The Mediating Effects of Resilience between Symptom Experience and Depression in Patients with Chronic Obstructive Pulmonary Disease (만성 폐쇄성 폐질환 환자의 증상경험과 우울의 관계에 미치는 극복력의 매개효과)

  • Purpose: The purpose of this study was to identify the relationship of resiliency and symptom experience and depression. Subjects (N=152) were patients with chronic obstructive pulmonary disease (COPD).admitted to D University Medical Center in B City. Methods: Data collection was conducted from July 20 to August 25, 2013. The levels of symptom, resilience, and depression were measured using a seven question symptom experience measuring instrument, a 25-question resilience instrument, and a 15-question Short Geriatric Depression Scale (SGDS). The collected data were analyzed by descriptive analysis, Pearson's correlation coefficients, and path analysis using SPSS/Win 19.0 and AMOS 5.0. Results: There was a significant negative correlation between symptom and resilience, a significant positive correlation between symptom and depression, and a significant negative correlation between resilience and depression. Symptom showed a direct effect on depression and an indirect effect on depression scores through resilience as a mediating variable. Conclusion: It is necessary to develop nursing intervention programs to relieve symptom in COPD patients, and to develop appropriate resilience enhancement programs to reduce their depression.

Effects of a Short-term Home-based Pulmonary Rehabilitation Program in Patients with Chronic Lung Disease (만성폐질환자를 위한 가정 호흡재활 단기 프로그램의 효과)

  • Purpose: The purpose of this study is to exam the effects of a short-term pulmonary program on lung function, exercise tolerance, and quality of life in chronic lung patients. Method: Randomized controlled pre-post test design was used. The outcome measures were forced expiratory volume in one second (FEV1, % predicted), 6 min walking distance (6MWD), Borg score after 6MWD, and Chronic Respiratory Disease Questionnaire (CRDQ). Experimental group performed the 4-week home-based pulmonary rehabilitation program composed of inspiratory muscle training, upper and lower extremity exercise, relaxation, and telephone visit. Patients in control group were only given education about self-management strategies. Thirty four patients with moderate-to-severe respiratory impairment were recruited, and 28 patients (19 in experiments, 15 in control) completed the study. Result: Significant improvements in lung function, exercise tolerance, and health related quality of life were found only in the experiment group. Conclusion: This study yielded evidence for the potential and beneficial effects of home-based pulmonary rehabilitation program in patients with moderate to severe chronic lung disease. The program could be adequately utilized for improvement of health related quality of life in chronic lung patients.

Factors Influencing Posttraumatic Growth in Fathers of Chronically ill Children (만성질환아 아버지의 외상 후 성장에 영향을 미치는 요인)

  • Purpose: The purpose of this study was to identify the level of distress and posttraumatic growth in fathers of chronically ill children and also, to identify the relation between characteristics of the fathers and children and their posttraumatic growth and to investigate factors that influence posttraumatic growth. Methods: In this study, 48 fathers who visited a university hospital in Seoul, Korea and who gave written consent completed the questionnaire between September 23 and November 19, 2013. Data were analyzed using Mann-Whitney U test, Kruskal-Wallis test, Pearson correlation coefficient and stepwise multiple regression. Results: The level of distress in fathers of chronically ill children was relatively high and the majority of them were experiencing posttraumatic growth. Models including the variable (deliberate rumination, religiousness, optimism) explained 64.3% (F=26.38, p <.001) of the variance for posttraumatic growth. Deliberate rumination (${\beta}=.59$, p <.001) was the most influential factor. Conclusion: The findings demonstrate that it is essential for nurses to intervene and facilitate continuously so as to promote posttraumatic growth and relieve distress in fathers of chronically ill children. Furthermore, it is also necessary for nurses to find ways to develop ideal interventions to activate deliberate rumination and offer spiritual care and help maintain optimism in these individuals.

