Use of Mouthguard for Prevention of Oral and Maxillofacial Injury (구강악안면영역의 외상방지를 위한 마우스가드의 사용)

  • With today's heightened interest in quality of life, leisure and sports activities were popular in the general public. Accordingly, the incidence of oral and maxillofacial injury are also rising. Use of a mouth protector to prevent the trauma of the oral and maxillofacial region is growing in importance, and among the mouth protector the mouthguard is the most commonly used. Mouthguard has been suggested to protect injuries by (1) preventing tooth injuries by absorbing and deflecting blows to the teeth; (2) shielding the lips, tongue, and gingival tissues from laceration; (3) preventing opposing teeth from coming into violent contact; (4) providing the mandible with resilient support, which absorbs an impact that might fracture the unsupported angle or condyle of the mandible; (5) preventing neck and cerebral brain injuries. Although mouthguard is effective for prevention of oral and maxillofacial injury, it is not widespread to athletes or general public and they are lack of awareness about the importance of mouthguard. We present the types and materials of mouthguard, things to consider when mouthguard fabrication, and the usage. This should be helpful in awareness about the importance and popularization of mouthguard.

Protecting Effects by Rooibos Tea against Immobilization Stress-induced Cellular Damage in Rat (흰 쥐의 고정화 스트레스에 대한 루이보스티의 방어 효과)

  • Stress will induce various changes in human metabolism. The remarkable phenomenon of these changes is increased energy metabolism that can induce many reactive oxygen species (ROS) production. ROS can peroxidize cellular macromolecules including lipid and protein. The object of this study was to investigate that stress may induce cellular damage by producing ROS and that Rooibos tea can protect cells against reactive oxygen species by immobilization stress in SD rat. The stress group significantly increased in 5-hydroxyindole acetic acid (5-HIAA), one of the stress hormone. Rooibos tea treatment had no effects on 5-HIAA contents, but body weight of Rooibos tea treated rat more increased than that of only the stress group. It was suggested that Rooibos tea colud not affect stress response itself, but protect against the another mechanism. We thought that the oxidative damage was caused by increased energy metabolism. Protein degradation level and lipid peroxide formation on index of oxidative damage significantly increased in the stress group. But the stress-induced activity change could not be observed in antioxidative enzymes such as superoxide dismutase, glutathione peroxidase and glutathione reductase. But the catalase activity of the brain significantly was inhibited by the stress. From these results, it was suggested that the immobilization stress induce the brain oxidative damage. However the oxidative damage was inhibited by feeding Rooibos tea containing various antioxidants, such as polyphenol, flavonoid and so on. Therefore, Rooibos tea have the protective effects against the stress caused by the ROS mediated cellular damage.

Functionality Analysis of Rhus javanica Fermented by Lactobacillus spp. (Lactobacillus spp. 이용 발효 붉나무의 기능성물질 검색에 대한 연구)

  • To determine the potential function of Rhus javanica in Korean medicine, it was fermented with each strain of Lactobacillus spp. Each strain of Lactobacillus spp. was inoculated in lactobacilli MRS broth, and 5 mg/ml of methanol extract of Rhus javanica was added. In mouse hippocampal HT22 cells, ethyl acetate extract of R. javanica fermented with L. brevis KCTC 3498 induced heme oxygenase-1 expression and showed a significant cytoprotective effect on glutamate-induced oxidative damage. The cytoprotective effect was related to the transcription of the nuclear factor E2-related factor2 (Nrf2), which is responsible for the induction of heme oxygenase-1 within the nucleus. The antimicrobial, antioxidant, and heme oxygenase-1 expression activities of fermented R. javanica were measured after extraction with ethyl acetate. R. javanica fermented with L. plantarum subsp. plantarum KCTC 3108, L. fermentum KCTC 3112, and L. brevis KCTC 3498 had higher antioxidant activity than nonfermented R. javanica. The fermented R. javanica with L. plantarum subsp. plantarum KCTC 3108, L. casei KCTC 3109 after ethyl acetate extraction showed antibacterial activity against Bacillus subtilis PCI 219, Escherichia coli KCTC 1682, Shigella flexneri KCTC 2517, Vibrio parahaemolyticus KCTC 7471, and Pseudomonas aeruginosa KCTC 2004. An ethyl acetate extract of the fermented R. javanica with Lactobacillus brevis KCTC 3498 exhibited stronger antibacterial activity than a nonfermented one against strains of B. subtilis PCI 219, E. coli KCTC 1682, S. flexneri KCTC 2517, and V. parahaemolyticus KCTC 7471.

