  • This research was done to classify lower body types by direct measurements, side and back view types from waistline to gluteal furrow line individually, and to analyze correspondence these lower body types with side and back view types from waistline to gluteal furrow line. The subjects of this study were 191 female college students from 18 to 25 years old residing in Pusan urban area. Type 1(long and slim) of lower body types by direct measurements was correspond with Type 1(slim curve) and Type 4(slim and protrusive hip) of side view and Type 3(wide and curve) and Type 4(trapezoid shape) of back view. Type 2(short and thick) of lower body types was correspond with Type 3(thick and droopy hip) of side view and Type 1(wide and straight) of back view. Type 3(short and slim) of lower body types was correspond with Type 2(slim and flat hip) of side view and Type 2(narrow and curve) of back view.

Local Complications after Intramuscular Buttock Injections in Children (소아에서 둔부 근육주사 후 발생한 국소 합병증)

  • Intramuscular injection(IM) into the gluteal muscles is a common route of medication, but may lead to complications. A retrospective review of 32 patients who required surgical treatment for local complications of buttock injections in children was made at the Taegu Fatima Hospital during a seven-year nine-month period (March 1990 to December 1997). Local complications included acute inflammation, cellulitis and abscess(71.9 %), and fat necrosis(21.9 %), and injection granuloma(6.2 %). Over the half of injections were on the upper and outer quadrant of the buttock, but the other 43.7 % were in the upper and inner or lower and outer quadrant which are considered unsuitable sites for intramuscular injection. The majority of complications developed within fat tissue(90.6 %) rather than within muscle(9.4 %). Two-thirds of the patients were under 2 years of age, this suggests that it is technically difficult to accurately administer IM injections in small children because muscle mass is smaller compared to subcutaneous. In addition subcutaneous fat is more susceptible to chemical irritation. Staph. aureus was the predominant organism, isolated in 84.6 % of the patients with abscesses. Treatment consisted of needle aspiration, incision and drainage, curettage, or surgical excision. In conclusion, the major factor that contributes to complications following IM of the buttock appears to be the inadvertent intrafat rather than of IM injection. Accurate injection into the muscles based on a knowledge of pelvic anatomy as well as the potential complications is necessary to prevent complications.

The Effect of Gluteal Taping on Posture and Balance During Standing in Children with Hemiplegic Cerebral Palsy (경직성 편마비 아동의 둔부 테이핑 적용이 선 자세에서 자세 및 균형에 미치는 영향)

  • PURPOSE: The purpose of the present study was to examine the effect of gluteal taping on posture and balance during standing in children with hemiplegic cerebral palsy (CP). METHODS: The subjects of this study were 13 children (six females, seven males; mean aged 8.5) with hemiplegic CP who were gross motor function classification system (GMFCS) level I. The change of posture and balance during standing before and after gluteal taping were measured using inclinometer, gross motor function measure, and functional reaching test. The collected data were analyzed using the paired t-test. RESULTS: The results of this study were as follows : 1) There were statistically significant decrease in the ant. tilt angle of pelvis after gluteal taping in children with hemiplegic CP (p<.05). 2) There were statistically significant increase in functional reaching test after gluteal taping (p<.05). 3) There was no statistically significant difference in gross motor function measure, but significant increase in one leg standing was observed (p<.05). CONCLUSION: As the above results, we suggest that gluteal taping could be effective on improving body alignment and dynamic balance ability during standing in children with hemiplegic CP. Further studies will be required for the short and long term effects of gluteal taping on improving postural symmetry and balance.

The Combined Effect of Autologous Fat Injection and Liposculpture for Gluteal Reshaping in Koreans (한국인에 있어 둔부몸매교정(Gluteal Reshaping)을 위한 자가지방주입술 및 지방흡입술의 효과)

  • Purpose: Together with the breast, buttocks are an important element of attractive body contour. To make a beautiful buttocks, improvement of body contour around the buttock as well as buttock augmentation and lifting is also important. The authors investigated characteristic features of buttocks in Koreans and report about the results of liposculpture and autologous fat injection for improving buttock's contour. Methods: We performed a retrospective study of 21 patients who would like to gluteal reshaping. We checked about buttock's ptosis, projection, depression, gluteal retraction and excessive fat accumulation around buttocks. Depending on it, we performed liposculpture and autologous fat injection. Under general anesthesia, we harvested fat from excessive fat accumulation areas around buttocks, and injected into buttocks medio-superiorly. Postoperatively, pillows were positioned on the bed not to press the buttocks which were injected with the fat. Results: Based on the shape of buttocks, A-shape is seen in 4 cases (19%), V-shape 3 cases (14%), squareshape 9 cases (43%), round-shape 5 cases (24%). Based on the Gonzalez's ptosis grading method, 1 degree ptosis is 1 case (4%), 2 degree ptosis is 6 cases (29%), 3 degree ptosis is 8 cases (38%), 4 degree ptosis is 6 cases (29%). There were no complications such as infection, hematoma, pain, dysparethesia. The subjective assessment of surgical results by patients was excellent. Conclusion: To make a beautiful buttock, improvement of body contour around the buttock as well as buttock augmentation and lifting is also important. Liposculpture and autologous fat grafting are very safe, useful and easy methods for improving buttock's contour.

