Decision-Making and Principle of Management in Periprosthetic Femoral Fracture after Total Hip Arthroplasty (고관절 치환술 후 삽입물의 안정성 판단과 대퇴 삽입물 주위 골절의 치료 원칙)

  • Periprosthetic femoral fractures remain as one of the most challenging complications following total hip arthroplasty. A thorough clinical and radiographic evaluation, precise classification, and understanding of modern management principles are essential to obtain optimal results for these fractures. The Vancouver classification system is a simple, effective, and reproducible method for the planning treatments of these injuries. The fractures associated with a stable femoral stem can be treated effectively with osteosynthesis, but periprosthetic femoral fractures associated with a loose stem require revision arthroplasty. This paper describes the principle of the treatment of patients with periprosthetic femoral fractures and how to assess the stability of the femoral stem.

The effect of steroid for fibrous dysplasia of the humerus combined with multiple cystic lesion (낭종성 병변을 동반한 상완골의 섬유성 골이형성증에서 steroid의 효과 - 증례 보고 -)

  • Fibrous dysplasia is a benign pathologic condition in which the normal cancellous bone is replaced by the fibro-osseous tissue. It is found mostly in the femur, tibia, skull, rib, and humerus. Clinically it may develops pain, progressive deformity, and pathologic fracture. Curettage with bone graft has been the most popular treatment method thesedays. A 17-year-old female who had fibrous dysplasia of the humerus combined with multiple cystic lesion was treated by intralesional steroid injection into the lesion total 2 times. The follow-up plain X-ray which was taken 11 months after steroid injection reveals decrease in size and increase in bone density and cortex thickness. The follow-up MRI reveals significant decrease in size and signal intensity. The signal intensity was decreased to that of normal bone marrow in T2 weighted image. She complains no pain and lives symptom free in last follow-up at 2 years and 8 months after steroid injection.

Estimation of Natural frequencies in Osteoporotic Mouse Femur: A finite Element Analysis and a Vibration Test (골다공증에 걸린 쥐 대퇴골의 고유진동수 예측: 유한 요소 해석 및 진동 실험)

  • In this study, a finite element analysis and a vibration test were performed to estimate the natural frequencies of mouse femurs with osteoporosis. Three groups of the femurs include the osteoporotic group, the treated group and the normal group. For the finite element analysis, the micro finite element model of the femur was reconstructed using the Micro-CT images and the Voxel mesh generation algorithm. In the vibration test, the natural frequencies were measured by the mobility test. from the results, the averaged natural frequencies in the osteoporotic group were the highest, followed by those in the treated group. The finite element models were validated within 15% errors by comparing the natural frequencies in the finite element analysis with those in the vibration test. The developed Micro-CT system, the Yokel mesh generation algorithm, the presented finite element analysis, and vibration test could be useful for the investigation of the structural change of the bone tissue, and the diagnosis and the treatment in the osteoporosis.

Histopathological Study of Cartilage in the Bone Bruise of the Lateral Femoral Condyle Associated with Anterior Cruciate Ligament Rupture (전방 십자 인대 파열시 손상된 대퇴골 외과 연골의 병리조직학적 연구)

  • Purpose: to describe the histologic appearance of the type III bone bruise in knees which had sustained an acute anterior cruciate ligament (ACL) rupture. Materials and Method: Twenty-five patients who sustained acute ACL rupture were prospectively enrolled in this study. On MRI, 14 patients demonstrated type III bone bruise on lateral femoral condyle, and 11 patients didn't demonstrated bone bruise. Arthroscopic evaluation and biopsy of the articular cartilage and subchondral bone wert performed before ACL reconstruction. Histologic and immunohistochemical evaluations were done. Results: There was no difference between the bone bruise and control group in the hematoxylin-eosin staining for cell distribution, Masson's trichrome staining for collagen and immunohistochemical staining for type I and type II collagen (p>0.05). But in the safranin-O staining for glycosaminoglycan distribution, the bone bruise group had an evidence of decreased staining at the superficial and middle layers, compared with the control group (p<0.05). We also found fatty change of bone marrow in calcified zone of the bone bruise group with safranin-O staining. Conclusion: We suggest that the type III bone bruise found on MRI indicates a substantial damage to normal articular cartilage homeostasis, and may induce further damage of the articular cartilage.

  • Autologous chondrocyte implantation is a widely used technique for treating cartilage defect or osteochondral lesion, which is the method of transplantation of self chondrocytes after cultivation in the laboratory. We experienced the rare case of heterotophic ossification in the recipient site of the knee after autologous chondrocyte implantation. So we want to report this case with review of the relevant literatures.

