고지혈증의 원인과 치료 - 당뇨병환자의 지실대사 이상

  • 비당뇨인에게서도 이상지질형증이 발견될 경우 적합한 치료가 시행되어야 하겠지만, 당뇨병 환자의 경우에는 비당뇨인에 비해 심혈관계 질환을 일으킬 가능성이 2~4배에 이르고, 심혈관계 질환이 발생한 후에도 사망률이 비당뇨인에 비해 높음을 감안하면, 당뇨병 환자에서 심혈관계 질환의 강력한 위험인자인 이상지질혈증의 적절한 치료는 매우 중요하다고 하겠다. 당뇨병 환자의 이상지질혈증의 적절한 치료를 위해서는 지질대사 이상에 대한 지식이 필요하다. 본 란에서는 정상 지질대사와 당뇨병환자에서 이상지질혈증의 발생기전과 특징은 어떠하며 당뇨병 환자에서 잘 나타나는 이상지질혈증의 특성이 심혈관계에 미치는 영향이 비당뇨인에 비해서 왜 더 크며, 더 적극적인 치료가 필요한가에 대한 정보를 제공하고자 한다.

Effect of Education of Diabetes Mellitus Patients on Health Behaviors Practice (당뇨병환자 교육이 건강행위실천에 영향을 미치는 요인)

  • 본 연구는 당뇨병환자의 교육이 건강행위실천에 영향을 미치는 요인을 분석하기 위해 시도하였다. 연구대상자는 서울에 소재한 한 종합병원 내과 외래를 방문한 당뇨병 환자를 대상으로 실험군 51명, 대조군 51명인 총 102명이다. 교육 2회 실시 후 2010년 1월 5일부터 2월 5일까지 설문조사하였다. 연구결과 첫째, 당뇨병지식은 실험군에서 교육전 평균은 7.46점에서 교육후 10.12점으로 효과적으로 나타났다(P<0.01). 둘째, 당뇨병관리는 실험군에서 교육전 평균점수는 23.41점에서 교육후 26.17점으로 유의하게 높았다(P<0.01). 셋째, 상관관계 결과 당뇨병지식과 건강실천의지(r=0.26, P=0.03), 건강실천의지와 당뇨병관리(r=0.67, P=0.00)는 유의한 양의 상관관계가 있는 것으로 나타났다. 따라서 당뇨병 예방을 포함한 다양한 당뇨병 지식 내용의 프로그램을 개발하여 집중적인 교육이 필요하다.

Factors Influencing Diabetes Educational Needs in Patients with Diabetes Mellitus (당뇨병환자의 교육요구도 영향요인)

  • This study was carried out to identify the perceived educational needs of diabetes mellitus type 2(DM 2) patients and the factors influencing their diabetes educational needs. The descriptive and correlation study design was used. The data was collected from January 2, 2012 to February 29 10, 2012 and a total of 346 diabetes patients responded in a structured instrument regarding their characteristics, diabetes knowledge and needs for diabetes education. The data was analyzed using ANOVA, t-test, Scheffe test, Pearson correlation, and stepwise multiple regression. The mean score of the diabetes educational needs was 3.03 (range: 1.09-4.00). Of the factors significantly influencing the diabetes educational needs, age (${\beta}$=-.25, p<.001), diabetes education participation of the family (${\beta}$=.21, p=.003), and self blood sugar test (${\beta}$=-.13, p=.045) explained the 11% variance of the diabetes educational needs. These three factors need to be considered when implementing diabetes education programs for diabetes management.

Relationship between Knowledge on Diabetes, Self-care Activities, and Self Effectiveness of Diabetics Patients (당뇨병 환자의 당뇨지식, 자가간호활동과 자기효능감의 관계 - 일 종합병원 환자를 대상으로 -)

  • This study provides some basic information on the improvement of diabetics' self-care activities through examining the patients' knowledge on diabetes, self-care activities, and how much they feel those activities are effective and through studying the correlation among the three factors. One hundred and fifty four patients were subjected to this study, who were treated or hospitalized in a hospital in Pusan. Researches were conducted from October 21 to November 9, 2002, and information were gathered by questionnaire and direct conversation with the patients. The data was processed through SPSS WIN 10.0 program. The Date were analyzed by descriptive statistics, ANOVA, t-test, Pearson correlation coefficient and Spearman rank correlation coefficient. The results of this study was the followings; 1) The subjects' knowledge on diabetes ranged from 6 to 23 points with a mean of 17.01 points. 2) The subjects' self-care activities ranged from 21 to 103 points with a mean of 67.72 points. That of the patients with regular treatments appeared to have the highest points (3.51) and the lowest points appeared in the measurement of sugar level in blood (2.60) 3) The subjects' feelings of effectiveness ranged from 21 to 103 points with a mean of 70.01 points. Drug therapy had the highest point (3.94) and diet showed the lowest (3.37) 4) In the general characteristics of the subjects, the more education a subject had, the more knowledge on diabetes he or she appeared to have (F=3.36, p=0.012). The self-care activities were significantly high among non-alcoholic subjects (F=4.301. p=.016). The subjects' feelings of effectiveness were higher in female patients than in male patients(t=2.132 , p=.035), and those in the ages between 51 and 60 felt more effectiveness (F=4.198. p=.003). In addition, married subjects felt more effectiveness than the unmarried (t=2.856, p=0.005), and the more education a patient had. the more effectivess he or she felt (F=2.879, p=.025). 5) In the subjects' pathological characteristic, the subjects who experienced diabetes education showed more knowledge on diabetes, more self-care activity, and more effectiveness than those without the education (t=3.193, p=.002; t=2.299, p=.023; t=2.128, p=.035). 6) As the results of the analyses of correlations among knowledge on diabetes, self-care activity, and effectiveness, the more knowledge on diabetes a patient had, the more self-care activity and the more effectiveness he or she showed (p=0.283. p=0.002; p=0.182, p=0.036). Self-care activity became higher as the feeling of effectiveness appeared higher (r=.550, p=.000). Based on the results above, several suggestions were made; 1) This study was conducted in one hospital, so it is difficult to generalize the results. It is necessary to gather more information from more patients through repeated researches. 2) The study on the other factors which can affect a patient's self-care activity is required. 3) The development of a program which improves a patient's self-care activity, knowledge on diabetes. and effectiveness is needed.

