A Study of the relationship between Chronic Pain and Quality of Life for Elderly in Long-term Care Service -Focused on the Mediating Effect of Depression- (장기요양 재가서비스이용 노인의 만성통증과 삶의 질 관계연구 -우울의 매개효과를 중심으로-)

  • In this study, the effect of chronic pain on the lives of elderly people in long-term care service was analyzed based on the mediated effect of depression. The research data was sampled from elderly people in long-term care services, 204 people participated. From mediated regression analysis, depression was the most relevant factor on the quality of life, followed by chronic pain. With chronic pain and depression as independent variables and quality of life as a dependent variable, depression was proved to have had a fully mediated effect on quality of life. The result of this study suggested that convergence of various support systems should be implemented for the elderly in long-term care services.

The Effects of Lumbar Stabilization Exercise on Pain-related Functions of Industrial Workers with Chronic Low Back Pain and Lumbar Instability (요부안정화운동이 요부불안정성을 가진 산업체 만성요통환자의 통증관련 기능에 미치는 효과)

  • The purpose of this study is to investigate the effects of lumbar stabilization exercise on pain-related function of industrial workers with chronic low back pain and lumbar instability. 20 industrial workers with chronic low back pain were divided into two groups, control group(n=10) and experimental group(n=10). Back muscle strength, flexibility, and balance ability were measured to assess physical functions, and visual analog scale(VAS) was used to evaluate pain levels. In both groups, back muscle strength and balance ability increased significantly, and pain levels decreased significantly. In comparison between the groups, the experimental group compared to the control group showed significant improvement in balance ability and significant diminishment in pain levels. In conclusion, the lumbar stabilization exercise has positive effects on industrial workers with lumbar instability and chronic low back pain by improving balance ability and reducing pain.

Effects of Chronic Pain and Social support on Depression and Suicide in the Elderly (만성통증과 사회적지지가 노인의 우울과 자살의도에 미치는 영향)

  • This research is aged 65 or older with chronic pain and social support, are subject to the elderly wanted to know the effect on depression and suicide intention. This research was done through targeting elderly with chronic pain who use welfare center. The data collection period from April 20 in 2014, implemented by May 20. finally 178 kinds of types of data were analyzed. The collected data is Person's correlation coefficient, and multiple regression analysis by using SPSS 18.0, t-test, ANOVA. In this research results chronic pain, depression and suicide was observed a significant correlation between the degree of social support appear also showed a significant negative correlation of depression and suicide. Also, chronic pain can affect the degree of social support, suicide was identified as the most powerful variable impact on depression. The elderly with Chronic pain is more severe the higher the degree of suicide, The results were lower social support is also of increasing depression and suicide. There fore family and social care and social support services need to develop for prevent depression and suicide for the elderly, also needs programs invention for chronic pain as well.

Chronic Injuries and Types of Injuries Related to Adolescent Taekwondo Athletes (성장기 태권도 선수의 만성 통증과 손상 유형)

  • Purpose: This study was designed to analyze? the chronic pain of adolescent Taekwondo athletes, to research the understanding about chronic pain of coaches and athletes, to? reduce the development of chronic pain. Materials and Methods: From March. 2006 to June 2007, 210 athletics and 12 coaches of elementary, middle and high school in Kwang-ju city were surveyed. They was evaluated by physical examination, simple radiographs and MRI. Results: In 210 athletics, 162(77.1%) athletes had chronic pain. 19(11.7%) athletes had over three regions of pain, 74(45.7%) athletes had two regions of pain, 69(42.6%) athletes had one region of pain. In 274 cases of 164 athletes, the most common region was foot and ankle(145 cases, 52.9%), followed by hand(47 cases, 17.2%) and knee(38 cases,13.9%), hip(25 cases, 9.1%), and lumbar area(19 cases, 6.9%) The chronic pain was classified by injury type. Contusion of foot was 103 cases(most common), sprain of ankles was 40cases, Contusion of hands and wrists was 28 cases, and so on. The etiologies of injury that causes the chronic pain were match injury in training that is most common, intensive exercise and match injury in competition. Conclusion: The chronic pain was common in adolescent Taekwondo athletes. The ankle and foot were the most common region of chronic pain and the match injury was the most common injury, caused the chronic pain. Although athletes had considerable mental stress about chronic pain, the treatment of chronic pain and rest were insufficient. Therefore the variable effort to prevent injury and to treatment injury must be considered importantly.

