  • 외인성 췌장부전은 췌장에서 분비되는 소화효소의 부족한 합성 및 분비에 기인한다. 대부분의 개체에서 보이는 임상증상은 체중감소, 다량의 연변, 지방변이다. 혈청 고양이 유사트립신 면역활성 농도검사는 외인성 췌장부전 진단에 있어 매우 유용한 검사법이다. 외인성 췌장부전 환자의 치료는 췌장 소화효소의 공급이다. 대부분의 외인성 췌장부전 환자에서 심각한 혈청 코발라민의 감소와 동반되기 때문에 코발라민의 투여는 반드시 필요하다. 마지막으로 개에 비해 흔하게 확인되진 않지만, 만성적인 연변 및 체중감소를 보이는 환자에서는 진단에 있어 외인성 췌장부전에 대한 고려가 필요하다.

  • 쥐의 단일클론항체는 기초연구에 뿐만 아니라 질병의 진단시약으로서도 광범위하게 유용되어 왔다. 반면, 쥐의 단일클론항체를 인체에 임상적으로 사용하는 것은 인체에 면역반응을 유발시킨다는 근본적인 문제점 때문에 어려운 일이라 인식되어 왔다. 그러나, 최근 2-3년간 유전자 기술과 단백질공학적인 방법에 의하여 쥐의 단일클론항체를 인간화시키거나 항체절편의 생산을 시도하게 되어, 단일클론항체의 임상적인 이용에 대한 가능성이 긍정적으로 진단되고 있다. 본 난에서는 항체의 구조와 기능을 간략히 살펴보고, 최근 항체의 단백질공학에 대한 경향과 방법 및 engineered 항체의 특성들에 관하여 논의하고자 한다.

  • 이상 방사면역측정(放射免疫測定)의 기본원리(基本原理), 순수(純粹)한 항원(抗原), 표지항원(標識抗原), 항체생성(抗體生成), 표준곡선(表準曲線)의 작성(作成), 항원항체복합체(抗原抗體複合體)의 분리방법(分離方法)에 대(對)하여 설명(說明)하였다. 생물학적방법(生物學的方法),화학적방법(化學的方法), 기계적(機械的)인 측정방법(測定方法)에 의(依)해 생체내(生體內)의 미량(微量)으로 존재(存在)하고 있는 물질(物質)들을 정확(正確)하게 측정(測定)할 수 없다. 그러나 방사면역측정법(放射免疫測定法)에 의(依)해 쉽게 측정(測定)되어 기관(器管)의 기능검사(機能檢査)는 물론(勿論) 각(各) 기관(器管)과의 상관관계(相關關係)를 분석(分析)하여 질병(疾病)의 진단(診斷), 치료경과(治療經過)을 평가(評價)하는데 매우 중요(重要)한 정보(情報)를 제공(提供)해주고 있다. 이러한 점(點)을 고려(考慮)하여 이 방법(方法)을 잘 습득(習得)하고 진료(診療)의 수단(手段)으로 널리 이용(利用)되어 의료기술발전(醫療技術發展)에 토대(土臺)가 되기를 바란다.

Detection of Helicobacter pylori by Pre-embedding Immunoelectron Microscopy - Comparison with Immunoblotting Method (Pre-embedding Immunoelectron Microscopy를 이용한 Helicobacter pylori의 발견 - Immunoblotting법과 비교 -)

  • Purpose : We tried to evaluate whether the detection rate of Helicobacter pylori in gastric biopsy specimens could be improved by using pre-embedding immunoelectron microscopy. Methods : A total of 119 children who complained of upper gastrointestinal symptoms were endoscoped at the Gyeongsang National University Hospital from July, 1996 to July, 1999. Five biopsy specimens(three for urease test, one for hematoxylin-eosin(H & E) staining, and one for preembedding immunoelectron microscopy) were obtained from each antrum and body. Immunoblotting analysis were also performed. Results : Among the 119 patients, H. pylori were found in 116 patients(97.5%) by the immunoelectron microscopy. Among three patients who were found H. pylori negative in immunoelectron microscopy, two patients showed H. pylori in H & E stained slides and one patient was urease test positive(color change within six hours). Urease tests were positive in 107 patients(89.9 %). The positive rate of immunoblotting tests was 81.5%. However, only 13 patients(10.9%) showed H. pylori on the H & E stained antrum or body tissue. Conclusion : In this study, we found H. pylori histopathologically in most of the pediatric patients who complained of upper gastrointestinal symptoms. This study showed that pre-embedding immunoelectron microscopic examinations can be used as a gold standard in the diagnosis of childhood H. pylori infection. However, this method also has limited capacity to detect widely scattered H. pylori compared to the other histopathologic diagnostic methods.