Effect of Resveratrol on the Induction of Cdk Inhibitor p21 and Pro-apoptotic Bax Expression by amyloid-β in Astroglioma C6 Cells (신경교 세포에서 resveratrol이 amyloid-β에 의해 유도되는 Cdk inhibitor p21 및 Bax 발현의 감소 효과)

  • Resveratrol (3,4',5-trihydroxy-trans-stilbene), a phytoalexin found in grape skins, peanuts, and red wine, has been reported to have a wide range of biological and pharmacological properties. $Amyloid-\beta$ deposition and senile plaque-associated astrocytes are common neuropathological features of Alzheimer's disease. In this study, we have explored the effects of resveratrol on $amyloid-\beta-peptide-mediated$ cytotoxicity in vitro and modulation of cell growth-regulatory gene products in astroglioma C6 cells to elucidate its possible mechanism for anti-cytotoxicity. Exposure of C6 cells to $Amyloid-\beta$ resulted in dose-dependent growth inhibition and morphological changes of C6 cells, which were recovered by pre-treatment with resveratrol. The anti-proliferative effect of $amyloid-\beta$ was associated with the induction of tumor suppressor p53 and cyclin-dependent kinase (Cdk) inhibitor p21 (WAF1/CIP1) expression assessed by RT-PCR and Western blot analysis in time-dependent manner in C6 cells. In addition, the pro-apoptotic Bax expression was also up-regulated in $amyloid-\beta-treated$ C6 cells without alteration of anti-apoptotic Bcl-2 and $Bcl-X_L$ expression. However, pre-treatment of resveratrol significantly inhibited $amyloid-\beta-induced$ p53, p21 and Bax levels, suggesting that the modulation of p53, p21 and Bax levels could be one of the possible pathways by which resveratrol functions as anti-cytotoxic agent. Our results demonstrate that resveratrol may enhance the protection against $amyloid-\beta-induced$ cytotoxicity by promoting the survival of glial cells.

Protective effect of Codium fragile extract on fine dust (PM2.5)-induced toxicity in nasal cavity, lung, and brain cells (미세먼지(PM2.5)로 유도된 세포(비강, 폐, 뇌)독성에 대한 청각(Codium fragile)의 보호효과)

  • To evaluate the protective effect of Codium fragile on fine dust (PM2.5)-induced cytotoxicity, we investigated its antioxidant activity and cell protective effect on PM2.5-exposed cells. The 40% ethanolic extract of C. fragile showed the highest total phenolic content, whereas the water extract of C. fragile showed the highest total polysaccharide content. The protective effect of the extracts on PM2.5-induced oxidative damage in nasal cavity (RPMI2650), lung (A549), brain (MC-IXC), hippocampus (HT-22), and microglia (BV-2) cells was evaluated by measuring the intracellular reactive oxygen species (ROS) content and cell viability. The results showed that the 40% ethanolic extract more efficiently inhibited ROS production than the water extract. In contrast, PM2.5-exposed cells treated with the water extract showed higher viability than those treated with the 40% ethanolic extract.

Scalp Dose Evaluation According Radiation Therapy Technique of Whole Brain Radiation Therapy (전뇌 방사선치료 시 치료방법에 따른 두피선량평가)