Psychosocial Impact of Chronic Orofacial Pain (만성 구강안면통증의 사회심리적 영향)

  • The aim of the study was to evaluate psychosocial impact of non-dental chronic orofacial pain (OFP) on daily living using the graded chronic pain (GCP) scale. It is also investigated the clinical profile such as demographics, event related to initiation of OFP and prior treatments for patients. During previous 6 months since September 2008, 572 patients (M:F=1:1.5, mean age=34.7 years) with non-dental OFP attended university-based specialist orofacial pain clinic (Dankook University Dental Hospital, Cheonan) to seek care although 63% of them already experienced related treatment for their OFP problem. They visited the most frequently general dental practitioner and orthopedic doctors due to their pain problem and medication was the most commonly employed modality. Most of the patients (89.2%) had TMD and the most common related event to initiation of their pain was trauma, followed by dental treatment. Almost half of the patients (46%) suffered from chronic pain(${\geq}6\;M$) and 40% of them exhibited relatively high disability due to chronic OFP. GCP pain intensity and disability days were significantly different for age and diagnosis (p<0.05) but not for gender and duration. GCP grades were affected by all the factors including gender, age, pain duration and diagnosis.(p=0.000) Female gender, elders, and long lasting pain were closely related to high disability. The patients with neuropathic Pain and mixed OFP rather than TMD were graded as being highly disabled. Conclusively, a considerable percentage of chronic OFP patients reports high pain-related disability in their daily, social and work activity, which suggest a need for psychosocial support and importance of earlier referral for appropriate diagnosis and tailored management.

Mediating Effect of Coping Strategies in the Relationship between Pain Beliefs and Depression, Pain Disability among Chronic Back Pain Patients (만성요통환자에서의 통증신념과 우울, 통증생활방해와의 관계에서 대처전략의 매개효과)

  • The Purpose of this study was to investigate of coping strategies(active and passive) between pain beliefs and depression, pain disability among chronic back pain patients. Data were analyzed by the SPSS-WIN 21.0 program. Indirect SPSS macro(Bootsrapping)was used to analyze the multiple-mediation model of this study. The result showed that the mean score for pain belief was $3.42{\pm}9.67$, and he passive coping strategies was $29.68{\pm}8.04$, active coping was $25.49{\pm}4.22$. The mean score of depression was $25.49{\pm}11.56$. The pain disability index was $46.94{\pm}12.65$. It found that there were significant correlations among the 5 variables. The multiple mediated effects of passive coping and active coping on pain beliefs and depression were (b=.453, 95% CI=.228, .703) and on pain beliefs and pain disability were (b = .285, 95% CI = .131, .519) in chronic low back pain patients. This study discovered that the active coping strategies had a positive mediating effect in the relationship between pain beliefs and depression, pain beliefs and pain disability. And passive coping strategies had a negative mediating effect. Based on findings of this study, improving the active coping strategy programs or management is highly recommended in chronic back pain patients.

The effects of deep neck flexor exercise on pain and neck disability index of the patients with chronic neck pain (심부목굽힘근 운동이 만성 목통증 환자의 통증과 기능에 미치는 영향)

  • This study was to investigate the effectiveness of deep neck flexor exercise with using modalities in pain reduction and functional improvement to those who have chronic neck pain. The subjects were instructed the patients with chronic neck pain (37 people). Randomized study design of two groups was used: Only using modalities group and deep neck flexor exercise group with using modalities, and each group was taken by three times per week for 12 weeks. To evaluate the effects of therapies was to research the questionnaire about VAS(visual analog scale), NDI(neck disability index), and endurance of deep neck flexor before experiment, after 6 weeks and after 12 weeks. In visual analog scale(VAS) and neck disability index(NDI), there were significant decreased in using modalities group and deep neck flexor exercise group after 6 weeks more than before experiments but were only significant decrease in deep neck flexor exercise group after 12 weeks. The endurance of deep neck flexor was significantly increased in deep neck flexor exercise group after 6 weeks and 12 weeks more than before experiments. Thorough these results deep neck flexor exercise has the effectiveness to the therapies of chronic neck pain. Using the deep neck flexor exercise would be high effects on pain reduction and functional improvement and also considered in improving the living qualities of those who have chronic neck pain.

