Purpose: This retrospective study was conduced to analyze the treatment results and to evaluate the prognostic factors affecting the survival of nasopharyngeal carcinoma patients. Materials and Methods: From 1987 to 2002, we analyzed 43 patients who had nasopharyngeal carcinomas that were histologically confirmed and who had also completed the planned radiation therapy course at Keimyung University Dongsan Medical Center According to the 6th edition of American Joint Committee on Cancer staging system, 12 patients ($27.9\%$) were at Stage 11, 13 ($30.2\%$) were at Stage III and 18 ($41.9\%$) were at Stage IV Histopathologically, there were 15 ($34.9\%$) squamous cell carcinomas, 8 ($18.5\%$) nonkeratinizing carcinomas, 17 ($39.5\%$) undifferentiated carcinomas, and 3 ($7.0\%$) lymphoepitheliomas. Among the total 43 patients, 31 patients ($72.1\%$) were treated with only radiation therapy. Neoadjuvant chemotherapy was peformed on 7 patients ($16.3\%$) and concurrent chemoradiotherapy was performed on S patients ($11.6\%$). Cisplatin and 5-Fluorouracil were administered to 11 patients for 4 cycles, and Cisplatin and Taxotere were administered to 1 patient for 6 cycles. The range of the total radiation dose delivered to the primary tumor was from 61.2 to 84 Gy (median 70.4 Gy), The follow-up period ranged from 2 to 197 months with median follow-up of 84 months. Results: The local control rate at 6 months after radiation therapy was $90.7\%$. The five year overall survival and disease free survival rates were $50.7\%$ and $48.9\%$, respectively. On the multivariate analysis, the age, T-stage ($T_{1-3}\;vs\;T_4$), N-stage and AJCC stage were the statistically significant prognostic factors affecting survival (p<0.05). The patterns of failure were as follows: local failure only in 3 patients ($7.0\%$), local and systemic failure in 1 patient ($2.3\%$), and distant metastasis only in 11 patients ($25.6\%$). Conclusion: The prognostic factors affecting the outcome of nasopharyngeal carcinoma were age, T-stage (7$T_{1-3}\;vs\;T_4$), N-stage and stage. Because systemic metastasis was the main failure pattern noted for nasopharyngeal carcinoma, systemic chemotherapy is needed to decrease the rate of distant metastasis for nasopharyngeal carcinoma. In audition, research for more effective chemotherapeutical regimens and schedules is also needed.
Depression in the medically ill is a common clinical problem that primary physicians and psychiatric consultants encounter. Treatment of such patients begins with a careful evaluation of the patient's medical and psychiatric conditions. The assessment of depression in the medical patients requires a multidimensional approach. Psychological instruments are also used as a method of assessment in these patients. First of all, what the therapists have to do is to find and remove organic causes. Psychosoical treatment includes dealing with the patient's resistance and despondency relevant to physical diseases. For biological treatment, it is important to select appropriate antidepressants. Therapists should be familiar with the side effects of the antidepressants as well as the patient's primary depressive symptoms, pharmacokinetics and pharmacodynamics of the available agents. In addition, special attention should be paid to the potential for drug-illness and drug-drug interactions. Tricyclic antidepressants can be still effectively used for patients with pain disorder, although a variety of new antidepressants such as selective serotonin reuptake inhibitors (SSRI), bupropion and venlafaxine could have more benefits in depression of the medically ill. However, electroconvulsive therapy can be recommended for refractory cases of depression in patients with medical illness.
Background: First-line drugs, if sensitive, are the most potent drugs in the treatment of multidrug-resistant tuberculosis (MDR-TB). This study examined the frequency and risk factors associated with acquired drug resistance to first-line drugs during a standardized treatment using first-line drugs in patients with MDR-TB. Methods: This study included patients who were diagnosed with MDR-TB at the National Masan Tuberculosis Hospital between January 2004 and May 2008, treated with standardized first-line drugs, and for whom the preand post-treatment results of the drug susceptibility test were available. Their medical records were reviewed retrospectively. Results: Of 41 MDR-TB patients, 14 (34.1%) acquired additional resistance to ethambutol (EMB) or pyrazinamide (PZA). Of 11 patients initially resistant to isoniazid (INH) and rifampicin (RFP), 3 (27.3%) acquired additional resistance to both EMB and PZA, and 3 (27.3%) to PZA. Of 18 patients initially resistant to INH, RFP and EMB, 6 (33.3%) acquired additional resistance to PZA. Of 6 patients initially resistant to INH, RFP and PZA, 2 (33.3%) acquired additional resistance to EMB. Ten of the 41 MDR-TB patients (24.4%) changed from resistant to susceptible. No statistically significant risk factors associated with acquired resistance could be found. Conclusion: First-line drugs should be used cautiously in the treatment of MDR-TB in Korea considering the potential acquisition of drug resistance.
