Effect of Occlusal Stabilizing Splint for Osteoarthritis of Temporomandibular Joint (측두하악관절의 골관절염에 대한 교합안정장치의 치료효과)

  • To evaluate the treatment outcome of occlusal stabilizing splint in patients with TMJ osteoarthritis, the 76 subjects were chosen among the patients who presented to the Department of Oral medicine of Pusan National University Hospital, diagnosed as TMJ osteoarthritis by cone beam computed tomography, x-ray and clinical exam, and treated with occlusal stabilizing splint from 2009 to 2011. They were treated with physical therapy and medication before occlusal stabilizing splint delivery and checked monthly after occlusal stabilizing splint delivery. Subjective symptoms and clinical findings were investigated to evaluate and compare the subjects' status at the first visit, splint delivery visit and the last visit. The results were as follows; 1. Pain, noise, LOM and MCO were significantly improved between the first visit and occlusal stabilizing splint delivery visit, and between occlusal stabilizing splint delivery and the last visit. 2. In the acute group, pain and noise were significantly improved between the first visit and occlusal stabilizing splint delivery visit. Pain, LOM and MCO were significantly improved between splint delivery visit and the last visit 3. In the chronic group, pain, noise and LOM were significantly improved between occlusal stabilizing splint delivery visit and the last visit.

Comparative Evaluation of Resolution according to Frequency Change for SPECT (단일광자방출전산화단층촬영(SPECT)을 위한 주파수 변동에 따른 분해능 비교평가)

  • In clinical SPECT, FWHM provides quality information of the images obtained under different observing conditions. Therefore, the purpose of this study was to determine the optimum cut off level with comparison of FWHM according to cut off levels in each filter - Band limited, Sheep-Logan, Hanning, Hamming, Low pass cosine, Parazen and Butterworth filter in a SPECT camera. I recorded images along the X, Y, Z-axis with 99mTcO4 point source and measured FWHM with profile curves. In conclusion, all filters showed the longest figures of FWHM with cut off level 0.4, which has the worst image resolution. The images with cut off level 0.7 showed best image resolution. The shortest average of FWHM in MS2 was $11.07\pm0.07mm$ using the Butterworth filter, in MS3 it was $8.44\pm0.19mm$ through using the Hanning filter.

Influence of CT Reconstruction on Spatial Resolution (CT 영상 재구성의 공간분해능에 대한 영향)

  • Computed tomography, which obtains section images from reconstruction process using projection images, has been applied to various fields. The spatial resolution of the reconstructed image depends on the device used in CT system, the object, and the reconstruction process. In this paper, we investigates the effect of the number of projection images and the pixel size of the detector on the spatial resolution of the reconstructed image under the parallel beam geometry. The reconstruction program was written in Visual C++, and the matrix size of the reconstructed image was $512{\times}512$. The numerical bar phantom was constructed and the Min-Max method was introduced to evaluate the spatial resolution on the reconstructed image. When the number of projections used in reconstruction process was small, artifact like streak appeared and Min-Max was also low. The Min-Max showed upper saturation when the number of projections is increased. If the pixel size of the detector is reduced to 50% of the pixel size of the reconstructed image, the reconstructed image was perfectly recovered as the original phantom and the Min-Max decreased as increasing the detector pixel size. This study will be useful in determining the detector and the accuracy of rotation stage needed to achieve the spatial resolution required in the CT system.

  • Non-invasive technique CT, called automated computed tomography, is used to detect lesion of a patient when diagnosing human body. Information obtained from CT plays an important role in assembling 3 dimensional images. Recently, new equipment, operated by CT, is required which can be appliable to physical and biological research. In accordance to this quest, micro-CT is invented that produce more detail and concrete information. Images supplied by CT are even more detailed and concrete, so it contributes much to the development of biology and polymer material engineering field. However, there has been little reliable reports regarding measuring information of space dose distribution about exposure dose limit of users operating micro-CT. In addition, little reports regarding space dose distribution of exposure has been known about unwanted diffraction light produced by usage of micro-CT. The exterior of micro-CT is covered by lead, which is for removing exposure of diffraction light. Thus, even if it is good enough to prevent exposure of diffraction light, consistent management of equipment will be required as time goes by and equipment are getting old as well. We measured space dose distribution regarding exposure of diffraction light of users operating micro-CT directly. Therefore, we suggest that proper management should be necessary for users operating micro-CT not to be exposed by unwanted diffraction light.

