당뇨병성 족부질환 - 당뇨발의 치료: 처치와 수술 및 관리

  • '당뇨발'은 '당뇨환자에서 신경학적 이상이나 말초혈관 이상 때문에 발생하는 발의 궤양으로 감염이나 심부 조직의 파괴를 동반하기도 하는 발의 질병'이라고 정의할 수 있다. 하지만 실제로는 발에 궤양이 발생한 상태가 아니더라도 매우 포괄적인 용어(umbrella term)로서 당뇨병환자에게서 발생하는 발의 만성 합병증을 총칭하는 것이 일반적이다. 왜냐하면 발의 궤양을 동반하지 않는 괴저(괴사), 심부 감염, 신경병증성 관절증(Charcot arthropathy) 등도 당뇨발의 범주에 포함시켜야 하기 때문이다. 이렇게 보면 '당뇨발'이란 의학적 진단명으로는 적절하지 않은 용어라 할 수도 있다. 다만 이미 일반화된 용어로서 환자의 교육과 설명에는 더 유용하다고 할 수 있다.

  • 당뇨발의 발생원인으로 신경병증 및 혈관병증이 있다. 신경병증의 경우 발의 자기 방어기전의 소실로 족부에 궤양이 유발되나, 혈관병증에 의한 혈액순환 장애는 궤양의 발생뿐 아니라, 산소 및 영양공급을 차단 시켜 궤양의 치료까지 어렵게 만들어 발이 까맣게 썩어가는 괴사까지 초래하게 된다.

  • 당뇨병이 증가함에 따라 당뇨환자의 발에 궤양(당뇨발)이 생겨 발을 절단하는 경우가 증가하고 있다. 이는 당뇨환자에게 심리적 문제나 경제적 비용뿐만 아니라 육체적 장애 및 사망의 주요 원인이 되고 있다. 그러나 당뇨환자의 건강한 발관리가 중요하다는 인식은 아직 미미한 상태이다. 당뇨발이 발생하는 원인을 잘 알고 미리 발 관리를 시행하는 것은 중요하다. 또한 당뇨발 증상을 조기 인식하는 것만이 발을 절단할 가능성을 줄일 수 있는 방법이다. 이 쳅터에서는 당뇨발 발생 위험요인에 대해 알아보고 당뇨발을 예방하기 위한 발 관리법은 무엇인지 살펴보도록 하겠다.

Foot Ulcer Risk, Foot Care Knowledge, and Foot Care Practice in Patients with Type 2 Diabetics (제2형 당뇨병 환자의 발궤양 위험, 발관리 지식 및 발관리 수행)

  • Purpose: The purpose of this study was to investigate foot ulcer risk factors, foot care knowledge, and foot care practice in patients with type 2 diabetes. Method: One hundred fifty type 2 diabetic patients were in and out-patients in a large urban hospital. The data were collected using a self-report questionnaire, chart review and foot examination. The questionnaires were developed by the researchers through the experts consultation and literature review. High risk for foot ulcer was evaluated by peripheral neuropathy(PN), peripheral vascular disease(PVD), and prior foot ulcer. Foot risk scores(FRS) means numbers of present risk factors. Results: 31.3% of subjects show 1 FRS, and 13.3% showed 2 FRS. Mean foot care frequency was 3.5 times per week. There were significant differences in foot care knowledge according to DM education (t=2.96, p=.004) and foot care education (t=3.65, p=.001). There were significant differences in the foot care practice activities according to duration of DM (t=3.48, p=.010) and educational levels. Conclusion: There were high proportion of foot ulcer risk among the patients. It is necessary to screen high risk foot ulcer patients and provide practical education for foot care practice of diabetic patients.

  • 당뇨병성 족부질환은 크게 당뇨병성 족부 혈관병증, 당뇨병성 족부 신경병증, 당뇨병성 족부궤양 등으로 나누어지며, 당뇨병환자에서 하지에 궤양 및 괴사가 발생 위험도는 정상인에 비하여 15~17배나 높다. 이러한 당뇨병성 족부질환을 제대로 치료하지 않으면 발의 일부나 전부를 절단하게 되며, 외상에 의한 절단을 제외하면 하지 절단의 가장 주요한 원인이 된다. 당뇨병성 족부질환으로 인해 불가피하게 절단이 되었다 하더라도 수술 직후부터 적극적인 약물치료 및 재활치료는 정상인과 거의 동일하게 일상생활 수행 및 독립보행이 가능하며, 스포츠를 포함한 여가활동까지 가능하다.