Anchorage plays an important role in orthodontic treatment. Because of limited anchorage Potential and acceptance problems of intra- or extraoral anchorage aids, endosseous implants have been suggested and used. However, clinicians have hesitated to use endosseous implants as orthodontic anchorage because of limited implantation space, high cost, and long waiting period for osseointegration. Titanium miniscrews and microscrews were introduced as orthodontic anchorage due to their many advantages such as ease of insertion and removal, low cost, immediate loading, and their ability to be placed in any area of the alveolar bone. In this study, a skeletal Class II Patient was treated with sliding mechanics using M.I.A.(micro-implant anchorage). The maxillary micro-implants provide anchorage for retraction of the upper anterior teeth. The mandibular micro-implants induced uprighting and intrusion of the lower molars. The upward and forward movement of the chin followed. This resulted in an increase of the SNB angle, and a decrease of the ANB angle. The micro-implants remained firm and stable throughout treatment. This new approach to the treatment of skeletal Class II malocclusion has the following characteristics . Independent of Patient cooperation. . Shorter treatment time due to the simultaneous retraction of the six anterior teeth . Early change of facial Profile motivating greater cooperation from patients These results indicate that the M.I.A. can be used as anchorage for orthodontic treatment. The use of M.I.A. with sliding mechanics in the treatment of skeletal Class II malocclusion increases the treatment simplicity and efficiency.
The purpose of this study was to evaluate the changes of skeletodental patterns during Class II treatment and its retention period. Forty two patients of Class II malocclusion, which was treated with nonextraction or first premolar-extraction were selected and their lateral cephalograms were examined in this study. Various skeletodental changes in lateral cephalograms of pre-treatment, post-treatment and retention were measured by superimposition in reference to the cranial base for jaws, the palatal plane for maxillary teeth, and mandibular plane for mandibular teeth. The data were analyzed by paired t-test. In this study, occlusal plane showed the significant anterior downward steepening after active treatment, and remained during retention period. In the nonextraction group, maxillary incisors were retracted and extruded during treatment. Maxillary molars were extended, and mandibular molar were uprighted, with no mesial movement. In the extraction group, both maxillary and mandibular incisors were retracted and extruded. Maxillary molars were extruded and moved mesially, and mandibular molars were extruded and moved mesially with no mesial tilting. During retention period in both groups, there were tendencies of labial tipping of maxillary incisor, and mesial tipping of maxillary and mandibular molar. But the changes were not significant and most of teeth showed no change in vortical and horizontal direction.

Purpose: To investigate the quality of training hospital based treatment, we evaluated the soft tissue sarcoma treatments afforded by general orthopedic surgeon rather than orthopedic oncologist. Materials and Methods: We reviewed the details of 25 patients with pathologically confirmed soft tissue sarcoma who registered in our hospital between July 1997 and 2006 September. We evaluated initial diagnoses, the surgical treatment (including adjuvant therapy) and the follow up method used and related these to the principles of soft tissue sarcoma treatment. Results: The study cohort comprised 16 men and 9 women of mean age of 50.2 years. A diagnostic biopsy was performed in 9(36%) cases before definitive surgical treatment. Wide excision was performed in 13(52%) cases. For the 12 cases in which the grade of sarcoma was estimated, adequate surgical treatment with adjuvant therapy was performed only in 4(33.3%) cases. In addition, an adequate follow up schedule was adopted in only 4(16%) of the 25 study subjects. Conclusion: Unexpectedly, many cases of soft tissue sarcoma were treated inadequately even in a training hospital. An intensive education program on the treatment of soft tissue sarcoma is necessary for all orthopedic surgeons.