  • In study suggested clinical availability to shoulder forced traction method in term of quality of image, the patient's convenience and stability, according to whether to use of shoulder forced traction bend using computed tomography(CT) that X-ray calibration and various mathematic calibration algorithm application can be applied by AEC. To achieve this, 79 patients is complaining of cervical pain oriented that shoulder forced traction bend use the before and after acquires lateral projection scout image and transverse image. transverse image of a fixed size in concern field of pixel and figure the average HU value compare that quantitative analysis. Artifact and pixel and resolution to qualitative clinical estimation image analysis. the patient feel inconvenience degree that self-diagnosis survey that estimate. As a result, lateral projection scout image if you used shoulder forced traction bend for the depicted has been an increase in the number of a cervical vertebrae. transverse image concern field shoulder forced traction bend use the before and after for pixel and the average HU-value changes was judged to be almost irrelevant. Artifact and resolution and contrast, in qualitative analysis of the results relating the observer to the unusual result. So, the patients of 82.27% complained discomfort that use of shoulder forced traction bend in self-diagnosis survey. No merit of medical image by using of bend from result was analyzed quality of image to quantitative and qualitative method judged. Nowadays, CT is supplied possible revision of quality of radiation by reduction of slice and automatic exposure controller, etc and application of preconditioning filter process due to various mathematic revision algorithm. So, image noise by beam hardening artifact should not be a problem. shoulder forced traction bend of use no longer judged clinically availability because have not influence of image quality and give discomfort, have extra dangerousness.

  • The purpose of this study was to manufacture a wrist brace using a computerized tomography system, clinical design software (MediACE 3D Program), and 3D printer. After acquiring the Dicom file of the upper limb with a computed tomography, the wrist brace was designed using the MediACE 3D Program to create a "stereolithography" file. The designed wrist brace was printed using a 3D printer. To verify the effectiveness of wrist assistive device manufactured by 3D printing technology, the stress distribution of the pressure and orthosis applied to bone and skin is represented by finite element analysis. It is expected that the wrist brace can be manufactured by reinforcing the part where the damage caused by pressure and breakage of the brace frequently occurs with the result of finite element analysis when producing the wrist brace.

  • 국민건강보험재정건전화특별법 (제정 2002.1.19 법률 제 6620호)은 건강보험의 재정적자를 조기에 해소하고 재정수지의 균형을 이루도록 함으로써 건강보험제도의 발전과 국민건강 증진 도모를 그 목적으로 하며, 제14조(특수의료장비의 설치ㆍ운영)에서 의료기관은 보건복지부장관이 고시한 특수의료장비를 설치ㆍ운영하고자 하는 때에는 보건복지부령이 정하는 바에 따라 이를 등록하여야 하며 설치 인정 기준에 적합하게 설치ㆍ운영하여야 하고 정기적인 품질관리를 받아야한다고 명시하였다. 특수 의료장비의 설치 및 운영에 관한 규칙 (제정 2003.1.14 보건복지부령 제235호)은 국민건강보험재정건전화특별법이 제정됨에 따라 특수의료장비의 적정 한 설치와 활용을 위하여 의료기관에서 설치ㆍ운영하는 특수의료장비의 등록절차 설치인정기준 및 품질관리 절차 등을 정하고 특수의료장비에 대한 관리체계를 확립하려는 것이다. 특수의료장비의 설치 및 운영에 관한 규칙의 주요 골자는 가. 의료기관에서 특수장비를 설치ㆍ운영하고자 하는 경우 보건복지부장관 또는 시ㆍ도 지사에게 등록하도록 하였는바, 이 등록에 대한 절차와 특수의료장비의 설치인정기준을 정함, 나. 특수의료 장비에 대한 정기적인 품질관리검사를 서류검사와 정밀감사로 구분하여 서류검사는 1 년마다, 정밀검사는 3 년마다 받도록 함, 다. 품질관리검사기관의 장은 특수의료장비품질관리검사성적서를 신청인에게 교부하고, 보건복지부 또는 시ㆍ도지사 및 건강보험심사평가위원장에게 검사결과를 통보하도록 함, 라. 특수의료정비를 설치ㆍ운영하는 의료기관의 개설자 또는 관리자 및 품질관리검사기관의 장이 작성ㆍ비치ㆍ보존하여야할 서류를 정함이다. 보건복지부장관에게 등록하여야할 특수의료장비는 자기공명영상촬영장치와 전산화단층촬영 장치이며, 시ㆍ도지사에게 등록하여야할 특수의료장비는 유방촬영용장치이다. 본 발표에서는 특수의료장비의 설치 및 운영에 관한 규칙에 대한 개요와 연세의료원 세브란스병원에서 시행하고 있는 특수의료장비의 정도관리 검사, 팬텀영상검사, 그리고 임상영상검사를 소개하고자한다.

