  • Purpose: The purpose of this study was to analyze the radiological and clinical outcomes in elderly patients with unstable intertrochanteric femur fractures in accordance with the length of intramedullary nail. Materials and Methods: Between August 2009 and December 2014, a total of 139 patients-older than 65 years of age with AO/OTA classification of 31-A2 unstable intertrochanteric femur fracture-who has been followed-up for at least 1 year after the treatment with internal fixation by using an intramedullary nail were enrolled for this retrospective control study. The subjects were classified into two groups according to the length of intramedullary nail: 106 patients in the short group (group I) and 33 patients in the long group (group II). For radiological assessments, the reduction state, time to union, and implant related complications were examined. The clinical outcomes were assessed by preoperative hemoglobin, operating time, intraoperative bleeding amount, blood transfusion rate, hospitalization period, and Charnley hip pain scoring system at the final follow-up. Results: The postoperative radiographs showed good or acceptable reduction in all cases. The mean time of radiologic bone union was 4.8 months, and there was no difference between the two groups. With respect to surgical time, the group II was found to take longer (57.87 minutes) than the group I (45.65 minutes) (p=0.003). The bleeding amount during surgery of the group II was greater (288.78 ml) than that of the group I (209.90 ml) (p=0.046). The clinical results at the final follow-up were found to be satisfactory in both groups. Conclusion: In cases of good reduction of the fracture from the treatment of unstable intertrochanteric femur fracture accompanying the posteromedial fragment in elderly patients, both groups-long and short intramedullary nails-showed satisfactory radiological and clinical outcomes.

  • This paper proposes an automatic segmentation method of cortical bone and trabecular bone and describes an implementation of structural analysis method of trabecular bone in micro-CT images. The proposed segmentation method extract bone region with binarization using a threshold value. Next, it finds adjacent contour lines from outer boundary line into inward direction and sets candidate regions of cortical bone. Next it remove cortical bone region by finding the candidate cortical region of which the average pixel value is maximum. We implemented the method which computes four structural indicators BV/TV, Tb.Th, Tb.Sp, Tb.N by using VTK(Visualization ToolKit) and sphere fitting algorithm. We applied the implemented method to twenty proximal femur of mouses and compared with the manual segmentation method. Experimental result shows that the average error rates between the proposed segmentation method and the manual segmentation method are less than 3% for the four structural indicatiors. This result means that the proposed method can be used instead of the combersome and time consuming manual segmentation method.

  • The purpose of this study was to determined the converge effects of weight-bearing exercise on lumbar, femur neck BMD and body compositions in intellectual disabilities and normal men. Nineteen men were participated for this study, they are divided into two groups(intellectual disabilities, n=9, and normal men, n=10). All subjects are accomplished on weight-bearing exercise (music rope-jumping) program for 60 minutes a day, 3 days a week throughout 12 weeks. Two-way ANOVA with repeated measures was used to determine differences between and within the intellectual and normal groups for dependent variables. The statistically significant level was set at 0.05. In this study, there were a significant differences on the bone mineral density of lumbar and femur neck level after exercise training in both groups (respectively p<.01, p<.001). Also, I found that there were effectively decreased on body weight, percent of body fat, and BMI levels after exercise training for 12 weeks in both groups (respectively p<.001). Therefore, music rope-jump exercise program may suggested to be one of the ideal training methods for enhancing and the convergence positive effect of bone mineral density and body compositions in intellectual and normal person.

