Development of Comprehensive Nursing Intervention for the Client with Lymphedema (통합적 림프부종 간호중재 개발 연구)

  • Purpose: The purpose of this study was the development of a comprehensive nursing intervention program for the client with acute lymph stasis and stage I lymphedema. Method: The Quasi-experimental design using a non-equivalent control group was used. The subjects were 22 stroke patients with lymph stasis in the control group and 23 patients in the experimental group. The complex physical therapy of Casley-Smith was carried out to the control group for 10 hours, and comprehensive nursing intervention for the experimental group was carried out for 60 minutes. The data for this study was gathered from Feb. 2002 until June 2002 and pertains knowledge about lymphedema, self-care for managing lymphedema, and circumferences of affected limbs. Data was analyzed by mean, standard deviation, ${\chi}^2-test$, and t-test. Result: The changes in knowledge about lymphedema, self-care practices, and circumference of affected limbs after nursing intervention did not show significant differences between control group and experiment group. Conclusion: It can be concluded that comprehensive nursing intervention had more efficiency than complex physical therapy in the treatment of edema for stroke patients because of it's simplicity and time saving. Thereby, the comprehensive nursing intervention program developed in this study would be a useful therapy for the clients with lymph stasis and early stage lymphedema.

Early Diagnosis of Burkitt Lymphoma on the Mandible: A Case Report (하악골에서 발생한 Burkitt 림프종의 조기발견과 진단)

  • Burkitt lymphoma (BL) is an aggressive form of non-Hodgkin's B-cell lymphoma found primarily in the pediatric population. In the oral cavity, this tumor can grow rapidly and often brings about facial swelling or development of an exophytic mass involving the jaws. A 5-year-old boy was referred for swelling and pain in the left mandibular area. The patient showed diffuse swelling on the left side of the mandible and firm-moderate tenderness upon palpation. An intraoral examination showed moderate mobility and sensitivity to percussion on the left primary first and second molars, without severe caries. A radiographic examination revealed complete loss of the lamina dura on the left primary second molar and permanent first molar. There was a radiolucent osteolytic lesion and destruction of the cortical bone of the left mandibular body. Based on the clinical, radiographic, and immunohistochemical findings, the patient was diagnosed with BL, and was referred to a pediatrician for systemic evaluation and intensive chemotherapy. Even before the completion of chemotherapy, the swelling resolved and the displaced teeth were relocated to a normal position. This patient showed a good prognosis due to prompt diagnosis and intensive chemotherapy. Early diagnosis and referral for treatment can prevent the development of BL.

  • 마사지 치료는 한국에서 가장 일반적인 치료 도구의 하나로써 물리치료에서 적용되고 있으며 방법도 빠르게 변하고 있다. 지금까지 여러 형태의 마사지 방법들이 국내 물리치료사에게 소개 되어왔지만 림프흡수 마사지는 임상결과가 이전의 연구에서 제시되어 왔음에도 제대로 소개되지 않아서 일반적인 치료적 마사지 방법의 하나로 사용되고 있지는 않았다. 이에 본 연구자들은 현재 국제 림프학회에서 공인된 보더 스쿨의 엠엘디 코스를 임상 물리치료사들에게 소개함으로 해서, 국내에서의 치료의 질을 한 층 더 높이는 기회가 되고자 한다. 공식적으로 공인된 보더 방법의 엠엘디 치료사가 되기 위해서는 보더 스쿨에서 교육하고 있는 모든 교육과정들(기초, 치료 I, 치료 II+III) 치료를 이수해야만 한다. 이수 후 지속적으로 자격을 인정받기 위해서는 2년에 한 번씩 재이수 과정을 마쳐야 하며 이런 과정을 통해 만성 부비동염, 부종, 화상, 관절염, 림프부종 등의 다양한 종류의 질환을 엠엘디를 이용하여 치료할 수 있게 된다. 결과적으로, 이 연구를 통해 보더의 엠엘디 과정을 소개함으로 해서 앞으로 국내 물리치료 분야에서 더 많은 림프와 암에 관련된 전문 엠엘디 치료사가 배출되기를 바라며 그러므로 한 층 국내 물리치료 분야의 질을 올릴 수 있는 기회가 되었으면 한다.

Development and Validation of a Knowledge Scale for Lymphedema in Patients with Breast Cancer (유방암 환자의 상지 림프부종 지식 측정도구 개발)

  • Purpose: The purpose of this study was to develop and validate of a knowledge scale for lymphedema in patients with breast cancer. Methods: 34 preliminary items were made according to literature review, then verified content validity, construct validity, and reliability of the scale. 28 items among them were confirmed through content analysis by 4 experts. After a preliminary test, a survey for 156 breast cancer patients was performed for confirming construct validity and reliability. The data were analyzed using factor analysis, independent t-test, and KR-20. Results: This scale had 28 items consisting of 2 categories: prevention and self-care of lymphedema. Construct validity was confirmed by known-group technique because there were some categories consisted of just one or two items, and some mismatches between categories and items in factor analysis. Women who were educated about lymphedema scored significantly higher than women who had not (t=-3.92, p<.001). Reliability was appropriate (KR-20=.81). A percentage of correct answers was 74.6%, but it was from 31.8% to 96.1%. Conclusion: The study shows that this scale is reliable and valid to measure the knowledge of lymphedema. This scale can be effective to assess and educate the patients with breast cancer.

Factors Influencing the Emotional State of Patients with Lymphedema (림프부종 환자의 정서상태와 관련요인)

  • Purpose: The purpose of this study was to investigate the emotional state and related factors in patients with lymphedema. Methods: The subjects of this study consisted of 95 patients with lymphedema at 8 hospitals in Busan and Seoul. Data was collected by a self-administered questionnaire between March 2001 to December 2001. Data was analyzed by Pearson correlation coefficient, and stepwise multiple regression using SPSS Win 12.0. Results: The mean score of the emotional state of the subjects was 3.06; of their physical symptoms, 1.84; of their Activities of Daily Living(ADLs), 2.30; and of their social activities, 3.67. The emotional states of lymphedema patients correlated with their physical symptoms, their ADLs, and their social activities. The Factor influencing the emotional state of the subjects was social activities. Conclusion: These results suggest that a negative emotional state is very common in patients with lymphedema, to which appropriate attention should be given. Rehabilitation programs must be implemented to improve lymphedema patients' emotional state, physical symptoms, ADLs, and social activities.
