Effects of Intensive Clinical Training for Nursing Students in Nursing Practice on their Clinical Competence, State Anxiety, and Clinical Practice Stress (간호학생의 밀착형 실무진입 임상실습교육이 임상수행능력, 상태불안 및 임상실습 스트레스에 미치는 효과)

  • Purpose: The purpose of this study was to examine the effects of a intensive clinical training program for nursing practice on the clinical competence, state anxiety, clinical practice stress in nursing students who were beginning their first clinical practice. Methods: This practice has been processed as a 'Pre-Post design with a non-equivalent control group'. The participants were 177 students in a college of nursing. The students were divided into two groups: 89 students in the experimental group and 88 students in the control group. Results: The experimental group showed significant improvement in their clinical competence scores compared to the control group. The difference between the two groups was also significant. State anxiety in the experimental group significantly decreased compared to the control group. However, scores for stress during practice were not significantly different. Conclusion: The findings in this research reveal that the program, 'Close clinical practice training for nursing students' practice entry, contributes to improving students' overall ability and performance, and reduces anxiety of nursing students as they start their first clinical practice. These findings can be utilized in helping nursing students improve the quality of their performance.

Influence of Professional Self-concept and Self-leadership on Clinical Competence in Nursing Students (간호대학생의 전문직 자아개념과 셀프리더십이 임상수행능력에 미치는 영향)

  • Purpose: This study was done in order to identify nursing students' professional self-concept, self-leadership and clinical competence and to analyze the correlation among the variables and the factors influencing clinical competence. Methods: The research participants were 294 senior nursing students in the nursing departments of 3 universities located in Jeollabuk-do. The students had completed 3 semesters of clinical practice. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation coefficient, and Multiple Regression. Results: Participants' scores for professional self-concept, self-leadership, clinical competence were $2.78{\pm}0.36$, $3.63{\pm}0.47$, $3.80{\pm}0.40$ respectively. Professional self-concept, self-leadership and clinical competence had positive correlations. Factors influencing nursing students' clinical competence included professional self-concepts in professional practice, self-expectations in self-leadership, constructive thinking, self-compensation and self-criticism in that order, and these variables explained 48%of the variance in clinical competence. Conclusion: Based on these results, it is important to develop and apply educational programs to increase professional self-concept and self-leadership in order to improve nursing students' clinical competence.

Effects of OSCE Method on Clinical Skills Performance of Nursing Students (OSCE 평가방법이 기본간호 수행능력에 미치는 효과 - 고열 대상자를 중심으로 -)

  • Purpose: The main purpose of this study was to compare the effects of OSCE method and traditional evaluation method on nursing skills performance of students for the fever client. Method: The study was designed as a quasi-experimental, nonequivalent control group post test design with two classes of sophomore students attending fundamentals of nursing courses at one baccaleureate nursing school located in a metropolitan Seoul area. Control group was evaluated by using a traditional method and the experimental group was evaluated by using the OSCE method. Data were collected from April, 2001 to November, 2002. There were 35 students in the OSCE method group, 39 students in the control group. Data analysis was done using SPSS WIN 11.0. Result: The results showed that the students in the OSCE method group did significantly better in clinical nursing skills performance than the students in the traditional method group. And the OSCE method group showed significantly higher satisfaction score in the evaluation method than the control group. Conclusion: The OSCE method is an effective in evaluating clinical nursing skills performance for student nurses. It is necessary to explore more reliable ways to compare the evaluation method by the same student. Also, it is recommended to replicate similar studies in nursing education.

