Effects of Cervical Spinal Stabilization Training in Private Security on Chronic Neck Pain and Cervical Function, Neck Pain, ROM (경부안정화 운동이 민간 경비원의 목통증, 경부장애지수, 관절가동범위에 미치는 효과)

  • The purpose of this study was to evaluate the effects of using cervical spinal stabilization exercise for the improvement of pain intensity, cervical range of motion, neck disability index, reposition sense, muscle tenderness with chronic neck pain in private security. For 21 patients diagnosed with chronic cervical pain and divided into cervical spinal stabilization exercise group and postural correction exercise group. Each exercise was conducted for 8 weeks. Pain and neck disability index were measured before and after exercise using the visual analogue scale(VAS) and the neck disability index(NDI). Range of motion were measured electronic goniometer, muscle tenderness of upper trapezius ad sternocleidomatoid were measured algometer, reposition sense were measured reposition panel before and after exercise. After 8 weeks of exercise, the cervical stabilization exercise group pain and neck disability were significantly decreased(p<0.05). Also there was significant difference in both group(p<0.05). In addition, range of motion, muscle tenderness reduce rate, reposition sense were significantly increase as compared to the pre-post exercise in cervical stabilization exercise group(p<0.05). But there was no significantly difference in postural correction group before and after exercise(p>0.05). And there was significantly increase more cervical stabilization exercise group than postural correction exercise group in range of motion, muscle tenderness reduce, reposition sense. In summary, cervical spinal stabilization exercise is more effective in improving cervical range of motion, muscle pain, reposition sense in private security on chronic cervical pain patients, in reducing patients' pain and disability. It is an effective treatment to aid rehabilitation in these cases.

A Convergence Study on Factors Influencing Health-related Quality of Life in Patients with Chronic Neck Pain (만성경부통증 환자의 건강관련 삶의 질 영향요인에 대한 융합연구)

  • A Convergence study was conducted to investigate the factors influencing the health-related quality of life in patients with chronic neck pain and to present a strategy for effective program development. The participants of this study were 92 patients with chronic neck pain in a region and collected data by self-reported questionnaire. Data were analyzed using PASW 18.0 program, that descriptive statistics, t-test, ANOVA, correlation analysis and stepwise multiple regression analysis were performed. According to the results of stepwise multiple regression, the identified influencing factors were disability(${\beta}=-.403$, p<.001), depression(${\beta}=-.313$, p<.001), age(${\beta}=-.194$, p=.008), muscle disorder(${\beta}=-.176$, p=.009), education(${\beta}=-.151$, p=.043) with health-related quality of life in patients with chronic neck pain. The explanatory power($R^2$) by 5 variables was 67.1%(F=38.118, p<.001). It was found that it is important to consider the individual characteristics, physical function improvement and psychological support for improving the health-related quality of life in patients with chronic neck pain. Therefore, it is necessary to develop a health promotion program based on the factors influencing on the health-related quality of life and analyze its application effect.

  • MyoFascial Pain Syndrome(MFPS) is defined as a regional pain syndrome characterized by muscle pain caused by myofascial trigger points (MTrPs). Myofascial pain is a common cause of persistent regional pain such as neck pain, shoulder pain, headaches, and orofacial pain. Clinicians who deal with orofacial pain must also understand the role of myofascial pain. This case report presents the treatment of botulinum toxin A for chronic myofascial pain.

  • This study was conducted to investigate the effects of upper thoracic joint mobilization technique using Kaltenborn-Evjenth concept on the range of cervical and thoracic motion and pain in patients with chronic neck pain. The subjects were divided into a thoracic joint mobilization group(n=7) and a conservative physical therapy group(n=7). Each of the groups received thoracic joint mobilization or conservative physical therapy three times a week lasted for four weeks. The measurements were performed for the range of thoracic segmental motion(SpinalMouse), the pain(visual analogue scale) and the range of cervical joint motion(Inclinometer, Dualer IQ). They were made four times: before experiment, at 2weeks, 4weeks, and 4weeks after experiment.

  • With the medical progress that has given spinal cord injured(SCI) individuals greater longevity and better overall health, chronic pain is emerged as major challenge in treating this population. According to past reports, estimates of prevalance of severe/disabling chronic pain in SCI patients have ranged from 18% to 63%. In etiologies of chronic pain in SCI patients, psychic or psychogenic pain categories should be included and more recent data have demonstrated that the persistant pain is directly related to higher levels of psychosocial distress and impairment. Recently, neurophysiological classification of the SCI pain syndrome into three etiologic groups(a; mechanical pain, b; radicular pain, c; deafferentation pain) is more frequently adopted for the classification of chronic SCI pain syndrome. The deafferentation pain is most common of the pain syndromes associated with SCI. After cervical epidural anesthesia for the surgical intervention of decubitus ulcer on the hip of two SCI patients, there were much reduction of existing chronic deafferentation character pain.

