Survey of Perception of Complementary and Alternative Therapies (CAT) (암환자를 돌보는 일부 종합병원 간호사의 보완대체요법에 대한 인식도)

  • Purpose: This study was done to investigate oncology nurses' perception, attitude, knowledge and practice of CAT and to describe nurses' needs for education on CAT Method: A cross-sectional descriptive study was conducted. A total of 270 oncology nurses working at four medical centers in Seoul and Kyungki participated in this study. They completed the questionnaire between December 2005 and February 2006. Results: Oncology nurses perceived CAT as possible nursing interventions, but felt that there was no conclusive evidence on effectiveness/safety. Of the participants 44.4% have used CAT for themselves, and 73.9% of them were satisfied after using CAT. Two biggest reasons for CAT use were symptom control and emotional support. However, 80.8% of the participants had not had any training related to CAT Both nurses who have used CAT and those who have been trained to practice CAT represented a more positive perception toward CAT (p=.000). The biggest perceived obstacles to the practice of CAT were concerns about side effects (71.2%) and lack of knowledge and skill (69.6%). The most favorable CAT modalities in both practice and training were mind-body medicine. Conclusion: Overall oncology nurses' perception toward CAT was encouraging. However a high number of the nurses reported lack of knowledge and skill in CAT Education and training strategy for CAT need to be developed.

The Attitude, Use and Adverse Effects of Complementary and Alternative Medicine (CAM) Therapies among Vulnerable, Community Dwelling Old Adults (재가 취약계층 노인의 보완대체요법에 대한 태도, 이용 및 유해반응)

  • Purpose: The purpose of this study is to obtain information of use of complementary and alternative medicines (CAMs) and any reported adverse effects of them on vulnerable elders in a community. Methods: 1,837 elderly subjects aged 65 or higher with health problems were selected from those who were enrolled in a public health care center and received visiting health management services. A proportional stratified sampling method was used. Data collection was completed by face to face interviews using a structured questionnaire. The descriptive statistics and ${\chi}^2$-test were used to analyze the data. Results: The prevalence of CAM use last year was 57.5%, and 10.4% of the subjects reported adverse effects of the therapies. The mean score of the subjects' attitudes towards CAM was $4.1{\pm}3.44$ (range 1~10). The major sources of advice and exposure to CAM therapies were most likely to be from family and friends (72.5%). The primary reason for choosing CAM was to manage symptoms and relieve pain (86.6%). Conclusion: These findings suggest the need to develop nursing strategies for reducing and preventing adverse effects of CAM use by promoting awareness of using safe and appropriate CAM therapies.

  • 본 연구는 MRI 검사를 받은 환자를 대상으로 영상매체(Visual equipment & Audio system)를 이용하여 영상물을 제공함으로서 불안과 불편감에 대한 효과에 대해 규명함으로써 MRI 검사로 인한 심리적, 정신적 불안과 불편 감을 감소시킬 수 있는 대체요법을 제공하기 위하여 본 연구를 시도하였다. 연구대상은 대전광역시에 위치한 K 대학병원에서 MRI 검사를 받은 환자 중 영상물을 제공받은 실험군 30명과 영상물을 제공받지 않은 대조군 30명을 대상하였다. 연구의 도구는 Spielberger의 기질불안 도구와 Cline, Herman, Shaw와 Morton이 고안한 불안점수 도구인 시각적 상사척도(VAS)를 이용하였고, 영상물을 제공하여 두 군 간의 활력징후를 측정하였고 불편감은 대상자의 주관적 불편감(어지러움, 공포감. 긴장감)점수와 객관적 불편감 행동 점수를 사용하였다. 자료 분석 방법은 SPSS12K for Windows program을 이용하였으며, 두 군의 일반적 특성 및 수술에 관련된 특성의 동질성 검증은 $\chi^2$ 검정, 가설검증은 t-test로 분석하였다. 연구결과를 요약해 보면 영상매체(Visual equipment & Audio system)를 이용하여 영상물(영화, 애니메이션)을 제공하면서 MRI 검사를 진행했을 경우 검사 대상자의 혈압 및 객관적 불편감에서는 유의한 영향을 미치지 못했으나 불안과 활력징후의 하나인 맥박의 감소, 또한 주관적 불편감이 감소됨을 알 수 있었다. 따라서 환자의 심리적인 긴장감을 완화시키며 안정감을 느끼게 하는데 효과적인 대체요법이 될 수 있을 것으로 사료 된다.

