  • The purpose of this study is to investigate prosodic characteristics of cerebral palsic adults' speech. The results showed some correlations between their articulation scores and prosodic properties of their speech: speakers with low articulation scores showed slower speech rate, larger number of IPs and pauses, and longer duration of pauses. They also showed steeper slopes of [L +H] in their APs.

  • 뇌성마비는 뇌 발달시기중 초기의 장애로 인하여 운동기능과 자세의 이상 등을 초래하는 질환으로 임상적으로 여러 유형으로 나누어진다. 뇌성마비 환자중에서 MRI 상 롤란도야의 이상소견을 보이는 경우 특이한 임상적 특징을 나타낸다고 보고되어있다. 본 논문에서는 이들을 대상으로 하여 뇌 단일광자컴퓨터촬영에서 나타나는 뇌혈류 이상 소견과 MRI 소견, 그리고 임상양상들을 비교해보고자 하였다. 임상적으로 뇌성마비로 진단된 80여명의 환자중 MRI상 롤란도야에 저산소성 뇌질환의 소견을 보인 7명을 대상으로 하였다. 이들에게 5-10mCi의 Tc-99m-ECD를 정맥주사후 뇌 단일광자컴퓨터촬영을 하였다. 뇌 단일광자컴퓨터촬영상에서는 특히 뇌피질, 선조체, 시상, 뇌간 및 소뇌의 혈류이상 소견을 분석하였고, 이를 MRI 소견 및 임상증상과 비교하였다. 모든 환자들은 만삭 또는 만삭에 가깝게 태어났고, 주산기 아사가 여섯명의 환자에서 있었다. 임상적으로는 긴장성 사지마비로 나타났다. Tc-99m ECD를 이용한 뇌 단일광자 컴퓨터촬영상 롤란도야의 혈류감소는 모두에서 확연하게 나타났다 MRI상에서 롤란도야의 이상소견은 5명에서는 확실하게 나타났고, 2명에서는 의심되었다. MRI상에서 이상소견을 보인 것보다 더 광범위한 부분에서 뇌단일광자컴퓨터촬영상 이상소견이 보였다. 결론적으로, 롤란도야에서 이상소견을 보인 환자는 임상적, 구조적, 그리고 기능적으로 특이한 소견을 보였다. Tc-99m-ECD를 이용한 뇌 단일광자컴퓨터촬영은 매우 민감하게 반영하였고, MRI보다 뇌의 더 광범위한 부위에서 이상소견을 보였다.

Changes in Activities of Daily Living of Children with Spastic Cerebral Palsy According to Gross Motor Function Classification System After One Year of Physical and Occupational Therapy (물리 및 작업치료 1년 후 대동작 기능분류체계에 따른 경직성 뇌성마비 아동의 일상생활동작 변화)

  • This study was conducted to investigate changes in activities of daily living (ADLs) according to the Gross Motor Function Classification System (GMFCS) after one year of physical and occupational therapy and to compare the responsiveness of ADL tools. A total of 48 children with spastic cerebral palsy participated in the study. The GMFCS, Functional Independence Measure for Children (WeeFIM), and Pediatric Evaluation of Disability Inventory (PEDI) were measured. The results showed that the GMFCS was significantly correlated with the PEDI (p<0.05), while there was a significant difference in the change of ADLs measured by the PEDI, but not the WeeFIM. There was a significant difference in the changes in ADLs according to the GMFCS, and the change in ADLs in the high functional level group was significantly higher than in the low functional level group (p<0.05). After physical and occupational therapy, the degree of improvement of ADLs varied according to the GMFCS, but seemed to be improved in a clinically meaningfully way. The PEDI is sensitive to changes in ADLs, so it may be used widely in clinical practice.

