  • 발음성 삼두근 증후군은 주관절을 굴곡하거나 신전할 때 삼두근 말단부 일부와 척골 신경이 내상과 전방으로 탈구되는 드문 질환이다. 이학적 검사 만으로는 다른 질환으로 오진되는 경우가 많아 정확한 진단이 필수적이며 확진을 위하여는 척골 신경 및 삼두근의 비정상적인 움직임을 관찰할 수 있는 영상 검사가 필요하다. 초음파 검사는 자기 공명 영상 촬영 등 다른 검사에 비해 간편하고 효과적으로 연부 조직의 동적 검사를 시행할 수 있는 장점이 있다. 저자들은 발음성 삼두근 증후군 환자를 동적 초음파 검사를 사용하여 진단하고 척골 신경 전방 전위술 및 삼두근 내두의 이전술을 시행하여 치유하였기에 문헌 고찰과 함께 초음파 검사의 유용성을 보고하고자 한다.

  • Snapping triceps syndrome is a rare condition in which a distal portion of triceps and ulnar nerve dislocate over the medial epicondyle as the elbow is flexed or extended from flexed position. Because it is frequently misdiagnosed as other elbow pathologies, accurate diagnosis is essential and imaging study is often needed to confirm the abnormal movement of ulnar nerve and triceps. Ultraonography is a convenient and effective method which is able to allow continual visualization of soft tissue movement compared to the other imaging modality including MRI. We reported one patient of snapping triceps syndrome who was diagnosed with the use of dynamic ultrasonography and treated with ulna nerve anterior transposition and repositioning of medial head of triceps. And we also provide the usefulness of musculoskeletal ultrasonography for the diagnosis of snapping triceps syndrome.

  • Musculoskeletal ultrasound has unique advantages that may be free from exposure to radiation, low price compared to MRI, outpatient procedure that can be easily accessible, and better accuracy combined with physical examination. Dynamic ultrasound performed with stress tests are known to be useful for detecting the hidden lesions in the tendons, ligaments, nerves. Ultrasound in the elbow can be used easily in the outpatient for evaluation of the joint surface and synovial space; diagnosis for tendon diseases such as lateral epicondylitis, medial epicondylitis and morbidity of peripheral nerves; guide for anterior-posterior bursal and intra-articular injections.

  • Ultrasound is a first line examination of the knee and useful diagnostic tool in assessing the various knee lesions. The advantages of ultrasound are that it is non-invasive, easily available at bed side, cheap, well accepted by patients, and that it has technical benefits including dynamic evaluation. Its limitations include reliability, which is largely operator dependent and its inability to evaluate deep structures. Technical guidelines of the ultrasound released by European Society of MusculoSkeletal Radiology (ESSR) and ultrasonographic findings of various knee lesions are described.

  • Magnetic resonance imaging and static ultrasound imaging do not indicate some cases of partial thickness tears of the supraspinatus tendon. The authors observed a partial thickness tear of the supraspinatus tendon that was not found using other imaging tools but was observed by resisted scaption in shoulder extension position dynamic ultrasound in several cases. This paper outlines this technique is reported by describing two cases.

  • Ultrasonographic examination for the rotator cuff disease is dynamic noninvasive study and the results is comparable to MRI on the behalf of recent development. It also can be used as a tool for guided injection and follow up study after rotator cuff repair. The development of equipment, learning curve and knowledge for the disease are the three most important factors for the critical role of the ultrasonography in the diagnosis and treatment of the rotator cuff disease.

  • Musculoskeletal sonography is rapidly developing due to the merits such as relatively low cost and possibility of dynamic study. Sonography can be helpful and easily introduced for the diagnosis of the foot and ankle disease. This review tried to clarify the cause and sonographic diagnosis of common foot and ankle diseases.

The Diagnostic Value of Dynamic US in the Extensor Tendon Dislocation at the Metacarpophalangeal Joint (중수지 관절부 신전건 탈구에서 실시간 표시 초음파의 진단적 가치)

  • Purpose: We studied the diagnostic value of dynamic US in the extensor tendon dislocation at the metacarpophalangeal joint. Materials and Methods: From January 2007 to October, we studied 6 cases that had been diagnosed and followed over 5 months (2-10) in average. US examination using a 10-MHz linear transducer were performed in three cases. The causes of dislocations were traumatic in 5 cases and congenital in one case. Results: In only 3 cases which could not be diagnosed clinically, we performed US. In dynamic US, all three cases showed the extensor tendon dislocation evidently. Operative findings were sagittal band rupture in 4 cases, capsular loosening in one case and sagittal band thinning in one case. Sagittal band repair was performed in 4 cases and capsular augmentation in one case. In case of congenital dislocation showing 4 digital extensor tendon dislocations in right hand, we operated only the second extensor by sagittal band repair with augmentation by looping. At last follow-up, no case showed recurrence or limitation of motion. Conclusion: In case of extensor tendon dislocation without apparent clinical finding, US with dynamic study has so great value that it can detect the dislocation in real time, which is superior to MRI.

  • The purpose of this article is review about the use of ultrasonography in the evaluation of the elbow joint. Ultrasonography has several key advantages in elbow joint, including easy assessment of target structure in elbow joint and ability to perform real time dynamic examination and to compare with opposite site. Ultrasonography is easy available to visualize abnormalities affecting tendons, muscles, ligaments, bursae, and occult fractures around the elbow joint and also allows accurate assessment of ulnar nerve in cubital tunnel. The role of ultrasonography will increase further with regards to evaluation of soft-tissue abnormalities of the elbow joint.

  • Musculoskeletal ultrasonography has been used as a convenient and simple tool for diagnosis of various conditions of orthopaedic diseases for many years. Generally magnetic resonance imaging (MRI) is thought to be the best method to search for anatomical structures or variations. However, for dynamic conditions such as dislocation or subluxation of tendons and nerves, MRI is not superior to ultrasonography, especially dynamic ultrasonography. So we present such a patient with an ulnar nerve subluxation at the elbow who has symtoms mimicking cubital tunnel syndrome diagnosed by dynamic ultrasonography and treated successfully by ulnar nerve anterior transposition and think that dynamic ultrasonography is a useful method for diagnosing dynamic condition such as ulnar nerve subluxation mimicking cubital tunnel syndrome.