Factors Influencing Self-Management Compliance of Patients with Chronic Hepatitis B (만성B형간염 환자의 자가관리이행에 대한 영향요인)

  • Purpose: Patients with chronic hepatitis B need lifelong health care because of progressive liver damage. The purpose of this study was to identify factors associated with self-management compliance among patients with chronic hepatitis B. Methods: The research was a cross-sectional, descriptive design using questionnaires. The participants were 118 patients with chronic hepatitis B from outpatient department in a tertiary hospital. Their ages ranged from 20 to 64. Data were analyzed using descriptive statistics, t-test, ANOVA and multiple regression with PASW statistics 19.0 program. Results: There were statistically significant differences in self management compliance by gender, age, job, alcohol consumption, and smoking. A positive correlation existed between self-management compliance, awareness of the importance of management, and self efficacy. Stepwise multiple regression analysis for self-management compliance revealed that the most powerful predictor was self efficacy. Self efficacy, age, and alcohol consumption explained 37.5% of the variance. Conclusion: The results indicate a need to promote self-management compliance with these participants. The findings also suggest that consideration be given to characteristics of age and alcohol consumption in developing strategies for enhancing self efficacy and developing programs to promote self-management for patients with chronic hepatitis B.

An Exploration of the Life Experiences of Patients with Chronic Pain : Women with Rheumatoid Arthritis (만성통증환자의 생활경험에 관한 연구-여성 류마티스관절염 환자를 중심으로-)

  • Rheumatoid Arthritis(RA), a chronic and systemic inflammatory disorder, is characterized by joint pain, swelling and stiffness. Patients with RA suffer from joint pain and overall pain. The painful and disabling consequences of RA are accompanied by a variety of affective, cognitive, and behavioral changes. The purpose of this paper was to explore and describe the life experiences of RA patients by eliciting verbal description of their experiences. Participants were nine persons who were diagnosed with RA, and had had it for more than six months. They were asked open ended and descriptive questions in order for them to talk about their experiences in their on terms. Interviews were tape recorded and transcribed verbatim. The transcripts were analyzed by Colaizzi method. From the protocols, 213 significant statements about life experience were organized into 83 formulating meanings which were then grouped into five theme clusters. The major themes that emgerged from the analysis were' Emotional Disturbances', 'Trying to Adapt to the Progress of Disease', 'Progress of Treatment'. 'Change of Role Performances', 'Experiences related to Family'. The result of this study showed that RA patients and families need nursing care based on a deep understanding of their lived experiences in everyday life. Nurses and other health workers must develop rehabilitation programs that focus on the pain control, functional independence and psychosocial factors.

Developing a Psychosocial Rehabilitation Model for Persons with Chronic Mental Illness (만성정신질환자의 정신사회재활모형 구축)

  • Purpose: The purpose of this study was to identify factors influencing the outcome of psychosocial rehabilitation for persons with chronic mental illness and to suggest a hypothetical model of psychosocial rehabilitation which is based on the relationship between influencing factors and then to test its fitness empirically. Method: Data was collected from persons with a chronic mental illness using a self-report questionnaire, and from their case managers using an observed rating questionnaire. A total of 220 cases were used in the final analysis, and a hypothetical model was verified through covariance structure analysis using LISREL. Result: 1) The fitness indices of the model were $X^2=133.77$ (df=48, p=.00), GFI=0.93, AGFI=0.87, RMR=2.32, NNFI=0.95 and NFI=0.95. Twelve paths among fifteen proved to be significant. 2) Psychosocial rehabilitation was influenced directly by symptoms, daily living skill and self-concept, and was influenced indirectly by the psychosocial rehabilitation program service. Psychosocial rehabilitation accounted for 32% of variance of these factors. Conclusion: This study is expected to contribute to understanding the psychosocial rehabilitation phenomena. Besides it will provide basic information for developing strategies of bio-psycho-sociological interventions and evaluations in regard to influential factors of psychosocial rehabilitation.

Related Factors of Quality of Life in Male Patients with Chronic Obstructive Pulmonary Disease (남성 만성폐쇄성폐질환자의 삶의 질 관련요인)

  • Purpose: The purpose of the study was to examine the related factors of quality of life (QOL) among patients with Chronic Obstructive Pulmonary Disease (COPD). Methods: Patients diagnosed with COPD (N=230) were recruited from four hospitals in Kyeong-Nam province, from March 2 to November 30, 2010. The data collection instruments were the Short Form 36, perceived dyspnea measure by Modified Medical Research Council, COPD and Asthma Sleep Impact Scale, COPD Self-efficacy Scale, and Center for Epidemiologic Studies Depression Scale were used. Following the completion of the data collection instruments Pulmonary function was tested. Data were analyzed with descriptive statistics, Pearson correlation and simultaneous multiple regression using SPSS/WIN. Results: The mean QOL of this study was 68.24. Using a multivariate approach, the significant correlates of QOL were depression (${\beta}$=-.37), dyspnea (${\beta}$=-.28), self-efficacy (${\beta}$=.20), and a sufficient degree of household income (${\beta}$=.16). These variables explained 49% of variance in QOL. Conclusion: The study suggests that psychological aspects are an important factor in explaining QOL of the patients. Screening and minimizing depression could be effective strategies in enhancing QOL of patients with COPD and further investigation to reduce depression could warrant the improvement of QOL in patients with COPD.

