Consideration of maxillary sinus bone thickness when installing miniscrews (미니스크류 식립 시 상악동의 골두께에 대한 고려)

  • Objective: Miniscrews are widely used in orthodontic treatment for the purpose of anchorage control. Maximum anchorage can be acquired by the use of miniscrews. Maxillary miniscrew has many clinical advantage for orthodontic treatment. Maxillary sinus, tooth root can be an obstacle for maxillary miniscrew installation. The purpose of this study was to find the safest area and direction of miniscrew insertion in consideration of the maxillary sinus. Methods: The maxillary sinus area of 40 patients (20 male, 20 female) was measured using 3D computed tomography and 3D reconstruction program. Results: The maxillary sinus floor was located most inferiorly between the 1st molar and 2nd molar and located most superiorly between the 1st premolar and 2nd premolar. Buccal bone thickness from the maxillary sinus is significantly thicker between the 1st molar and 2nd molar and significantly thinner between the 1st premolar and 2nd premolar. The area between the 1st premolar and 2nd premolar has a significantly longer vertical distance from CEJ to sinus in consideration of buccal bone thickness. Conclusions: Considering maxillary bone thickness, the posterior area has advantages over the anterior area for installing miniscrews safely and preventing perforation.

  • In benign exophytic lesion in TMJ such as osteoma, chondroma, osteochondroma, synovial chondromatosis etc, symptom such as pain, mouth opening limitation, Most case of condylar exophytic lesion manifest with facial asymmetry, malocclusion, mandibular midline deviation. An osteochondroma and synovial chondromatosis are most commom benign condylar tumor. However this tumor is most frequently found on long bone and flat bone and is unusual on the skull. We report cases of osteochondroma, synovial chondromatosis of TMJ and review literatures.

  • Multiple primary cancer is a disease of more than two cancers occurring in an individual indepen- dently. We experienced a case of triple primary cancer, that is, lung cancer, malignant thymoma and bladder cancer which has not been reported in Korea. The patient was a 60 year old man with dyspnea and chest discomfort. He was rirst diagn sed bladder cancer and received Bricker's operation 3 months ago. At that time the chest roentgenography and computerized tomographic scan revealed as a preaortic, retrosternill medidstin;11, nlass and a lung mass at the posterior portion of the left lower lobe. On operation, there was An identillable lung mass in the left lower lobe and a thymoma already invaded the surrounding structures. So, left lower lobectomy and thymectomy were performed simul- taneously. He received one cycle of postoperative chemotherapy, but refused further management and self-discharged. He died, about one year later.

The effect of mandibular position on measurement in spiral tomography (나선형 단층방사선사진촬영에서 하악골 위치가 측정치에 미치는 영향)

  • Purpose : To evaluate the effect of deviation of mandibular positioning, by changing the mandibular plane inclination, on the measured height and width of mandible in spiral conventional tomography. Materials and Methods : By means of the Scanora multifunctional unit, cross-sectional tomograms were taken from two human dried mandibles at the mandibular angulations: -15 degree, -10 degree, -5 degree, and 0 degree. Twenty-eight sites in two dried mandibles were imaged. One examiner measured the bone heights and widths at selected sites on the images and the actual bone heights were recorded. Results : The bone heights at the four mandibular inclinations overestimated real bone heights and the mean difference between actual heights and image heights on 0 degrees was the smallest (P<0.01). The bone widths on -15 degrees were narrowest and there were significant differences between bone widths measured at the four mandibular inclinations (P<0.001). We found statistically significant differences between both bone heights and widths as measured according to the mandibular plane angle for the posterior region (P<0.01). Conclusion : The use of different mandibular positioning may result in discrepancies in heights and widths when measured from the cross-sectional tomographic images. It is suggested that the mandibular positioning may play a significant role in the measurement of mandibular heights and widths.

  • A 49-year-old man visited our hospital via the emergency room. He had suffered chest trauma by falling down. His chest X-Ray showed pneumomediastinum with pneumopericardium. We checked the Chest CT, and it showed pneumopericardium without any injury to the other organs, the compressed heart and a minimal pneumothorax on the left hemithorax. Closed thoracostomy was then done under local anesthesia. We then performed open pericardiostomy under general anesthesia. We got a good result and so we report on this case.