  • From 1980 to 1997, we experienced 3 skip metastasis in femur out of 19 osteosarcoma patients. After diagnostic incisional biopsy and chemotherapy, hip disarticulations for all patients were performed. Still the mechanism of skip metastasis is unclear, but there have been two suggestions including medullary extension through an intraosseous embolism of tumor cells, or trans articular extension along the planes of fascia, capsule, ligaments, and venous channels around joints. The mechanisms of skip metastasis in our cases were not clear, but it was considered as an initial preliminary stage of metachronous osteosarcoma. If there are evidences of skip metastasis, it is strongly recommended to disarticulate proximal to the skip lesion rather than to amputate transmedullary.

Anatomic Double-Bundle Posterior Cruciate Ligament Reconstruction Using Achilles Allograft - Preliminary Report - (이중고리 동종 아킬레스건을 이용한 후방십자인대 재건 - 예비 보고 -)

  • Purpose : Preliminary report of the technique and trial of double bundle PCL reconstruction using Achilles allograft. Materials and Methods : From May 1999 to July 2000, 8 cases of PCL insufficient patients were treated with Achilles allograft reconstruction using the double bundle and double femoral tunnel technique. The tibial tunnel was prepared anteromedially. All other combined injuries within the knees were treated accordingly. Minimal follow-up period was 1 year. The results was assessed from the point of function and stability using Lysholm knee score and KT-2000 arthrometer. Results : Up to present follow up. 8 patients showed good sign of recovery with no instability (translation less than 2 mm) except olio that has been grafted-ligament rupture. In addition, none showed any sign of infection nor ROM limitation. Two complications were seen, which one had grafted-bone fracture and the other grafted-ligament rupture. The former occurred during operation and the latter occurred due to improper protection. Conclusion : Presently the follow up period is too short to draw any conclusive opinion but it is essential to select healthy and well sterilized allografts fur successful outcome. Double femoral tunnel technique seems to be more physiologic in PCL reconstruction. With these prerequisites, it seems to be a good alternative to use Achilles allografts fur the reconstruction of PCL. However, a longer follow-up is needed.

Arthroscopically Assited Reconstruction of the Anterior Cruciate Ligament using the Flexible Reamer (유연성 연마기를 이용한 관절경적 전방 십자 인대 재건술)

  • Purpose : The purpose of this study is to describe the surgical technique of ACL reconstruction with the flexible reamer and evaluate the clinical results of this method. Materials and Methods : Twenty four patients who followed for 12 months were included. The results were evaluated by Lachman test, KT-2000 arthrometer, Lysholm score, and postoperative roentgenogram of the knee. Results : The range of motion of the affected knee was acceptable except one case(10 to 90 degree) and at the last follow up, two cases were positive in Lachman test. Mean Lysholm knee scoring scale was 63.7 point preoperatively and 91.4 point at the last follow up. The results of KT-2000 arthrometer was 6.6mm preoperatively and 1.5mm at the last follow up. In operative time, there were decrease of mean 13.5 minute than ACL reconstruction without the flexible reamer, Conclusion : ACL reconstruction with the flexible reamer achieves the ideal isometric point of tibia and femur with no difficulty, proper notchplasty preventing from the impingement, and lessens the operative time.

  • We report a case of a girl with a symptomatic anomaly of medial meniscus. The complaint of the patient was pain and snapping of the knee. The anterior horn of medial meniscus has anomalous insertion which was extended to the intercondylar notch of the femur on the surface of the anterior cruciate ligament, it attached to lateral wall of medial femoral condyle. This anomalous band was not detected in MRI of knee but found during the operation. After resection of anomalous band, the symptoms completely disappeared.

The Effect of the Position Change of the Magnification Marker and the Stitching Range on the Image in the Examination of the Femur Radiography Using Stitching Technique (스티칭 기법을 이용한 대퇴골 방사선검사 시 확대 표식자 위치 및 스티칭 범위 변화가 영상에 미치는 영향)

  • In this study, we would like to examine the effect of the location of the magnification marker and the change in the stitching range on the magnification marker and the expansion of the anatomical structure in the examination of femoral stitching radiation using the stitching technique, and to find out the usability of the femoral stitching radiation examination in the preoperative examination for THRA. The measured values measured by 5 researchers were summed and presented through descriptive statistics. When the position of the magnification marker was changed, the average value of the measured value was in the range of 113.54 to 113.90 mm. The anatomical structures of the femur were measured in the range of 57.24 to 57.78 mm, 27.20 to 27.56 mm, and 70.11 to 70.81 mm, respectively. This showed a similar deviation from the expansion rate of previous related papers using markers. Therefore, it is considered that femoral stitching X-ray examination using magnification markers can be used for preoperative examination for THRA.
