목적 : 유방 절제 생검 후 유방암이 진단된 환자에서 잔류종양과 추가 병변을 진단하는데 있어 자기공명영상의 유용성을 알아보고자 하였다. 대상 및 방법 : 2005년 3월부터 2007년 8월까지 유방 절제 생검으로 유방암이 진단되고 수술 전 자기공명영상을 시행한 16명의 환자를 대상으로 하였다. 생검 후 생긴 공동 주위로 두껍고 불규칙한 조영증강 테두리를 보이거나 공동에 연하여 조영증강되는 종괴가 있는 경우를 잔류종양이 있다고 보았다. 또한 공동과 분리된 다초점성, 다발성, 혹은 반대측 유방 병변 등의 추가 병변을 확인하였다. 수술 후 조직학적 소견을 비교하여 자기공명영상의 정확성을 평가하였고 자기 공명영상소견에 따라 향후 치료에 변화가 있었는지를 알아보았다. 결과 : 공동주위 조영증강 소견으로 잔류종양을 평가하는 자기공명영상의 민감도와 특이도는 각각 85.7%(6/7)와 55.6%(5/9)이었다. 자기공명영상으로 6명의 환자에서 다초점성(multifocal), 다발성(multicentric), 혹은 반대측 유방에 추가 병변을 발견하였다. 자기공명 영상의 결과에 따라 7명의 환자에서 치료 범위가 변화되었다. 결론 : 절제 생검 후 자기공명영상에서 보인 소견 중 공동주위 조영증강 소견만으로 잔류종양 유무를 감별하기 어렵다. 그러나 유방 자기공명영상은 다초점성 및 다발성 혹은 반대측 병변을 포함한 추가병변을 발견하는 데 도움을 주어 수술적 치료에도 유용한 정보를 제공한다.

현미경적 다발성 동맥염은 폐출혈과 급속 진행성 사구체심염을 특징으로 하는 전신성 혈관염의 일종으로 소아에서는 매우 드문 질환이다. 저자들은 폐출혈과 급성 신부전을 동반한 7세 여아에서 신장조직검사와 p-ANCA 검사로 현미경적 다발성 동백염으로 진단 후 5년 뒤 2차 신장조직검사와 경구 ACE inhibitor, angiotensin II receptor blocker와 저용량의 cyclophosphamide를 투여 받은 환아를 7년간 추적관찰하였다. 발병당시에는 BUN 117 mg/dL, Cr 2.3 mg/dL이었으나, 퇴원시 BUN 20.8 mg/dL, Cr 1.6 mg/dL이었고, 최근 검사에서는 BUN 51.7 mg/dL, Cr 3.2 mg/dL으로 만성 신병증 소견을 보이고 있으며, 외래 추적 관찰 지속 중에 있다. 이에 문헌 고찰과 함께 증례 보고하는 바이다.

Pulmonary histiocytosis X is an idiopathic benign disease characterized by proliferation and infiltration of lung tissue by characteristic Langerhans cells and eosinophils. Pulmonary histiocytosis X is common in young male adults, and shows variable clinical characteristics. We experienced a case of pathologically proven pulmonary histiocytosis X in a 30-year-old man who visit to our hospital due to chest discomfort and cough. The chest radiograph of our patient shows right pneumothorax and characteristic multiple thin-walled cysts on the both upper lung fields. The HRCT shows multiple thin-walled cysts, a few scattered nodules in both upper and right middle lung, and right pneumothorax.

봄바람이 불면서 구제역 발생 방지를 위한 양돈장의 방역관리가 더욱 절실해지고 있다. 이에 농림부는 3월부터 5월까지를 '구제역 특별대책기간'으로 설정, 강도 높은 국격검역과 국내 방역활동을 추진하고 있다. 구제역은 국내에서는 2000년과 2002년도에 발생하여 4,500억원의 직접 손실을 입은 바 있으며 최근에는 발생하지 않았고 국제적으로도 발생이 감소하고 있지만 우리나라와 인접한 중국과 북한에서 계속 발생하고 있고 베트남 등 다발지역인 아시아 국가들과의 교류도 증가하여 국내에 유입될 가능성을 배제할 수 없는 상황이다. 특히 최근에는 사료 값이 큰 폭 인상되는 등 양돈농가의 경영여건이 악화되었고, 환절기 질병 발생률이 높아 방역에 소홀해질 수 있다는 우려가 높아지고 있다. 게다가 더 올봄에는 예년보다 황사가 자주 발생할 것으로 예보되어 구제역 방역에 더욱 고삐를 조여야 할 것으로 보인다. 또한 양돈농가에서 방역활동을 철저히 하는 한편, 특별대책기간 중 외국을 여행하는 농가는 중국, 몽골, 메트남 등 구제역 발생국가 여행이나 현지 축산농장 방문을 자제하고 입국시 육류 등 축산물을 가지고 들어오지 않도록 해야한다.

