Relevance between Geriatric Oral health Assessment (GOHAI) and Falls : Using KLoSA 2018 7th Data (노인구강건강평가지수(GOHAI)와 낙상 경험의 관련성: 고령화연구패널조사(KLoSA) 2018년도 제7차 자료를 이용하여)

  • Purpose: This study is based on the results of previous studies that falls, which is a frequent social problem, causes physical, mental and social health problems in the elderly. In order to identify risk factors for falls in the elderly, this study derives a relationship with oral health and proposes the necessity of oral health care as part of countermeasures to prevent falls in the elderly. Methodology: The final 6,936 people were analysed using the 2018 Korean Longitudinal Study of Ageing(KLoSA) survey data, and the analysis was conducted using the multiple logistic and multiple regression analysis to investigate the relationship between Geriatric Oral Health Assessment Index (GOHAI) and falls experience. Findings: As a result of the analysis in this study, the number of falls experiences and whether or not falls were higher in the elderly female group than in the male group. The likelihood of falls experience has been statistically reduced as Geriatric Oral Health Assessment Index increases by one unit.(OR : 0.991) The number of falls has also been statistically reduced as Geriatric Oral Health Assessment Index increase by one unit.(B : -0.001) Practical Implications: The results of this study, which showed that higher Geriatric Oral Health Assessment Index was lower risk of falls, explained by the connection that oral health may affect nutritional intake, which leads to sarcopenia and physical loss, which in turn increases the risk of falls. As a way to solve the fall problem, efforts should be made to improve the oral health of the elderly, and furthermore, the importance of nutrition management thorugh oral health care of the elderly is increasd.

A Study on the Relationships of Oral Status and Self-perceived Oral Health Status in Elderly (일부 노인의 구강내 상태와 자가인지 구강건강상태와의 관련성)

  • Purpose: The recent proliferation of the aging population, oral health care is closely related to the mental health of the elderly is very important being recognized. Provide basic data for improving the quality of life for the elderly, to determine whether the oral health of the elderly whether oral self-perception of health and any related wish. Methods: Surveys to May 20 from April 15, 2011, subjects were elderly and living in Daejeon and Chungnam 277 people were examined. All the statistics using SPSS 18.0 for the determination of statistical significance, and the significance level was 0.05. Results: 1. Association between masticatory ability, oral health-related quality of life and cognitive function in the elderly population using structural equation modeling (구조방정식을 이용한 대도시 일부 노인들의 저작능력 및 구강건강관련 삶의 질과 인지기능)

  • Objectives: The aim of this study was to investigate the association between masticatory ability, oral health-related quality of life, and cognitive function in an elderly population using structural equation modeling. Methods: A total of 308 subjects, aged 65 years and over, were recruited from the senior citizen center in Daegu, South Korea and provided consent for inclusion in the study. The Korean version of the Mini-Mental State Examination (K-MMSE) was used to assess the level of cognitive function. Masticatory ability was evaluated through chewing tests, patient-perceived masticatory function and, measurement of relative occlusal forces on molar teeth using the T-Scan $III^{(R)}$ system. All data were analyzed using ${\chi}^2$ tests, t-tests, and one-way ANOVA in SPSS version 23.0 for windows. Structural equation modeling was performed using SPSS AMOS version 22.0. Results: There was a significant association between cognitive function and molar teeth occlusal force. Ability of food chewing score and patient-perceived masticatory function could affect oral health-related quality of life (P>0.05). Higher occlusal forces on molar teeth, compared to anterior teeth, positively correlated with higher cognitive function. Conclusions: These results suggest that an individual's oral health condition could affect molar teeth occlusal forces and may be related to a risk of developing dementia. Therefore, there is a need for implementing nation-wide policies to improve oral health, such as masticatory function, in the elderly population.

Oral health status of institutionalized elderly in Korea (노인장기요양시설 입소노인의 구강건강실태와 시사점)

  • Purpose: The institutionalized elderly have placed the most vulnerable state, and the report about the oral health status of the institutionalized elderly was scarce. The aims of the current study were to investigate the oral health status of institutionalized elderly. Materials and methods: The survey of the oral health status was carried out on 487 institutionalized elderly from 4 longterm care facilities of Seoul metropolitan city, Gyunggi province, and Gangwon province in Korea. Results: The prevalence of dental caries, root caries, and dental calculus of the institutionalized elderly was 19.2%, 15.3%, and 23.7%, respectively. The percentage of edentulism among the institutionalized elderly was 26.2%. Those who had poor oral hygiene and tongue coating were 43.9% and 90.5%, respectively. The percentages wearing complete and removable partial denture on maxilla were 12.8% and 3.3%, respectively. The percentages wearing complete and removable partial denture on mandible were 7.8% and 5.6%, respectively. The percentages of institutionalized elderly needing complete and removable partial denture on maxilla were 29.7% and 27.2%, respectively. The percentages of institutionalized elderly needing complete and removable partial denture on mandible were 24.5% and 30.9%, respectively. Conclusion: The oral health stuatus of institutionalized elderly was poor. For the oral health promotion of elderly in longterm care facilities, it is essential to educate nurses and caregivers about the importance of the oral health and oral hygiene method.

