Functional MRI of Visual cortex in the Patients with Occipital Lobe Ischemia (후두엽의 허혈성 뇌졸중 환자에서 시각피질의 기능적 자기공명영상)

  • Purpose : To evaluate the usefulness of functional MRI (fMRI) of visual cortex in patients with ischemic infarction in the occipital lobe. Materials and Methods : Four patients with the symptoms and signs of visual cortical ischemia were included. Functional MRI was performed by 2D-FLASH technique with the parameter of 90/56msec TR/TE, $40^{\circ}$ flip angle, $240{\times}240{\;}FOV,{\;}64{\times}128$ matrix number, 8.32 seconds acquisition time, 8mm slice thickness. An axial slice including both visual cortices was selected and alternative activation and resting of the visual cortex was performed using red color photostimulator. all patients undertook visual field test, and vascular abnormality was examined by MRA (n=4) and DSA (n=2). fMRI results were compared with the results of a visual field test, conventional MRI and cerebral angiography. Results : On fMRI, decreased activity of the visual cortex was found in the occipital lobe corresponding to stenosis of the posterior cerebral artery or its branch noted on angiogram. However, 2 of 4 patients showed no abnormal findings on conventional MRI. Visual field defect was noted in 3 patients, one and of whom showed no abnormality on conventional MRI and diffusion-weighted image, but revealed decreased activity in the corresponding visual cortex on fMRI. Conclusion : fMRI may be a sensitive method for detection of the status of decreased blood flow or vascular reserve which other methods can not.

Comparative Study of Functional Magnetic Resonance Imaging by Global Scaling Analysis (Global Scaling 분석방법에 따른 기능적 자기공명영상의 비교 연구)

  • Purpose : To evaluate the effect of global scaling analysis on brain activation for sensory and motor functional MR imaging study. Materials and methods : Four normal subjects without abnormal neurological history were included. Arm extension-flexion movement was used for motor function and 1KHz pure tone stimulation was used for auditory function. Functional magnetic resonance imaging was performed at 3T MRI (GE, Milwaukee, USA) using BOLD-EPI technique and SPM2 was employed for data analysis. On data analysis, the brain activation images were obtained with and without global scaling by fixing other parameters such as motion correction and realignment. Results : The difference in brain activation between no scaling and global scaling was not large in case of right upper extremity movement (p<0.000001). For auditory test, brain activation with global scaling showed larger activation than that of without global scaling (p<0.05). Conclusion : A caution must be taken into account when analyzing functional imaging data with global scaling especially for functional study of small local BOLD signal change.

  • 허혈성 심질환 환자의 평가도구로서 MRI의 역할이 확대된 데에는 소위 "one-stop shop"이라는 통합적 심장검사법의 개발이 계기가 되었다. 즉 한 번의 MRI 검사로서 심실벽운동을 평가하여 심근의 기능과 예비능을 측정하여 만성 심근경색을 평가할 수 있고, 심근의 관류를 평가할 수 있고, 관동맥의 혈류 예비능과 심근의 viability를 평가하고, 관동맥 조영술을 통해, 수술 후의 우회로 평가 및 관동맥의 협착 여부의 진단이 가능한 프로토콜이 MRI장비마다 각각 개발되어 보급되고 있다. 하드웨어와 소프트웨어의 개발과 지속적인 연구를 통해서 MRI는 허혈성 심질환의 진단적 검사로서 기존의 다른 검사들과 비교하여 높은 시간적, 공간적 해상력으로 정확한 구조와 기능의 평가를 제공할 수 있는 가장 유용한 검사가 될 것으로 사료된다 특히, MR 관동맥조영술, 심근관류검사, 심근 표지법, dobutamine 부하검사, viability 평가, plaque characterization, MR spectroscopy 분야에서 현재 활발히 연구가 진행되고 있으며 향후 임상적용이 확대될 것으로 예상된다.

