Review of Balance in Elderly by Vestivulo-ocular Reflex (안뜰눈반사를 통한 노인 균형 연구)

  • The purpose of this paper is to review vestibulo-ocular reflex and vestibular rehabilitation for balace problem in elderly. Falling is a common problem associated with aging. Falling and the resulting consequences constitutes one of the most common and serious problems. Balance deficits are one of the top risk factors for falling. Three systems(visual, vestibular and somatosensory) need to be integrated and used to coordinate and control balance. During normal ambulation and head movement, the vestibulo-ocular reflex(VOR) stabilizes gaze, and helps to keep the image of the retina. There is a direct association between deficits in VOR and falls. Older adults have a decreased ability to enhance and suppress the VOR. Vestibular rehabilitation(VR) is an exercise-based group of approaches that began with the aim of central nervous system compensation. VR restores VOR gain and reduces error using repetitive movements of the head and eyes. Many researches showd balance and gait tests were superior in the VR group. It is considered this article helps to prevent elderly falling with vestibular rehabilitation.

  • 일반적으로 출생 후 1세 전후의 영유아들은 신체의 발달이 온전치 않아 수면중 질식사하는 영유아 돌연사증후군(SIDS)에 이를 수 있어, 보호자의 세심한 관심이 필요하다. 더욱이 보호자가 있더라도 오히려 더 높은 질식에 의한 사망률이 늘어나고 있다. 이를 위해서, 본 논문에서는 수면시 발생할 수 있는 질식사 방지를 위한 하드웨어 장치와 이와 연계된 스마트폰 앱(App) 시스템을 설계하고 구현한다. 제안된 시스템으로써 하드웨어 장치는 초음파 거리 센서(sonsor)를 이용하여 실시간으로 뒤집힘 여부의 정보를 수집하여 무선 통신을 통해 스마트폰 앱(App)에 전달한다. 또한 스마트폰 앱 시스템은 수집된 정보를 분석 판단하여, 비정상적인 상태의 영유아에 대해 긴급 경보와 긴급 전화를 자동 호출한다. 따라서 여러 위험에 노출되어 있는 영유아들의 수면중에 뒤척이다 엎드려 자는 경우, 호흡기를 막거나 가슴압박으로 인한 질식사를 미연에 방지한다. 향후, 본 논문에서 제안된 시스템은 졸음방지나 낙상방지시스템 같은 유비쿼터스 헬스케어(ubiquitous healthcare) 환경 구현에도 적용될 수 있다.

  • In recent years, the importance of the characterization of fall for a fracture prevention system keeps increasing since fracture from a fall can lead to serious health problems. Fall is one of the major sources which increase morbidity in elderly people. In terms of the cost and the influence to the quality of life, the most serious injury with hip fractures is caused by falls. The traditional methods in characterizing fall patterns have been mainly by the epidemiological surveys. With surveys, the exact data of fall patterns can not been acquired. In this paper, we measured and analyzed with the parameters related to fall pattern such as velocities and accelerations during the motion of falls using 3D motion capture program. We acquired the parameters of the fall pattern of intentional and unexpected fall. The result showed that the variation of velocity and acceleration during fall was very important in characterizing fall pattern, which of vital importance for the development of a fracture prevention system and for the safety of the elderly

Association between dynamic balance and ankle muscle characteristics (동적 균형과 발목 근육 특성 사이의 관계)

  • The relationship between balance and ankle muscle characteristics (strength and flexibility) may be important to identify those at an increased risk of falling and to develop fall-prevention training programs. The association between ankle muscle characteristics (strength and flexibility) and balance has not previously been studied. The purpose of this study assessed the relationship between ankle muscle characteristics and balance. Sixteen healthy participants volunteered to participate in the study. Dynamic balance measured using Y-balance kit. Ankle muscle characteristics (strength and flexibility) measured using hand-held dynamometer and goniometry. The results indicated a positive correlation between ankle invertor muscle strength and Y-balance test performance. Clinicians should consider ankle muscle strength during therapeutic interventions to improve dynamic balance.

Effects of Biofeedback Training for Prevention of Falling in Elderly Persons (노인의 낙상방지를 위한 바이오피드백 훈련의 효과)

  • Background: The purpose of this study was to investigate effects of six-week biofeedback training for prevention of falling in elderly persons. Biofeedback training for using the TETRAX system. Methods: Thirty healthy elderly persons(men=17, women=13) who were 79 years of mean age participated in sensory regulation training. They were trained for thirty minutes a day, three times per week. We measured subjects sensory regulated function by TETRAX system, and analyzed mean difference of observed variables by paired t-test between the pre and post test. Results: The first experimental group were significantly difference between pre and post test. The second control group were not significantly between the pre and post test. The third there are significant between group. Conclusion: The results of this study reveal that biofeedback training exercise will improve sensory balance function, and further studies needs to identify which specific factors are related to fall in the elderly population, and it is expected this study may contribute in reducing fall and therapeutic exercise in falling.

Development of block-type sidewalk pavement system using snow-melting system (융설시스템을 이용한 조립식 보도포장 기술 개발)

  • Snow-melting system has been applied not only to roads for car traffic but also to pavement for the pedestrians safety reason in some of the developed countries such as USA and Canada based on countermeasures against Natural Disasters Act revised in 2000. Even though this system was introduced in korea in 2006 and has been partly applied to car traffic roads, there is few places that the system has been applied. Therefore, in this research a snow-melting system with a block-type to cover a pavement that efficiently transfers heat form heat rays to the top of a pavement and protects the heat rays. A quality check showed that compression and bending strength was improved approximately 5 times stronger and 7 to 10 times more absorption rate than the KS(Korea Industrial Standard) requirement. Moreover, only 10 minute was required to increase temperature above zero with a block-type snow-melting system whereas approximately 180 minute was spent with the existing system. This research is expected to contribute to environmental issues and reduce accidents on a slippery road.

