Role of Interventional Managements for Malignant Colorectal Obstruction (소화관 폐색을 동반한 대장암 환자에서 중재적 치료의 역할)

  • Incidence of colorectal cancer has been increasing in Korea probably due to the westernized life style. Although the technical development of colonoscopy and introduction of screening examination has led to the detection of early colorectal cancer, considerable patients still have clinical symptoms of colorectal obstruction. Most of these patients are old and they have advances stage of cancer or severe co-morbidities. In addition, the emergency operation under poor preparation state of colon can lead to serious mortality or complications. Since the introduction of colorectal stent, there have been a large number of studies for recent 10 years. It seems that the role of colorectal stent in the palliative indication or bridge to surgery has been recognized. A well designed, randomized prospective study with long term data is necessary to support the role of colorectal stent in the malignant colorectal obstruction.

Radiologic Analysis of Congenital Origin Intestinal Obstruction in Neonate and Childhood (선천성 장관폐쇄의 방사선학적 분석)

  • Congenital origin intestinal obstruction are important disease due to required emergency operation. So accurate and rapid diagnosis is needed for decreased mortality and morbidity. Radiologist must detect to accurate obstruction site and also associated other congenital anomalies. And also embriological bases are very important role to the diagnosis of theses diseases. We were analysed radiologically and clinically 25 cases with congenital origin intestinal obstruction with review of literature. 1. Hypertrophic pyloric stenosis 6 cases, midgut malrotation 4 cases, congenital megacolon 8 case, imperforated anus 5 cases, ileal atresia 1 case and duodenal atresia 1 case. 2. Male and female radio were 16:9. Especially on hypertrophic pyloric stenosis, 5 cases were male infants. 3. All cases of hypertrophic pyloric stenosis represented string sign and also pyloric beak sign shoulder sign on UGI. 4. I case duodenal atresia showed double bubble sign on simple abdomen x-ray and ileal atresia showed mechanical small bowel obstruction sign with microcolon. 5. On midgut malrotation, cecum was located in right upper abdomen on 4 cases. And 2 cases were associated with Ladd's band. I case with volvulus and 1 case with mesenteric defect. 6. Involved site of all congenital megacolon were localized to rectosigmoid colon. 7. On 5 cases imperforated anus, 3 cases were low type and 2 case high type. Rectoperineal and rectourogenital fistula were demonstrated on 4 cases.

Radiographic Diagnosis of Atresia Coli in a Korean Native Calf (한우 송아지에서 결장폐쇄의 방사선 진단례)

  • A 5-day-old male Korean native calf was referred with the primary complaint of anorexia, absence of feces, depression, and abdominal distention. Although this case was tentatively diagnosed as atresia coli, plain and contrast radiography was performed to confirm site and appearance of obstruction. A plain radiograph revealed gas distension in the large intestine, with absence of fecal material in the descending colon. In a contrast radiograph, the blind part of descending colon that was accumulated with contrast media smoothly protruded to distended colon with gas and the middle part was seen as narrow. These findings were well corresponded with the mucosal blockage junction between distended spiral loop with gas and descending colon that was hypoplasia or atropy in necropsy. In conclusion, contrast radiography was considered very useful method to identify the site and type of obstruction in diagnosis of atresia coli.

  • Intussusception and Meckel's diverticulum are very rare disorders in intrauterine or neonatal periods, which are causes of intestinal obstruction. We experienced a case of intussusception due to Meckel's diverticulum which caused intestinal obstruction in the neonate who had bilious vomiting a few hours after birth. We report this case with a brief review of the literature.

  • 비만과 관련된 질환은 당뇨병, 고혈압, 고지혈증, 죽상경화증(동맥경화증), 심근경색증, 지방간, 담석증, 골 관절염, 통풍, 폐쇄성 수면 무호흡증, 대장암, 직장암, 전립선암, 유방암, 자궁내막암 등의 각종 암, 불안, 우울, 적응장애, 히스테리 등의 심리적 질환 등이 있다. 비만이 있을 때 우리 몸에서는 인슐린 자체에 대한 저항성이 생겨서 말초조직에서의 포도당 이용이 감소된다. 연구에 의하면 표준체중의 45$\%$ 이상을 초과한 비만환자는 당뇨병 발생의 위험도가 30배 증가된다고 알려져 있다. 비만이 있을 때 흔히 동반되는 질환들 중 특히 고혈압, 고지혈증, 죽상경화증 등의 질환은 당뇨가 있을 때 흔히 동반될 수 있는 질환들이고, 인슐린 저항성과 깊은 관계가 있는 질환들로 당뇨인들에게 비만은 매우 중요한 질환이라고 할 수 있겠다.

