Rheumatoid arthritis (RA) is a chronic inflammatory disease, which is mainly characterized by disease of joints affected with synovial hyperplasia, pathological immune response, and progressive destruction; all of which represent an important social health problem. These provide new insights in its pathogenesis of rheumatoid arthritis diagnosis and disease progression in molecular changes. This review focuses on new serological and immunological markers which seem to be useful in early diagnosis and prognosis of rheumatoid arthritis. Therefore, such tests are widely conducted for serological biomarkers and the developments with such immunological factors to identify patients who are at risk for disease progression. This evidence of the disease based on laboratory medicine could provide the best outcome for patients. Finally, data from recent studies will help to refine the ultimate usefulness of this novel approach for early diagnosis, treatment, and helping clinicians to optimize therapy by using this approach.

Juvenile rheumatoid arthritis(JRA) is the most common major connective tissue disease in children. Renal involvement in JRA is rare. Among the renal lesions that have been reported in JRA, amyloidosis and drug-induced nephropathy are the most common. Crescentic glomerulonephritis in JRA has rarely been reported. We report a case of ANCA-associated pauci-immune crescentic glomerulonephritis in JRA. The patient was a 15-year old boy with a 3-year history of JRA. He presented with gross hematuria, proteinuria, positive p-ANCA and elevation of BUN and creatinine. Pathologic findings revealed focal necrotizing and crescentic glomerulonephritis. There were no significant immunoglobulin or complement deposits. His renal function recovered after intravenous methylprednisolone pulse therapy and oral steroid use. In Korea, this is the first reported case of pauci-immune crescentic glomerulonephritis in JRA. (J Korean Soc Pediatr Nephrol 2005;9:231-236)

Rheumatoid arthritis(RA) is an of autoimmune inflammatory systemic disease. It is characterized by uncontrolled proliferation of synovial tissue and a wide array multisystem comorbidities. The disease may involve any joint of the body, but often statrs in the peripheral joints. It was reported that more than 50% of RA patients exhibit clinical involvement of TMJ. This report is a case report of dental management and progression for 16 months in patients who had severe bony change in TMJ involved rheumatoid arthritis Dental management was included palliative treatment such as interocclusal splints, physical therapy, mouth opening exercise. Although it was progressed rapidly osteolytic bone change during follow-up, no more advanced occulsal change and improved symptom and jaw motion. Further investigations about rule of dentistry in TMJ involvement in RA maybe needed.