Consonant Confusions Matrices in Adults with Dysarthria Associated with Cerebral Palsy (뇌성마비로 인한 마비말장애 성인의 자음 오류 분석)

  • The aim of this study was to analyze consonant articulation errors produced by 90 speakers with cerebral palsy (CP). Phonetic transcriptions were made for 37 single-word utterances containing 70 phonemes: 48 initial consonants and 22 final consonants. Errors of substitution, omission, and distortion were analyzed using a confusion matrix paradigm showing the visualization of error patterns. Results showed that substitution errors in initial and final consonants were most frequent, followed by omission and distortion. Consonant omission occurred more frequently on final consonants. In both initial and final consonants, the within-place errors were more prominent than the within-manner errors. The current results suggest that consonant confusion matrices for dysarthric speech may provide useful information for evaluating speech intelligibility and developing automatic speech recognition system of adults with CP associated dysarthria.

GENERALIZED HYPERCEMENTOSIS WITH ARRESTED DENTAL ERUPTION IN A CEREBRAL PALSY PATIENT : A CASE REPORT (뇌성마비 환자의 지연맹출을 동반한 다발성 과백악질증 : 증례보고)

  • Hypercementosis is an excessive deposition of secondary cementum on the root of a tooth. It is mostly presented as a solitary lesion or in rare cases as a generalized type, but which is seldom recognized; typically it is discovered during regular dental X-ray. Increased thickness of cementum is not uncommon but generalized hypercementosis on impacted permanent teeth which may cause delayed eruption is rarely reported. This case report discusses a patient with cerebral palsy, epilepsy and mental retardation that presents multiple hypercementosis with delayed eruption. On intraoral examination, multiple retained primary molar teeth were found. As there was no any further symptoms, regular dental checkup had been done for several years. In 2015, a surgical opening was performed in the second molar area, but there was no specific change. Panoramic view showed multiple impacted permanent teeth with increased thickness of roots due to excessive deposition of cementum. Hypercementosis was also observed in the root of the erupted tooth. Several laboratory test results including hormone, urine, complete blood count test were reviewed. The patient was also diagnosed with subclinical hypothyroidism, impaired fasting glucose and had been taken valproic acid($Orfil^{(R)}$) for 10 years. However, none of them clearly explained generalized hypercementosis or delayed eruption. The patient is now 24 years old and regular dental checkups and radiographs are taken to confirm that there is no change in the lesion.

Effect of Articulation Abilities on the Articulator Strength Training by IOPI of Spasticity Dysarthric Speech (IOPI를 활용한 조음기관 훈련 프로그램이 경직형 마비말장애의 조음 능력에 미치는 영향)

  • Objective : The purpose of this study was to investigate the effects of the IOPI articulator strength training program on articulator(tongue and lip) muscle strength, numbers of /l, s, ʨ/ articulation accuracy, articulatory numbers, articulation regularity and accuracy in the alternate motion rates, and sequential motion rate changes in patients with spastic dysarthria. Methods : Three cases of patients with spastic dysarthria living in Jeju, Korea, were included in this study. A single subject design was selected to study changes in articulator(tongue and lip) muscle strength, numbers of /ㄹ, ㅅ, ㅈ/ articulation accuracy, articulatory numbers, articulation regularity and accuracy in the alternate motion rates and sequential motion rates. Results : After the articulator strength training program was conducted on patients with spastic dysarthria, there were positive changes in articulator(tongue and lip) muscle strength, numbers of /ㄹ, ㅅ, ㅈ/ articulation accuracy, articulatory numbers, articulation regularity and accuracy on the alternate motion rates and sequential motion rates. Conclusion : Our findings suggest that IOPI articulator strength training program could be very useful for the most representative childeren with cerebral palsy if conducted in various subtypes of dysarthric patients and linked with articulatory function training with IOPI at home.

