  • Purpose : Early identification and treatment of developmental disabilities is of considerable importance in the health care of children. Screening of development is aimed at identifying infants who may need more comprehensive evaluations. Methods : A new test, the Ewha Infant Development Screening Test(EDST) has been created to screen the development of infants, 0-4 years of age. EDST was constructed so that results can be calculated into developmental ages and developmental quotients. The test consists of three sectors, e.g. language, social-adaptive and motor, and of 158 test items. A total of 104 infants, aged from one month to four years, including healthy infants as well as 10 with chief complaints of developmental delay, who visited the pediatric clinic of Ewha Womans University Dongdaemun Hospital, from June, 25 to November 30, 2001, were given the Bayley Scale of Infant Development as a base test and EDST. Results : The result showed the appropriate cut-off of EDST was 90 with better sensitivity and specificity, compared to cut-offs of 85 or 80. Conclusion : Further study with a large number of infants in the future is needed to make EDST more reliable and accurate.

  • Purpose : Early identification of developmental disabilities allows intervention at the earliest possible point to improve the developmental potential. The Ages and Stages Questionnaire (ASQ), a parent- completed questionnaire, can be used as a substitute for formal screening tests. The purpose of this study was to evaluate the validity of the Korean version of the ASQ (K-ASQ) as a screening tool for detecting developmental delay of young Korean children in the setting of a busy pediatric outpatient clinic. Methods : Parents completed the K-ASQ in the waiting room of the pediatric outpatient clinic of St. Mary's Hospital, Catholic University Medical College. Out of 150 completed the ASQ, 67 who were born term and had no previous diagnosis of developmental delay, congenital anomalies, or neurological abnormalities were enrolled. The cut-off values of less than 2 standard deviations (SD) below the mean for the ASQ were used to define a "fail", and children who failed in one or more domains tested were classified as "screen-positive". Diagnosis of developmental delay was made when the developmental indices fell below -1 SD of the Bayley Scales of Infant Development-II. Results : (1) The mean age of children was $16.4{\pm}7.4$ months. Ten children (14.9%) were small-for- gestational age infants. The mean birth weight and gestational age were $3.1{\pm}0.6kg$ and $38.8{\pm}1.4$ weeks. Nine children (13.4%) were twins and 33 (49.0%) were male. The mean maternal education in years was $13.6{\pm}2.4$, and 31.3% had full-time jobs. The time for completing the ASQ was $10.2{\pm}3.0$ minutes. (2) Seventeen children (25.4%) were classified as screen-positive, four of them were delayed in development. Among eight children diagnosed with developmental delay, four were screen-positive and the other four were screen-negative by the ASQ. (3) The test characteristics of the ASQ were as follows: sensitivity (50.0%); specificity (78.0%); positive predictive value (23.5%); negative predictive value (92.0%). Conclusion : The high negative predictive value of the K-ASQ supports its use as a screening tool for developmental delay in the setting of a pediatric outpatient clinic.

  • 이상에서 고찰하였듯이 현재까지는 어느 하나로 결정할 만한 선별 검사 방법이 없지만, 그 중에서 경질 초음파 검사와 color-flow Doppler 초음파 검사가 시행하기가 쉬우면서 민감하고 비교적 특이도가 높은 방법이라고 할 수 있다. 그러나 역학적인 면에서 조기 진단 혹은 선별 검사의 효율은 검사의 시행과 그에 따른 처치에 의해 사망률이 실제로 감소되었을 때 유의하다고 할 수 있으며, 이런 면에서 아직까지 난소암으로 인한 사망률을 감소시킬 만한 결정적인 선별 검사 방법은 알려져 있지 않다. 사망률이 1/3 감소되었음을 확인하는데 100,000명의 선별 검사자와 100,000명의 대조군이 필요하므로 앞으로 보다 많은 인구를 대상으로 한 역학적인 연구가 필요하다. 앞으로의 선별 검사에는 보다 특이도가 높은 종양 표지 물질의 개발, 초음파를 비롯한 진단 기기의 혁신적인 발달이 필요하며 이는 현재까지의 발전 상황으로 보아 실제로 가능할 것으로 생각된다. 이와 더불어 돌연변이를 일으킨 난소 상피 세포가 수 차례의 분열만 일으키더라도 그 유전자 산물을 검색해 낼 수 있고, 나아가서는 DNA 진단까지 가능한 분자 생물학적 혹은 세포 유전학전 진단 방법의 개발과 이용도 기대된다.