  • Purpose: Opposing portal irradiation with helmet field shape that has been given to a patient with brain metastasis can cause excess dose in patient's scalp, resulting in hair loss. For this reason, this study is to quantitatively analyze scalp dose for effective prevention of hair loss by comparing opposing portal irradiation with scalp-shielding shape and tomotherapy designed to protect patient's scalp with conventional radiation therapy. Materials and Methods: Scalp dose was measured by using three therapies (HELMET, MLC, TOMO) after five thermo-luminescence dosimeters were positioned along center line of frontal lobe by using RANDO Phantom. Scalp dose and change in dose distribution were compared and analyzed with DVH after radiation therapy plan was made by using Radiation Treatment Planning System (Pinnacle3, Philips Medical System, USA) and 6 MV X-ray (Clinac 6EX, VARIAN, USA). Results: When surface dose of scalp by using thermo-luminescence dosimeters was measured, it was revealed that scalp dose decreased by average 87.44% at each point in MLC technique and that scalp dose decreased by average 88.03% at each point in TOMO compared with HELMET field therapy. In addition, when percentage of volume (V95%, V100%, V105% of prescribed dose) was calculated by using Dose Volume Histogram (DVH) in order to evaluate the existence or nonexistence of hotspot in scalp as to three therapies (HELMET, MLC, TOMO), it was revealed that MLC technique and TOMO plan had good dose coverage and did not have hot spot. Conclusion: Reducing hair loss of a patient who receives whole brain radiotherapy treatment can make a contribution to improve life quality of the patient. It is expected that making good use of opposing portal irradiation with scalp-shielding shape and tomotherapy to protect scalp of a patient based on this study will reduce hair loss of a patient.

Neuronal Cell Protective Effect of Dendropanax morbifera Extract against High Glucose-Induced Oxidative Stress (High Glucose로 유도된 산화 스트레스에 대한 황칠나무 잎 추출물의 뇌신경세포 보호 효과)

  • Antioxidant activities and neuroprotective effects of ethyl acetate fraction from Dendropanax morbifera (EFDM) against high glucose-induced oxidative stress and neurotoxicity were investigated to confirm their physiological activities. An 80% ethanolic extract of D. morbifera showed the highest contents of total phenolic compounds as well as 2,2'-azino-bis(3-ethylbenzothiazoline-6-sulfonic acid) and 1,1-diphenyl-2-picrylhydrazyl radical scavenging activities. The extract was fractionated using several solvents, and the ethyl acetate fraction showed the highest activities in ferric reducing/antioxidant power and malondialdehyde inhibitory assays. To evaluate the neuroprotective effect based on antioxidant activities, cell viability was assessed using PC12 and MC-IXC cells in $H_2O_2$- and high glucose-induced cytotoxic assays, respectively. EFDM evidently showed neuroprotective effects in all cells (neuron-like PC12 cells and human brain-originated neuroblastoma MC-IXC cells). Inhibitory effect of the extract on acetylcholinesterase (AChE) as an acetylcholine-hydrolyzing enzyme was performed to examine the effect on cognitive function. EFDM presented an AChE inhibitory effect. Finally, high-performance liquid chromatography analysis showed that the major phenolic compound of EFDM is probably a rutin.

Cognitive-enhancing Effects of a Fermented Milk Product, LHFM on Scopolamine-induced Amnesia (발효유 산물인 LHFM의 인지기능 개선 효과)

  • Probiotics and their products, such as yogurt and cheese have been widely consumed in many countries with proven health benefits including anti-microbial activity and anti-diarrheal activity. LHFM (Lactobacillus helveticus - fermented milk) is a processed skim milk powder, fermented by a probiotics, L. helveticus IDCC3801. In the present study, we aimed to investigate the neuroprotective effects and the cognitive improvements of LHFM. LHFM itself did not show any cytotoxicity to the human neuroblastoma cell line, SH-SY5Y; however, it dose-dependently protected against glutamate-induced neuronal cell death. LHFM also attenuated scopolamine-induced memory deficit in Y-maze and Morris-water maze. In the analysis of hippocampus after a behavior test, LHFM significantly increased the acetylcholine level and also inhibited acetylcholine esterase activity. Therefore, the raised acetylcholine release partially contributes to the improvement of learning and memory by a treatment with LHFM. These results suggest that LHFM is an effective material for prevention or improvement of cognitive impairments caused by neuronal cell damage and central cholinergic dysfunction.

