  • Prosthetic rehabilitation of an edentulous patient who has a maxillary defect is difficult to achieve for many clinicians. The maxillary defect causes leakage of air during pronunciation and compromises denture stability, support and retention by reducing denture-bearing area and breaking peripheral seal. In these patients, the sizes and shapes of defects are very important factors which attribute to prognosis of maxillary obturators. This case report shows the prosthetic rehabilitation of the patient who had maxillectomy on the right maxillary sinus because of squamous cell carcinoma. The patient had a stepwise treatment protocol which started with pre-operative dental examination and followed by surgical, interim and definitive obturation phase. In this case, an acceptable level of retention could be obtained due to well-defined static defects and the preserved premaxillae and the patient was satisfied with the result of the treatment in the aspect of function and esthetics.

  • This study is about design for the Rotational Instrument of the Industry factory which is used management safety and maintenance. We developed the multichannel vibration monitering system of the self-diagnosis for middle level CMS(Condition Monitoring System) market, and that system are new features to the expandability and flexibility. Normally one channel is used for treating one signal, but developed instrument can treat four channel with one signal processing card. One rack have redundant power supply and displace and it can check vibration measurement value in field without computer. Bearing fault detection is fundamental of vibration surveillance, but sometimes can not check with vibration velocity and acceleration. So it need the filtering and the amplitude modulation on the acceleration enveloping technology when irregular vibration is happened. We developed the vibration analysis instrument which is applied such technology. And the development prototype shows activated within the vibration error limit.

  • Maxillary bone defects may follow surgical treatment of benign and malignant tumors, trauma, and infection. Palatal defects often lead to problems with swallowing and pronunciation from the leakage of air into the nasal cavity and sinus. Obturators have been commonly used to solve these problems, but long-term use of the device may cause irritation of the oral mucosa or damage to the abutment teeth. Utilizing implants in the edentulous area for the fabrication of the obturators has gained attention. This case report describes a patient, who had undergone partial resection of the maxilla due to adenocarcinoma, in need of a new obturator after losing abutment teeth after long-term use of the previous obturator. Implants were placed in strategic locations, and an implant-retained maxillary obturator was fabricated, showing satisfactory results in the rehabilitation of multiple aspects, including palatal defect, masticatory function, swallowing, pronunciation, and aesthetics.

  • Maxillectomy is performed to remove the tumor in the palate, maxillary sinus, buccal mucosa or nasal cavity. The resection range depends on the size and the extent of the tumor and it affects speech production or cause nasal regurgitation during feeding. Obturator can occlude an opening such as an oro-nasal fistula and protect the defect area. Successful reconstrucion of the patient's oral cavity who have gone over the maxillectomy is a difficult task. The condition and number of teeth, the remaining support area, and the extent of the defect area have a great influence on manufacturing the obturator. If these factors are disadvantageous, the prognosis of the prosthesis is uncertain. The final obturator must have a sufficient retention in the patient's oral cavity and must not irritate the surrounding tissue and support area where the resection was performed. In this case, a 55 year old female went through the maxillectomy and the only 3 teeth remained. And the retention of the maxillary prosthesis seems to be poor. So that, we fabricated the closed hollow obturator which has reduced weight compared to the conventional obturator. Consequently the closed hollow obturator can give better sealing and the adaptation.

  • Surgery on the distal trachea or the carina presents special problems for maintaining the airway and systemic oxygenation. Cardiopulmonary bypass is an alternative method for respiratory support for the patients with these conditions. Percutaneous cardiopulmonary support (PCPS) applied under local anesthesia has recently been used for respiratory support in tracheal surgery and the outcome is satisfactory. We encountered a patient who had severe distal tracheal stenosis after prolonged intubation. We had a gratifying result with performing tracheal resection and repair under the support of PCPS.

  • As stated in Part(I), the use of the isolator is meant to protect a structure from seismic risk, by concentrating the inelastic deformations to relatively cheap and replaceable devices while the rest of the structures remains elastic. This research has been carried out to investigate the effects of various structural parameters and isolator characteristics on the seismic response of Base Isolated Bridges. Simplified analysis method for practical design is developed by using the results. The Proposed Code-Type approach method can be used to estimate the inertial forces accurately, not only at the isolator but throughout the height of the Base-Isolated Bridges. The proposed method is recommended to use in preliminary design tool or even a final design tool for Base Isolated Bridges. For the validation of simplified design method, examples with artificial earthquake time history and design response spectrum for P.C Box Bridge with bilinear hysteretic steel damper are evaluated.
