  • Rubinstein-Tabyi syndrome(RTS) is a congenital multisystem dysmorphic syndrome with many non-specific features, making diagnosis occasionally difficult. The major features of this syndrome include mental retardation, characteristic facial appearance, short stature, microcephaly, and broad thumbs and halluces. This syndrome was first described by Rubinstein and Tabyi in 1963, and many studies have been continued about this syndrome, but specific pathogenesis of the Rubinstein-Tabyi syndrome phenotype is still not clear. High arched palate, micrognathia and multiple caries etc have been reported in Rubinstein-Tabyi syndrome. In this report, a 6-year and 5-month-old boy visited at our department due to multiple dental caries, who showed broad thumbs, mental and physical development retardation, and characteristic facial appearance including both ptosis and ear deformity. This patient was diagnosed as a Rubinstein-Tabyi syndrome, and treated the multiple dental caries under general anesthesia. This study was aimed to observe the relationship between medical and dental characteristics.

  • Two cases of rampant dental caries patients, 5 year old and 3 year old male, were treated; and got following results: 1) Early treatment of rampant dentral caries is emphasized to obtain favorable treatment results. 2) Premedication prior to treatment is an important factor due to the long operative procedures which may elicit the child's phychological trauma. 3) The full cast gold crown restoration for rampant dental caries in primary dentition indicated because of its precise work for maintaining normal occlusion and gingiva.

  • Biliary atresia is defined as a complete obstruction of bile flow owing to destruction or absence of all or part of the extrahepatic bile ducts. This disease is occurring in approximately 1:10,000 live births and moderate predominance of female is noted. The etiology of biliary atresia remained unsolved. The signs and symptoms are hyperbilirubinemla, jaundice, clay-colored stools, steatorrhea, dark yellow urine and hepatomegaly. Currently biliary atresia is best managed by hepatic portoenterostomy with or without liver transplantation. Biliary atresia patients with these cases showed staining of the teeth. The stains ranged in color from yellowish-brown to deep green. Enamel hypoplasia was all erupted teeth present. Patients had poor oral hygiene and rampant caries.

  • Hydrocephalus is characterized by accumulation of cerebrospinal fluid in the ventricles of the brain causing progressive ventricular dilatation. Accumulation of cerebrospinal fluid typically causes increased pressure inside the skull, this may result in headaches, vomiting, nausea, seizures, sleepiness and mental impairment. Patients with hydrocephalus may have dental manifestations such as changes in the occlusion and greater accumulation of plaque. The prevalence of dental caries is high due to the difficulty of oral care. The purpose of this case report is to describe the dental care for an uncooperative child with hydrocephalus under general anesthesia.

  • Clinical therapy that combines full-mouth rehabilitation with immediate implantation and orthognathic surgery poses a challenge to prosthodontists. This clinical report describes a multidisciplinary approach to the diagnosis and treatment of a patient presenting with skeletal discrepancy and rampant caries. The results thus achieved indicate that full-mouth rehabilitation by fixed immediate and early loading implantation accompanied by orthognathic surgery can be a predictable and effective treatment procedure.

  • Loss of multiple teeth by multiple dental caries, traumatic influence or hereditable disease can cause many problem in children. Using removable appliance can be used for treatment of this problem. But conventional removable appliance has limits of retention by wire-clasp, non esthetic appearance, difficulty in pronunciation by its volume. Flexible denture using $Valplast^{(R)}$ system could be a excellent partial denture for restorarion in child because of various advantage such as retention from soft tissue, natural esthetic aspect, biocompatibility, durability to compare with conventional acrylic appliance. The present case report describe properties and consideration of flexible denture using Valplast $system^{(R)}$ in comparison with conventional acrylic appliance and report successful restoration of child using Valplast $system^{(R)}$.

Changes in Salivary Flow Rate and pH in Stressful Conditions (스트레스하에서 타액유출량과 수소이온농도의 변화)