A Mouse Colon Model to Investigate Stent-Induced Tissue Hyperplasia (쥐 대장에서 스텐트 삽입 후 발생하는 조직 과증식 모델 형성을 위한 연구)

  • The purpose of this study was to investigate the feasibility of stent placement and the evaluation of tissue hyperplasia caused by bare metallic stent placement in a mouse colon model. In a pilot study, C57BL/6 mouse were used to verify diameter of colon. Mean diameter size was 4.05 mm. Twenty C57BL/6 mice were divided into two groups to assess differing stent diameters (Group A, 5 mm diameter; Group B. Group B, 4 mm diameter). Follow-up, 1-week fluoroscopic imaging, 4-week endoscopic imaging were obtained. Mice were sacrificed 4-week after stent placement. Microscopic findings were evaluated. Stent placement was technically successful except one mouse in Group A. Data from the mouse was omitted. During follow-up, five mice in Group A died within 7 days after stent placement, and one stent in Group B was migrated into the rectum. The incidence of stent-related complication was 60% and 10% between group A and Group B, respectively. Gross and Endoscopic findings showed tissue hyperplasia through the mesh, and all the stents had become incorporated into the wall of the colon. Microscopic findings were no statistically significant difference. colonic stent placement was technically feasible, and stent-induced tissue hyperplasia was evident in a mouse model. With stent large size, there was the highest incidence of colon perforation.

Immunocytochemical Study on the Enteroendocrine Cells in the Gastrointestinal Tracts of the Korean Snakes (한국산 뱀목 위장관 점막이 장내분비세포에 대한 면역세포화학적 연구)

  • This study attempts to investigate several enteroendocrine cells in the gastrointestinal epithelia of the Korean snakes (Dinodon rufozonatum rufozonotum Rhabdophis tigrina tigrina, Enhydris rufodorsata, Agkistrodon blomhoffii brevicaudus, Agkistrodon saxatilis, Agkistodon calginosus). For a light-microscopical examination of immunocytochemistry, the paraffin sections (5 $\mu$ m) of tissue specimens taken from the various parts of the gastrointestinal tract were stained immunocytochemically by PAP procedure with 10 antisera. The frequency of enteroendocrine cells per unit area (mm$^2$) of each mucosa were counted and the shapes of the cells were observed. In Dinodon rufozonatum rufozonatum, Rhobdophis tigrina tigrina, Enhydris rufodorsata, Agkistrodon saxatilis and Agkistrodon caliginosus, cholecystokinin (CCK)-8, gastrin, pancreatic polypeptide (PP) and serotonin cells were observed. But the freuqency of these immunoreactive cells differ trom each portion of gastrointestinal tracts of all species, respectively. In Agkistrodon blomhoffii brevicaudus, CCK-8, gastrin and serotonin cells were observed. CCK-8 and serotonin cells were found in whole gastrointestinal tracts and gastrin cells were observed in pylorus and mucosa of small intestine. The frequency of these cells was different from each portion. The shapes of CCK-8, gastrin, PP and serotonin cells were pyramidal or oval and closed type in stomach. A large number of these cells were spindle in shape and open type in small intestine and anterior pant of large intestine, whereas some cells were closed type. In posterior part of large intestine and rectum, these cells were oval in shape and closed type.

A Clinical Study of Congenital Duodenal Obstruction (선천성 십이지장 폐쇄증의 임상적 고찰)

  • Congenital duodenal obstruction in the newborn infant may be due to a variety of causes. Duodenal obstruction often presents with bilious vomiting and upper abdominal distention. Diagnosis is usually established on plain x-ray of the abdomen by the classic finding of the double-bubble. In the period July 1986 to June 1990, 16 patients with congenital duodenal obstruction were operated and the following results were obtained. 1. Sixteen patients were comprised of 11 males and 5 females, the ratio of male and female was 2.2:1. 2. Thirteen patients(81%) had been admitted to our hospital during one month of life. 3. Congenital duodenal obstruction was in 16 cases; malrotation in eight(50%), annular pancreas in six(38%), type 1 atresia in one(6%), and wind-sock anomaly in one(6%). 4. There were two premature patients and six patients of small for gestational age. 5. Overall, bilious vomiting, occurring in three fourths, was the single most frequent presenting complaint. 6. Polyhydramnios occurred in two of the patients. 7. Diagnosis was possible with clinical symptom and simple abdomen. 8. The operative procedures performed were ; duodenoduodenostomy in five, duodenojejunostomy in two, excision of wind-sock membrane in one, and Ladd's procedure in eight. 9. A total of ten asociated congenital anomalies were found in six patients. 10. Postoperative complications occurred in five cases(31%).

End to End Anastomosis of Type A(long gap) Esophageal Atresia in 1,200 gram Premature Baby A Case Report (1,200gram 미숙아에서 Type A 식도 폐쇄증의 식도 단단문합술 -1례 보고-)

  • The management of neonate with long gap atresia without a fistula(type A) is complex and controversial. Various esophageal reconstruction include use of native esophagus or replacement with colon, stomach and small bowel. A severe premature male, at 28 weeks gestation weighing 1.2kg, was born with type A esophageal atresia in Chonnam University Hospital. Initial treatment consisted of gastrostomy under the local anesthesia and suctioning of proximal pouch, and than underwent delayed esophageal end to end anastomosis. A minimal leakage and mediastinitis ocurred postoperatively, but was treated by adequate drainage and negative suction from the leakage site through the gastrostomy. The patient was discharged in good general condition and normal weight of 5.4kg after 4 months after the surgery.