Research Trend of Complementary and Alternative Medicine (보완대체요법의 국내외 연구 동향 비교)

  • Purpose: This study was done to describe the research trend of Complementary and Alternative Medicine(CAM) in Korea and internationally. Method: A Systematic review of CAM related literature published from 2000 to 2005 was done. A comprehensive search by using 34 CAM modalities as search terms to identify all articles dealing with CAM was undertaken from 4 databases: PubMed, RICHIS, KoreaMed, and Riss4U. A Total of 588 abstracts and 223 original articles were reviewed by two independent reviewers using systematic analysis guidelines. Data regarding the types of study design, characteristics of subjects and researchers, and classification of CAM modalities was recorded. The outcome measures of CAM were identified. Results: CAM research has been rapidly increasing worldwide in the last decade. In Korea, nursing(53.5%) followed by medicine(42.7%) occupied the majority of CAM research in comparison to medicine(42.9%) and disciplines in CAM(25.0%), like homeopathy, internationally. Prevailing research designs in Korea were experimental and quasi-experimental, however, randomized clinical trials were found. Internationally, survey research was dominant, with the exception of randomized clinical trials that were mostly an experimental design. Conclusion: The findings suggest that a robust research design in CAM research is needed to accumulate strong scientific evidence and to adopt nursing interventions from CAM modalities.

Utilization of Alternative Complementary Remedies of Stroke Patients (뇌졸중 환자의 보완대체요법 이용실태)

  • Purpose: This descriptive survey was conducted to investigate out the utilization of alternative complementary remedies for stroke patients. Method: The subject for this study were 194 patients, selected from inpatients department of 11 major medical center at Busan and Ulsan. Data were collected from August 19th to November 12th, 2002 through interview schedule designed by the investigator. Data was analyzed with frequency, percentage, and ${\chi}^2$-test by using SPSS Win 10.0. Results: 71.6% of objects has used alternative complementary remedies. The use of alternative complementary remedies according to disease-related features was statistically significant in concurrent diseases(${\chi}^2=15.03$, p=0.001), rehabilitation treatment(${\chi}^2=6.341$, p=0.012) and the level of ADL(${\chi}^2=19.63$, p=0.000). The most frequently used therapies were diet and nutrition(31.8%). The patients with less than 3month onset and in the mid-period of treatment frequently used remedies. The reason for using remedies were the belief in it's effects on treatment, but most patients responded was not effective(30.9%) or not much improvement after using remedies. Most of the patients were recommended to use remedies by their neighbors(27.3%) and family members(25.2%). The family members were most supportive in therapy(56.1%). The 28.1% of patients responded that the remedies were effective in promoting blood circulation and 95% of the patients had not experienced side effects. Conclusion: These results could be the basic material in developing nursing intervention for cerebral apoplexy patients.

The Perception and Utilization patterns of Complementary and Alternative Medicines in Patients with Fibromyalgia Syndrome (섬유근통 증후군 환자의 보완대체요법에 대한 인식과 이용행태)

  • Purpose: The purpose of this study was to investigate the perception and utilization patterns of complementary and alternative medicines (CAM) in patients with fibromyalgia syndrome. Methods: A total of 92 fibromyalgia syndrome patients participated in this study. Data were collected using a structured questionnaire and were analyzed with SPSS/WIN 23.0 program. Results: As a result, 59.8% of subjects had an experience of CAM and 33.7% of subjects have used until now. There were significant differences of the use of CAM between the group of CAM users and non-CAM users by education ($x^2$=4.04, p=.044), period of illness ($x^2$=15.03, p=.001), and period of treatment ($x^2$=12.10, p=.002). Relatively large numbers of patients understanded as that CAM was effective (58.7%) and fibromyalgia syndrome was controllable (75.0%). Moreover, there were significant differences of the use of CAM by the perception of CAM effects ($x^2$=4.15, p=.042), fibromyalgia syndrome ($x^2$=6.55, p=.038), and best treatment for fibromyalgia ($x^2$=11.03, p=.001). Conclusion: These results could be utilized as a basic data for developing nursing intervention for fibromyalgia syndrome.

A Study on the Use of Complementary and Alternative Therapies in Korean Adults (한국 성인의 보완대체요법 이용에 관한 조사)

  • Purpose: This descriptive study was conducted to identify the use of complementary and alternative therapies, and the perception and attitudes toward CAT in Korean adults. Methods: The 1,010 study subjects were recruited from ten regions in South Korea. Data were collected from July to September, 2004. Results: The 43.2% of the sample(n=436) had an experience of using CAT. The subjects using CAT was more likely to have high score in the CAT perception compared to the subjects with no experience of CAT(p=.001). The primary source of information about CAT was mass media(59.7%). The 52.4% of the sample answered that CAT was supplementary and the 27.7% answered CAT was applicable to chronic pain or musculoskeletal disorders. Subjects(34.8%) pointed out that the greatest problem was no scientific informant in the use of CAT. The primary reason of using CAT was to maintain their health or to prevent disease(44.0%), and the 59.0% of the subjects responded that CAT was effective. Majority of subjects were satisfied with CAT(63.3%) and experienced no side effects of CAT(90.6%). Conclusion: This study suggests that professional counseling and education for CAT is needed for Korean public as well as the research approach to test the effects of CAT.