ORTHODONTIC APPROACH TO THE CEREBRAL PALSY PATIENT WITH MAXILLARY PROTRUSION IN THE MIXED DENTITION : A CASE (혼합치열기 뇌성마비환자의 상악전돌에 대한 교정치료 : 치험례)

  • Cerebral Palsy is a genetic term referring to abnormalities of motor control caused by damage to a child's brain early in the course of development. Due to the impairment of balanced perioral muscle development, the prevalence of malocclusions in patients with cerebral palsy such as maxillary protrusion is high. But most clinicians may feel uncomfortable to treatment of these problems. Here a case report about mitigation of maxillary anterior teeth protruded in patient with cerebral palsy. 8y 4m old boy who have cerebral palsy visited our dental hospital. He showed severely protrusive maxillary anterior teeth with mouth breathing and could not close his mouth. He and his mother wanted to improve dental and facial esthetic problem. Specially designed or modified intraoral fixed appliance and rubber elastic chain was used in the therapy. Treatment carried out for 8 months and we could observe maxillary incisor angle was improved and mouth breathing habit was stopped. In conclusion, modified fixed appliance therapy for the patients with cerebral palsy might be useful. Continuous rehabilitation training of lips should be followed after treatment to correct imbalance of muscle tone.

Changes of neurodevelopmental outcomes and risk factors of very low birth weight infants below 1,500 g, in the last 10 years (최근 10년간 1,500 g이하 극소 저출생 체중아의 신경학적 위험 요인 및 예후 변화에 관하여)

  • Purpose : As a result of advances in neonatal intensive care and perinatal care, neurodevelopmental outcomes of very low birth weight infant(VLBWIS) is expected to lead to improvement. The aim of this study was to report neurodevelopmental outcomes and risk factors of neurologic impairment of very low birth weight infants during the past 10 years. Method : We performed a retrospective study of 447 newborn infants below 1,500 gm admitted to neonatal intensive care unit of Taegu Fatima Hospital between Janury 1996 and December 2004. Infants were subdivided into group 1(Jan. 1996 to Dec. 1998), group 2(Jan. 1999 to Dec. 2001), and group3(Jan. 2002 to Dec. 2004). We analyzed epidemiologic data to study changes of neurodevelopmental outcomes and risk factors of neurologic impairment. Result : The incidence of cerebral palsy and developmental delay decreased significantly in periods 2 and 3(vs period 1; cerebral palsy 10 percent, developmental delay; 18 percent, P<0.05). Periventricular leukomalacia incidence decreased in period 3(vs period 1; 14.5 percent, P<0.05). The overall survival rate of VLBWIS increased significantly in period 2 and 3(vs period 1; 90.0 percent, P<0.05). The risk factors of neurologic impairment are long-term ventilator care(above 1 wk), low Apgar score, low gestational age and low birth weight. Conclusion : In the most recent 10 years, neurologic impairments of VLBWIS significantly decreased, as a result of advances in neonatal intensive care and perinatal care.

The Changes of Respiratory Functions Following Postures in Cerebral Palsy : Spastic Diplegia (뇌성마비의 유형별 자세에 따른 호흡기능의 변화 : 경직성 양하지마비)

  • The purpose of this study was to evaluate respiratory functions in relation to the gross motor functions(total value of GMFM), the difference of chest girth, and the changing position in spastic children. The respiratory functions(FVC, FEV1, $FEV1\%$, and PEF) were measured in the supine, the $45^{\circ}$semi-sitting, and the $45^{\circ}$sitting in 9 subjects. In the supine position, the mean difference of chest girth was $1.56{\pm}0.80cm$, the total value of GMFM was $45.41{\pm}17.79\%$. In the supine position, there was significant positive relationship in FVC-FEV1, FVC-PEF, and FEV1-PEF, but there was no significant relationship in GMFM and all respiratory functions. In the $45^{\circ}$semi-sitting, there was significant positive relationship in GMFM-FVC, FVC-FEV1, FVC-PEF, FEV1-PEF, and $FEV1\%-PEF$. In the $90^{\circ}$sitting, there was significant positive relationship in GMFM-FEV1, $GMFM-FEV1\%$, FVC-FEV1, FVC-PEF, and FEV1-PEF. In results of measured respiratory functions according to the postures, the supine position had highest value in all respiratory functions, but there were no significant (p<0.05).