  • The effects tea beverages (TBs) prepared from powdered green tea (Gt), oolong tea (Ot), black tea (Bt), or pure tea (Pt) with lemon, orange, grenadine etc on cadmium toxicity in rats were investigated Sensory evaluations of the TBs are better than those of each water extracted teas. Cadmium (50 ppm) was administered to experimental rats fed a basic diet, or a diet with various TBs (15% w/v), for 5 weeks. Although body weight gains, feed intakes, and fecal weights in all Cd-treated groups were lower than those in the normal control group (NC), feed efficiency ratio, urine volumes, liver weights, and kidney weights did not differ significantly between groups. The serum ALT and AST levels in the Cd-treated control group (Cd-Co) were higher than those in the NC animals. Serum ALT and AST levels in all Cd-treated rats fed TB-supplemented diets were lower than in animals of the Cd-Co group. Tibia and femur weights in Cd-Co animals were lower than those in NC rats. Tibia and femur weights in Cd-treated rats fed TB-supplemented diets were higher than those in Cd-Co animals. There were no between-group differences in tibia lengths; animals in the NC and TB-supplemented diet groups showed femur lengths longer than those of Cd-Co rats. Although the contents of crude ash and cadmium in the femurs of Cd-Co mts was markedly higher than in the femurs of NC animals, the cadmium content in femurs of Cd-Co rats was significantly lower than that in the femurs of NC animals. The changes in mineral levels caused by Cd administration were alleviated by every TB-supplemented diet tested Whereas fecal calcium excretion by Cd-Co animals was significantly higher than that of NC rats, calcium excretion by Cd-treated rats fed TB-supplemented diets was significantly lower than that of Cd-Co animals. Fecal cadmium excretion by all Cd-treated rats fed TB-supplemented diets was significantly higher than that of Cd-Co animals. In conclusion, this study provides experimental evidence that various TBs may regulate cadmium-induced organ toxicity by reducing cadmium accumulation in tissues through the mechanism of increasing the fecal excretion of cadmium.

  • This study was performed to assess the effects of physiological factors and lifestyles on bone mineral density (BMD) in 64 postmenopausal women. Sixty four subjects were selected out of 223 postmenopausal women in Seoul and Kyunggido. The BMD of the lumbar spine (L2 ${\rightarrow}$ L4) and femoral neck were measured dual energy X-ray absorptiometry (DEXA). Subjects were assigned to one of three groups such as normal (T-score > -1, n = 20), osteopenia (-2.5 < Tscore ${\leq}$ -1, n = 24), and osteoporosis (T-score ${\leq}$ -2.5, n = 20). Anthropometric measurements and questionares were administered to these women. The mean age, height, weight and BMI were 62.09 yrs, 153.78 cm, 56.09 kg and 23.70 $kg/m^2$ respectively. The BMDs of lumbar spines (L2 ${\rightarrow}$ L4), femoral neck were 0.84 $g/cm^2$, 0.71 $g/cm^2$ respectively. Years after menopause and age of last delivery of the osteoporosis and osteopenia group were significantly longer than the normal group (p < 0.05). The hours of exercise and outdoor activity of the normal group were longer than the osteoporosis and osteopenia group, but there were no significant differences among the three groups. The BMDs of these two sites were positively correlated with weight, BMI, hip and body fat and negatively correlated with LBM, TBW. These results show there are no consistent effects on bone mineral density, adjusting for age and BMI, of physiological factors and lifestyles in postmenopausal women. Therefore. this study confirmed that one of the most effective ways to minimize bone loss in postmenopausal women would be to maintain an adequate body weight.

  • In this study, a new method that automatically segments trabecular bone for its morphological analysis using micro-computed tomography imaging was proposed. In the proposed method, the bone region was extracted using a threshold value, and the outer boundary of the bone was detected. The sphere of maximum size with the corresponding voxel as the center was obtained by applying the sphere-fitting method to each voxel of the bone region. If this sphere includes the outer boundary of the bone, the voxels included in the sphere are classified as cortical bone; otherwise, they are classified as trabecular bone. The proposed method was applied to images of the distal femurs of 15 mice, and comparative experiments, with results manually divided by a person, were performed. Four morphological parameters-BV/TV, Tb.Th, Tb.Sp, and Tb.N-for the segmented trabecular bone were measured. The results were compared by regression analysis and the Bland-Altman method; BV/TV, Tb.Th, Tb.Sp, and Tb.N were all in the credible range. In addition, not only can the sphere-fitting method be simply implemented, but trabecular bone can also be divided precisely by using the three-dimensional information.

