위한 동기는 정의 상관관계를 보였다. 본 연구의 결과를 통해 손씻기에 대한 건강신념을 고려한 지속적인 병원에서의 감염관리교육과 올바른 손씻기 교육을 진행한다면 손씻기 수행도가 높아질 것으로 판단된다.