Skin ulcer and immunoblot patterns by inoculation sites in BALB/c mice infected with Leishmania major (BALB/c 마우스에서 큰리슈만편모충의 감염부위에 따른 궤양형성과 혈청 면역반응)

  • The skin ulcer in Leishmcnio mcior infection is known to be variable according to the inoculation sites even in a susceptible host. The present study traced the immunoblot patterns by the site of inoculation and duration of infection in BALB/c mice. L. mqior were subcutaneously inoculated on the nose, footpad, and back of the mice, in a dose of 3 × 106 promastigotes. Sera of the mice were collected every 10 days after inoculation. SDS-PAGE separated soluble protein bands of the promastigotes and immunoblot was carried out with the infection sera. The skin ulcer first appeared on the nose at 15 days, and on the footpad at 17 days after inoculation. The ulcer on the back appeared after 90 days. In the mice with ulcer on the nose or footpad, serum IgG antibody reacted to 202, 139, 98, 83, 81, 67, 65, 62, 59, 54, 52, 42, 26, and 23 kDa bands at 20 days after inoculation. In mice inoculated on the back, however, the immunoblot showed visible reactions with 202, 83, 81 74, 67, 65, 62, 59, 54, 52,20 and 17 kDa bands at 90 days after inoculation. The present result showed that the antigenic protein bands of L. mqior promastigotes were differed by the inoculation site and duration of infection. Since the skin ulcer and the serum antibodies to antigenic bands between 67-52 kDa appeared simultaneously, it is suggested that the serum IgG antibodies may play a role in formation of the skin ulcer in BALB/c mice.

Development of a Diabetic Foot Ulceration Prediction Model and Nomogram (당뇨병성 발궤양 발생 위험 예측모형과 노모그램 개발)

  • Purpose: This study aimed to identify the risk factors for diabetic foot ulceration (DFU) to develop and evaluate the performance of a DFU prediction model and nomogram among people with diabetes mellitus (DM). Methods: This unmatched case-control study was conducted with 379 adult patients (118 patients with DM and 261 controls) from four general hospitals in South Korea. Data were collected through a structured questionnaire, foot examination, and review of patients' electronic health records. Multiple logistic regression analysis was performed to build the DFU prediction model and nomogram. Further, their performance was analyzed using the Lemeshow-Hosmer test, concordance statistic (C-statistic), and sensitivity/specificity analyses in training and test samples. Results: The prediction model was based on risk factors including previous foot ulcer or amputation, peripheral vascular disease, peripheral neuropathy, current smoking, and chronic kidney disease. The calibration of the DFU nomogram was appropriate (χ2 = 5.85, p = .321). The C-statistic of the DFU nomogram was .95 (95% confidence interval .93~.97) for both the training and test samples. For clinical usefulness, the sensitivity and specificity obtained were 88.5% and 85.7%, respectively at 110 points in the training sample. The performance of the nomogram was better in male patients or those having DM for more than 10 years. Conclusion: The nomogram of the DFU prediction model shows good performance, and is thereby recommended for monitoring the risk of DFU and preventing the occurrence of DFU in people with DM.

Development of Insole for AI-Based Diagnosis of Diabetic Foot Ulcers in IoT Environment (IoT 환경에서 AI 기반의 당뇨발 진단을 위한 깔창 개발)

  • Diabetes is a common disease today, and there are also many cases of developing into serious complications called Diabetic Foot Ulcers(DFU). Diagnosis and prevention of DFU in advance is an important task, and this paper proposes the method. Based on existing studies introduced in the paper, it can be seen that foot pressure and temperature information are deeply correlated with DFU. Introduce the process and architecture of SmarTinsole, an IoT device that measures these indicators. Also, the paper describes the preprocessing process for AI-based diagnosis of DFU. Through the comparison of the measured pressure graph and the actual human step distribution, it presents the results that multiple information collected in real-time from SmarTinsole are more efficient and reliable than the previous study.