Considerations for Postoperative Results of Pressure Sores around Buttock Region by Musculocutaneous Flaps and Perforator Flaps (근피피판과 천공지피판을 이용한 둔부 주위 욕창 재건술의 술후 결과에 대한 고찰)

  • Purpose: Perforator flaps have been widely used for reconstruction of pressure sores because they have many benefits, especially reducing tension. Otherwise in order to prevent recurrence of a pressure sore, sufficient thickness of a flap is desirable, so a musculocutaneous flap is also useful for reconstruction of a pressure sore. Therefore, the authors considered about the postoperative results of reconstruction of pressure sores between using perforator flaps and musculocutaneous flaps. Methods: In this study, 33 patients (46 flaps) who underwent reconstructive operation of pressure sores from January 2007 to February 2011 were reviewed. Patients operated by using perforator flaps were 18 (18 flaps), and musculocutaneous flaps were 17 (28 flaps). We studied postoperative complications and recurrence. Results: We experienced five patients (10 flaps) with complications or recurrences who were operated using musculocutaneous flaps and one patient using a perforator flap. One case using a perforator flap stemmed from dehiscence caused by a urinary fistula occurring in two months after the surgery. Among ten cases using musculocutaneous flaps, one case was caused by total necrosis of flap, five cases by partial necrosis of flap and dehiscence, and four cases by recurrences during follow-up period. Conclusion: Even if more cases were required, it can be more considerable to operate using the perforator flap rather than the musculocutaneous flap to reduce the complication or recurrence of pressure sore.

Evaluation of Usefulness in New Immobilization Device with 3D CT Angiography for Lower Extremity (전산화단층촬영검사에서 하지의 3D 혈관조영검사를 위한 새로운 고정기구의 유용성 평가)

  • Our objective was to develop and evaluate a non-invasive device for rigid immobilization and surface disease non-contact of the table in the lower extremity during CT angiography. The immobilization device consists of two components. The patient's lower limb device stabilizing elements made of polyethylene resin soft materials, and pelvis parts foam pad is used for non-contact surface. In a Prospective study the lower extremity device was used in patients who underwent a CT angiography of the lower limb. Immobilization with our device was well tolerated by all patients. The quality of the resulting images in the popliteal and infrapopliteal region was rated by five-point scale. The rigid immobilization resulted in a complete absence of motion artifacts. The new device is an effective, well tolerated and easily used immobilization that is suitable of use in 3D lower extremity CT angiography.

  • The use of the autogenous free fat is a well-known procedure to fill in superficial depressions resulting from the traumatic or congenital defects. The major donor site for this procedure was the abdominal subcutaneous fat or buttocks. In 1977, Egyedi was the first to report the use of the buccal fat pad as a pedicled graft. The buccal fat pad is a structure usually considered to be a nuisance when encountered in intraoral procedures such as facial bone osteotomies, elevation of buccal falp, or procedures on Stensen's duct. In these operations, appearance of the buccal fat pad complicates surgical exposure. The buccal fat pad is a lobulated convex mass of fatty tissue covered by a very delicate membrane, and is described as having a body from which four processes extend. These projection serve as a filling material between the various muscular structures in the area. Recently malar depression was augmented with the pedicled buccal fat pad in 3 cases, and it was used for the reconstruction of the nasolabial fold in one case.

