The Effect of Meniscectomy on Clinical Result After ACL Reconstruction (전방십자인대 재건술에서 반월상 연골 절제술의 영향)

  • Purpose: To evaluate the effects of total or subtotal meniscectomy on anterior cruciate ligament reconstruction. Materials and Methods: We reviewed the 455 cases of arthroscopic ACLR (anterior cruciate ligament reconstruction) from February, 2003 to February, 2007 and followed-up more than 1 year. The 93 cases were enrolled. The 45 cases who underwent only ACLR were included and the 48 cases who underwent ACLR with total or subtotal meniscectomy were included in this study except grade 3 or 4 chondral lesion, partial meniscetomy or meniscal repair. We divided the patient into 4 groups which were isolated ACLR group (group I, 45cases), ACLR with lateral meniscectomy group (group II, 10cases), ACLR with medial meniscectomy group (group III, 28cases) and ACLR with both medial and lateral meniscectomy group (group IV, 10cases). The clinical evaluation was done by range of motion (ROM), IKDC subjective score, Lysholm score, anterior drawer test, Lachman test, Pivot shift test and KT-1000 arthrometer. Results: At final follow up, group IV was inferior than group I in IKDC subjective score and Lysholm score, and inferior than group II in IKDC subjective score. In KT-1000 arhtometric test, group I had better results than group III and group IV. Also in anterior drawer test and Lachman test, group 1 had better result than group III and group IV. In pivot shift test, there was no significant difference among four groups. Conclusion: Medial or both medial and lateral meniscectomy had greater laxity in anterior drawer test, Lachman test and KT-1000 arthrometric test and both medial and lateral meniscectomy had a lower subjective score than both meniscus intact group.

The comparative study of arthroscopic meniscectomy with or without high tibial osteotomy in patients with degenerative medial meniscus posterior horn tear (내반 변형을 지닌 내측 반월상 연골판 후방 골 기시부 퇴행성 파열 환자에서 반월상 연골판 절제술 단독과 근위 경골 절골술 동반 수술의 결과 비교)

  • Purpose: We compared the clinical and radiological results of meniscectomy with HTO or without HTO for degenerative medial meniscus posterior horn with varus deformity. Materials and Methods: Forty-two patients who had medial meniscus degenerative root tear with varus deformity more than 3 degrees were included for this study. Among them, 30 patients were performed meniscectomy combined with open wedge HTO and 12 patients were performed only meniscectomy without HTO. The mean follow-up period was 52.5 months. The clinical results were evaluated based on symptom improvement, patients' subjective satisfaction for surgery and HSS score. We also compared the osteoarthritic progression between the group on preoperative and at the final follow up radiographs. Results: Symptom improvement was achieved in 83.3% (25 cases) with HTO group and 66.7% (8 cases) without HTO group at final follow up with a significant difference. Patients' satisfaction was achieved in 83.3% (25 cases) with HTO group and 58.3% (7 cases) without HTO group which has a significant difference. The HSS score was improved in both group (90.8: with HTO group, 89.0: without HTO group) at the final follow up without significant difference. WOMAC score was improved in both groups at the final follow up without significant difference. There were no significant differences in the osteoarthritic progression between two groups. Conclusion: The good clinical result for treatment of patient who have medial meniscus degenerative root tear with varus deformity, proximal high tibial osteotomy is considered absolutely necessary. However, the progression of degenerative arthritis, its effect on long term follow up will be needed.

Magnetic resonance imaging Usefulness after Medial Meniscus Posterior Root Tear Repair (내측 반월상 연골판 후각 기시부 파열 봉합 후 추시 자기공명영상 검사의 유용성)

  • Purpose: This study intends to verify the usefulness of magnetic resonance imaging (MRI) for estimate recovery after arthroscopic pull-out repair at root tears of medial meniscus. Materials and Methods: We performed 17 patients who examined MRI and arthroscopy among patients who had received repair of medial meniscus from November, 2007 to June, 2011. To determine restoration meniscus, we performed arthroscopy and MRI. Results: Lysholm knee scores before and after operation were average 56.4 and 79.0 and visual analogue scale (VAS) score was improved from 8 points to 3 points. From secondary look arthroscopy performed after operation, 17 cases showed stabilization after regeneration. However, In MRI, cleft sign implying root tears of medial meniscus was observed in all cases before and after operation, ghost sign was observed in 10 cases and 9 cases respectively, radial linear defect was showed 17 cases and 15 cases respectively. Conclusion: It was not useful that MRI after medial meniscus repair in non-anatomical site, to consider restoration of medial meniscus. To evaluate for recovery medial meniscus after repair more exactly, secondary arthroscopy would be required.