  • A case with a bronchial foreign body(wood) which had an uncommon way of entry through a wound In the chest wall was described. A 41-year-old man was admitted to our hospital because of chronic cough and profuse purulent sputum. He had been injured by a fragment of wood which penetrated his anterolateral chest wall at 30 years ago. A chest x-ray film reavealed a calcific foreign body, measuring 3.0 by 1.0 cm, in the posterobasal segment of the left lung. Computed nomograp y of the chest demonstrated a calcific foreign body with bronchiectatic change near the diaphragmatic pleura In the left lower lobe. Left lower lobectomy was performed, and the patient remains well without any thoracic symptoms after the operation.

  • Image fusion is fast catching attention as Wagner pointed out in his 2006 version of the recent progress and development presented at the annual meeting of Society of Nuclear Medicine. Prototypical fusion of bone scan and radiograph was already attempted at in 1961 when Fleming et al. published an article on strontium-85 bone scan. They simply superimposed dot scan on radiograph enabling simultaneous assessment of altered bone metabolism and local bone anatomy. Indeed the parallel reading of images of bone scan and radiography, CT, MRI or ultrasonography has been practiced in nuclear medicine long since. It is fortunate that recent development of computer science and technology along with the availability of refined CT and SPECT machines has permitted us to open a new avenue to digitally produce precise fusion image so that they can readily be read, exchanged and disseminated using internet. Ten years ago fusion was performed using Bresstrahlung SPECT/CT and it is now achievable by PET/CT and SPECT/CT software and SPECT/CT hardware. The merit of image fusion is its feasibility of reliable assessment of morphological and metabolic change. It is now applicable not only to stationary organs such as brain and skeleton but also to moving organs such as the heart, lung and stomach. Recently, we could create useful fusion image of cardiac SPECT and 64-channel CT angiograph. The former provided myocardial metabolic profile and the latter vascular narrowing in two patients with coronary artery stenosis and myocardial ischemia. Arterial stenosis was severe in Case 1 and mild in Case 2.

  • Introduction: Accurate diagnosis and treatment planning are very important for orthognathic surgery. A small error in diagnosis can cause postoperative functional and esthetic problems. Pre-existing 2-dimensional (D) chephalogram analysis has a high likelihood of error due to its intrinsic and extrinsic problems. A cephalogram can also be inaccurate due to the limited anatomic points, superimposition of the image, and the considerable time and effort required. Recently, an improvement in technology and popularization of computed tomography (CT) provides patients with 3-D computer based cephalometric analysis, which complements traditional analysis in many ways. However, the results are affected by the experience and the subject of the investigator. Materials and Methods: The effects of the sources human error in 2-D cephalogram analysis and 3-D computerized tomography cephalometric analysis were compared using Simplant CMF program. From 2008 Jan to 2009 June, patients who had undergone CT, cephalo AP, lat were investigated. Results: 1. In the 3 D and 2 D images, 10 out of 93 variables (10.4%) and 11 out 44 variables (25%), respectively, showed a significant difference. 2. Landmarks that showed a significant difference in the 2 D image were the points frequently superimposed anatomically. 3. Go Po Orb landmarks, which showed a significant difference in the 3 D images, were found to be the artificial points for analysis in the 2 D image, and in the current definition, these points cannot be used for reproducibility in the 3 D image. Conclusion: Generally, 3-D CT images provide more precise identification of the traditional cephalometric landmark. Greater variability of certain landmarks in the mediolateral direction is probably related to the inadequate definition of the landmarks in the third dimension.

A Morphological Study of Intrachondral Articulations in the Korean Water Deer (한국고라니 갈비연골속관절에 관한 형태학적 연구)

  • Intrachondral articulations are the synovial joints that occur within costal cartilage in artiodactyls. The anatomical structure of Korean water deer differs from that of other deer. However, there have been no reports on the occurrence and shape of intrachondral articulations in the deer. To provide information on these articulations, we examined the occurrence and shape of intrachondral articulations in the Korean water deer by gross findings, radiography, computed tomography (CT), and histological observation. These joints often occur in the second to the tenth ribs. Morphologically, they are spheroidal joints, especially from the third to the eighth ribs, and their configuration is discernable in gross findings, radiography, CT, and histological sections. These basic results would be helpful in the diagnosis and treatment of diseases in the lateral thoracic wall of the Korean water deer.

  • We experienced a case of thymolipoma that is a rare benign mediastinal tumor, composed of normal thymic tissue and matured adipose tissue. A 46-years-old woman was admitted to the department of medicine due to lower abdominal pain. Simple chest PA showed a large mass shadow at right cardiac border. Chest CT scanning showed well defined large fatty mass at right cardiac border which was suggested thymolipoma. The mass was resected and confirmed as thymolipoma histopathologically.
