Evaluation of the Efficiency of the Foxtail Millet Vacuum Cushion in Skin Cancer Radiation Treatment (자체 제작한 Foxtail Millet Vacuum Cushion의 광자선 피부암 치료 시 유용성 검증)

  • Purpose: The sufficiency of skin dose and the reemergence of patient set-up position to the success of skin cancer radiation treatment is a very important element. But the conventional methods to increase the skin dose were used to vacuum cushion, bolus and water tank have several weak points. For this reason, we producted Foxtail Millet Vacuum Cushion and evaluated the efficiency of the Foxtail Millet Vacuum Cushion in skin cancer Radiation treatment. Materials and Methods: We measured absolute dose for 3 materials (Foxtail Millet Vacuum Cushion, bolus and solid water phantom) and compared each dose distribution. We irradiated 6 MV 100 MU photon radiation to every material of 1 cm, 2 cm, 3 cm thickness at three times. We measured absolute dose and compared dose distribution. Finally we inspected the CT simulation and radiation therapy planing using the Foxtail Millet Vacuum Cushion. Results: Absolute dose of Foxtail Millet Vacuum Cushion was similar to absolute dose of bolus and solid water phantom's result in each thickness. it Showed only the difference of 0.1~0.2% between each material. Also the same result in dose distribution comparison. About 97% of the dose distribution was within the margin of error in the prescribed ranges ($100{\pm}3%$), and achieved the enough skin dose (Gross Tumor Volume dose : $100{\pm}5%$) in radiation therapy planing. Conclusion: We evaluated important fact that Foxtail Millet Vacuum Cushion is no shortage of time to replace the soft tissue equivalent material and normal vacuum cushion at the low energy radiation transmittance. Foxtail Millet Vacuum Cushion can simultaneously achieve the enough skin dose in radiation therapy planing with maintaining normal vacuum cushion' function. Therefore as above We think that Foxtail Millet Vacuum Cushion is very useful in skin cancer radiation treatment.

  • To compare 2 beam arrangements, circumferential equally angles (EA) beams or partially angles (PA) beams for stereotactic body radiation therapy (SBRT) of primary lung cancer for intensity-modulated radiation therapy (IMRT) and volumetric-modulated arc therapy (VMAT) delivery techniques with respect to target, ipsilateral lung, contralateral lung, and organs-at-risk (OAR) dose-volume metrics, as well as treatment delivery efficiency. Data from 12 patients, four treatment plans were generated per data sets ($IMRT_{EA}$, $IMRT_{PA}$, $VMAT_{EA}$, $VMAT_{PA}$). The prescribed dose (PD) was 60 Gy in 4 fractions to 95% of the planning target volume (PTV) for a 6-MV photon beam. When compared with the IMRT and VMAT treatment plan for 2 beams, conformity index, homogeneity index, high dose spillage, D2 cm (Dmax at a distance ${\geq}2cm$ beyond the PTV), R50 (ratio of volume circumscribed by the 50% isodose line and the PTV), resulted in similar. But Dmax of the Organ at risk (OAR), spinal cord, trachea, resulted in differ between four treatment plans. Especially $HDS_{location}$ showed big difference in 21.63% vs. 26.46%.

  • Dose distribution of Korean radiation workers classified by occupational categories was analyzed. Statistics of the occupational radiation exposure(ORE) in 2002 of the radiation workers in diagnostic and dental radiology were obtained from the Korea Food and Drug Agency(KFDA) who maintains the database for individual radiation dose records. Corresponding statistics for the rest of radiation workers were obtained by processing the individual annual doses provided by the Korea Radioisotope Association(KRIA) after deletion of individual information. The ORE distribution was classified in term of 28 occupational categories, annual individual dose levels, age groups and gender of 52733 radiation workers as of the year of 2002. The total collective dose was 66.4 man-Sv and resulting average individual ORE was 1.26 mSv. Around 80% of the workers were exposed to minimal doses less than 1.2 mSv. However, it appeared that the recorded doses exceeded 20 mSv for 43 workers in the industrial radiography and for 147 workers in the field of radiology. Particularly, recorded doses of 23 workers in radiology exceeded the annual dose limits of 50 mSv, which is extraordinary when the working environment is considered. It is uncertain whether those doses are real or caused by careless placing of dosimeters in the imaging rooms while the X-ray units are in operation. No one in the workforce of 16 operating nuclear power plant units was exposed over 20 mSv in 2002. Number of workers was the largest in their 30's of age and the mean individual dose was the highest in their 20's. Women were around 20% of the radiation workers and their average dose was around one half of that of man workers.

