A Study on the Status of Dental Trauma in 14 - 16 Year-Old Adolescents in Yangsan (양산시 14~16세 청소년의 전치부 외상 상태에 대한 조사연구)

  • The number of reported traumatic injuries has been increasing in recent years, particularly that of dental trauma in children and adolescents. While the risks associated with dental trauma in this population have been increasingly realized, domestic reports on this issue seem to be insufficient. The purpose of this study was to investigate the prevalence of dental trauma and to evaluate the relevant risk factors and the level of self-recognition. 1,371 adolescents attending middle schools at Yangsan were surveyed via clinical examination and questionnaire; the results were as follows: According to the clinical examination, the overall prevalence of dental trauma was 16.8% with preponderance of males (19.2%) compared to females (13.7%) (p < 0.05). The average number of injured teeth per adolescent with a history of trauma was 1.34. Comparing the prevalence, maxillary central incisors and enamel fracture occupied the highest ranks by tooth type and mode of trauma, respectively. Class II division 1 malocclusion and overjet exceeding 8.0 mm were identified as significant risk factors (p < 0.05). The degree of self-recognition of dental trauma showed a low coincidence rate compared with results of the clinical examination. Males expressed a higher level of satisfaction toward the results of treatment for dental trauma than females (p < 0.05).

Clinical manifestations patterns of allergic disease in Korean children under the age of 6 : multi-center study (다 기관 설문 조사를 통한 알레르기 질환의 연령별 임상 양상)

  • Purpose : It is widely known that allergic diseases progress through an allergic march. However, there have not been any recent reports in Korea on how the diseases progress. Methods : Parents who visited one of the Pediatric Allergy Clinics of four university hospitals in Seoul with a child under the age of 6 years during the period from May 1-30, 2006 were asked to complete a questionnaire. Results : A total of 229 parents answered the questionnaire about their children, of which 122 were male and 107 were female. The most common allergic disease before 2 years of age was atopic dermatitis (79%). Meanwhile, in 2- to 4-year age group, allergic rhinitis (38%) and asthma (37%) were predominant. The most common allergic disease in the 4- to 6-year age group was asthma (72%), followed by allergic rhinitis (64%). Seventy-three percent of the children had a family history of allergic disease, most often in the fathers (39%). Among patients with allergic rhinitis, 50% had experienced asthma in their earlier days and 30% had experienced atopic dermatitis. In addition, 57% of the children with asthma had suffered from atopic dermatitis. Parents believed that asthma was the most serious allergic disease. Conclusion : In the present study, allergic disease showed a tendency to march from atopic dermatitis to asthma and then to allergic rhinitis. Early diagnosis and treatment of atopic dermatitis is, therefore, considered important for prevention of the allergic march.

Left to Right Ratio of Autopneumonectomy State in Pulmonary Tuberculosis Patients (폐결핵 환자의 편측 전폐 파괴에 대한 좌우 비교)

  • Background: There are several causes that the total destruction of unilateral lung can occur. Pulmonary tuberculosis of these causes may lead to destruction through chronic inflammation and fibrosis. In such circumstances, the left to right ratio is supposed to be different. We performed the study with 224 cases for total destruction of unilateral lung, who were admitted at National Kongju Tuberculosis Hospital for recent 4 years. Method: On admission chest film, radiologic pattern was classified into 4 types. And we analyzed patients' age, sex, duration of illness, sputum AFB smear and culture. Result: 1) The male to female ratio was 2:1 and 74% of the patients were between 31 and 60 years of age. 2) One hundred and ninety eight cases(88%) had the duration of illness over 3 years. Namely, most of patients had long history. 3) Sputum AFB smear and/or culture were positive mostly (80%). 4) As for the radiologic pattern, 181 cases (81%) had the total destroyed lung in left and partial lesion in right lung, 31 cases (14%) total destroyed lung in right and partial lesion in left lung, 11 cases (5%) total destroyed lung in left only, and 1 case total destroyed lung in right only. 5) In the radiologic pattern, there is no significant difference in terms of age, sex, duration of illness and sputum examination. Conclusion: From these results, we found that the total destruction of unilateral lung in pulmonary tuberculosis occurred generally in the left side.

  • This study survey research of infection control, wastewater management, and instrument disinfection according to characteristic at Daegu, Gyeongbuk Province In 2012 and 2017, the same 114 dental clinics to identify the infection management behavior and prevent infection of dental medical institutions. Scored mean 3.37 points on 8 items of infection control, 95.5% in "records of the patient's medical history", 1.8% in "presence of a wastewater facility." Scored 94.7% in "disinfection of metal trays", 17.5% in "storage from a spitting receptacle in the waste bin and commissioned management.", Scored higher in of infection control, wastewater management and instrument disinfection according to general characteristics and dental characteristics in 2017 than in 2012. Points were higher dental hygienist, University graduation, Type of duty was counseling and management, hospital or higher, Number of dentist(dental hygienist, chair, patient) was high. Therefore raise a need for infection control into consideration the dental characteristics and education and promotion regardless of the hospital size.