The Effect of Electrical Muscle Stimulation Therapy on Chronic Knee Pain for Aged (노인의 만성 무릎 통증에 대한 전기 근육 자극요법의 적용 효과)

  • Purpose: This study is to examine the effect of electrical muscle stimulation therapy on chronic knee pain for aged. Method: Design was randomized one-group pretest-posttest design. Samples were total 15 elderly on 60 years old and above with chronic knee pain. Measures were S-F McGill Pain Questionnaire and Arthritis Impact Measurement Scale for knee pain. Electrical muscle stimulation therapy, experimental treatment was applied on chronic knee pain for 4 weeks, 3 times/week, 15 min/time. Data were collected from half March 2005 to May 2005. Data were analyzed using SPSS PC+ 12 version. Descriptive statistics was used for analysis of general characteristics in sample, and paired t-test was used to analysis the effect of electrical muscle stimulation therapy. Results: After receiving the electrical muscle stimulation therapy chronic knee pain was significantly decreased (t=-29.163, P=.000 in S-F MPQ; t=-37.005, P=.000 in AIMS). Conclusion: Electrical muscle stimulation therapy can be a better effective primary nursing intervention on chronic knee pain for aged in community.

Structural Equation Modeling on Quality of Life in Pre-dialysis Patients with Chronic Kidney Disease (투석 전 만성 신부전 환자의 삶의 질 구조 모형)

  • Purpose: This study was designed to test structural equation modeling of the quality of life of pre-dialysis patients, in order to provide guidelines for the development of interventions and strategies to improve the quality of life of patients with Chronic Kidney Disease (CKD). Methods: Participants were patients who visited the nephrology outpatient department of a tertiary hospital located in Seoul. Data on demographic factors, social support, nutritional status, physical factors and biobehavioral factors and quality of life were collected between March 4 and March 31, 2011. Results: In the final analysis 208 patients were included. Of the patients 42% were in a malnourished state. Anxious or depressed patients accounted for 62.0%, 72.6%, respectively. Model fit indices for the hypothetical model were in good agreement with the recommended levels (GFI=.94 and CFI=.99). Quality of life in pre-dialysis patients with CKD was significantly affected by demographic factors, social support, nutritional status, physical factors and biobehavioral factors. Biobehavioral factors had the strongest and most direct influence on quality of life of patients with CKD. Conclusion: In order to improve the quality of life in pre-dialysis patients with CKD, comprehensive interventions are necessary to assess and manage biobehavioral factors, physical factors and nutritional status.

Literature Review about Sexual Adjustment for Chronic Illness and Disabilities (만성질환 및 장애자의 성(性) 재활에 관한 문헌 고찰)

  • Improving quality of life can be achieved by rehabilitating one socially and restoring their sexual life. Holistic care for persons with disabilities means the rehabilitation nurse intervenes when problems of sexual function are discovered, observed, or expressed. However, most people with chronic illnesses and disabilities must confront prevalent myths about their sexual nature and abilities. These myths often have their basis in the notion, common in our society, that the only people who are sexual are those who are young and beautiful. People who are obese, old, or do not conform to current standards of sexual attractiveness are usually deemed as asexual or undesirable. These narrow sterotypes have been particularly damaging to physically or mentally disabled people. People with chronic illnesses and disabilities often have special needs regarding their sexual behavior. Most of us will confront chronic illness or disability in our own lives to some degree. In addition, illnss or disability can occur at any age. In this study, I discuss some of the special needs of people with chronic illnesses and disabilities and the methods of coping with those needs. I also discuss some of the sexual concerns that people with chronic illnesses and disabilities have in common, and the assessment of sexual function and nursing strategies through literature review.
