Risk Factors Associated with Cataract and Macular degeneration by in Korean aged 60 years and over (한국인 60세 이상 노년층의 황반변성과 백내장의 위험요인 -국민건강보험공단 노인코호트 자료를 활용하여-)

  • Purpose: This study was conducted to identify factors associated with cataract and age-related macular degeneration in elders. Methods: In total, 341,588 men(44.18%,) and women(61.887%), aged over 60, were recruited from Korean National Health Insurance Service-Senior(2002-2013). We also analysed the factors which determine the prevalence of contract (ICD-10: H25) and age-related macular degeneration(ICD-10: H353) using Cox proportional hazard regression model Results: The subjects who were women, in older age, the group of higher income level, with hypertension, with heart disease, and with diabetes, the prevalence of both contract and age-related macular were increased(p<0.0001). Conclusion: The prevalence of contract and age-related macular degeneration were higher in old age of Korean who has chronic diseases such as hypertension, heart disease, and diabetes. The management of chronic diseases are essentially required in elderly for more healthy eye in aged society.

  • The aim of this study was to evaluate the correlation between senile eye diseases such as age-related macular degeneration (AMD), glaucoma and oral health behaviors. Data from the Seventh Korea National Health and Nutrition Examination Survey (KNHANES VII-2) were used to analyze oral health behaviors according to the presence of AMD and glaucoma. Demographics and oral behaviors were analyzed using the complex chi-square test and complex logistic regression to compare participants with AMD and glaucoma with those without AMD and glaucoma. According to the presence or absence of AMD and glaucoma, there were statistically significant differences in age, education level, oral care product use, and difficulty in speaking variables. The results of this study provide evidence of a significant association in some variables between oral behaviors and senile eye diseases.

  • Object : The purpose of this study is to understand age-related maculardegeneration(AMD) with both western and Korean medicine.Methods : We investigated the comprehension of general AMD degenation in both western and Korean medicine through literature review.Results : The results are as follows.1. AMD prevalent increasing as the population ages; however, treatment options remain limited and incompletely defined.2. Generally macular degeneration has been affected by aging and is associate with the function of kidney(腎) and liver(肝) in Korean medicine.Conclusion : Further studies are needed to apply comprehension of AMD in Korean medicine to clinical stage.

Long-term Results of Taking Anti-oxidant Nutritional Supplement in Intermediate Age-related Macular Degeneration (중기 나이관련황반변성 환자에서 항산화영양제 복용 후 장기 관찰 결과)

  • Purpose: We prospectively investigated clinical changes and long-term outcomes after administration of the drugs recommended by the Age-Related Eye Disease Study-2 to patients with intermediate age-related macular degeneration (AMD). Methods: This prospective multicenter study enrolled 79 eyes of 55 patients taking lutein and zeaxanthin. The primary endpoint was contrast sensitivity; this was checked every 12 months for a total of 36 months after treatment commenced. The secondary endpoints were visual acuity, central macular thickness, and drusen volume; the latter two parameters were assessed using spectral domain optical coherence tomography. Results: The mean patient age was $72.46{\pm}7.16years$. Contrast sensitivity gradually improved at both three and six cycles per degree. The corrected visual acuity was $0.13{\pm}0.14logMAR$ and did not change significantly over the 36 months. Neither the central macular thickness nor drusen volume changed significantly. Conclusions: Contrast sensitivity markedly improved after treatment, improving vision and patient satisfaction. Visual acuity, central retinal thickness, and drusen volume did not deteriorate. Therefore, progression of AMD and visual function deterioration were halted.

Intravitreal Anti-vascular Endothelial Growth Factor Injections to Treat Neovascular Age-related Macular Degeneration: Long-term Treatment Outcomes (삼출 나이관련황반변성에 대한 항혈관내피성장인자 유리체내주사 치료의 장기 임상 결과)

  • Purpose: We assessed the visual and anatomical outcomes, and the safety profile of long-term intravitreal anti-vascular endothelial growth factor (VEGF) injections (aflibercept, ranibizumab, and bevacizumab) given to treat neovascular age-related macular degeneration (NAMD). Methods: We analyzed medical records collected over 7 years of treatment-naive NAMD patients who received outpatient clinic-based intravitreal anti-VEGF injections. All were treated employing either "treat-and-extend" or "as needed" protocols at the discretion of the retinal specialist. The number of injections, adverse events associated with injection, and measures of visual acuity (VA), central foveal thickness (CFT), and intraocular pressure (IOP) were recorded. Results: Overall, we assessed 196 eyes of 196 patients (average age $68.6{\pm}9.6years$; 77 females). Patients received an average of $17.3{\pm}13.5$ injections over $78.0{\pm}16.5months$ of clinical follow-up. The initial mean VA (logMAR) was $0.75{\pm}0.58$ and the CFT was $349.7{\pm}152.6{\mu}m$. Both parameters exhibited maximal improvements at the 6-month visit (p < 0.05). However, the clinical outcomes worsened over the 7-year clinical course; the best-corrected visual acuity (BCVA) was $0.91{\pm}0.78$ and the CFT was $284.5{\pm}105.8{\mu}m$ at 7 years. The BCVA at 7 years was significantly correlated with the initial BCVA. IOP-related events increased 11-fold and anterior chamber reactions increased 3-fold over the years, but no significant complications such as endophthalmitis were recorded. Conclusions: The use of intravitreal anti-VEGF agents was associated with initial visual improvements over 6 months but did not prevent the worsening of NAMD over 5 years. The BCVA at the initial visit was a strong predictor of the final BCVA. A more intensive injection schedule might improve long-term outcomes.

Study on Relationship between Eye Health and Household Income of the Elderly (노인의 가구 소득과 눈 건강과의 관계)

  • Purpose: In this study, eye-health inequity was investigated by analyzing the relationship between household incomes and eye-health of senior citizens. Further, this study suggested the preliminary data for establishment of public eye-health policy in order to improve low income senior citizens' life quality. Methods: The data from the 2009 Survey of Korea National Health and Nutrition Examination were analyzed in this study. The objectives of the KNHNE survey were over 65 year old group (1,668 people). Main factors of eye-health (visual acuity, cataract, pterygium, intraocular pressure, retinophathy, age-related macular degeneration, diabetic retinophathy, myopia, hyperopia, astigmatism, and anisometropia prevalence) were analyzed with t-test and chi square test. Results: Low income group revealed that refractive error rate and intraocular pressure were low, however, naked eye visual acuity and corrected visual acuity were high at 0.1 to less than 0.5. On the other hands, in the high income group, there was high prevalence of hyperopia. Cataract mainly occurred at low income group besides group which maximum corrected visual acuity was below 0.8 also highly showed cataract. Moreover, the prevalence of cataract showed that it related with smoking, drinking, occupation, and education level. Conclusions: Results revealed that there was inequity of eye-health which related with socioeconomic status of the elderly. Especially, the prevalence of cataract was correlated with life quality. Consequently, establishment of public eye-health policy seems to be required for eye-health inequity of low income senior citizens.