A feasibility study on new stimulation method in fMRI language examinations using custom designed images (기능적 자기공명영상의 언어기능검사 시 image를 이용한 자극방법의 타당성 연구)

  • The purpose of this work is to know the validity of a new stimulation method in cognitive functional imaging using custom-designed images correspond to words or syllables improving the shortcomings of existing method using text. From March 2011 to May five Subjects in need of language related functional MRI scanning were selected and both of text stimulating method and image stimulating method sacanning were carried out three times each. Using 3.0T Philps MRI machine and Invivo Co's Eloquence system, data acquisition was performed with EPI-BOLD technique. Post processing was performed with SPM 99 while the activated signals were determined within 95 percent confidence level.The number of activation clusters and the activation ratio inside ROI were compared. As as result, all of the subject showed activation inside Broca area but it did not have statistical significance. In conclusion, the image sitimulation method has potential because image itself is a common means of recognition and it can be recognised easily even if there language barrier. This stimulation method can be applied to replacing the exising scanning method especially in the elderly, infants, foerigners who may not fully understand about the examination.

  • This study involves an experiment using functional magnetic resonance imaging(fMRI) to delineate brain activation for execution functional performance. Participates to this experiment of the normal adult (man 4, woman 6) of 10 people, is not inserts the metal all closed phobia and 24.5 year-old average ages which the operating surgeon experience which are not they were. The subject for a functional MRI experiment word -color test prosecuting attorney subject rightly at magnetic pole presentation time of 30 first editions and after presenting, uses SPM 99 programs and the image realignment, after executing a standardization (nomalization), a difference which the signal burglar considers the timely order as lattice does, pixel each image will count there probably is, in order to examine rest and active crossroad dividing independence sample t-test (p<.05). Overlapped in this standard anatomic image and got a brain activation image from level of significance 95%. With functional MRI resultant execution function inside being relation, the prefrontal lobe, anterior cingulate gyrus, parietal lobe, orbitofrontal gyrus, temporal lobe, parietal lobe was activated. The execution function promotes a recovery major role from occupational therapy, understanding about the damage mechanism is important. When confirms the brain active area which accomplishes an execution function brain plasticity develops the cognitive therapeutic method which is effective increases usefully very, will be used.

  • The obstacles for cardiac imaging are motion artifacts due to cardiac motion, respiration, and blood flow, and low signal due to small tissue volume of heart. To overcome these obstacles, fast imaging technique with ECG gating is utilized. Cardiac exam using MRI comprises of morphology, ventricular function, myocardial perfusion, metabolism, and coronary artery morphology. During cardiac morphology evaluation, double and triple inversion recovery techniques are used to depict myocardial fluidity and soft tissue structure such as fat tissue, respectively. By checking the first-pass enhancement of myocardium using contrast-enhanced fast gradient echo technique, myocardial blood flow can be evaluated. In addition, delayed imaging in 10 - 15 minutes can inform myocardial destruction such as chronic myocardial infarction. Ventricular function including regional and global wall motion can be checked by fast gradient echo cine imaging in quantitative way. MRI is acknowledged to be practical for integrated cardiac evaluation technique except coronary angiography. Especially delay imaging is the greatest merit of MRI in myocardial viability evaluation.

Functional MRI of Language Area (언어영역의 기능적 자기공명영상)

  • Purpose : To evaluate the usefulness of functional MR imaging (fMRI) for language mapping and determination of language lateralization. Materials and Methods : Functional maps of the language area were obtained during word generation tasks and decision task in ten volunteers (7 right handed, 3 left-handed). MR examinations were performed at 1.5T scanner with EPI BOLD technique. Each task consisted of three resting periods and two activation periods with each period of 30 seconds. Total acquisition time was 162 sec. SPM program was used for the postprocessing of images. Statistical comparisons were performed by using t-statistics on a pixel-by- pixel basis after global normalization by ANCOVA. Activation areas were topographically analyzed (p>0.001) and activated pixels in each hemisphere were compared quantitatively by lateralization index. Results : Significant activation signals were demonstrated in 9 of 10 volunteers. Activation signals were found in the premotor and motor cortices, the inferior frontal, inferior parietal, and mid-temporal lobes during stimulation tasks. In the right handed seven volunteers, activation of language areas was lateralized to the left side. Verb generation task produced stronger activation in the language areas and higher value of lateralization index than noun generation task or decision task. Conclusion : fMRI could be a useful non-invasive method for language mapping and determination of language dominance.