A Study on Nursing Students' Attitudes toward Complementary and Alternative Medicine (CAM) (간호학생이 지각하는 보완대체요법에 대한 태도 조사연구)

  • The purpose of this study was to help in explaining the necessity of education on complementary and alternative medicine(CAM) and opening relevant curriculums in nursing education by grasping the kinds of CAM in which nursing students want to be opened. Data were collected from September 27, 2003 through October 20, 2003. A group of 865 nursing students were surveyed using questionnaires in seven universities whose curriculums on CAM have not been available. Collected data were statistically analyzed by using SAS 8.02 system, and the study results are as follows: 1. The average of subjects' behavior toward CAM was 3.52 of the total 5 with practical affairs 3.79. medical treatment 3.79, social concerns 3.84, and communication 3.78. 2. Most nursing students $(90.6\%)$ did not precisely understand the meaning of CAM although they had heard of it. Most of them $(85.5\%)$ had no experience of applying CAM. The study also revealed that more qualified CAM lecturers in the future are needed: specialist in the relevant field $(83.2\%)$, doctor and herb doctor $(6.5\%)$, clinical nurse $(6.0\%)$ and professor of the department of nursing science $(4.3\%)$. 3. Concerning students' behavior toward CAM, there were statistically significant difference between students from agrarian and fishing villages (F=4.91. p=.007) and the others. 4. CAM courses that students want were therapeutic touch (massage) $(22.9\%)$. aroma therapy $(14.8\%)$, manual healing methods $(14.7\%)$, treatment of blood vessel $(8.3\%)$, foot reflexology $(7.9\%)$, and heart remedy $(6.8\%)$. In conclusion, the study indicated that nursing students had positive attitudes toward CAM and for this. healers should have a frank talk with patients in order to maximize symptom-relieving effects. However, the study showed that most nursing students did not understand the exact meaning of CAM. Considered the fact that improvement of both health status and quality of life will be an important issue in the future, active introduction and education of CAM in nursing curriculum are a matter of necessity.

The Analysis of Trends in Complementary and Alternative Therapy (CAT) in Nursing Research in Korea (보완. 대체요법관련 국내 간호연구의 동향분석)

  • Purpose: The purpose of this study was to analyze trends in complementary and alternative therapy in nursing research in Korea. Method: The researchers examined academic theses and dissertations published from 1990 to 2002, and 151 articles were used for the analysis. Result: Nursing research on CAT increased rapidly from 1995 Articles with quantitative research designs made up 93.9% of the total and there were more experimental studies than non-experimental studies. Patients who had surgery, mental disorders, renal failure, hypertension, arthritis, dementia & cancer were the most frequently participants in studies on CAT. The type of CAT used in nursing research were mind-body therapy (65.8%), manual healing therapy (28.7%), phamacologic & biological therapy (3.7%), bioelectromagnetics (0.9%) and herbal therapy (0.9%). In 44 articles both psychological and physiological parameters were used as dependent variables. In 34 articles only physiological parameters were used and in 13 only psychological parameters. The most frequently used physiological parameters in CAT were pain, physiological function and vital signs, while the most used psychological parameters were anxiety, depression and stress. Conclusion: More studies about CAT are needed to extend the role and fields for professional nursing. There is a need to conduct qualitative studies in nursing about the experiences of patients who receive CAT and nurses who use CAT.

Nursing Activities on Continuous Renal Replacement Therapy in the Intensive Care Units (중환자실 내 지속적 신대체 요법과 관련된 간호업무 분석)

  • Purpose: The purpose of the study was to analyze the nursing activities related to continuous renal replacement therapy (CRRT) in the intensive care units (ICU). Methods: We developed a list of 31 CRRT related nursing activities based on literature review and reviewed by expert group. A total of 109 direct time measurement records by 43 ICU nurses were collected and analyzed in terms of total time per shift, frequency, standard time, difficulty and performance levels of each CRRT nursing activity. Results: The mean time for CRRT nursing activity was 85.60 minutes per 8 hour shift. Nurses have spent average $9.46{\pm}6.98$ minutes in a shift for "waste fluid bag change" activity. In addition, "check catheter location" was the most time consuming single activity. The most difficult activity was "counseling-answer" and the most competent one was "dialysis solutions change". Conclusion: The CRRT nursing activities accounted for a significant portion of total nursing workload. Practical allocation of nursing staff for CRRT patient along with development of a new nursing cost system need to be considered. Continuous nursing educational and training programs on CRRT should be developed.