The Effect of Korean Medical Treatments including Shinbaro Pharmacopuncture for Medial Meniscus Tear Patients: Five Cases Report (내측 반월상 연골판 손상에 대한 신바로 약침을 포함한 한의학적 치료효과 증례보고 5례)

  • 연구 목적 이 연구는 내측 반월상 연골판 손상으로 인한 무릎 통증을 호소하는 5명의 환자에서 한의학적 치료, 특히 신바로 약침으로 인한 통증 개선이 있었기에 보고함이다. 연구 방법 무릎 통증으로 본원 입원치료를 받은 환자 중, MRI 영상 검사상 내측 반월상 연골 파열 진단을 받은 환자들의 차트를 분석하였다. 슬관절 통증 호전 및 기능 개선은 Numeric Rating Scale (NRS), Western Ontario and McMaster Universities Arthritis Index (WOMAC Index), 슬관절 Range Of Motion(ROM) 및 special tests 측정으로 판단하였다. 연구 결과 5증례에서 NRS는 평균 3.4의 호전, WOMAC Index는 평균 64.4%의 호전을 보였다. 슬관절 ROM 측정에서 환자 모두 입원시보다 증가되었으나, 무릎에 손상에 사용되는 보편적 검사인 Stress valgus, Stress varus, Drawer test, Apley compression test, Mcmurray's test 상 전후비교에서 4명의 환자는 호전되었고, 1명 환자에서는 변화가 없었다. 결과 이 연구는 내측 반월상 연골판 손상에 대한 신바로 약침을 포함한 한의학적 치료 후 단기간 내 통증 및 기능 향상을 보여주었다.

Clinical outcome in relation to stability of longitudinal meniscal tear associated with anterior cruciate ligament rupture (전방 십자인대 파열에 동반된 반월상 연골판 종파열의 파열부위 안정성에 따른 치료 결과)

  • Purpose: The purpose of this study was to evaluate the results of treatment for longitudinal meniscal tear in ACL rupture patients by clinical evaluation and $2^{nd}$ look arthroscopy. Materials and Methods: Among the patients who underwent surgery due to ACL rupture and longitudinal meniscal tear between 2002~2009, 32 patients were selected for the study. The patients were devided into three groups; group 1 had repair, group 2 had partial menisectomy due to impossible to repair, group 3 had conservative treatment due to stable meniscus. Comparative retrospective analysis was applied with KT 1000 arthrometer, Lysolm' score, Tegner activity score, IKDC subjective score, and $2^{nd}$ look arthroscopy. Results: Lysolm' score, Tegner activity score, IKDC subjective score showed no statistical difference in all three groups. KT-1000 side to side difference improved from 6.2, 6.15, 6.38 before surgery to 2.56, 2.49, 2.58 after surgery in group 1, 2, and 3, respectively. In group 1, second-look arthroscopy revealed complete healing in 12 patients, incomplete healing without any clinical symptoms in 2, and healing failure in 1. In group 2, second-look arthroscopy showed partial regeneration on margin of meniscectomy. In group 3, second-look arthroscopy revealed complete healing in 2 patients, while 4 showed incomplete healing so that have repaired. Conclusion: In the study, the outcome was better when the stable longitudinal meniscal tear on posterior horn with ACL rupture had active treatment.

  • We report a case of a girl with a symptomatic anomaly of medial meniscus. The complaint of the patient was pain and snapping of the knee. The anterior horn of medial meniscus has anomalous insertion which was extended to the intercondylar notch of the femur on the surface of the anterior cruciate ligament, it attached to lateral wall of medial femoral condyle. This anomalous band was not detected in MRI of knee but found during the operation. After resection of anomalous band, the symptoms completely disappeared.

  • Although total meniscectomy had been performed as a treatment of meniscal tear, many published articles reported progression of the degeneration process in the articular cartilage and development of the osteoarthritis after meniscectomy. With the increased knowledge of the biomechanical properties of the meniscus, meniscal repair has been performed increasingly to maintain the important functions of the meniscus. Outside-in, inside-out, all-inside technique have been used as meniscal repair methods. In this review, indications, operative technique, and outcomes of the all-inside meniscal repair are discussed.

  • As awareness of root tear of meniscus have been increased, it became necessary to have an intensive repair. Posterior horn tear, especially for a lot of repair methods and the results have been reported recently. The report of the anterior root tear has yet to be found. We recently encountered a patient with an iatrogenic meniscal subluxation caused by past experienced partial menisectomy for discoid meniscus. Thus, we repots the technique and clinical results of arthroscopic repair of subluxated lateral meniscus anterior root tear using suture anchor.

  • Recently, meniscal allograft transplantation has been regarded as a successful procedure in terms of pain relief and functional improvement for the symptomatic patients previously underwent subtotal or total meniscectomy. However, the likelihood of a successful outcome would be reduced by various complications including graft tear, shrinkage, extrusion, infection, progressive chondral injury, and granuloma due to nonabsorbable suture material. Therefore, knee surgeons need to be all aware of the complications and to make an effort to minimize them. The purpose of this article is to review the current literatures regarding clinical results and complications after meniscus allograft transplantation.

Incarcerated Flap Tear of the Medial Meniscus into the Inferior Joint Capsule - A Report of Two Cases - (하방 관절낭 내로 감돈된 내측 반월상 연골의 판상파열 - 2예 보고 -)

  • Flap tear is a type of displaced meniscal fragments that is often clinically significant lesions requiring surgical intervention. If the displaced tear is located inferomedial to the tibial plateau and incarcerated into the articular capsule, it can be overlooked from preoperative magnetic resonance imaging evaluation and escape detection during arthroscopic examination. In addition, the clinical feature and treatment result of incarcerated flap tear has not been reported. We present 2 cases of medial meniscus flap tear incarcerated into the articular capsule that showed specific clinical features, in order to emphasize the importance of clinical suspicion of such a lesion for accurate preoperative diagnosis.
