Evaluation of remineralization and acid resistance effect in fluoride varnish by Micro-computed tomography (미세전산화단층촬영술을 이용한 불소바니쉬의 재광화 및 내산성 평가)

  • Objectives : The purpose of this investigation was to evaluate the remineralization and acid resistance in fluoride varnish by Micro-computed tomography (micro CT). Methods : Specimens of bovine teeth enamel were embedded in resin, polished and randomly divided into 3 groups (a control group, a NaF solution group, a fluoride varnish group). Each group has 3 specimens that was standardized according to Vickers hardness number (VHN). Specimens were immersed in demineralization solution for 72 hours. The control group had no treatment, the NaF solution group was treated by a 5% NaF solution for 4 minutes, and the fluoride varnish group was treated by a fluoride varnish for one hour. All specimens were subjected to a chemical pH cycling method for 14 days. After a chemical pH cycling method, the density were measured using micro CT. Then, specimens were immersed in each demineralization solution for 72 hours. After demineralization processed, the density were measured using micro CT. Results : 1. The density was significantly higher in the fluoride varnish and 5% NaF solution group than that of the control group after 14 days cycling (p<0.05). And the density value of the fluoride varnish group was higher than that of the 5% NaF solution, with no significant difference. 2. The differences of density after acid resistance treatment were statistically significant among 3 groups(p<0.05). Conclusions : It is suggested that fluoride varnish showed the remineralizing effect and acid resistance effect on the enamel, and micro CT could be used to evaluate the change of enamel lesion.

A Rare Case of Acquired Arteriovenous Malformation in Transarterial Chemoembolization for Hepatocellular Carcinoma (간세포암의 경동맥 화학색전술 중 발견된 후천성 동정맥 기형에 관한 드문 증례보고)

  • Transarterial chemoembolization (TACE) is a commonly used and rapidly evolving non-invasive treatment for hepatocellular carcinoma (HCC). It is important that understanding individual anatomical variants and planning for tumor-feeding artery access to acquire adequate treatment effectiveness. In this study, we will report acquired arteriovenous malformation which interferes with TACE for HCC. A 72-year-old man with persistent abdominal pain for 2 days visited our hospital. The patient was chronic hepatitis B carrier and had a history of HCC treated with conventional TACE 10 years ago. Hypervascular nodular HCC in the liver segment 8 and aberrant right hepatic artery from the superior mesenteric artery were detected on computed tomography (CT). When first TACE was performed, the tumor-feeding artery originating from the left hepatic artery was found and embolized. There was no tumor-feeding artery from the right hepatic artery but arteriovenous malformation was found. After a month, follow up CT showed necrotic lesion and residual HCC and we performed secondary TACE. On secondary TACE, we selected the right hepatic artery and passed through arteriovenous malformation. Superselective-angiogram showed remnant tumoral staining and remnant tumor was embolized using drug-eluting bead and Adriamycin. Final angiogram showed no remnant tumoral staining and the patient was discharged without complication. We found the rare case of arteriovenous malformation adjacent to HCC, and we performed superselective TACE beyond arteriovenous malformation to treat HCC.

The effects of bamboo salt solution on remineralization of artificial caries-like lesions (죽염이 초기인공우식법랑질의 재광화에 미치는 영향)

  • Objectives : The aim of this investigation was to evaluate the remineralization effect of Bamboo salt and NaF+Bamboo salt solutions on bovine enamel formed incipient artificial enamel caries by microcomputed tomography (micro CT). Methods : Experimental solutions were distilled water(negative control), 2% sodium fluoride solution (2% NaF group), 3% bamboo salt solution (3% BS group) and the solution mixed 2% sodium fluoride solution and 3% bamboo salt solution (2% NaF+3% BS group). Specimens were prepared from extracted bovine teeth and divided into 4 groups of 10 specimens each by randomized blocks according to density. Then the specimens surface were divided equally into three parts to observe sound enamel area, incipient enamel carious area and remineralized enamel area. Only one-third of specimen surface was coated with nail varnish and these were exposed to a lactate carbopol buffer system for 72 hours. Then one-half of the demineralized enamel area was coated with nail varnish. The specimens were carried out under pH cycling model for 14 days as follows; samples were immersed in each experimental solution for 2 mins 3 times per day, demineralized for 4 hours and in mixed saliva for the remaining hours. After pH cycling, density was measured using micro CT. Results : All experimental groups showed remineralization effects except for negative control group(p<0.05). The differences of density after experimental solution treatment were statistically significant difference among 4 groups (p<0.01). The density difference values of groups were $0.04{\pm}0.01$ in negative control group, $0.19{\pm}0.01$ in 2% NaF group, $0.14{\pm}0.01$ in 3% BS group, and $0.21{\pm}0.01$ in 2% NaF+3% BS group. Conclusions : The bamboo salt solution showed remineralization effects on incipient artificial enamel caries and the solution mixed sodium fluoride solution and bamboo salt solution showed more remineralization effects than the bamboo salt solution. Thus, it is suggested that bamboo salt can be used as remineralization agent in incipient enamel caries lesion.

A Fat Density Mass in the Mediastinum (지방음영을 보인 종격동내 종괴)

  • A 47-year-old female was admitted for an evaluation of a left mediastinal mass. The chest X-ray performed 16 months ago was normal, but the chest X-ray upon admission showed a large mass adjacent to the anterior aspect of the left hemidiaphragm. The CT scan demonstrated a large mass with a fat density in the left lower hemithorax. A focal diaphragmatic defect behind the xiphoid process was suspected. A thoracoscopic examination revealed omental herniation through the diaphragmatic defect. Therefore, a left thoracotomy was performed and the defect was repaired. We believe that a differential diagnosis should be needed to include a diaphragmatic omental hernia when a fat density mass is observed in the mediastinum.

A Case of Bronchus-Associated Lymphoid Tissue(BALT) Lymphoma Treated with Lobectomy (폐엽절제를 통한 BALT 림프종 치료 1 예)

  • The bronchus-asociated lymphoid tissue(BALT) lymphoma is a low-grade primary malignant lymphoma that originates from bronchus associated lymphoid tissue. A 67-year-old woman was admitted for evaluation of cough, sputum, rhinorrhea which had persisted for one month. Physical examination showed decreased breathing sound on the left upper lung field. High resolution chest computed tomography demonstrated consolidation which showed air-bronchogram and surrounding ground glass opacity in left upper lobe. These findings implicated inactive tuberculosis, organizing pneumonia, or bronchiolo-alveolar carcinoma. The histologic findings from percutaneous needle aspiration biopsy revealed aggregated atypical small lymphoid cells with lymphoepithelial lesions. With immunohistochemical staining, the atypical lymphoid cells reacted positively with CD 20 antibody and negatively with CD 3 antibody. Thus, we could diagnosed her as a patient with BALT lymphoma. After left upper lobectomy, she has been well without recurrence of the disease for 14 months. In this country of Republic of Korea, it was the 1st case of BALT lymphoma surgically treated when histological diagnosis had been done. Based on this case, we wanted to demonstrate the importance of early histological diagnosis and treatment of BALT lymphoma.