Evaluation of difference in respiratory phase between amplitude- and phase-based four-dimensional computed tomography (위상 기반 사차원전산화단층촬영과 진폭 기반 사차원전산화단층촬영 영상에서의 위상차 평가)

  • Purpose : Under the assumption of change to the amplitude based sorting, the study will use four dimensional computed tomography imaging (4DCT) arrayed using the phase based sorting to analyze the respiratory phase difference. Materials and Methods : The study analyzed the 4DCT (4-dimensional computed tomography) images of 10 liver cancer patients that were treated with respiratory gated radiotherapy from 2015 February to March. Using RPM respiratory gating (RPM 1.7.5, Varian, USA) equipment, imaging according to respiratory cycle of phase based sorting was acquired and using a treatment planning system (Pinnacle 9.2, Philips, USA) the acquired imaging according to respiratory cycle was used to measure the abdominal movement value by respiratory cycle. The measuring point was the point where the center point of the Marker Block and the body surface met in the 50% phase image and here the coordinate values Lateral, Vertical, Longitudinal (X, Y, Z) were set as reference points, and on the X, Z plane identical to the reference point, using the identical method the Y axis coordinate value of each 0%, 30%, 40%, 50%, 60%, 80% phase images were acquired to quantitatively measure the variation of distance to the Y axis. The abdominal movement value according to respiration was applied to the theoretical model that the value decreases linearly from maximum inhalation to maximum exhalation to divide the variation of my value to predict as amplitude value by respiratory cycle and conversely the variation in amplitude was recalculated with the phase variation deviation value to analyze. Results : The deviation value between expected value and actual location was the largest in the 30% phase with 0.24 cm, and standard deviation was also the largest in 30% phase with 0.13 cm. The effective value of the deviation value derived from the average of the deviation squared value of each patient appeared as minimum 0.7 cm, maximum 0.18 cm, average 0.12 cm, and standard deviation 0.4 cm. Also by dividing the actual movement distance value with the peak expiration value then converting it into %Phase, the deviation value with actual phase 16.5% in 30% phase, 10.0% and 40% phase, 10.0% and 60% phase, 15.4% and 80% phase, and overall average about 13%, and arraying based on amplitude, phase shift occurred and further it was from peak expiration the chance of deviation occurrence was increasingly measured. Conclusion : Based on the results of the study there were differences between value acquired based on theoretical model and actual value. Therefore in respiratory gated radiotherapy using external surrogates, there needs to be establishment of respiration gated radiation system that avoids the combination of two Sorting methods considering that there will be occurrence of treatment and corresponding clinical differences due to the phase difference that occur due to the Amplitude based Phase Sorting.

