Study of nosocomial rotavirus infection in neonates admitted to a postpartum-care center (서울시내 1개 산후 조리원에서 시행한 로타바이러스 선별검사에 대한 분석)

  • Purpose : Rotavirus is one of the most important etiologic agents of nosocomial infections among the neonates. This study was designed to investigate nosocomial rotavirus infection in neonates who were admitted to a postpartum-care center after birth. Methods : From March 2005 to September 2006, 957 healthy neonates were examined for rotavirus antigen in stool by immunochromatographic method and 216 neonates were rotavirus antigen positive within 24 hours after admitted to a postpartum-care center. We reviewed the nursing charts retrospectively such as characteristics, monthly distribution, birth hospitals, delivery methods, feeding types and clinical manifestations. Results : Among 957 neonates, 216 neonates (22.6%) were rotavirus antigen positive and there were no differences in sex, birth weight, gestational age. Monthly positive rate of rotavirus antigen showed diversity from 10% to 36%. According to birth hospitals, positive rate showed diversity from 3.5% to 53.6%. Out of 957 neonates, 655 cases (68.4%) were born of vaginal delivery and mean hospitalized duration was 2.4 days, 302 cases (31.6%) were born of cesarean section and mean hospitalized duration was 5.7 days. 17.6% of vaginal delivery and 33.4% of cesarean section were rotavirus antigen positive. The positive rate was higher in neonates by cesarean section than vaginal delivery (P<0.001). According to feeding types, positive rate of rotavirus antigen was lower in breast-fed group than formula-fed group (P<0.001). Proportion of symptomatic case among rotavirus antigen positive was 34.7%. Most common clinical manifestation was diarrhea (61.3%), following poor feeding (45.3%), fever (40.0%), vomiting (25.3%), delayed weight gain (12.0%), and decreased urine amount (5.3%). Conclusion : Some neonates were already infected before admission to a postpartum-care center. Without meticulous management, nosocomial rotavirus infection would transmit rapidly in a postpartum-care center spreading to the community. Recommendation of breast-feeding, routine rotavirus screeing test with or without symptom, and isolation of all rotavirus antigen positive neonates in a postpartum-care center seem to be necessary. Also attentive hygiene education and further investigations of rotavirus infection in a postpartum-care center would be needed.

Influence of Climate Factors and PM10 on Rotaviral Infection: A Seasonal Variation Study (Rotavirus 감염의 연도별 유행시기의 변동양상 및 기후요소와 PM10과의 관계)

  • Purpose: Recently, while the authors were experiencing that the epidemic period of rotaviral infection happened more in the early spring, we tried to find out how the outbreaks of rotaviral infection are changing in detail depending on the weather condition since it has something to do with the climate factors and PM10. Methods: Fourteen hundreds seventy nine patients who were proved to be positive to rotavirus were chosen among children less than 5 years old from January 1995 to June 2003. Among various climate factors, monthly average temperature, humidity, rainfall and PM10 were selected. Results: Rotaviral infection was most active in 2002 as 309 (20.9%) patients. It has been the spring that is the most active period of rotaviral infection since 2000. The temperature (RR=0.9423, CI=0.933424~0.951163), rainfall (RR=1.0024, CI=1.001523~1.003228) and PM10 (RR=1.0123, CI=1.009385~1.015248) were significantly associated with the monthly distribution of rotaviral infection. Conclusion: Through this study we determined that the epidemic period of rotaviral infection is changed to spring, which is different from the usual seasonal periods such as late fall or winter as reported in previous articles. As increased PM10 which could give serious influence to the human body, and changing pattern of climate factors such as monthly average temperature and rainfall have something to do with the rotaviral infection, we suppose that further study concerning this result is required in the aspects of epidemiology, biology and atmospheric science.