Effect of Walk Training on Physical Fitness for Prevention in A home Bound Elderly (걷기 훈련이 재택 노인의 낙상방지 체력에 미치는 영향)

  • The purpose of this study was to determine the effect of walk training on leg strength, flexibility, postural stability, balance and gait in home bound elderly women. Eighteen elderly women of the experimental group aged between 70 and 90 years image who have normal vision, hearing and Romberg test. They participated in the 12 week walk training. The subjects of the experimental group practiced walk training 3 times a week for during 12 weeks. During the 40 minute workout, the subjects practiced 5 minutes of warming-up exercises, 30 minutes of conditioning exercises and 10 minutes of a cool-down exercise. The intensity for the conditioning phase was determined by subject' heart rates, which ranged from 60% to 70% of age-adjusted maximum heart rates. The body composition, leg strength, flexibility, postural stability, balance and gait were measured prior to and after the experimental treatment. The body fat, lean body mass, leg strength (ankle dorsiflexor, plantarflexor, inversor and eversir, knee flexor, extensior), flexibility (range of motion of ankle dorsiflexion, plantarflexion, inversion and eversion), and postural stability of the experimental group were significantly greater than those of the control group. Duration of standing on the right foot and that of standing on the left foot of the experimental group was greater than that of the control group. Total balance scores of the experimental group were significantly higher than those of the control group. Among 13 items for balance, the scores of experimental group in balance with eyes closes, turning balance, sternal nudge, neck turning, one leg standing balance and back extension were higher than those of the control group. Total scores of gait of the experimental group were significantly higher than those of the control group following the walking training. Scores of experimental group in step height, step length and walk stance while walking among 9 items for gait were significantly higher than those of the control group. The results suggest that walk training can improve physical fitness for prevention in home bound elderly women.

  • This paper suggests detection system for the movement of patient on bed based on IEEE802.15.4 by using pressure pad and guard sensor. The system is designed to detect ordinary activities of patients on bed as well as patients' falling from the bed while sleeping at night. The node that is installed at bed sends data to gather when the pressure pad and sensor of guard detect patients' activities and falling. These data sent to gather are transmitted to monitor at help desk by TCP/IP communication. To remove unnecessary data that occurred due to switch chattering during tossing and turning, timer of MCU is used. Also, Communication module can change transmission power to apply this system to various environments of hospital room. Therefore, the nurse can take care of patients on bed in real time with data about patients' conditions.

The Effects of Coordinative Locomotor Training Program for Life-Care Promotion on Balance of Obese Elderly Women (라이프케어 증진을 위한 협응적 이동훈련 프로그램이 비만 여성노인의 균형에 미치는 영향)

  • The purpose of this study was to examine the effects of coordinative locomotor training (CLT) program on the balance for the promotion of life care of elderly obese women. Ten participants of elderly obese women who were able to walk independently without surgery experience of lumbar, hip or knee joint within the past year were recruited and under went the pretest, CLT (20 sessions), followed by the post-test. The test included BMI test using In-body, joint kinematics using myoVIDEO, muscle activation using surface EMG test (erector spinae (ER), external oblique abdominalis (EO), quadriceps femoris (Quad), hamstring muscle (Ham)) and balance tests including dynamic balance test using forced treadmill, Berg balance scale (BBS) and timed up go (TUG). The CLT program was conducted 60 minutes a day, 5 days a week, over 4 weeks period. As a result of this study, The the trunk and hip joints kinematics during the stance and swing phases of gait were a statistical significance levels were set at p <0.05. The ER and EO muscle activation were significantly improved after intervention (p <0.05). The length of gait line and single support line of change of center of pressure (COP) were significantly increased after intervention (p <0.05). The BBS and TUG were also significantly enhanced after intervention (p <0.05). The results of this study showed that CLT program for the improvement of life care had significant effects on improving postural instability, muscle weakness, reduced balance ability and falling risk of obese elderly women. Therefore, it is recommended to apply CLT program to improve life-care through improving balance ability and preventing fall of obese elderly women.

Navigation Trajectory Control of Security Robots to Restrict Access to Potential Falling Accident Areas for the Elderly (노약자의 낙상가능지역 진입방지를 위한 보안로봇의 주행경로제어)

  • One of the goals in the field of mobile robotics is the development of personal service robots for the elderly which behave in populated environments. In this paper, we describe a security robot system and ongoing research results that minimize the risk of the elderly and the infirm to access an area to enter restricted areas with high potential for falls, such as stairs, steps, and wet floors. The proposed robot system surveys a potential falling area with an equipped laser scanner sensor. When it detects walking in elderly or infirm patients who in restricted areas, the robot calculates the velocity vector, plans its own path to forestall the patient in order to prevent them from heading to the restricted area and starts to move along the estimated trajectory. The walking human is assumed to be a point-object and projected onto a scanning plane to form a geometrical constraint equation that provides position data of the human based on the kinematics of the mobile robot. While moving, the robot continues these processes in order to adapt to the changing situation. After arriving at an opposite position to the human's walking direction, the robot advises them to change course. The simulation and experimental results of estimating and tracking of the human in the wrong direction with the mobile robot are presented.