Apatient with multiple dissecting aneurysms of a posterior inferior cerebellar artery trunk who presented with SAH is reported. A 58-year-old woman presented with sudden occipital headache, dizziness and vomiting. The intial vertebral angiography revealed a suspicious pearl and string sign at the proximal posterior inferior cerebellar artery(PICA) segment. After 2 weeks, follow up angiography showed a progression of the proximal PICA dissection and newly developed dissecting aneurysm of the distal PICA segment. A far lateral suboccipital transcondylar appoach confirmed two dissecting aneurysms at distant sites of the PICA trunk. The dissection segments were wrapped with muslin wrap, which preserved the flow through the PICA and brain stem perforators. The angiographys at 3 weeks and 6 months after operation revealed serial disappearance of the dissecting aneurysms which is distal to proximal. The diagnosis, course and treatment of the dissecting aneurysms of the PICA are discussed with literature review.

Anorexia nervosa(AN) is a disorder characterized by disturbance of body image, fear of gaining weight, severe weight loss and, in female, amenorrhea. Compared with normal persons, patients with AN have neuropathic symptoms more frequently. But electrophysiologic abnormalities have rarely been reported. We experienced a case with recurrent neuropathic symptoms after severe weight loss. Further evaluation revealed AN. Electrophysiologic study showed sensorimotor polyneuropathy and focal neuropathy with conduction block. As far as we know, this feature of neuropathy in AN has not been described. We describe unusual feature of neuropathy in our patient with literature review.

오리에 대한 전문가가 없이 단순하게 오리를 과거 양계장이나 또는 육계농장이 그랬듯이 보온덮개 건물에 어떠한 최적의 환경 조건도 맞추어주지 않고 자연환기에 의해서 사양관리로 우리 스스로 오리를 사육하면서 많은 문제에 봉착해 있는 것 또한 오늘날 우리나라 오리 농장의 현실이라고 할 수 있다. 이렇게 열악한 환경에서 오리 농장을 경영하다 보니 자연스럽게 문제가 되는 것은 조류독감과 같은 외래성 질병의 영향으로부터 벗어나기가 어렵다는 것이라고 할 수 있다. 특히 조류독감은 이제 세계적인 문제를 동시다발적으로 일으킬 수 있는 중요한 바이러스성질환으로 우리 오리 농가 모두가 합심해서 그 발생을 막아야 하는 중요한 문제라고 할 수 있을것이다. 우리가 더 위생적이고 또한 더 품질이 좋은 오리고기의 생산을 위해서는 반드시 제대로 된 시설을 하지 않을 수 없다는 것을 알고 이 부분에 대하여 공부하고 연구할 필요가 있는것이다.

When the chest roentgenogram reveals the presence of multiple pulmonary nodules, the basic investigation includes a history, physical examination, routine hematologic and urine studies, and sputum specimens to search the etiology. We have experienced a case of endometrial sarcoma with metastasis to the lung.

Polyneuropathy that is associated with monoclonal gammopathy of undetermined significance (MGUS) similar to chronic inflammatory demyelinating polyneuropathy (CIDP) has been reported before, whereas a connection to acute inflammatory demyelinating polyneuropathy (AIDP) has not been. A 52 year-old man was presented with ascending paralysis beginning 1 day ago. Neurological examinations showed facial diplegia and decreased motor power and deep tendon reflexes in all extremities. On electrophysiologic study, sensorimotor polyneuropathy was observed. Protein-and immunoelectrophoresis revealed IgA $\lambda$ monoclonal gammopathy. High dose steroid therapy was given and the symptoms improved slightly.