Developing the speech screening test for 4-year-old children and application of Korean speech sound analysis tool (KSAT) (4세 말소리발달 선별검사 개발과 한국어말소리분석도구(Korean Speech Sound Analysis Tool, KSAT)의 활용)

  • This study aims to develop a three-sentence speech screening test to evaluate speech development in 4-year-old children and provide standards for comparison with peers. Screening tests were conducted on 24 children each in the first and second halves of 4 years old. The screening test results showed a correlation of .7 with the existing speech disorder evaluation test results. We compared whether there was a difference between the two groups of 4-year-old in the phonological development indicators and error patterns obtained through the screening test. The developmental indicators of the children in the second half were high, but there were no statistically significant differences. The Korean Speech Sound Analysis Tool (KSAT) was used for all analyses, and the automatic analysis results and contents of the clinician's manual analysis were compared. The degree of agreement between the automatic and manual error pattern analyses was 93.63%. The significance of this study is that the standard of speech of a 4-year-old child of the speech screening test according to three sentences at the level of elicited sentences, and the applicability of the KSAT were reviewed in both clinical and research fields.

A Study on the Validity and Utility of Korean-Age and Stage Questionnaire for Children in Community (지역사회 아동의 Korean-Age and Stage Questionnaire 타당도와 효용성 연구)

  • The purpose of this study was to evaluate the validity and utility of the Korean-Ages and Stages Questionnaires(K-ASQ) as a screening tool for detecting developmental delay of preschool child in community. Informed consents from parents of 229 children were received for the process of the study. The subjects were 229 parents and the children of three to five years old who did not have reported genetic diseases or developmental delays. The result of this study, validity were calculated for the K-ASQ sensitivity 0.41%, specificity 0.76%, hyper-referral 0.14%, hypo-referral 0.21%. Predictive value of delayed development were K-ASQ 0.70% and that of normal development were K-ASQ 0.77%. K-ASQ can be feasibly used in the setting of health clinic and be the highly predictive criteria for delayed development and normal development.

Relation of Developmental Delay and Family Environment of Children in Community (아동의 발달지연과 가족환경과의 관련성)

  • The purpose of this study is to investigate the relationship between family environment and developmental delay in a sample of normal 3-5 years old children. Developmental evaluation is performed using K-DDST II and K-ASQ. Family environment is researched by survey. The survey questions include children's order, family numbers, religion, patient's age, patient's education history. Subject is selected between the age of 3-5 years old children. The test has been conducted to find an interrelationship between a developmental delayed result and family environment. Study has found that there is a strong relationship between developmental delay and children's family environment. Family environment factor includes children's gender, birth order, parent's age, education history. Therefore, developmental evaluation must have consideration on the element of children's family environment for developmental delay test because of a strong relationship between family environment and developmental delay result.

Development of Designs for an Early Developmental Disorder Screening Questionnaire for Multicultural Families (다문화가정을 위한 발달장애 조기 선별검사지 디자인 개발)

  • This study aimed to develop designs for an early screening questionnaire for developmental disorders among children from multicultural families in the Republic of Korea, who are at an increased risk of developmental disorders due to cultural and language barriers. Research on early screening questionnaires for multicultural families is extremely scanty, unlike that on ordinary early detection tests designed for the same category of disorders. Worse still, there have been no attempts made at overcoming the limitations of language-based and intercultural communication that are endured by multicultural parents and social workers in the field. Given the challenges, this study confirmed through professional seminars the present status of early developmental disorder screening questionnaires and the necessity for developing specialized versions for multicultural children. Then the study identified the needs of the stakeholders by employing surveys and interviews, and obtained insights and core design elements. These preceding implementations led to the creation of an early developmental disorder screening questionnaire for multicultural families. The test kit incorporates the style of illustrations preferred by multicultural parents, as well as a system of language-specific interpretation services. Produced in a leaflet format, the questionnaire will be used at support centers for multicultural families and for disabled persons in each district for the practical purpose of early screening of developmental disorders among multicultural infants and preschool children.