Immunohistochemical Study of NSE in Small Cell Lung Cancer (SCLC) Combined with Serum Assay (소세포폐암에서 Neuron Specific Enolase의 면역조직 화학염색과 혈청농도에 관한 연구)

  • Background: Neuron specific enolase (NSE) is a neuronal form of the glycolytic enzyme enolase which was first found in extracts of brain tissue, and later in a variety of APUD cells and neurons of the diffuse endocrine system. SCLC shares many APUD properties with normal neuroendocrine cells. NSE immunostaining and serum NSE measurement may be a useful marker of neuroendocrine differentiation in lung tumors and diagnosis of small cell carcinoma. Methods: NSE immunohistochemical staining was done and at the same time serum NSE levels were measured in 22 small cell lung cancer and 21 non small cell lung cancer which were confirmed histologically. Results: 1) NSE immunoreactivity was detected in 9 of the 18 (50%) small cell lung cancer, in 5 of the 16 non small cell lung cancer. 2) Whereas the mean value in non-small cell lung cancer group was $11.79{\pm}4.47\;ng/ml$, the mean level of serum NSE in small cell lung cancer increased up to $59.3{\pm}77.8\;ng/ml$. In small cell lung cancer patients, mean value of limited disease group was $20.19{\pm}12.91\;ng/ml$, while mean value of extended disease group was $91.9{\pm}94.2\;ng/ml$ showing statistically significant difference. If serum levels above 20 ng/ml were tentatively defined as positive, 16 of 22 (73%) patients with SCLC had positive serum NSE level, but only one patient with NSCLC did. There was no correlation between serum NSE level and immunoreactivity of NSE. Conclusion: These studies indicate that serum NSE measurement may be a useful marker for the diagnosis and disease extent and NSE immunostaining can be used to demonstrate the neuroendocrine components of lung tumor.

소 해면형 뇌증/양 스크래피 -병리학적 감별진단과 진단방법 국제표준-

  • 양의 스크래피(scrapie)는 우리나라에서의 발생보고가 없는 해외가축전염병의 일종이며, 동물의 전염성 해면형 뇌증(Transmissible Spomgifrom Encephalopathies; TSEs)중 역사가 가장 오랜 질병인데, 현재 영국에서 문제되고 있는 새로운 전염병인 소 해면형 뇌증(Bovine Spongiform Encephalopathy; BSE) 즉 일명 광우병(mad cow disease)과의 어떤 연관 가능성 때문에 수의학계의 관심의 대상이 되고 있는 질병이다. 일단 감염되어 발병되면 치료대책 없이 100$\%$ 폐사되는 세기의 불치병으로 알려진, 소 해면형 뇌증(BSE)은 영국에서 현재 사람의 크로이츠휄트-야콥병(Creutzfeldt-Jakob disease; CJD)과의 관련 가능성 여부를 놓고 독특한 문제가 되고 있는 세계적인 희귀질병이다. 이들 질병에 대하여는 아직까지 확실한 병인체가 밝혀져 있지도 않으며, 그렇기 때문에 면역 혈청학적 진단방법도 확립되어 있지 못할 뿐만아니라 예방백신의 개발 또한 불가능하다. 다만, 임상적인 병력과 임상소견, 뇌조직 표본에 대한 현미경 검사 또는 전자현미경 검사에 의한 특이소견 관찰 등 조직병리학적 진단만이 가능할 뿐이다. 본편에서는 소 해면형 뇌증(BSE)의 병리학적 감별진단과 관련, 지금까지 보고된 임상증상을 검토해보고, 우리나라에서 경험한 소의 광견병에 대한 조직병리학적 진단 재료를 근거로하여 감별진단을 위한 참고자료로 설명하고, 국제수역사무국(Office de International Epizooties; O.I.E.)에서 발생한 Manual of Standards for Diagnostic Tests and Vaccines for List A and B Diseases of Mammals, Birds and Bees(1992)(포유류, 조류, 꿀벌에 있어서의 A급 및 B급 질병에 대한 진단시약 및 예방백신에 대한 표준지침) 중에서 소 해면형 뇌증(B 83; p 742-747)과 스크래피(B 32; p 424-427)에 관한 내용 (Chapter 22, 205-215)을 기본자료로 제공하고자 한다.