Effects of Teaching Effectiveness and Clinical Learning Environment on Clinical Practice Competency in Nursing Students (임상실습 교수효율성과 임상실습교육환경이 간호학생의 임상실습수행능력에 미치는 영향)

  • Purpose: The purpose of this study was to examine the effects of the teaching effectiveness and clinical learning environment on the clinical practice competency in nursing students. Method: Undergraduate nursing students (268) enrolled in one of 4 universities in 2 cities completed a survey questionnaire. Collected data were analyzed using descriptive statistics, Pearson's correlation coefficient and multiple regression with the SPSS/Win 11.0 program. Results: The mean score for teaching effectiveness was 3.09 (${\pm}.53$) on a 5-point scale, for clinical learning environment, 3.09 (${\pm}.50$) on a 5-point scale, and for clinical practice competency, 3.94 (${\pm}.52$) on a 6-point scale. Significant positive correlations were found between teaching effectiveness and clinical learning environment, and clinical practice competency. The regression model explained 8.8% of clinical practice competency. Teaching effectiveness and clinical learning environment were significant predictors of clinical practice competency. Conclusion: The findings of the study suggest that teaching effectiveness and clinical learning environment be considered when developing strategies to increase clinical practice competency in nursing students.

Effects of a Simulation-based Stroke Care Education on Nursing Performance Ability and Satisfaction in Nursing Students (뇌졸중 대상자간호 시뮬레이션 교육이 간호학생의 수행능력과 만족도에 미치는 효과)

  • The aim of the study was to verify the effects of simulation-based stroke care education on the nursing performance ability and satisfaction among nursing students. The study used a nonequivalent control group pretest-posttest design. A total of 64 nursing students in a nursing college participated in this study. The participants were assigned conveniently according to the academic year to either the experimental group in 2013 (n = 32) or control group in 2014 (n = 32). The experimental group received a simulation-based stroke care education, and the control group received a clinical placement with self-directed practicum. The data was analyzed using the Mann-Whitney U test. The results showed that the scores of the overall nursing performance ability (z = -3.373, p < .001) and satisfaction (z = -3.245, p = .001) in the experimental group were significantly higher than those in the control group. Overall, simulation-based stroke care education is an effective teaching strategy of improving the nursing performance ability and satisfaction among nursing students.

Development of Handoff Education Program using SBAR for Nursing Students and Its Effect on Self-efficacy, Communication Ability and Clinical Performance Ability (간호대학생 대상의 SBAR를 활용한 인수인계 교육 프로그램이 자기효능감, 의사소통능력과 임상수행능력에 미치는 효과)

  • Purpose: This study was conducted to develop a handoff education program for nursing students and examine the effects of it on nursing students' self-efficacy, communication ability, and clinical performance ability. Methods: A quasi-experimental design was used with a nonequivalent control group pretest-posttest method. The experimental group (n=31) received handoff education using SBAR; the control group (n=31) received non-SBAR handoff education. Self-efficacy, communication ability, clinical performance ability were measured to evaluate the effects of the program. Results: The experimental group showed significant improvements in self-efficacy (p<.001), and communication ability (p=.025) compared to the control group. However, there was no significant difference in the clinical performance ability between the groups (p=.618). Conclusion: The results indicate that the handoff education program using SBAR is effective in improving nursing students'self-efficacy and communication ability.

  • 진료 및 간호서비스는 다른 제조업과는 달리 무형의 서비스에 대한 간접비의 비율이 두드러지는 특성을 지니며, 이러한 특성으로 인해 의료기관의 전략적 계획수립, 수익성 분석, 경제성 분석, 의료서비스 제공의 질 관리 등의 의사결정에 적극 활용되어야 하는 원가 분석에 어려움을 겪어왔다. 전문 간호 분야인 가정간호는 1회 방문 시에 다양한 활동들이 모여서 가정간호서비스가 수행되므로 가정간호의 원가분석에는 각 행위들이 모인 활동에 대한 분석에 근거한 정확한 원가 정보가 요구되고 있다. 활동기준원가의 기본 요소는 자원, 활동, 자원 동인, 활동 동인, 원가 대상이며 이에 따라 가정간호사업에서 활동기준원가의 각 요소를 정의하고 전통적 원가 계산이 아닌 활동을 중심으로 가정간호사업의 업무 알고리즘을 분석하여 활동기준원가관리시스템을 설계한다. 가정간호사업의 활동기준원가관리시스템은 가정간호의 효율적 운영과 의료기관의 경영 개선과 항께 실질적 원가계산을 통해 정부가 건강 보험 수가를 결정하고, 보험 수가의 적절성을 평가하는데 중요한 기초 자료를 제공하게 될 것으로 기대한다.