  • The aim of this study was to determine the prevalence of self-assessment halitosis and to offer the basic information that establishes a strategy of effective health promotion and to enhance the quality of life. This study performed a questionnaire survey. The questionnaire was filled out by the 362 adults, who lived in Incheon Metropolitan City from May to July, 2010. The data were analyzed using a Chi-square test in the SPSS version 12.0 program. The prevalence rate of self-assessment halitosis was 24.3%. Hypertension, diabetes, medication and inadequate oral hygiene practice were significantly associated with self-assessment halitosis. Other factors of self-assessment halitosis were older age and occupation. As a result, dental professionals have to investigate a systemic disease history thoroughly. And they have to educate regular checkup of oral health and to perform a continuous feedback. The self-assessment halitosis has a negative influence on mental health and social life, so dental professionals must take into consideration about the halitosis.

  • Conversion disorder and factitious disorder should be ruled out before making diagnosis of malingering. For this work, inspection of patient's behavior along with complete neurological examinations, psychological tests, and meticulous psychiatric interview are necessary. Facial expression test, thermography, dynamometry were failed differentiating conversion disorders to the malingered pain and motor symptoms, however, controlled diagnostic block showed positive result partly in patients with regional or cervical pain syndrome. Chronic pain patients who are related to the process of litigation encounter stressful life situations which lead them into various neurobehavioral and psychosocial complications. Most of suspected malingered patients would not revealed pure form rather mixed with factitious and/or conversion features. At the time of increasing number of chronic patients associated with traffic accidents or industrial injuries who are involved in litigation, psychiatrist are needed sharp eyes and tenacity for evaluating malingering or medically unexplained symptoms.

  • Histiocytosis-X is a disorder characterized by proliferation of cells, phenotype of Langerhans' cells. Traditionally, these diseases include three subtypes of commen histologic features and clinical findings respectively. Eosinophilic granuloma shows a solitary or multiple skeletal lesions. Hand-Schuller-Christian disease often fatal due to widespread extraskeletal lesions. Patient' chief compaints are frequent bone pain and swelling. During rescent 10 years in our hospital, 13 patients, 11 male and 2 female, were diagnosed as Histiocytosis-X. And we evaluated clinical features, radiographic finding, histological characteristics and treatment modalities according to the each subtypes. Major treatment methods of the eosinophic granulma was a chemotherapy combined with local currettage, wide resection of the lesion should be performed carefully in selective cases.

  • Nontyphoidal Salmonella are gram negative bacilli organism, which may induce systemic infection such as febrile enteritis, bacteremia, and osteomyelitis. Main route of infection is known as food but also possible through reptile, amphibian, and fish raised as pets in the house. There is no known cases report of Salmonella infection through pets in Korea and also rare in the overseas. We report 2 patients who visited Severance children's hospital with chief complaint of fever and diagnosed as nontyphoidal salmonellosis. Each case had a history of raising turtle or tropical fish with possibility of Salmonella infection through these pets. Increasing incidence of raising pet reptile and fish lately, contact precaution and proper prevention and control of Salmonella infection of these pets especially in children under 5 years old are necessary due to higher risk of serious complications of salmonellosis.

  • Dentinogenesis imperfecta is a hereditary disorder that causes abnormal dentin formation and tooth morphology. Affected teeth show very thin dentin and large pulp chamber. Immediately after their eruption, the teeth tend to be destroyed rapidly. Early dental intervention is recommended to improve the feeding capacity and quality of life for the child, to maintain vertical dimension, and to promote healthy and functional dentition. This case report describes a treatment for a 4-year-2-month-old girl whose primary and permanent dentition were affected by Dentinogenesis Imperfecta. Shell teeth were observed in the entire primary dentition. The pulp of most teeth was exposed and the patient complained of pain. Most paternal family members were transferred to the Dentinogenesis Imperfecta. She needed an extensive dental treatment, but her compliance could not be expected because she was very young and anxious. So we decided to treat her under general anesthesia. All molar teeth are restored with Stainless Steel crown. Maxillary anterior teeth were extracted and mandibular anterior teeth are restored with composite resin. The patient's pain disappeared and masticatory function was restored.

A Case Report of Referral Pain on Mandibular Toothache Originated from Myofascial Pain (근막동통으로 인한 하악 구치부 연관통의 임상증례)

  • Majority of toothache is caused by abnormality of pulpal or periodontal tissues. However, there are numerous nonodontogenic sources that may be responsible for pain felt in the tooth. Nonodontogenic toothache may result from muscle, maxillary sinus, neuropathic, neurovascular, even cardiac and psychogenic problems. Myofascial pain is one of the most common abnormality characterized by palpation of a hard band which acts as the trigger point that causes stiffness and fatigue of the muscle, referral pain in tooth, tension-type headache, and hyperalgesia. Masseter muscle particularly induce referral pain in maxillary and mandibular molars. This case reported the treatment of the pain in right mandibular molar originated from myofascial pain of the right masseter. The pain is improved by general and reversible treatments such as muscle exercise, physical therapy, and medication. Nonodontogenic toothache should undergo proper differential diagnosis in order to avoid unnecessary dental treatments, such as endodontic, periodontic treatment, and tooth extraction, which would fail to alleviate the symptom of the patient.
