The Usefulness of Al Face Block Fabrication for Reducing Exposure Dose of Thyroid Glands in Mammography (유방촬영 시 갑상샘의 피폭선량 경감을 위한 Al Face Block 제작 및 유용성에 관한 연구)

  • Currently, there are many studies being conducted around the world to reduce exposure dose to radiation for patients to receive medical treatments in a safe environment. We developed and fabricated of this shield that the patients are protected from the radiation and are need of safety control during breast imaging. In this study, for breast imaging, GE Senography 2000D were used and set at SID 65cm, 28kVp, and 63mAs. The measuring instrument was Fluke's Victoreen 6000-529. And we performed Face Block on with 30 patients. The chamber on the actual thyroid glands to take CC and MLO and measure the dosage before and after wearing the Face Block. For the results, after wearing the Face Block, exposure was decreased by 53.8%-100% and 65.8% in average in CC View and by 50%-100% and 60.7% in average in MLO View. The development of the Face Block that practically decreased the exposure dose of thyroid glands, crystalline eyes during breast imaging and reduced the patients' anxiety during breast imaging. The Face Block is expected to improve patients' satisfaction and contribute to reducing patients' exposure dose, but more efforts should be made to reduce exposure dose to medical radiation.

  • In medical institutions, there are radiation-related workers such as radiological technologists, physicians, dentists, and dental hygienists who handle diagnostic radiation generators. Also, there are work assistants, such as nurses and assistant nurses, who assist in radiation treatment or transfer patients to the radiation examination room. Radiation exposure management for radiation-related workers is carried out under the 「Medical Service Act」, but there is no legal basis for work assistants, etc. And the management of radiation exposure for diagnosis is regulated by the 「Medical Service Act」, and the management of radiation exposure by therapeutic radiation and nuclear medical examination is governed by the 「Nuclear Safety Act」. Thus, to improve the management of radiation exposure for diagnosis, the regulations on radiation exposure management for diagnosis under the 「Medical Service Act」 were compared and reviewed with those of the 「Nuclear Safety Act」. As a result, the main contents are as follows. First, it is necessary to legislate to include nurses, assistant nurses, and clinical practice students who are likely to be exposed to radiation besides radiationrelated workers as subjects of radiation exposure management for diagnosis. Second, when a radiation-related worker for diagnosis is confirmed to be pregnant, the exposure dose limit should be defined. Third, it is necessary to revise the regulations on the types of personal exposure dosimeters in the 「Rules on the Safety Management of Radiation Generators for Diagnostics」. Fourth, it seems that health examination items for radiation-related workers, radiation workers, and frequent visitors should be the same. Fifth, It is necessary to unify and regulate diagnostic radiation and all medical radiation, including therapeutic radiation and nuclear medicine, in one legal system.

A Measurement of Exposure Dose for Patient Transporter (환자 이송원의 피폭선량 측정)

  • The medical institutions use radiation generating devices and radioactive isotopes to diagnose and treat patients. The patient transporter performs work in an environment that is more likely to be exposed to radiation when compared with the general public, such as inevitably entering the radiation management area for patient transfer, or transferring the isotope-administered patient at a short distance. For this reason, we conducted a study to determine the degree of exposure of the patient transporter. The 12 patient transporters working at Incheon A General Hospital are eligible. From April 1, 2019 to April 30, 2019, the dosimeter was used in the chest for one month and the accumulated dose was measured. The dosimeter used was a Optically Stimulated Luminescence Dosimetry (OSLD) and the dose reading was OSLD Microstar Reading System. As a result of cumulative dose measurement for one month, the average of the deep dose was 0.13 mSv and the surface dose was 0.13 mSv, and the cumulative dose for one month was multiplied by 12 to estimate the cumulative dose expectation As a result, the average of the deep dose and the surface dose were 1.52 mSv and 1.51 mSv, respectively. It is necessary to classify the patient transporter as a frequent visitor in order to measure and manage the exposure dose, increase the knowledge of protection against radiation through education and training, and prevent radiation trouble through medical examination.

  • 유방촬영은 날로 증가추세에 있으며 압박을통해 영상을 얻는 것이 화질을 향상시키고 피폭선량을 줄이는데 필수적이다. 그러나 압박대 자체의 두께로 인해 산란선과 피폭선량을 증가시킬 수 있으므로 압박대 재질에 대한 고찰이 필요하다. 현재 임상에서 쓰이고 있는 재질은 폴리카보네이트이며 플라스틱 계열이다. 환자의 피폭선량을 줄이기위해 노력한다면 이보다 더 좋은 재질에 대해 고려해볼 필요가 있기에 본 연구에서는 플라스틱 계열 물질 중 비결정 플라스틱에 대한 방사선투과성에 대해 비교해 보고자 한다.

Shielding Effect of Radiation Protector for Interventional Procedure (중재적 방사선 분야 방호용구 차폐효과)

  • The purpose of this study is to evaluate shielding effect of radiation protector for interventional radiologists in procedures by measuring inside and outside of radiation protector. In this study, we measured the radiation dose of 4 interventional radiologists during TACE and PTBD procedure for 4 month(2005.05-2005.09). Absorbed dose were measured by TLD placed underneath and over radiation protector such as Goggle, Thyroid protector, Apron and placed on the 4th finger of Hand. In addition, we measured background radiation dose in the control room using TLD. During TACE procedure, using 0.07 mmPb Goggle decreased average 53.8% of radiation dose rate in continuous fluoroscopic mode and decreased average 77.6% of radiation dose rate in pulse fluoroscopic mode. Using 0.5 mmPb Thyroid protector decreased average 88.9% of radiation dose rate in continuous fluoroscopic mode and decreased average 92.8% in pulse fluoroscopic mode. During PTBD procedure, using 0.07 mmPb Goggle decreased radiation dose rate average 62.7%, 87.9% by 0.5 mmPb Thyroid protector, 90.5% by 0.5 mmPb Apron. The average fluoroscopic time of PTBD was 6.14 min. shorter than TACE procedure, but radiation exposure dose rate of PTBD was 3 times higher in total body dose, and 40 times higher in hand dose rate than TACE. Interventional radiologists must wear thicker protector recommended over 0.5 mmPb. Also, they must use lead Goggle during interventional procedure. Abdomen dose decreased average 38.4% by drawing a lead curtain under the patient's table, therefore, they must draw a lead curtain to shield scattering ray. Radiation exposure dose decreased average 59.0% by using pulse fluoroscopic mode. So radiologists would better use pulse fluoroscopic mode than continuous fluoroscopic mode to decrease exposure dose.

  • Most of the spatial scattered dose caused by the scattered rays generated by the collision between the object and X-rays is relatively easily absorbed by the human body as electromagnetic waves in the low energy region, thereby increasing the degree of radiation exposure. Such spatial scattering dose is also used as an indicator of the degree of radiation exposure of radiation workers and patients, and there is a need for a method to reduce exposure by reducing the spatial scattered dose that occurs indirectly. Therefore, in this study, a lead-free radiation shielding sheet was proposed as a way to reduce the spatial scattering dose, and a Monte Carlo (MC) simulation was performed based on a chest X-ray examination. The absorbed dose was calculated and the measured value and the shielding rate were compared and evaluated.
