Foot pressure analysis of laterally & medially wedged rearfoot insole in gait (보행 시 발 뒤축 내/외측 경사진 안창의 족부압력 분석)

  • 경사진 안창은 안창의 경사로 발의 자세를 교정함으로써 하지 통증을 완화시키는 데 사용되는 보조치료기구로 내측 경사진 안창(MWI: medially wedged insole)과 외측 경사진 안창(LWI: laterlly wedged insole)으로 나뉜다. 본 연구는 내/외측 경사진 안창이 족부압력에 미치는 영향을 정량적으로 분석하고자 한다. 분석변수는 압력중심(COP: center of pressure)의 궤적 변화, 발바닥 내 세 개 구획에 상대적으로 작용하는 수직 힘(RVF: relative vertical force) 그리고 여덟 개 해부학적 지점의 압력이다. 경사진 안창의 경사조건은 내/외측 각각 $5^{\circ},\;8^{\circ},\;15^{\circ}$이며, 경사가 없는 안창을 포함해 총 일곱 가지 안창이 실험에 사용되었다. 실험참여자로는 20대 성인 남성 10명이 참여하였다. 실험결과 앞발 누름기에서 MWI와 LWI는 모두 COP를 내측으로 이동시키고 외측 압력을 감소시켰다. 뒤축 접지기에서 MWI는 COP를 내측으로 이동 시키고 내측압력을 증가시키는 반면, LWI는 COP를 외측으로 이동시키고 외측압력을 증가시켰다. 그리고 RVF 분석결과 MWI와 LWI는 모두 앞발 부위 힘의 크기를 감소시켰고, LWI만이 발 뒤축 부위 힘의 크기를 증가시켰다.

The Effect of Protective Socks Combined with Functional Insole on Plantar Foot Pressure in Healthy Adults: A Pilot Study (정상인 대상으로 기능성 인솔과 결합한 보호형 양말이 발바닥 압력에 미치는 영향: 예비 실험 연구)

  • PURPOSE: This study examined the ability of experimental socks combined with a functional insole to reduce plantar foot pressure in healthy subjects. METHODS: The study enrolled 15 subjects. An in-shoe measurement device was used to measure the peak plantar pressure while walking. The peak forefoot, midfoot, and hindfoot plantar pressure data were collected under two conditions: the subjects were wearing their own socks and while they were wearing the experimental socks. The paired t-test was used to determine the differences in peak plantar pressure between the two conditions at all three positions. RESULTS: Wearing the experimental socks resulted in a significantly higher peak plantar pressure in the medial forefoot and midfoot areas than wearing one's own socks (p<.05), and also in significantly lower peak plantar pressure in the medial and lateral hindfoot (p<.05). However, there were no significant differences of peak plantar pressure between experimental and own socks in middle and lateral forefoot (p>.05). CONCLUSION: The experimental socks combined with a functional insole decreased plantar pressure in the hindfoot and supported the medial longitudinal arch. However, it is necessary to change the design and material of the forefoot area in the functional insole to prevent foot ulcer at that location in people with diabetes mellitus.

Evaluation method in gait analysis (보행분석 시스템을 이용한 보행평가)

  • This paper suggests the evaluation method of gait analysis in measurements obtained using the "Foot Scanner" and "Foot Analyzer" system. Previous examination method with the unaided eye on the sole of the foot and analysis method of pressure distribution in gait have been discussed by many researchers. Also they have concerned with pressure curve, COP(center of pressure) trace, and velocity in COP. However experiment results depend on test environment and conditions of subjects. Consequently we need to regard the special energy parameter for solving the problem. The kinetic energy and impulse parameter can be used as parameters of gait analysis. The results of this study confirmed the validity of presented of the parameters through the experiment with eight subjects.

  • 착의행동을 일으키는 전단계의 감각으로서 국소의 냉감각과 체표 각 부위에 있어서의 냉점분포에 대하여 측정을 하였다. 피험자는, 냉점분포 측정에 한국인 여자 대학생 30명, 냉감각측정에는 한국인과 일본인 여자 대학생 8명을 대상으로 하였으며, 환경조건은 실온 $29^{\circ}C{\pm}1^{\circ}C$, 상대습도 $55{\pm}5\%$의 인공기후실로 하였다. 냉자격으로서는 $0^{\circ}C$의 냉각이 사용되었으며, 측정부위는 냉점분포 측정의 경우 체표 20부위, 냉감각 측정의 경우 체표 13부위로 하였고 측정결과는 다음과 같다. (1) 체표면의 냉점분포 밀도는 부위에 따라서 다르며, 체간부와 두부는 냉점분포 밀도가 높고 말단부에 갈수록 그 분포가 낮아졌다. 특히 가장 민감한 부위는 복부였다. 즉 체간부와 두부는 중핵온을 일정하게 보지하므로서 중핵부 체내온의 변화를 방어하기 위한 온도 수용 부위로서의 역할을 하고 있으며, 말초부는 냉점분포가 낮아서 기온에 따라 체표온(외각온)을 광범위하게 변화시키는 사지부의 온도정보입력의 역할을 담당하고 있다고 하겠다. (2) 각 부위에 있어서의 냉감각 순위는 일본인과 한국인 피험자간에 고도의 상관(r=0.93)이 보여져, 양 피험자군간에 차이는 볼 수 없었다. 3) 냉점분포 밀도가 높은 부위인 복부는 냉감각도 가장 민감하였으며, 냉점분포 밀도가 낮은 하퇴부는 냉감각 감수성도 낮게 나타났다. (4) 발바닥과 손바닥은 냉감각을 민감하게 느끼는 부위임에도 불구하고 냉점분포 밀도가 낮게 나타났으며, 이는 특수성이 있는 부위로서 주목되어지는 결과라고 하겠다.