  • Botulinum toxin has been used for treating strabismus, blepharospasm, cerebral palsy, cervical dystonia, hyperhydrosis, facial wrinkle and chronic migraine under US Food and Drug administration approval. Also it has been tried spasticity-induced pain, post-herpetic neuralgia, myofascial pain and aphthous ulcer as off-label use. In this study, we reviewed recent studies that suggested effects of botulinum toxin on snoring and sleep apnea.

The Effects of P-A Mobilization on The Cervical Range of Motion and Pain for Patients with Chronic Neck Pain (만성 경부 통증환자에 대한 후-전방 가동기법이 경부 가동범위와 통증에 미치는 영향)

  • The main purpose of this study is to evaluate the effects of manual therapies which are Posterior Anterior Central Vertebral Pressure (PACVP) and Posterior Anterior Unilateral Vertebral Pressure(PAUVP} in patients having a chronic pain in the neck. This study focused on sixty patients having a chronic pain in the neck from 19 years old and 65years old. There are 33 females and 27 males each. This study randomize two groups from all subjects that have been mentioned above. Group 1 received a cervical spine manual therapy, Group 2 received a conservative physical therapy. Each therapy was held for 3 times a week, totally, 6 times for two weeks. The tape was used to measure cervical range of motion(ROM) in six areas-flexion, extension, left lateral flexion, right lateral flexion, left rotation, right rotation. 100mm Visual Analogue Scale(100mm VAS) was used to measure the subjective pain level. All measurements of each patient were measured at pre-treatment and 2 weeks post-treatment. The results of this study would be summarized as follow: 1. The manual treatment group has much more recovery than that of conservative physical therapy group in terms of the degree of the ROM improvement showing a significant difference between two groups(p<0.05) 2. The manual treatment group has more recovery than that of the conservative physical therapy group in the improvement of pain(p<0.05).

The Effects of Stretching and Isometric Exercise for Chronic Neck Pain Patient in Strength and Pain (스트레칭과 등척성 운동이 만성 경부통증환자의 근력과 통증에 미치는 영향)

  • Purpose : The purpose of this study was to assess more effective exercise prescription for the people with chronic neck pain by comparing stretching exercise and isometric exercise about strength and pain. Method : The research design of the study was that 20 young adults with mild neck disability (5-14 out of 50 in Neck Disability Index) were randomized into 2 groups and underwent stretching exercise(10 peoples) and isometric exercise(10 peoples) at 3 times a week for 5weeks. Measures of pain scale (Visual Analog Scale) and Muscle strength of deep neck flexor (Pressure Biofeedback) were assessed before and after intervention. The pre and post exercise results were compared using paired t-test and the effectiveness of intervention of two groups were compared using Independent t test. Results : The results of this study were that pain reduction was seen after stretching and isometric exercise and significantly difference. There was significantly improvement of cervical flexor strength after stretching and isometric exercise. But, there was no significant difference between stretching and isometric exercise groups. Conclusion : The stretching and isometric exercise for chronic neck pain patients may use according to preference of patient because the effects of treatment was same. Further studies are needed to analyzed psycho social factors, cost effects, group approach.

  • Multifidus cervicis plane block has been effectively used to provide analgesia during and after cervical spine surgery, but not for any other purpose. Here, we report three cases of chronic axial neck pain unresponsive to medical treatment. We performed multifidus cervicis plane block bilaterally, which lowered numerical rating scale (NRS) score within 2 weeks. Thus, multifidus cervicis plane block is one of the novel options that may be used for chronic axial neck pain.

  • The purpose of this study is compare the effects of two therapeutic processes-i) conservative Myofacial Release Technique along and ii) Transcutaneous Electrical Nerve stimuliation alone - on the increase in the range of motion(ROM) and on the decrease in patient's pain with chronic Neck pain. The subjects of the study were student with choronic neck pain in Daegu health university. Half of them (10) took conservative Myofacial Release Technique along and the others (10) took Transcutaneous Electrical Nerve stimuliation alone. I used 3D Motion analysis. The range of motion (ROM) in six areas-flexion, extension, left lateral flexion, right lateral flexion, left rotation, right rotation-to check the effectiveness of the two combinations of methods, measured pain with visual analogue scale (VAS) to see the effect on pain reduce. Assement was conducted to the groups before the treatment began and affer the four weeks treatment ended. This study shows that both groups demonstrated significant improvement in ROM increase and pain reduction. Flexion ROM increases were significant in MFR and TENS after treatment 2 weeks and 4 weeks. Extension ROM increases were significant in MFR and TENS after treatment 2 weeks. Right rotation ROM was no significant differences were found in MFR and TENS after treatment all weeks. Right lateral flexion ROM increases were significant in MFR and TENS after treatment 3 weeks. Left lateral flexion ROM increases were significant in MFR and TENS after treatment 1 weeks. But the others no significant differences were in MFR group and TENS group during 4 weeks.