Correlation of the Neuropsychological Screening Battery (NSB) and Neuroanatomy for the Parkinson's Disease with Mild Cognitive Impairment by Using the Analysis of Cerebral Cortex Thickness in the Brain MRI (뇌 자기공명영상에서 대뇌 피질 두께 분석법을 이용한 파킨슨병의 경도인지장애 신경심리검사와 신경해부학적 상관관계)

  • This study is to investigate neuro-anatomical correlation between neuropsychological results and cerebral cortex thickness of cognitive ability in the brain MRI targeting the patients with mild cognitive impairment. It was that 78 people who were diagnosed as first Parkinson's disease followed by neuropsychological screening battery(Parkinson's disease with mild cognitive impairment: 39 people; Parkinson's disease with normal cognition: 39 people) and 32 people of normal group were selected. Correlation between mild cognitive impairment and normal cognitive impairment and correlation between neuropsychological screening battery and cerebral cortex thickness in the brain MRI were performed by independent sample t-test or Pearson correlation coefficient and then level of significance of collected data was verified in p<0.05. As a result, cerebral cortex thickness of the Parkinson's disease with mild cognitive impairment in both side precuneas and right inferiortemporal lobe had statistically significant decrease. In addition, function of visuospatial ability, verbal and visual memory was reduced in neuropsychological screening battery for cognitive assessment. Especially, there was correlation between neuropsychological screening battery of verbal and visual memory anatomical left precuneus.

  • 신경과 의사들은 실제 임상에서 환자들에게 대뇌피질감각의 이상 여부를 알기 위해 손바닥에 숫자나 문자를 쓰게 하여 알아 맞추게 하는 검사를 시행한다. 손바닥에 쓰여진 문자나 숫자를 인식하기 위해서는 몇 가지 단계의 인지과정이 필요할 것으로 생각된다. 첫번째로 손바닥에 닿는 감각을 인지해야 하고, 다음 단계로 이 감각들을 시공간적으로 통합하는 과정이 필요할 것이고, 이러한 정보들을 유지하면서 마지막 단계로 우리 머리 속에 있는 문자와 일치시키는 과정이 필요할 것이다. 본 연구에서는 위와 같은 가설 아래 각각에 해당할 수 있는 뇌영역을 밝히기 위해 기능적 뇌자기공명영상을 이용하였다. 손바닥의 일차적 감각을 인지하는 데는 일차감각영역이 활성화 되었고, 이차감각영역의 활성은 감각들의 시공간적 통합과 관련될 것으로 생각되었으며, 이것들을 유지하는 것은 작업기억의 하부구조인 시공간 그림판과 관련되는 영역이며, 문자를 일치시키는 과정은 브로카영역 부근의 활성과 관련되는 것으로 생각되었다. 위의 가설에 대한 추가검증 및 실제로 일어나는 인지과정에 대한 추가적인 연구가 필요할 것이다.

Effects of the Symmetric Upper Extremity Motion Trainer on the Motor Function Recovery after Brain Injury: An fMRI Study (뇌손상 후 운동신경기능 회복에 대한 대칭형 상지 운동기구의 효과: 기능적 뇌 자기공명영상 연구)

  • The effect of the developed symmetric upper extremity motion trainer on the cortical activation pattern was investigated in three chronic hemiparetic patients using both fMRI and Fugl-Meyer test. The training program was performed at 1 hr/day, 5 days/week during 6 weeks. Fugl-Meyer tests were performed every two weeks during the training. fMRI was performed at 3T scanner with wrist flexion-extension in two different tasks before and after the training program: the only unaffected hand movement (Task 1) and passive movements of affected hand by the active movement of unaffected hand (Task 2). fMRI studies in Task 1 showed that cortical activations decreased in ipsilateral SMC but increased in contralateral SMC. Task 2 showed cortical reorganizations in bilateral SMC, PMA and SMA. Therefore, it seems that the cortical reorganization in chronic hemiparetic patients can be induced by the training with the developed symmetric upper extremity motion trainer.