  • In total body irradiation (T81) for leukemia, we have a two methode. One is a AP (anterior-posterior) method and the other is a Lateral methode. Our hospital used lateral methode. T81 must consider about body contour, because of homogeneous dose distribution. For compensation about irregular body contour, we use compensator. For T81 treatment, we must be considered, accurate manufacture of compensator and accurate calculation of dose. We developed the automatic program for T81. This program accomplished for compensator design and dose calculation for irregular body. This program was developed for uses to use in a windows environment using the IDL language. In this program, it use energy data for each energy: TMR, output factor, inverse square law, spoiler, field size factor. This program reduces the error to happen due to the manual. As a development of program, we could decrease the time of treatment plan and care the patient accurately.

The Analysis of a Cerrobend Compensator and a Electronic Compensator Designed by a Radiation Treatment Planning System (방사선치료계획장치로 설계된 Cerrobend 선량보상체와 전자 선량보상체의 제작 및 특성 분석)

  • In this study, the physical compensator made with the high density material, Cerrobend, and the electronic compensator realized by the movement of a dynamic multileaf collimator were analyzed in order to verify the properness of a design function in the commercial RTP (radiation treatment planning) system, Eclipse. The CT images of a phantom composed of the regions of five different thickness were acquired and the proper compensator which can make homogeneous dose distribution at the reference depth was designed in the RTP. The frame for the casting of Cerrobend compensator was made with a computerized automatic styrofoam cutting device and the Millennium MLC-120 was used for the electronic compensator. All the dose values and isodose distributions were measured with a radiographic EDR2 film. The deviation of a dose distribution was $\pm0.99 cGy\;and\;\pm1.82cGy$ in each case of a Cerrobend compensator and a electronic compensator compared with a $\pm13.93 cGy$ deviation in an open beam condition. Which showed the proper function of the designed compensators in the view point of a homogeneous dose distribution. When the absolute dose value was analyzed, the Cerrobend compensator showed a $+3.83\%$ error and the electronic compensator showed a $-4.37\%$ error in comparison with a dose value which was calculated in the RTP. These errors can be admtted as an reasonable results that approve the accuracy of the compensator design in the RTP considering the error in the process of the manufacturing of the Cerrobend compensator and the limitation of a film in the absolute dosimetry.

  • 본 연구는 소아백혈병의 치료 방법 중 하나인 조혈모세포이식법의 전처치로서 사용되고 있는 전신방사선조사법에 대하여 선량분포에 대한 연구를 진행한 것으로, MCNPX 프로그램을 이용하여 모의실험을 하였다. 결과 피부선량은 평균 112.43 mGy/min, 심부장기선량은 평균 47.52 mGy/min으로 나타났으며, 조직보상체의 재질과 거리에 따라 다르게 나타나는 경향성을 볼 수 있었으며, 결과를 바탕으로 전신방사선조사를 임하기 전에 정량적인 선량평가를 할 수 있을 것으로 생각된다.

Evaluation of Dose Distributions Recalculated with Per-field Measurement Data under the Condition of Respiratory Motion during IMRT for Liver Cancer (간암 환자의 세기조절방사선치료 시 호흡에 의한 움직임 조건에서 측정된 조사면 별 선량결과를 기반으로 재계산한 체내 선량분포 평가)