  • 타액은 그 양과 조성 등에 있어 많은 인자들의 영향을 받는다. 타액분비 감소는 구강의 자정작용, 완충능, 치아우식저항성 등 타액의 고유한 기능을 변화시켜서 구강건조감, 구강작열감, 다발성 치아우식증 등의 소인이 된다. 이에 저자는 심리적 요인이 구강내 환경에 미치는 영향을 평가하고자 타액선질환을 포함한 전신질환이 없는 전북대학교 치과대학생 20명을 대상으로 일상생활시와 시험 직전의 비자극성 전타액을 5분간 추출하여, 그 유출량과 수소이온농도를 측정, 비교, 평가하였다. 타액유출량은 $25m{\ell}$의 메스실린더를 사용하였고, 수소이온농도는 pH/SEmeter(ORION, 720A model)를 이용하여 측정하였다. 평가 결과 남성의 비자극성 타액유출량과 수소이온농도는 $3.68{\pm}1.31m{\ell}/5min$$7.63{\pm}0.17$이었고, 여성에서는 각각 $4.93{\pm}1.47m{\ell}/5min$$7.43{\pm}0.29$로서 성별간 유의한 차이가 없었다. 그리고 일상생활시의 성인의 5분간 수집된 비자극성 타액유출량은 $4.18{\pm}1.48m{\ell}/5min$였고, 스트레스하에서의 양은 $2.20{\pm}0.95m{\ell}/5min$로 나타나 스트레스시 타액유출량이 감소하는 것으로 나타났다(p<0.01). 또한 일상생활시의 성인의 비자극성 타액의 수소이온농도는 $7.55{\pm}0.24$였고, 스트레스하에서의 수소이온농도는 $7.22{\pm}0.20$으로 나타나 스트레스시 수소이온농도가 감소하는 것으로 나타났다(p<0.01). 이상의 결과로 보아 스트레스는 타액유출량을 감소시키고 구강내를 보다 산성화시킨다고 사료된다.

  • Epidermolysis bullosa (EB) is an inherited skin disease characterized by blister formation of the skin following just minor trauma or spontaneously. The main classification of EB is based on the localization of the blistering. In epidermolysis bullosa simplex, the blister formation is intraepidermal; in epidermolysis bullosa junctionalis the blister formation in the epidermal-dermal junction; in the dystrophic forms of EB blister formation is intradermal. Oral manifestations of recessive dystrophic EB are characterized by rampant caries, crowding, microstomia, ankyloglossia, vestibular obliteration. Dental therapy may be well provided to patient with recessive dystrophic EB by using general anesthesia especially in case of poor cooperation. A two years old girl with recessive dystrophic EB visited our clinic for rampant caries. She was hospitalized for severe anemia and fever, and we provided dental therapy under general anesthesia. Extraction of incisors and stainless steel crown restoration of primary molars were performed.

  • Muscular dystrophy is a genetically heterogeneous group of disorders characterized by progressive muscle weakness of variable distribution and severity. Fukuyama type congenital muscular dystrophy (FCMD) is an unusual form of muscular dystrophy with autosomal recessive inheritance and is clinically characterized by an early age of onset, severe central nervous system involvement, facial muscle weakness, and multiple joint contractures. Muscular dystrophy is susceptible to perioperative respiratory, cardiac and other complications. Patients with FCMD have upper airway muscle weakness, therefore general anesthesia is preferred to sedation regarding maintaining the airway when treating these patients. The development of malignant hyperthermia in general anesthesia for patients with muscular dystrophy is a concern. Total intravenous anesthesia should be used instead of inhaled anesthetics because of the risk of malignant hyperthermia. A 3-year-9-month old, 13kg girl with Fukuyama type congenital muscular dystrophy was scheduled for dental treatment under general anesthesia. She had multiple caries and 14 primary teeth needed caries treatment. Prior to general anesthesia, oral premedication with 9 mg midazolam was given. General anesthesia was induced and maintained with target controlled infusion of propofol $3{\sim}3.5{\mu}g/mL$. The patient with progressive muscular dystrophy was successfully treated under total intravenous anesthesia with a target controlled infusion of propofol. There were no complications related to anesthesia and dental treatment during or after the operation. This case suggests that target controlled infusion of propofol is a safe and appropriate anesthetic technique in FCMD patients for dental treatment.

BIOCHEMICAL CHARACTERISTICS OF SALIVA TO BE LINKED TO THE MULTIPLE CARIES IN CHILDREN (소아의 다발성 치아우식증과 연관된 타액의 생화학적 특성)

  • Saliva is obviously potential medium to protect the dental caries by not only physical clearing effect, but aggregating action of protein with bacteria. Nevertheless, we still do not understand how the dental caries occur and what brings the individual difference in caries prevalence. In the regards of dental caries prevalence, we hypothesized that the composition of salivary protein might be different from caries susceptible group to caries resistant group. The purposes of this experiment were focused on the molecular analysis of salivary proteins from the subjects who were involved in multiple caries. Electrophoretic analysis was done on the whole saliva collected from the children with and without multiple caries. We found 86.2% of subjects with multiple caries has approximately 120 KDa protein band while 30.4% in the healthy subjects. And the concentration of the total protein on the subjects with multiple caries is significantly higher than that of the healthy group. However, it turned out that the difference of the salivary composition does not affect the bacterial adhesion to hydroxyapatite bead. With regards of enzymes in saliva, the activity of ${\alpha}-amylase$ and lactate dehydrogenase does not have any significant difference between both groups. However, the concentrations of $Na^+\;and\;Cl^-$ in saliva from multiple caries group is higher than that of the control group. Taken all together, it may be concluded that 120 KDa protein in saliva may be associated with the process of dental caries, also the high concentration of protein and $Na^+,\;Cl^-$ in saliva may be linked to dental caries development as a cofactors.