In this study, we compared dose distributions from simultaneously integrated boost (SIB) method versus the RTOG 0631 protocol for spine radiosurgery. Spine radiosurgery plans were performed in five patients with localized spinal metastases from hepatocellular carcinoma. The computed tomography (CT) and T1- and T2-weighted magnetic resonance imaging (MRI) were fused for delineating of GTV and spinal cord. In SIB plan, the clinical target volume (CTV1) was included the whole compartments of the involved spine, while RTOG 0631 protocol defines the CTV2 as the involved vertebral body and both left and right pedicles. The CTV2 includes transverse process and posterior element according to the extent of GTV. The doses were prescribed 18 Gy to GTV and 10 Gy to CTV1 in SIB plan, while the prescription of RTOG 0631 protocol was applied 18 Gy to CTV2. The results of dose-volume histogram (DVH) showed that there were competitive in target coverage, while the doses of spinal cord and other normal organs were lower in SIB method than in RTOG 0631 protocol. The 85% irradiated volume of VB in RTOG 0631 protocol was similar to that in the SIB plan. However, the dose to normal organs in RTOG 0631 had a tendency to higher than that in SIB plan. The SIB plan might be an alternative method in case of predictive serious complications of surrounded normal organs. In conclusion, although both approaches of SIB or RTOG 0631 showed competitive planning results, tumor control probability (TCP) and normal tissue complication probability (NTCP) through diverse clinical researches should be analyzed in the future.
The purpose of this study was to examine how a therapeutic drumming intervention would impact middle school students with regard to school violence prevention. Participants were all in the third-year class of a middle school in Korea. A school music teacher and a music therapist designed and implemented the program collaboratively, and mainly used dyadic, synchronized, and improvisational drumming based on the Social Emotional Learning core competencies. A total of 65 students participated in a weekly 45-minute program for 10 weeks. Ten participants out of 65 were selected for interviews and the rest of the 55 participants were asked to fill out an open-ended survey. Content analysis of the survey and interviews produced 492 meaningful statements, which were categorized into seven themes: somatic responses to drumming, emotional processing, group cohesion, empathy, relationship with peers, self-esteem, and self-regulation. The findings indicated that dyadic, synchronized, and improvisational drumming may promote prosocial behaviors in students of this age. The author discussed that drumming produces physical input directly from the instruments, which prompts students to identify and empathize with their own or others' emotions. This study therefore suggests that collaborative work between school music teachers and music therapists may positively impact middle school students' prosocial behaviors, as they pertain to school violence in Korea.

The prognosis for class III patients in growing child can be made in mixed dentition and the severity of the symptom is often amenable to early intervention. Class III malocclusion can be classified as functional class lit and skeletal origin. Skeletal Class III malocclusion is usually characterized by overdeveloped mandible, underdeveloped maxilla, but the cause of pseudo class III is most dentoalveolar or functional shift of mandible. The primary goal of early intervention of malocclusion is to supply an environment that is conducive to the development of favorable occlusal relationships and avoiding of worsening of the problems. Inverted labial bow appliance is introduced as an appliance to combine the advantage of active plate and activator. It is undemanding with this appliance to initiate not only dentoalveolar expansion of upper dentition but also to orient the functional retrusion of mandible. With simple design the compliance for patients such as mouth breathing problem can be improved. For successful use of this appliance it is utmost important to make accurate and early diagnosis between pseudo- and skeletal class III malocclusion. This article will demonstrate the use of an Inverted labial bow appliance for early treatment of a functional Class III malocclusion. After 4 month treatment, anterior crossbite was treated and the results were achieved mainly dentoalveolar change of upper and lower anterior teeth.
Purpose: The purpose of this study was to make a report on the clinical prognosis of post traumatic lateral and posterolateral instability of the knee after LCL augmentation and popliteal tenodesis. Materials and Methods: The assessment was made among 21 cases who underwent augmentation of lateral collateral ligament(LCL) and popliteal tenodesis with allograft or artificial ligament (synthetic polyester, ABC ligament, Surgicraft, U.K.) at this Medical Center during the period from July 1996 to July 2003 and whose follow-up period was longer than one year. The authors recorded and analysed the physical findings (external rotation recurvatum test & posterolateral drawer test), stress roentgenograms(preoperative and postoperative) and Lysholm score. Results: The lateral and posterolateral instability of the knee were improved in 20 cases postoperatively, acccording to the clinical test and stress roentgenograms . Average Lysholm score was 52.5 preoperatively and 86.7 postoperatively .Conclusion: Our study found the surgery of lateral and posterolateral instability of the knee with augmentation of LCL and popliteal tenodesis using allograft or artiflcial ligament is simple technique. Taking these results into consideration, it seems to be one of effective methods of treatment.