Therapeutic Effect of Anti-Rotavirus Chicken Egg Yolk Immunoglobulin(IgY) on Diarrhea by Infection of Rotavirus (로타바이러스 감염성 설사에 대한 항-로타바이러스 난황항체의 치료 효과)

  • Purpose : Rotavirus is an enteric pathogen that affects millions of children globally each year. But no specific therapy is available for the management of rotavirus diarrhea. Due to the clear need to define improved modality for treatment of rotavirus diarrhea, we evaluated the efficacy of antirotavirus IgY in the treatment of infants and children with gastroenteritis. Methods : First, the amount of viral particle in the stools of thirteen patients(seven were given IgY, 6 placebo) infected by rotavirus were evaluated for 3 days with the quantitative RT-PCR method. Second, 36 children with known rotavirus infection identified by ELISA or semi-quantitative RT-PCR were evaluated. We gave 5 g anti-rotavirus egg yolk daily in two equally divided doses for 3 days to two groups(an 18 IgY group and an 18 placebo group), respectively after parenteral consent. Daily vomiting frequency, stool frequency, oral intake and urine output were monitored for 3 days, and electrolyte and blood chemistry were checked at the first and third days. Results : First, in the placebo group, the amount of virus particles increased daily, but in the IgY group it decreased daily. Second, when IgY and placebos were given to children infected with rotavirus, diarrhea on the third day decreased significantly in the IgY group, compared with the placebo group. Conclusion : Treatment with antirotavirus immunoglobulin from immunized chicken's egg resulted in a decrease in the amount of viral particles in stools and diarrhea frequency in children. These results suggest that anti-rotavirus IgY is effective in the treatment of rotavirus gastroenteritis.

  • screw압축기는 진동이 적고, 소형이면도 그효율 유지가 용이하다는 특징을 가지고있으며, 왕복압축기와 같은 crank가 없고 원통형 밀폐용기 내에 암,수 로타가 서로 반대 방향으로 맞물려 회전하면서 로타홈과 케이싱 사이에 생기는 공간의 용적이 로타의 회전에 의해서 가스를 연속적으로 흡입, 압축, 배출하는 용적형압축기이다. 본 연구는 screw 압축기의 성능에 큰 영향을 미치는 로타의 치형함수의 기하학적성질 및 제한사항을 서술하였고, 로타의 형상이 특성값에 미치는 영향을 이론적으로 해석 하기 위하여, 임의의 치형을 갖는 로타를 만들고, 이것과 맞물리는 상대로타의 치형을 설계하여 seal line length와 blow hole area을 계산하였다.

  • 발전용 터빈은 현대생활의 기본을 이루는 전기를 만들기 위하여 보일러에서 생 산된 열에너지를 기계적 에너지로 전환하는 장치로서 발전효율 상승과 경제성 때문에 그 단위기 용량이 대형화 되고 있는 추세이다. 특히 원자력 발전 설비는 화력발전설 비에 비하여 거의 두배에 이르는 용량으로 건설되어 원자력발전용 터빈의 중요성이 증대 되고 있다. 본 연구에서는 원자력 발전용터빈로타의 운전유형별 열응력해석을 수행 하여 현재 운용중인 발전용 터빈로타의 운전조건을 검토하고부하추종 운전의 타당성 검토기본자료로 활용코자 하였다.

  • Rotavirus is a most common etiologic viral agent of pediatric gastroenteritis. Most newborns infected by rotavirus show no specific symptoms, yet, some of neonates can develop serious complications such as intestinal perforation, necrotizing enterocolitis or even death. And rotavirus infection may cause only neurologic manifestation such as seizure without gastrointestinal manifestations in some neonates. We experienced a case of neonatal rotavirus infection with benign convulsion without gastrointestinal manifestations, and report this case with literature review as followings.