A study on gait characteristics for the elderly footwear design (노인의 신발 디자인을 위한 보행 특성에 관한 연구)

  • The general characteristics of the elder gait are to show a tendency that stride length and cycle decrease compared with those of adult, then walk velocity decreases. Despite differences by age in design of elder footwear, the study on characteristics of elderly gait is few. This study aims to compare pressure distributions of elderly foot with those of young students by using EMED. The ground contact time on foot was longer than that of university students in Result. Elderly males and females have 1.12 times and 1.20 times greater maximum force over the whole areas on the foot during walking than that of younger males and females, respectively. The elderly have long ground contact time at the middle foot. The guidelines which should be considered in designing shoes for the elderly are proposed. The results can be applied to design of the elder footwear on the basis of ergonomics.

  • The brain is one of the most complex systems in nature. Brain waves, or the "EEG", are electrical signals that can be recorded from the brain, either directly or through the scalp. The kind of brain wave recorded depends on the behavior of the animal, and is the visible evidence of the kind of neuronal (brain cell) processing necessary for that behavior. But, EEG had been considered as a virtually infinite-dimensional random signal. However, nonlinear dynamics light on dynamical aspects of the human EEG. The methods of nonlinear dynamics provide excellent tolls for the study of multi-variable, complex system such as EEG. In this study, 20 persons seperated in 2 groups were examined with EEG, one group stimulated on specific area of the sole of the foot with footbed inside the shoes. This experiment resulted in at the group stimulated on specific area of the sole of the foot correlation dimension of P4 and O1 channels increased significantly. Therefore. we obserbed that stimulation on specific area of the body had a constant effections on the specific channels.

  • This paper describes the development of wearing intelligent shoe system to measure applied forces and moments (ground reaction forces and moments) on the soles of feet during human walking. In order to walk safely, robot must get the intelligent feet with 6-axis force/moment sensors (Fx sensor (x-direction force sensor), Fy sensor, Fz sensor, Mx sensor (Mx : x-direction moment sensor), My sensor, and Mz sensor) and detect the forces and moments data from the sensors. And the feet must be controlled with the data and controllers. While a human is walking, the forces and moments should be measured and analyzed for robot's intelligent feet. Therefore, the wearing intelligent shoe system should be developed. In this paper, four 6-axis farce/moment sensors and two high speed measuring devices were designed and fabricated, and the wearing intelligent shoe system was made using these. The characteristic tests of the wearing intelligent shoe system were performed, and the forces and moments were detected using it.

  • Basal cell nevus syndrome (BCNS), also known as Gorlin syndrome, is a rare autosomal dominant disorder characterized by variable manifestations, including multiple basal cell carcinomas, odontogenic keratocysts of the jaw, skeletal anomalies including scoliosis and bifid ribs, palmar and plantar pits, calcification of the falx cerebri, and biparietal frontal bossing. We report a case of a 9-year-old boy with the clinical features of basal cell nevus syndrome, in which a PTCH gene mutation was confirmed by DNA testing.

  • We undertook this study to determine the plantar pressure distribution characteristics of hallux valgus, major increasing foot disease. Twenty three valgus Patients were evaluated with clinical examinations, radiologic studies and dynamic plantar pressure distribution measurements. The present study also suggested a masking method for detailed analyses on plantar pressure distribution measurements. With higher grade of hallux valgus, pressure, contact area, and impulse on metartasus are significantly increased Pressure concentration is very important in foot diseases and an approximate plantar pressure distribution should be considered on any shoe design.

  • We studied the postural response induced by plantar sole vibration with various frequencies(20, 60, 100Hz) and vibration zone(the anterior and posterior foot zone) of both soles during standing. Eight healthy young adults were exposed to 15s periods of plantar sole vibration while blindfolded. Body sway(COM, center of mass), the angle of neck, trunk, hip, knee, ankle and EMG of four lower limb muscles(tibialis anterior, lateral and medial gastrocnemial, soleus muscle) were recorded during 15s plantar sole vibration using 3D motion analysis system. Simulating each zone separately resulted in spatially oriented body tilts; oppositely directed backward and forward, respectively, the amplitude of which was proportional to the vibration frequency. EMG activity of lower limb muscles also varied according to the direction of the vibration zone and linearly according to the frequency. These findings led us to consider the plantar sole vibration as useful method of postural balance control and adjustment.