Posttransplant Lymphoproliferative Disorder in Pediatric Liver Transplantation: Samsung Medical Center Experience (소아 간이식에서 Posttansplant Lymphoproliferative Disorder (PTLD): 삼성서울병원의 경험)

  • Purpose: In a retrospective study for the pediatric patients who underwent liver transplantation in the past 6 years at Samsung Medical Center, the clinical features of 5 patients with posttransplant lymphoproliferative disorder (PTLD) were analyzed. Methods: Between June 1996 and June 2002, 41 pediatric patients underwent liver transplantation. Seven of them died in the postoperative period. Thirty-five including one patient who died of PTLD were finally reviewed. Patients were divided into two groups: high risk group, EBV naive recipients of EBV-positive grafts; low risk group, the patients other than those in high risk group. The authors reviewed age at operation, immunosuppressive agent, postoperative duration until diagnosis, postoperative duration until EBV seroconversion, presence of treatment against rejection, and presenting symptoms of PTLD. Results: Five of 41 patients (12.2%) developed PTLD. All of them belonged to high risk group, and the incidence of PTLD in high risk group was 31.3% (5/16). The mean age at operation was 10.8 months old and the mean duration between operation and diagnosis for PTLD was 9.8 months. Primary EBV infection developed after a median of 6 months after transplantation. One patient was diagnosed as laryngeal and gastrointestinal PTLD and the other four, gastrointestinal PTLD. The following symptoms and signs were seen in the patients: anemia (100%), hypoalbuminemia (100%), fever (80%), diarrhea (80%), gastrointestinal bleeding (80%), and anorexia (60%). Conclusion: PTLD is one of the major complications after pediatric liver transplantation, especially in the group of high-risk recipients. Anemia, hypoalbuminemia, fever, diarrhea and gastrointestinal bleeding were features that are characteristic of PTLD. The common features of PTLD development were: (i) EBV-positive donors placed into EBV naive recipients, (ii) primary EBV infection about 6 months after transplantation, (iii) young age, about 1 year old at operation, and (iv) the requirement for intensive posttransplant immunosuppression.

Evaluation of Usefulness for Anti-TPO Antibody Test in Item of the Medical Examination (건강검진 종목으로서 항갑상선 과산화효소 항체검사의 유용성에 대한 평가)

  • Purpose: The test had been applied for outpatient by end of 2006, however, it has been included in the medical examination since January 2007, as demand and interests have been gradually increasing in the thyroid gland disease and cancer. thus, we would necessarily evaluate usefulness of the test by comparing the number of patients who are diagnosed as "benignancy" by the medical test with the number of outpatient who attend and are diagnosed as autoimmune thyroid disease among the benign patient, in samsung medical center for a certain period. Materials and Methods: Based on the result for Anti-TPO Antibody test by RIA for the 12,937 patients in samsung medical center from October 2007 to March 2008, for six months, benignancy rate classified by sex and age is measured statistically and number of the patients who are diagnosed as autoimmune thyroid disease are kept tracked on. Results: According to the analysis of the Anti-TPO antibody test 1,135 of 12,937, which is 8.77% are benign and 218 treated patient of them, which is 19.2%, were diagnosed as autoimmune thyroid disease. Conclusions: Based on the statistics, usefulness of the test seem to have co relationship with derivation of autoimmune thyroid disease. this is 19.2% of probability relatively high. this figure, however, does not have strong relationship with specialty of the disease. Thus screening test seems to have somewhat effectiveness, considering other experiments and their margin.

  • actin and some actin binding proteins such as tropomyosin, α-actinin and troponin T were localized by simultaneous double immunogold labeling in several developmental stages of Cryptosporidium parvum. All of the observed developmental stages have many paricles of tropomyosin and actin around pellicle and cytoplasm. Tropomyosin was labeled much more than the actin when these two proteins were labeled simultaneously. And α-actinin was labeled mostly in the pellicle, but troponin T labeling weas very rarely observed. From this study it was suggested that tropomyosin seemed to be one of the major proteins of C. parvum, so it must be playing important roles in C. parvum.

  • A 54-year-old homosexual man was diagnosed as human immunodeficiency virus-1-positive in 1992. He was admitted to a tertiary hospital in March, 2000 because of right flank pain, fever and intermittent cough. A chest roentgenogram showed right-sided pleural effusion, and closed thoracostomy was performed for drainage Salmonella species and Escherichia coli were isolated from the pleural fluid. In spite of 6 weeks of antibiotic treatment, fever did not subside and the general condition gradually deteriorated, and under the diagnosis of lung abscess with empyema thoracis, right pleuropneumonectomy was performed. The general condition improved postoperatively until day 10 when he showed sudden change in mental status to stuporous and developed focal seizure. Brain CT showed multiple abscesses in right frontal and left frontotemporal lobes and he expired on postoperative day 14.