Effect of 8 Direction Incline and Rotation Exercise on Pain and Dynamic Balance in the Patients with Chronic Low Back Pain (8방향 경사와 회전 운동이 만성요통환자의 통증과 동적균형에 미치는 영향)

  • The purpose of this study is to investigate the effect of 8 direction incline and rotation exercise on pain and dynamic balance in the patients with chronic low back pain(CLBP). 20 patients who were diagnosed with CLBP were included for the study. 10 patients(experimental group) were treated by modality{Hot Pack(H/P), Transcutaneous Electric Nerve Stimulation(TENS), Ultra Sound(U/S)} and then performed 8 direction incline and rotation exercise. The other 10 patients(control group) were only treated by modality. The therapeutic intervention was taken three times a week for 6 weeks. Dynamic balance was assessed by Star Excursion Balance Test(SEBT) and pain was assessed by Visual Analog Scale(VAS). VAS scores of both groups were decreased. However, the experimental group was more significantly decreased than the control group. The dynamic balance of both groups was significantly increased in anterior, posterior, medial, lateral direction. But experimental group was more significantly increased than the control group. In conclusion, 8 direction incline and rotation exercise was effective on pain and dynamic balance in CLPB.

The Effects of Aromatherapy on Elders' Chronic Pain, Sleep (아로마테라피가 노인의 만성통증과 수면에 미치는 효과)

  • This research applied Aromatherapy Foot Massage on Community elders and examined its effects on their chronic pain, sleep. The study subjects were 43 female elders in the community of including 22people for the experimental group and 21 for the control group. The aromatherapy was carried out 6times for 2weeks to the experimental group, 3times a week and 10minutes per session. The components of aroma essential oil included Chamomile roman, Lavender, Marjoram sweet, and Orange sweet that were diluted 20% into 100mL of Jojoba oil which was the carrier oil. The results, the experimental group that participated in the aromatherapy improved in chronic pain, quality of sleep, and the number of urination during sleep decreased compared to the control group. The research results demonstrated that due to the aroma foot massage, chronic pain decreased, sleep improved.

Effects of Cervical Spinal Stabilization Training in Private Security on Chronic Neck Pain and Cervical Function, Neck Pain, ROM (경부안정화 운동이 민간 경비원의 목통증, 경부장애지수, 관절가동범위에 미치는 효과)

  • The purpose of this study was to evaluate the effects of using cervical spinal stabilization exercise for the improvement of pain intensity, cervical range of motion, neck disability index, reposition sense, muscle tenderness with chronic neck pain in private security. For 21 patients diagnosed with chronic cervical pain and divided into cervical spinal stabilization exercise group and postural correction exercise group. Each exercise was conducted for 8 weeks. Pain and neck disability index were measured before and after exercise using the visual analogue scale(VAS) and the neck disability index(NDI). Range of motion were measured electronic goniometer, muscle tenderness of upper trapezius ad sternocleidomatoid were measured algometer, reposition sense were measured reposition panel before and after exercise. After 8 weeks of exercise, the cervical stabilization exercise group pain and neck disability were significantly decreased(p<0.05). Also there was significant difference in both group(p<0.05). In addition, range of motion, muscle tenderness reduce rate, reposition sense were significantly increase as compared to the pre-post exercise in cervical stabilization exercise group(p<0.05). But there was no significantly difference in postural correction group before and after exercise(p>0.05). And there was significantly increase more cervical stabilization exercise group than postural correction exercise group in range of motion, muscle tenderness reduce, reposition sense. In summary, cervical spinal stabilization exercise is more effective in improving cervical range of motion, muscle pain, reposition sense in private security on chronic cervical pain patients, in reducing patients' pain and disability. It is an effective treatment to aid rehabilitation in these cases.