Usefulness of Korean Infant Developmental Screening Test in Premature Infants (미숙아에서 한국형 영유아 발달검사의 유용성)

  • Purpose : This study aimed to evaluate the usefulness of Korean Infant Development Screening Test in predicting delayed development in premature infants. Methods : A total of 74 children with histories of prematurity, who visited the pediatric neurology clinic of Korean University Ansan Hospital from August, 2002 to July, 2004, were examined, using the Korean Infant Development Screening Test. They were divided into two groups; a normal group and a patient(neurologically compromised) group. Results : At 7 months of conceptional age, the normal and patient groups differed significantly in the fine motor, cognitive-adaptive sections. At 12 months of conceptional age both group, differed significantly in the gross motor, fine motor, personal-social, language and cognitive-adaptive sections. At 18-24 months, the group, differed significantly in gross motor, fine motor, personal-social, language and cognitive-adaptive sections. Conclusion : The Korean Developmental Screening Test was useful in predicting developmental delay in premature infants.

The usefulness of diagnostic tests in children with language delay (언어 발달지연 환아에서 진단적 검사의 유용성)

  • Purpose : To assess the usefulness of magnetic resonance imaging (MRI), karyotyping, brainstem auditory evoked potential (BAEP), electroencephalogram (EEG), tandem mass screening test, and newborn metabolic screening test in children with language delay for diagnosing underlying diseases. Methods : From January 2000 to June 2007, a retrospective chart review was performed for 122 children with language delay who visited the Child Neurology Clinic at Yeungnam University Hospital and who underwent neuropsychologic tests and other diagnostic evaluations for underlying diseases. They were grouped into phenomenological diagnostic categories, and test results were analyzed according to the underlying diseases. Results : Of 122 patients, 47 (38.5%) had mental retardation, 40 (32.8%) had developmental language disorders, 23 (18.9 %) had borderline IQ, and 12 (9.8%) had autism spectrum disorder. In 26 (21.3%) cases, the causes or relevant clinical findings to explain language delay were found. Eight (10.4%) of 77 MRIs, 6 (8.0%) of 75 EEGs, and 4 (5%) of 80 BAEPs showed abnormal results. Results directly attributed to diagnosing underlying diseases were 2 hearing defects in BAEPs and 1 bilateral perisylvian cortical dysplasia in MRIs. No abnormal results were found in karyotyping, tandem mass screening tests, and new-born screening tests. Conclusion : Commonly used tests to diagnose the cause of language delay are not very effective and should only be used selectively, according to patient characteristics. However, despite the low diagnostic yields from these tests, because many patients show abnormal results, these tests are useful when conducted in complete evaluation.

Analysis on the Present Support Conditions Based on Screening and Needs of Child Care and Education Teachers for Children with Language Developmental Delayed (보육교사의 언어발달지연 유아 선별에 따른 지원 실태와 요구 분석)

  • The purpose of this study was to examine the screening and its validation of children with language developmental delay in child care and education centers, the response of parents with children with language developmental delay and the assistance needs of child care and education teachers for those children. Teacher interview with semi-structural method, transcription and content analysis method were used. The subjects in this study were 13 teachers and 30 infants and toddlers with language developmental delay in 10 child care and education centers in Seoul. The findings of the study were as follows: First, the child care and education teachers screened children with language developmental delay in their classes based on their verbal characteristics and maladjusted behavior. Their screenings were turned out correct. Second, as for the response of parents, the parents who had children with language developmental delay were indifferent, were aware of the fact in advance or asked the teachers for counseling on their children's language developmental delay. Third, the teachers couldn't provide the appropriate supports for the children in needs. Fourth, regarding the assistance needs of the child care and education teachers, they called for assistance from specialists and professional institutions, and wanted to receive education on language development. Besides, they asked for relevant supporting materials and screening criteria, and considered it necessary to reduce the ratio of teacher to children.