  • In spite of various bio-functionalities of chitosan, the effects in vivo were still ambiguous because of its low absorption on organism. Therefore, chitosan oligosaccharides (COSs) are necessary to elucidate for an efficient utilization in vivo. COSs were prepared from chitosan using ultrafiltration membrane enzymatic reactor system with MWCO (molecular weight cut-off) 3,000 Da of membrane. Calcium absorption accelerating effect using COSs was examined by two methods, in vitro and in vivo. In vitro experiment, calcium absorption by the addition of COSs in a mixture solution of calcium and phosphate was higher approximately $50\%$ than that by control. In vivo using rats, group taken the diet contained $1\%$ COSs anil calcium chloride decreased about $75\%$ of calcium content excreted from feces, and then, showed about $15\%$ increase in breaking force of femur. These results demonstrated that COSs definitely involved in calcium metabolism in vivo.

  • Purpose: To evaluate the clinical usability of reconstructive methods, and how to select flap after wide excision of malignant soft tissue tumor in ankle and foot. Materials and Methods: The 15 cases shown in the 14 patients (In case of a male patient, reconstruction was performed two times due to local recurrence.) with malignant soft tissue tumor in ankle and foot, who underwent reconstruction after wide excision from March 2000 until March 2007. Oncologic, surgical and functional results were evaluated. Results: The method of reconstruction used were anterolateral thigh perforator flap (5cases), Reversed superficial sural artery flap (4 cases), dorsalis pedis flap (3 cases), local flap (3cases). The defect, mean size was $5.5{\times}5.7\;cm$, was reconstructed with rotation flap or free flap, mean size was $5.9{\times}6.0\;cm$, skin graft for remnant. The mean operation time was 310 minutes (120~540 minutes); it took 256 minutes to reconstruct by rotation flap, and 420 minutes by free flap. As oncologic results, 7 patients were no evidence of disease, 6 patients were alive with disease and 1 patient was expired by pulmonary metastasis at the time of the last follow-up. 4 patients had local recurrence and 4 pateints had distant metastases. As functional results, 14 patients were evaluated with average score of 68.8% using the system of the Musculoskeletal Tumor Society. Conclusion: The dorsalis pedis and reverse superficial sural artery rotation flap which is easy procedure, has less complication and takes short operation time, can be primarily considered to reconstruct a small defect. And the anterolateral thigh perforator flap is suitable for coverage of a large defects after wide excision of malignant soft tissue tumor in ankle and foot.

  • Purpose: To evaluate the timing of the improvement in surgical skills of the direct anterior approach for hip arthroplasty through an analysis of the clinical features and learning curve in 58 cases. Materials and Methods: From November 2016 to November 2018, 58 patients, who were divided into an early half and late half, and underwent hip arthroplasty by the direct anterior approach, were enrolled in this retrospective study. The operation time and complications (fracture, lateral femoral cutaneous nerve injury, heterotopic ossification, infection, and dislocation) were assessed using a chi-square test, paired t-test, and cumulative sum (CUSUM) test. Results: The mean operation times in total hip arthroplasty (26 cases) and bipolar hemi-arthroplasty were 132.1 minutes and 79.7 minutes, respectively, demonstrating a significant difference between the two groups. CUSUM analysis based on the results revealed breakthrough points of the operation time, decreasing to less than the mean operation time because of the 16th case in total hip arthroplasty and 14th case in bipolar hemiarthroplasty. Complications were encountered in the early phase and late phase: five cases of fractures in the early phase, no case in the late phase; eight and two cases of lateral femoral cutaneous nerve injury, respectively; three and two cases of heterotopic ossification, respectively; and one case of dislocation, one case of infection and three cases of others in the early phase. The CUSUM chart for the fracture rate during operation in the early phase revealed the following: five cases fracture (17.2%) in the early phase and no case in the late phase (0%). This highlights the learning curve and the need for monitoring the inadequacy of operation based on the complications. Conclusion: Hip arthroplasty performed by the direct anterior approach based on an anatomical understanding makes it difficult to observe the surgical field and requires a learning curve of at least 30 cases.