Effect of Ischemic Preconditioning for Preventing Ischemic Injury of the Spinal Cord (척추 신경의 허혈성 손상 예방을 위한 허혈성 전처치의 효과)

  • Background: Paraplegia is a serious complication of thoracic or thoracoabdominal aortic operations, which is related to ischemic injury of the spinal cord induced by low perfusion pressure during cross clamping of the aorta. Ischemic preconditioning of heart or brain with reversible sublethal ischemic injury induces resistance to subsequent lethal ischemia. The aim of this study is to investigate whether ischemic tolerance could be induced by the preconditioning of the spinal cord using swine model. Material and Method: The animals were randomly assigned to three groups: sham group(n=3), control group(n=6) and pre-conditioning group(n=8). In the sham group, we performed the left thoracotomy only without any ischemic injury. In the preconditioning group, the swine received reversible spinal cord ischemic injury by aortic clamping for 20 minutes, whereas control group had no previous aortic cross- clamping. Forty-eight hours later, the aorta was clamped for 30 minutes in both groups. Neurological examination was done 24 hours later, then the animals were euthanized for histopathology and malonedialdehyde(MDA) spectrophotometry assay of the spinal cord. Result: Statistically significant difference in neurological outcome was observed between the control and preconditioning groups at 24 hours after ischemic injury. The incidence of paraplegia and severe paresis was 100% in the control group, and 62.5% in the preconditing group(p=0.028). There was no statistically significant difference in histopathology and MDA assay of the ischemic spinal cord between these two groups with borderline statistical difference in MDA assay(p=0.0745). Conclusion: In the present swine study, ischemic preconditioning could induce tolerance against 30 minute ischemic insult of the spinal cord, although the animals did not completely recover(stand-up or walk). We expect that combining this preconditioning with other currently existing protection methods might lead to a synergistic effect, which warrants further investigation.

Optimization of Total Arc Degree for Stereotactic Radiotherapy by Using Integral Biologically Effective Dose and Irradiated Volume (정위방사선치료 시 적분 생물학적 유효선량 및 방사선조사용적을 이용한 Total Arc Degree의 최적화)

  • Purpoe : To find the optimal values of total arc degree to protect the normal brain tissue from high dose radiation in stereotactic radiotherapy planning. Methods and Materials : With Xknife-3 planning system & 4 MV linear accelerator, the authors planned under various values of parameters. One isocenter, 12, 20, 30, 40, 50, and 60 mm of collimator diameters, $100^{\circ},\;200^{\circ},\;300^{\circ},\;400^{\circ}C,\;500^{\circ},\;600^{\circ}$ or total arc degrees, and $30^{\circ}\;or\;45^{\circ}$ or arc intervals were used. After the completion of planning, the plans were compared each other using $V_{50}$ (the volume of normal brain that is delivered high dose radiation) and integral biologically effective dose. Results : At $30^{\circ}$ of arc interval, the values of $V_{50}$ had the decreased pattern with the increase of total arc degree in any collimator diameter. At 45 arc interval, up to $400^{\circ}$ of total arc degree, the values of $ V_{50}$ decreased with the increase of total arc degree, but at $500^{\circ}\;and\;600^{\circ}$ of total arc degrees, the values increased. At $30^{\circ}$ of arc interval, integral biologically effective dose showed the decreased pattern with the increase of total arc degree in any collimator diameter. At $45^{\circ}$ arc interval with less than 40 mm collimator diameter, the integral biologically effective dose decreased with the increase of total arc degree, but with n and n mm or collimator diameters, up to $400^{\circ}$ or total arc degree, integral biologically effective dose decreased with the increase of total arc degree, but at $500^{\circ}\;and\;600^{\circ}$ of total arc degrees, the values increased. Conclusion : In the stereotactic radiotherapy planning for brain lesions, planning with $400^{\circ}$ of total arc degree is optimal. Especially, when the larger collimator more than 50 mm diameter should be used, the uses of $500^{\circ}\;and\;600^{\circ}$ of total arc degrees make the increase of$V_{50}$ and integral biologically effective dose. Therefore stereotactic radiotherapy planning using $400^{\circ}$ of total arc degree can increase the therapeutic ratio and produce the effective outcome in the management of personal and mechanical sources in radiotherapy department.