Effects of Self Reflective Practice using Cellular Phone Video Recordings on Self-Confidence in Performance, Practice Satisfaction, Learning Self-efficacy and Core Basic Nursing Skills Achievement among Nursing Students (스마트폰 동영상을 활용한 자기성찰 실습이 간호학생의 핵심 간호술 수행자신감, 실습만족도, 학업적 자기효능감 및 성취도에 미치는 효과)

  • The purpose of this study was to examine the effects of self reflective practice using cellular phone video recordings on self-confidence in performance, practice satisfaction, learning self-efficacy and core basic nursing skills achievement. A quasi-experimental study with a non-equivalent control group posttest design was used. Participants were recruited from one nursing colleges. The sample consisted of an experimental group (n=43) that participated in the self reflective practice using cellular phone video recordings program and a control group (n=43). The data of self-confidence in performance, practice satisfaction, and learning self-efficacy were collected using self-administered questionnaires from March 2, 2018 to May 30. The core basic nursing skills achievement was evaluated by two researchers. Data analyzed using ${\chi}^2$-test and independent t-test. After the intervention, self-confidence in performance, learning self-efficacy and core basic nursing skills achievement significantly increased in the experimental group compared with the control group. Findings indicated that, this program can be recommended as an effective educational program for the nursing student to improve their fundamental nursing skills.

Cardiopulmonary Resuscitation Learning Experience, Knowledge, and Performance in Newly Graduated Nurses (일개 병원 신입간호사의 기본심폐소생술 학습경험, 지식 및 수행능력에 관한 연구)

  • Purpose: The purpose of this study was to evaluate the learning experience, knowledge, and performance of cardiopulmonary resuscitation (CPR) in newly graduated nurses, and to identify differences related to learning experience. Methods: The participants were 114 new nurses in the hospital. They were asked to complete a questionnaire, which included CPR learning experience. They were evaluated by a written test and a skill test using a manikin and check list. Results: All participants attended CPR lectures and underwent practice while in university. Only 12.28% of participants were taught by a certified Basic Life Support (BLS) instructor. The mean scores of the written and skill tests were $79.82{\pm}12.69$ and $64.41{\pm}11.71$, respectively. The nurses lacked CPR knowledge related to checking breathing, the frequency of 30 chest compressions, compression rate, and automated external defibrillator use. They also lacked skill in performing CPR related to checking breathing and pulse and giving 2 breaths. CPR performance differed according to learning time (p=.047) and BLS educator (p=.029). Conclusion: The findings of this study reveal that CPR performance by newly graduated nurses is poor and suggest that CPR education by trained instructors, practice-based education, and reeducation programs must be provided to newly graduated nurses in the hospital.

Influence of Nursing Students' Anxiety during Simulation Training on Personal Satisfaction of Simulation, Self-efficacy, Clinical Competence (간호대학생의 시뮬레이션 교육 시 발생하는 불안정도가 시뮬레이션 교육만족도, 자기효능감, 임상수행능력에 미치는 영향)

  • Purpose: The purposes of this study were to identify nursing students' levels of anxiety in simulation based nursing education and to understand the relationship of anxiety and personal satisfaction of simulation, self-efficacy, and clinical competence during simulation. Methods: This study was a descriptive study with 84 senior nursing students at a private university in South Korea. Level of anxiety was measured with STAI (the State- Trait Anxiety Inventory) prior to the stimulation practice. After the debriefing, satisfaction scores were measured with SSE (Satisfaction with Simulation Experience scale), self-efficacy scores with the modified measurement developed by Roh and Park, and scores for clinical skills with a scale developed by the Korea Association of Cardiopulmonary Resuscitation. Results: Nursing students exhibited moderate to severe anxiety in simulation practice (Mean 49.51, SD 8.60). Self-efficacy was negatively correlated to anxiety (r=-.25, p=.022). Conclusion: Decreasing students' anxiety levels needs to be addressed to increase the self-efficacy of the simulation practice.