  • The dose distributions within the real volumes of tumor targets and critical organs during internal target volume-based intensity-modulated radiation therapy (ITV-IMRT) for liver cancer were recalculated by applying the effects of actual respiratory organ motion, and the dosimetric features were analyzed through comparison with gating IMRT (Gate-IMRT) plan results. The ITV was created using MIM software, and a moving phantom was used to simulate respiratory motion. The doses were recalculated with a 3 dose-volume histogram (3DVH) program based on the per-field data measured with a MapCHECK2 2-dimensional diode detector array. Although a sufficient prescription dose covered the PTV during ITV-IMRT delivery, the dose homogeneity in the PTV was inferior to that with the Gate-IMRT plan. We confirmed that there were higher doses to the organs-at-risk (OARs) with ITV-IMRT, as expected when using an enlarged field, but the increased dose to the spinal cord was not significant and the increased doses to the liver and kidney could be considered as minor when the reinforced constraints were applied during IMRT plan optimization. Because the Gate-IMRT method also has disadvantages such as unsuspected dosimetric variations when applying the gating system and an increased treatment time, it is better to perform a prior analysis of the patient's respiratory condition and the importance and fulfillment of the IMRT plan dose constraints in order to select an optimal IMRT method with which to correct the respiratory organ motional effect.

  • The MCNP 4.2 code was used to calculate the thermal neutron flux distributions for $(n,\;{\gamma})$reaction in mainshell, annular plate, and subshell of the calandria of a CANDU 6 plant during operation. The thermal neutron flux distributions in calandria mainshell, annular plate, and subshell were in the range of $10^{11}{\sim}10^{13}\;neutrons/cm^2-sec$ which is somewhat higher than the previous estimates calculated by DOT 4.2 code. As an application to shielding analysis, photon dose rates outside the side and bottom shields were calculated. The resulting dose rates at the reactor accessible areas were below design target, $6 {\mu}Sv/h$. The methodology used in this study to evaluate the thermal neutron flux distribution for $(n,\;{\gamma})reaction$ can be applied to radiation shielding analysis of CANDU 6 type plants.

  • The purpose of this study is to confirm the effect of reducing the surface dose around the radiation field in breast cancer radiotherapy using the Field-in-Field (FIF) technique. X-ray was exposed from a linear accelerator (Linac) was used for irradiation, and the surface dose was measured with a glass dosimeter. The source-to-surface distance (SSD) was 90 cm, the field size is 10 × 10 cm2, and the X-ray energy was 6 MV and 10 MV, respectively. The surface dose of the FIF was compared with the dose measured in the physical wedge (PW) and dynamic wedge (DW). Wedge angles of 15° and 30° were used in the PW and DW, respectively. Surface dose was measured at 1 cm, 3 cm, and 5 cm from the center of the field size, respectively. According to the results, FIF showed lower surface dose compared to PW and DW regardless of the energy of the X-ray beam, wedge angle, and dose measurement point. Since FIF could reduce the radiation dose in periphery of the field size in breast cancer treatment, it is expected to be able to reduce the secondary damage caused by the radiation beam as well as to obtain a uniform dose distribution on the target.

The Development of a Multi-Purpose Irradiator and the Characteristic of Dose Distribution (다목적 방사선 조사장치 개발 및 선량분포특성)

  • The design, construction and performance test of a convenient multi-purpose irradiator is described. A multi-purpose irradiator using Cesium-137 has been developed for studies of low dose radiation effects in biology and for calibration of Thermo Luminescent dosimeter(TLD). During the operation, three rods of radioactive material which are 10cm in length revolve 180 degrees and irradiate biological samples, or TLD, and return to their shielded position, after the programmed time. A programmable Logic Controller(PLC) controls the sequence of operation, interlock, motor rotation and safety system. The rotation speed of biological samples can vary up to 20 RPM. A real time monitoring system was also incorporated to check and control the operation status of the irradiator. The capacity of the irradiation chamber was 4.5 liters. The isodose distribution at arbitrary vertical planes was measured by using film dosimetry. The dose-rate was 0.13 cGy/min in air and 0.11 cGy/min in water equivalent material in the case of Cesium-137. Range of activity was 2 Ci. The homogeneity of dose distribution in the chamber was ${\pm}$7%. The actual radiation level on the surface was within permissible levels. The irradiator had a maximum 0.35 mR/min radiation leakage on its surface.