Gluteal Subcutaneous Fat Thickness Measured by Computed Tomography as an Estimate of Proper Gluteal Intramuscular Injections in Korean Adults (전산화 단층촬영을 이용한 한국 성인의 둔부 피하지방두께 측정 -근육주사 바늘길이 산정을 위한 기초조사-)

  • Purpose: To study the thickness of gluteal subcutaneous fat (SCF) and propose an adequate length for needle for gluteal intramuscular injections based on computed tomography (CT) measurements. Methods: The thickness of gluteal SCF were measured and studied for 568 patients who visited a tertiary hospital in Seoul, Korea between January 2007 and February 2009 for routine health screening and who had abdominopelvic CT. Results: The average thickness of gluteal SCF was $15.92{\pm}4.08mm$ in males and $24.90{\pm}5.47mm$ in females. The thickness of gluteal SCF differed significantly according to gender. The gluteal SCF thickness was greater than 20.4 mm for 54 (12.3%) of the 440 male patients and 99 (77.3%) of the 128 female patients. Conclusion: The most common syringe needle used for gluteal intramuscular injections in Korea is a 23 G, 25.4 mm-needle. The SCF thickness must be less than 20.4 mm in order to reach the dorsogluteal muscles to a depth of at least 5 mm if this 25.4 mm needle is used. In many patients, especially in female patients, the 25.4 mm needles will result in improper intramuscular injections with the injection being into the SCF. Therefore an appropriate needle should be selected by considering the gender and SCF thickness of patients receiving gluteal intramuscular injections.

A Study on the Relationship between Vascular Perfusion and Interface Pressure on the Ischial Tuberosity in the Sitting Posture (않은 자세에서 둔부 좌골결절의 접촉압력과 혈류량과의 관계에 대한 연구)

  • Pressure-induced decubitus is a serious disease among the elderly people. Interface pressure occluding vascular perfusion is known to be a cause of decubitus. Therefore, it is essential to quantify the relationship between vascular perfusion and interface pressure among the elderly people to understand more about decubitus. Nine healthy elderly people (57.8$\pm$ 5.6 years, 63.3$\pm$ 7.0kg, 1.68$\pm$ 0.05m) were participated. Three healthy young people (31.7$\pm$ 3.2 years, 74.7$\pm$ 8.4kg, 1.75$\pm$ 0.04m) were also examined to be compared with the elderly group. Capillary vascular perfusion on the ischial tuberosity was recorded in the sitting posture as pressures were applied from 15mmHg to 135mmHg. The average interface pressure to occlude vascular perfusion (the average occlusion pressure) under the ischial tuberosity was 115.7mmHg in the elderly group. This value was not significantly different from the average occlusion pressure of the young group. Obesity effect on the occlusion pressure was investigated among the elderly group. The result was not significantly different between the obesity and the normal group in this study. This is a preliminary study to unveil the complicated cause of pressure-induced decubitus associated with occlusion of vascular perfusion. More subjects are required for the future study.

Pressure on Sacrum and Buttock according to Tilt Table Inclination (기립경사대 각도 증가에 따른 천골과 둔부 압력 변화)

  • Purpose: Although use of a tilt table is recommended in clinical practice, there are no published guidelines regarding pressure and inclination for tilt table use. The aim of the current study was to assess the changes of pressure on sacrum and buttock according to different inclination of the tilt table in healthy subjects. Methods: Thirty two healthy subjects participated in this study. Subjects were positioned supine on the tilt table and safety straps were secured across the chest, pelvic, and knee with sufficient tension to prevent the subjects from falling. Pressure and peak pressure of sacrum and buttock were measured using pressure mapping system with the tilt table standing at $0^{\circ}$, $15^{\circ}$, $30^{\circ}$, $45^{\circ}$, $60^{\circ}$, $75^{\circ}$, and $85^{\circ}$ inclination. Results: A significant decrease in the pressure of sacrum and buttock was achieved by increasing tilt table inclination (p<0.05): $0^{\circ}{\sim}15^{\circ}$ (8.16%), $15^{\circ}{\sim}30^{\circ}$ (8.02%), $30^{\circ}{\sim}45^{\circ}$ (11.61%), $45^{\circ}{\sim}60^{\circ}$ (16.18%), $60^{\circ}{\sim}75^{\circ}$ (16%), and $75^{\circ}{\sim}85^{\circ}$ (11.48%). A significant decrease in the peak pressure was achieved by increasing tilt table inclination (p<0.05): $30^{\circ}{\sim}45^{\circ}$ (9.91%), $45^{\circ}{\sim}60^{\circ}$ (19.24%), $60^{\circ}{\sim}75^{\circ}$ (19.93%), and $75^{\circ}{\sim}85^{\circ}$ (11.48%). No significant peak pressure change was observed in $0^{\circ}{\sim}15^{\circ}$, $15^{\circ}{\sim}30^{\circ}$ tilt table inclination (p>0.05). Conclusion: The results of this study showed that the pressure of sacrum and buttock were decreased according to increasing tilt table inclination in healthy subjects. Guidelines are needed in order to optimize patient safety and overall outcome for tilt table standing.