  • AC 부하를 갖는 동기전동기에 있어서, 동기전동기의 속도리플을 저감시키기 위한 문제를 풀기위해 3가지 제어기법에 대해 비교를 한 후 가장 강인한 제어기법에 대해 분석하였다. AC 부하를 갖는 특별한 제어 대상으로 엑스선 전산화 단층촬영 장치(CT)용 겐트리를 선정하였으며 시스템이 갖는 특별한 구조에 의해 이러한 시스템 특성을 갖는다. 동기전동기의 출력 축에 링(Fing) 모양의 원판 프레임을 갖으며, 이 원판 표면에 무게가 서로 다른 여러 장치(X_선 튜브, X-선 검출기, 고압발생장치, DAS 장치, 온도조절장치 등)를 부착하여 영상 획득 시스템의 회전부를 구축하기 때문이다. 이러한 시스템에서는 무게 평형을 갖지 못하는 편심 무게가 존재하게 되며 이로써 전동기 관점에서는 AC 부하처럼 인식되는 제어 조건으로 인식 될 수 있다. AC 부하를 갖는 동기전동기에 대해 일반적인 벡터제어 알고리즘으로 제어를 수행하면 정상상태에서도 속도 오차가 "0" 으로 줄어들지 않고 AC 형태의 오차 성분이 존재하며 편심 무게의 크기에 비례하여 진폭이 커지는 특성을 갖는다. 이러한 문제점을 해결하기 위하여 Sine파 보상전류를 갖는 속도제어기법, 펀심부하토크 관측기를 이용한 속도제어기법, 그리고 기준모델제어기법을 소개하였다. 각 방법에 대한 실험 결과로부터 편심무게의 변동과 편심 위치의 변동 조건에서 기준모델제어기법이 강인한 제어 특성과 리플저감 측면에서 가장 우수함을 검증하였다. 이로써 AC 부하 조건에서 고정도 속도 제어기가 요구되는 경우 좋은 선택의 지침이 될 수 있다고 본다.

Development of Infrared Imaging Measurement Device for Internal Red Ginseng Analysis. (홍삼 내부 분석을 위한 적외선 영상 측정기기 개발)

  • 본 연구는 홍삼 등급판정 과정에서 내부 조직 치밀도 분석을 위해 의료 영상 분석 방법을 이용하여 적외선 영상 측정기기를 개발하였다. 기존 홍삼 내부분석방법은 수동으로 암실환경에서 강한 조명을 홍삼에 투과하여 사람 눈으로 직접 판별하는 과정을 거친다. 이러한 기존 검사 과정은 작업 비효율성과 불균일한 검사기준으로 제품 품질 신뢰도 저하의 단점을 가져온다. 이러한 문제점을 해결하기 위해 본 연구는 적외선 조명환경에서 자동 측정이 가능한 홍삼 내부 측정기기를 개발하였다. 개발된 장치는 홍삼의 빛 투과 특성을 응용한 920nm 파장대역의 적외선 조명기구, 조명 제어회로, 적외선 대역 촬영이 가능한 영상 측정 카메라 장치, $0.9^{\circ}$의 간격으로 $360^{\circ}$ 홍삼 영상취득이 가능한 회전 엑츄에이터로 구성이 된다. 본 연구에서 제안 하는 홍삼 단층 영상분석 방법은, 홍삼을 $0.9^{\circ}$ 간격으로 회전시키어 $360^{\circ}$ 홍삼 내부영상을 취득하여 라돈 변환(Radon transform)을 통해 사이노그램(Sinogram)으로 재구성 하였으며, 역 라돈 변환(Inverse Radon transform)을 통해 단층 영상복원(Back-projection)알고리즘을 구현하였다. 이 결과 홍삼을 절단하지 않고 홍삼 내부 단면영상 획득이 가능하였으며 내공(內空), 내백(內白)의 유무를 판단하고 직경을 파악할 수 있었다. 이를 토대로 등급 판별 공식을 산출하면 신뢰성 있는 홍삼 등급 자동화 측정기기를 개발할 수 있을 것으로 기대된다.
