Microvascular Anastomosis Using Horizontal Mattress Suture Technique (Horizontal Mattress Suture Technique을 이용한 미세혈관 문합)

  • Horizontal mattress suture technique on microvascular anastomosis of rat(body weight : 200-250gm) femoral artery was evaluated. The present study was conducted to compare the horizontal mattress suture with simple interrupted suture on the suture time, patency rate of the sutured vessels, and the histological changes of surgical site of the vessel wall during wound healing period. The mean suture time of the vessel wall with horizontal mattress suture technique was 15min. 49sec. ${\pm}$ 2.14, which is significantly shorter than that of simple interrupted suture technique. The patency rate of the sutured vessel in both groups was statistically not different each other till post-operative 3rd day but patency rate of horizontal mattress suture was higher than that of simple interrupted suture at post-operative 3rd week. The histological findings such as intimal noss, medial degenention and intimal regeneration were similar in both groups.

  • The use of mechanical anastomosis coupler instead of sutures has been increasing in microvascular anastomosis surgery. However, non-biodegradable anastomosis coupler has problems such as not only inflammatory reaction but also remaining permanently in operation wound. Therefore, we fabricated biodegradable anastomosis coupler using injection molding process to overcome the limitation of non-biodegradable anastomosis coupler. In various injection molding process conditions, the shrinkage was calculated with different cylinder temperatures and injection molding pressures and anastomotic strength was measured. As a result, changes in shrinkage hole part larger than the pin part. In addition, the shrinkage in the cylinder at higher temperatures increase. However, the higher the injection pressure, shrinkage tends to decrease, respectively. In-vitro degradation behavior of PCL anastomotic coupler evaluated for 12 weeks, water uptake was increased and molecular weight was accompanied by a reduction in mass of the biological degradation and reduction of anastomotic strength was confirmed. However, decreased levels of anastomotic strength enough to exceed the requirements of preclinical surgery, PCL microvascular anastomosis coupler suitable candidate materials that could identify.

  • In clinical microsurgery, limitation of space often prohibits rotation of a double clamp in an end-to-end anastomosis, or shortage of length of the secondary vessel in an end-to-side anastomosis does not allow visualization of the back wall. In these situation, back wall repair is extremely difficult. To overcome this problem, we use continuous suture technique without rotation of double clamp for end-to-end and end-to-side anastomosis of the 40 rat femoral arteries. After continuous sutures for microarterial anastomosis, the following results were obtained: Continuous sutures are useful in anastomosis where there is unavoidable limited access to the posterior wall and for some of the larger vessels now being anastomosed in free flap and other reconstructive surgery.

  • In the assessment of patency of a small vessel anastomosis, micro-arteriography using dental X-ray and film was used as a method of testing the patency of arterial anastomoses in the rat. Micro-arteriography could lead to an objective evaluation of the patency in End-to-End and End-to Side anastomoses. The method used in this study is easily accessible for Oral and Maxillo-facial surgeon to practice the microvascular anastomosis, and requires materials available in every dental clinic.

  • An automated anastomotic ring-pin system consisting of both the anastomotic ring-pin system and the coupler device has eliminated the drawbacks of the suture method. High density polyethylene (HDPE), a material with outstanding biocompatibility and injection molding capability, was used in the ring. SUS316 stainless steel, Ti-6Al-4Nb, Ti-6Al-4V, and unalloyed titanium were used in FEM simulations of the micropin. The authors categorized the microvascular anastomotic ring micropins into short neck (SN) and long neck (LN) groups in order to evaluate the effect of the micropin's fillet radius and neck length on the von Mises stress. The micropins were further divided into those with and without fillet. On the basis of the fillet radius rate (FRR), which represents the rate of change in the von Mises stress with respect to the availability and shape of the fillet, and the neck length rate (NLR), which represents the rate of change in the von Mises stress with respect to changes in the length of the neck within the fillet shape, it can be concluded that the SN-3 neck design is the most stable.

  • The occurrence of thrombotic occlusion & endothelial injuries at the site of anastomosis have been considered as major problems in microvascular surgery. The purpose of this study was to ascertain whether a end-in-end(sleeve, telescope) anastomosis compared favorably with end-to-end anastomosis in healing procedures on the endothelium and to study the possibility of clinical application in end-in-end method. The microvascular anastomoses have been performed with end-in-end method in the femoral arteries of 20 rats group, also with end-to-end method on the same arteries of another 20 rats group, and then the four anastomosed vessels in subdivided groups on each group were taken after period of 1, 3, 7, 14, 21 days following by anastomoses for scanning electron microscope observation. The results were as following: 1. The patency rate was 90% in the end-in-end group and 85%in the end-to-end group and late thrombus occurred at 7, 14 days on both groups, which suggested -consistent monitoring of patency was required for two weeks at least. 2. Platelet aggregates at the site of anastomosis began to organize on post-operation 3 days and in the end-in-end group, the initially decreased lumen of inserted vessel was gradually increasing on 7 days due to atrophy of the medial layers. 3. Re-endothelialization was completed between 7 and 14 days in end-in-end group, whereas between 14 and 21 days in end-to-end group.

  • Far-infrared radiation therapies are becoming more popular for blood circulation disorders, cardiovascular disease, skin diseases, inhibit cancer cell, etc replacing conventional operations. In this research, thermal characteristics of heating part in panel radiators, which is effective on the blood circulation disorders were experimentally analyzed. The heating line supplies heat energy to insulation coatings with heat flux of $150mW/m^2$ in normal status and as a result the coatings reached 20% of the heating line temperature. In other words, the insulation itself could increase surface temperature of heating plates by 20% and raise thermal time constant promote blood circulation effect. We also found that space arrangement of the heating lines was an important factor in designing heating parts and both coefficient of heat conduction and density of the heating plate should be also considered for superimpose of thermal diffusion.