  • 본 연구에서 열발생탱크, 모타, 로타와 스탯, 순환펌프, 열교환기로 구성된 풍력-열변환시스템을 제작하여 열교환에 미치는 몇 가지 요인에 대하여 시험하였다. 주요결과는 다음과 같다. 가. 풍력열교환시스템은 발열부, 구동부, 열교환부로 구성하였다. 나. 열교환성능시험에서 열교환에 미치는 요인은 유체주입량 3수준, 점성유체의 종류 2수준, 로타의 졸류 3수준, 로타와 스텟의 간격 3수준으로 정하여 열교환시험을 하였다. 다. SAS GLM procedure를 사용하여 열교환량에 대한 각 처리의 효과에 대해 조사해본바 유체주입량이 열 교환량에 가장 큰 영향을 미친다는 것을 발견했다. 라. 최고열교환량은 처리조건 R3 로타, 유체주입량 110 L, 로타와 스텟의 간격 17mm, A 오일에서 발생했으며 7,800 kcal/h 가 되었다. 마. 열 변환효율을 극대화하려면 열발생탱크의 직경보다는 높이를 크게 하고 유체를 최대 높이까지 주입하는 것이 바람직하리라 사료된다.

Studies on the pathogenesis of group A avian rotavirus infection in turkeys (Group A 조류 로타 바이러스의 병원성에 관한 연구)

  • Commercial turkey poults not previously exposed to avian rotavirus were inoculated orally with the virus alone or in combination with E coki serotype 078 at 1, 7, 14 and 21 days of age. Turkey poults of 1, 7 and 14 days of age were susceptible to infection despite the presence of maternal antibodies against avian rotavirus in their serum. However, turkey poults at 21 days of age were less susceptible compared to those ages 1, 7 and 14 days. The clinical signs in poults of all ages were mild. Viral antigens were demonstrated in the mature villous epithelial cells of the duodenum, jejunum and ilem. Histopathological lesions were characterized by vacuolation of the epithelial cells and heterophil infiltration in infected turkey poults. A significant difference in D-xylose absorption was observed between control and rotavirus infected groups at 1 and 3 days post-infection in 14 days old turkey poults.

Comparison among Diagnostic Methods of Rotaviral Gastroenteritis in Children (유소아 로타바이러스 장염 진단 검사의 비교 연구)

  • Purpose: The purpose of this study was to evaluate the clinical efficacy of reverse transcription-polymerase chain reaction (RT-PCR) in detecting rotaviral gastroenteritis in children comparing with that of commercial immunoassays. Methods: Stools from 79 children admitted Korea University Hospital due to diarrhea were collected from December 1999 to February 2000. Immunoassays were done using commercial rotavirus Latex kit and Rotatec (ELISA) kit. RT-PCR was performed to amplify group A rotavirus, most commonly pathogenic to human, using VP4- and VP7-specific primers. The detection rates of immunoassays and RT-PCR were compared. Results: ELISA assay was superior to LA assay and moderately concordant with RT-PCR in detecting rotaviral gastroenteritis. Conclusion: Although RT-PCR is known very sensitive, it does not have significant advantage over immunoassay in detecting rotaviral gastroenteritis.

Fat Content in Stool of Children with Rotaviral Enteritis (로타바이러스 장염 환아의 대변 내 지방량에 관한 연구)

  • Purpose : Rotavirus is a leading cause of severe gastroenteritis in infants and young children around the world. The aim of this study is to investigate the fat content in stools of patients with rotaviral enteritis compared to the stools of children who had no gastroenteritis. Methods : Seventy two patients who were admitted to Konkuk University Hospital, College of Medicine from Jun 2001 to May 2002 due to rotaviral enteritis and seventy five patients who were admitted at the same time with other diseases with no gastrointestinal problems as control, were enrolled in this study. The age of patients was from one month to five years. The average age of children with rotaviral enteritis was $17{\pm}11months$ and the average age of control patients was $14{\pm}15months$. Fat content of stools was investigated by acid steatocrit tests in both patients with rotaviral enteritis and control. Results : Acid steatocrit value of patients with rotaviral enteritis was higher than that of control patients. There was no difference in acid steatocrit value of children with rotaviral enteritis among the age groups. In one month- to six month-old infants, there was no difference in acid steatocrit values between the children with rotaviral enteritis and control patients. But, over the age of seven months, the acid steatocrit value of children with rotaviral enteritis was higher than that of control patients. Conclusion : We are of the opinion that fat malabsorption in patients with rotaviral enteritis and steatorrhea in rotaviral enteritis may result from decreased fat absorption in the small intestine.
