노년기 건강문제는 의료적 관점에서뿐만 아니라, 사회경제적 지위와 같은 사회적 조건과의 상호작용 속에서 해법을 찾을 필요가 있다. 이러한 인식 하에, 본 연구에서는 사회경제적 지위가 노인의 건강불평등에 이르는 메커니즘을 파악하고자 하였다. 구체적으로는 사회경제적 지위(소득 및 교육)와 신체적 건강의 세 차원(의료적 건강/기능적 건강/주관적 건강) 간의 사이에서의 중간기제로서 건강행동, 의료시설 접근성, 사회참여, 사회적 관계망의 매개효과를 규명하는 것이다. 연구모형의 검증을 위해 2014년에 실시된 전국노인실태조사 데이터를 2차 분석하였다. 본 연구의 대상은 65세 이상 남녀 노인 10,451명으로, 매개모형 검증을 위해 Baron & Kenny(1986)가 제안하는 절차에 따라 위계적 다중회귀분석을 실시하였고 매개효과의 유의성 검증을 위해 Sobel test를 실시하였다. 분석결과, 노인의 소득수준과 교육수준의 건강에 대한 효과가 동일하지는 않았고 건강의 하위차원에 따라서도 다소 다른 결과가 도출되기는 했지만, 전반적인 결과의 방향은 노인의 사회경제적 지위는 건강격차를 야기하고 있었으며, 그 메커니즘은 건강행동, 의료시설 접근성, 사회참여, 사회적 관계망을 매개한다는 것이다. 특히 본 연구에서 교육은 소득보다도 건강에 대한 보다 강력한 효과를 나타내었다는 점, 사회참여와 같은 사회통합요인의 효과가 부각되었다는 점, 거주지역을 통제한 이후에도 잔존한 의료시설 접근성의 유의미한 매개효과 등은 본 연구의 결과 중에서도 주목할 만하다. 이러한 결과를 바탕으로 사회경제적 조건으로 인한 건강불평등의 메커니즘을 규명하고 이를 완화시킬 수 있는 대안을 모색할 수 있었다는 점에서 본 연구의 의의가 있다.

본 연구는 지역사회 재가노인을 대상으로 사전연명의료의향서(AD) 작성의도 영향 요인을 파악하기 위한 서술적 조사연구이다. 본 연구대상은 D 광역시에 거주하며 노인복지관을 이용하는 65세 이상의 남녀 노인 196명이다. 자료수집기간은 2016년 12월 8일에서 12월 19일까지였다. 수집된 자료는 SPSS 22.0 version을 이용하여, 서술통계, ${\chi}^2-test$, t-test, Logistic regression을 이용하여 분석하였다. 본 연구결과, 대상자 중 AD 작성 의도를 가진 경우는 58.2%였다. AD 작성의도에 따라 연령(${\chi}^2=4.92$, p=.020), 독거상태 (${\chi}^2=4.72$, p=.030 ), 주관적 건강상태(${\chi}^2=3.97$, p=.046), AD에 대한 인식 (t=-4.81, p<.001) 및 죽음에 대한 부정적 태도(t=2.01, p=.045)에서 통계적으로 유의한 차이가 나타났다. 회귀분석결과, AD 작성의도에 유의한 영향요인은 AD에 대한 인식으로 나타났다 (OR=11.87, p<.001, 95% CI=3.52~39.94). 따라서 노인의 AD에 대한 긍정적인 인식이 AD 작성 의도의 주요 예측변수임을 알 수 있다.

The purpose of this study is to grasp the design characteristics of welfare facilities for the aged in Japan in order to establish the basic data for such facilities in Korea. In conclusion of analysis, the design characteristics of welfare facilities for the aged can be summarized as follows; the dignity of the individual, the privacy, the small group units, the sociopsychological support, the succession of regional and cultural factors, the nature, the safety, the interior materials for the old men, the improvement of staves' environment, the energy reduction and earth environment. The aged welfare equipments in Japan are her products which reflect the society and culture of Japan, therefore our efforts to convert them to equipments which are suitable to our culture and emotion are desired urgently.

Being rapidly increased in population of the Elderly supporting of the Elderly is no longer family's and personal problems. It is social issue that demand measures by the government. Though many kinds of welfare policies for the Elderly have been established, it is not sufficient both in quality and quantity. In order to support residents of region, welfare facilities which based on community welfare have been supplied. This study is targeted on health facility for the elderly of Japan. First purpose, it is to acheive basic information about health facility. Second purpose of this study is to basic data for planning of facility.

This study has been started in order to provide basic information for the planning of Korean elderly care facilities by analysing For-profit elderly homes of Japan. Japan is famous for her high elderly proportion and also her fast aging speed. Therefore, Japanese experiences of trials and errors related to the supply of elderly homes will be a good guideline for Korea which is undergoing the sharp increase in elderly population. Data have been mainly collected from the official statistics of Japan and Korea. The outcomes of this study are as follows. At first, the definition of For-profit elderly homes of Japan has been proposed clearly. Secondly, the trends of them related to supply and operational characteristics have been analysed. Thirdly, the size and utilization rate of them have been analysed. And finally, the physical characteristics of elderly residences in For-profit elderly homes of Japan have been discussed.