Effects of Nutrition Education on Improvement of Blood Glucose in Type 2 Diabetic Patients with Low Income (저소득층 당뇨병 환자의 영양교육을 통한 혈당개선 효과)

  • This study was carried out to investigate the effects of nutrition education on blood glucose in type 2 diabetic patients with low income. There were 50 (27 male and 23 female) study subjects with type 2 diabetes under national medical support, and the monthly income in most of them was less than 500,000 won. Anthropometric measurement, biochemical blood indices, food intake, dietary habits, and nutrition knowledge of the subjects were investigated before nutrition education. Nutrition education was conducted 6 times within 12 weeks. After nutrition education, the glycosylated hemoglobin was significantly decreased (male: $8.4{\pm}1.6%$ to $8.0{\pm}1.7%$, female: $8.3{\pm}1.6%$ to $8.0{\pm}1.3%$) and the total cholesterol was also significantly decreased in both groups. The group of female resulted in a significant decrease in fasting blood glucose ($169.7{\pm}28.2$ mg/dL to $152.8{\pm}22.0$ mg/dL) and triglyceride ($177.8{\pm}56.3$ mg/dL to $162.3{\pm}36.1$ mg/dL), but the group of male did not show a significant decrease in fasting blood glucose and triglyceride. Nutritional knowledge and dietary habit scores of the subjects were significantly improved after nutrition education. The results demonstrate that nutrition education for type 2 diabetic patients with low income are effective in improving their blood glucose.

Effect of Nutrition Education on Diabetic Management in Diabetic Patients (영양교육이 당뇨병환자의 식사요법에 대한 지식과 실천 및 인지도, 혈당관리에 미치는 영향)

  • The purpose of this study was to investigate the effect of nutritional education on the knowledge and the practice & Perception of diet therapy, and on the level of FBG and HBAlc. The subjects were selected among the patients treated in the Inha University Hospital from June of 1996 until April of 1998. Educated group consisted of 20 patients who visited the diet-counseling department as a part of diabetic treatment. Non-educated group consisted of 20 patients who visited the diet-counseling department ignoring doctors prescription purpose. The knowledge and the practice & perception of diet therapy drinking & smoking habits, regular exercise, and general characteristics including family history were analyzed by questionnaires. FBG and HbAlc were measured before and 4 months after the treatment. The results were as follows : Both groups were composed of 12(60%) male and 8(40%) female respectively. The mean age of subjects was 51.4 years in educated group and 51.9 tears in non-educated group. The duration and %IBW were 5.9 years and 109.8 % in educated group and 5.5 years and 111% non-educated group respectively. There were no significant differences in drinking & smoking habits and regular exercise between groups. The diet therapy knowledge score was significantly higher in educated group(p<0.001, 5.8$\pm$2.3/10.0 vs 3.4$\pm$2.0/10.0). The diet therapy practice & perception score was also significantly higher in educated group(p<0.001, 5.6$\pm$2.3/10.0 vs 2.8$\pm$2.2/10.0). Levels of FBG and HbAlc in both group were reduced at 4 month after the treatment(-74.2 mg/dl and -1.4% in educated group vs -58.7 mg/dl and -2.0% in non-educated group). However, the reduced amounts of them were not significantly different. Therefore, nutritional education seems to be very important in improving the knowledge and the practice & perception of the diet therapy. However, it appears no direct influence in reducing FBG and HbAlc levels. Further studies on diabetic management by using more efficient, consecutive and intensive nutritional education methods are needed. (Korean J Nutrition 34(1):69-78, 2001)

  • Diabetes Mellitus is a worldwide disease affecting millions of persons and appears to be on the increase in Korea. At the present time it can not be cured but can be controlled. To achieve control the patient must know about the disease. Education of the patient is an essential part of the management of the diabetic and is the responsibility of the nurse. The educational program must be adapted according to patient's knowledge and attitude about their diseases. Therefore, the opportunity to assess the knowledge and attitude of diabetics must provided. The objectives of this study were : 1. To describe the general characteristics of the study population. 2. To find out the characteries of experience of treatment. 3. To exams the knowledge and attitude of diabetics about the self-control. 4. To learn the influencing factors affecting the knowledge and attitude of diabetics about self - control. The study population defined and selected was 65 diabetics registered at the Diabetic Clinic of 5. hospital who visited the Diabetic Clinic from April 16, 1976 to May 14, 1976. The direct interview method was used. Statistical analysis of the data was X²- test. The following results were obtained : A. General characteristics of the study population : The respondent's average age was 50. 8 years old, sex ratio(male to female) revealed as 1 : 1.1, economic background was upper middle class, the most respondents had better than- average education, 75.4 Percents of the respondents lived in Seoul, 47. 2 percents were unemployed, the average size of family was 5.3 and 47.7 percents of respondents had religions. B. The characteristics of clinical experience : The average duration was 7.0 years, 46.1 percents of respondents had experience of hospitalization, 56.9 percents was taking oral drugs, 67.7 percents examined urine- sugar at home, 60.0 Percents had treat compilations and 20.0 percents had experience of coma. C. There was significant difference in the relationship : Between complications and durations of diabetes, hospitalization and therapeutic methods.