Comparison of Oral Health Related Living Quality of the Elder with Physical Debilities & Ones Capable of Living at Homes (Using GOHAI Grouping) (거동불편노인과 자가 활동 가능노인의 구강건강관련 삶의 질 관련 비교 - GOHAI 군집화를 활용하여 -)

  • Purpose: This study conducted a survey on the elderly of 65 years and over with physical debilities and ones capable of living at home residing in Jeju Special Self-Government Province to compare and analyze demo-sociological characteristics and factors influencing on oral health related living quality. And also this study intended to provide basic data for developing effective public medical policies and health promotion programs to increase oral health related living quality of the elder. Methods: The elderly of 65 years and over living in Jeju Special Self-Government Province were interviewed individually from 7 February 2011 to 18 April and interview results of a total of 220 subjects were analysed for this study. Results: Current status of the elderly including ones with & without physical debilities influenced functional limitation and behavioral aspects of GOHAI criteria used in this study. The elderly with physical debilities experienced less limitation in food chewing and swallowing, and pronunciation than ones capable of living at home. On the analogy of the previous study(by Park, N. G., 2010) in which oral health related quality and satisfaction of life of the elderly with physical debilities were different due to their physical, circumstantial and psychological limitations, the former's oral health conditions are worse than the ones capable of living at home and because of medical care accessability limitation they suffer from deteriorated oral condition. By the comparison of factors influencing on the living quality relating to the oral health of the elderly with physical debilities and ones capable of living at home, 2 factors, age and living area, were meaningful factors commonly influencing on the oral health related living qualities of both. The elderly of 75 years and over were more affected by psychological and behavioral aspects of oral health related living quality than the ones of 65-74 years, and the ones living in country suffered from functional limitations, pains and discomfort more than ones in city. Additionally, being different from the elderly capable of living at home, the ones with physical debilities were influenced by the factors of average monthly income and medical security type. Conclusion: Improvement of programs and systems to increase oral health related quality of life needs to be carried out preferentially for the elderly of 75 years and over, and dwelling in country. Also this study suggests that the policy of paying the denture insurance allowance in 2012 need effective planning considering the elderly's current status, age, living area, medical security type.

Association between the self-reported periodontal health status and oral health-related quality of life among elderly Koreans (한국노인의 자가보고 치주건강상태와 구강건강관련 삶의 질의 연관성)

  • Purpose: This study assessed the impact of self-reported periodontal health on the oral health-related quality of life among elderly Koreans. Methods: Four hundred twenty one elderly Koreans in Seoul and suburban areas were selected with a cluster (institution) sampling method, and were requested to take oral examinations and finish questionnaires on the Oral Health Impact Profile-14(OHIP-14). and self-reported periodontal health status, such as periodontal symptoms, self-rated periodontal health and periodontal treatment need. As the dependent variable, OHIP-14 showed a positive skewed distribution (skewness: 1.17), we transformed to square-root form to apply parametric analyses. Bivariate analysis by t-test and ANOVA, and multivariate analysis with the two-level regression model accounting clusters were implemented. Results: Mean age of the subjects was 74.6 years and 66.5% were women. Fourteen items of OHIP-14 were summarized to one factor explaining 78.6% of total variance and produced the Chronbach alpha coefficient of 0.92. Results from the multivariate model, adjusting for age, sex, type of institutions, ability to pay, and number of teeth present, showed significantly lower OHIP-14 with reporting less than 3 periodontal symptoms (p(O.OOO1), rating their own periodontal health as above average level (p=O.0144), and thinking they don't need any periodontal treatments in the near future (p=O.0148), than their counterparts. The intraclass-corrrelation estimated by the final model was 0.028. Conclusion: This study demonstrates a significant association between self-reported periodontal health status and the oral health-related quality of life.

Factors Influencing the Dental Health and Living Quality of the Elderly With Physical Debilities (거동불편노인의 구강건강과 삶의 질에 영향을 미치는 요인)

  • Purpose: This study conducted a survey on the elderly with physical debilities, who are in a medically vulnerable social class, to examine closely their demo-sociological characteristics, unmet needs, dental states, and living qualities and satisfaction levels relating to oral health and social supports to them, and also to prepare the basis for effective public medical policies and health improvement programs aimed at improving the quality of life for the elderly with physical debilities. Methods: Twenty-two elderly care facilities within Jeju Special Self-Government Province participated in the survey. Between 11 January and 5 March 2010, a total of 250 elderly persons(65 and over) with physical debilities were interviewed and their dental health was checked. Results: The results of the survey are as follows. The need for social support for dental care of the elderly with physical debilities was high in the medical institution-supported service (49.6%). The unmet needs for physical care were high in bathing (49.6%) and using public transportation (71.6%). More than half of these surveyed had ten or fewer teeth. The survey found that 31.6% of the participants experienced problems eating, due to poor dental health. Concerning quality of life, 30.5% of those surveyed experienced physical pain. Conclusion: In summary, the ages of the survey participants directly relates to the degree of behavioral debility experienced. The more debility a participant exhibits, the greater is the need for social support and dental care. The dental health of a participant directly relates to a higher quality of life. Good dental health of a participant translate to better quality of life. In light of the fact that the elderly with physical debilities suffer from a lack of accessibility to medical care and worse oral health than do other elderly persons, it is essential to increase accessibility to medical institutions that can provide such services as door to door dental care. Current insurance policies, funding for denture insurance, and free denture and denture-upgrade programs desperately need to be expanded. Therefore, to improve effectively the quality of life for the elderly with physical debilities civil dental medical resources should be encouraged to provide inclusive and prevention-focused medical care. In the public domain, door to door dental care services and cooperation with civil dental care resources need to be improved to increase impartial accessibility to dental medical institutions.
