Treatment of Laryngeal Web (Laryngeal web의 치료)

  • Laryngeal web was first reported by Fleischmann in 1822, and it causes upper airway obstruction and abnormalities of phonation. Congenital webs result from an arrest of development of the larynx around the tenth week of fetal life. Acquired webs may result from cicatrical strictures of infectious lesion, traumatic and postoperative wounds. The most common site of webbing is glottic, followed by subglottic and supraglottic. We have experienced a case of simultaneous glottic and supraglottic laryngeal webs probably due to tuberculous lesions in 28 aged male who was treated by endolaryngeal microsurgery.

  • Sudden esophageal obstruction by a bolus of food is not a rare phenomenon. The individual is frequently elderly and has ill-fitting dentures through others may initiate this by bolting food. In adult patient there is a frequent history of alcoholic intake, the consumption of meat with improper chewing. A common anatomic abnormality leading to meat impaction of the esophagus is hiatal hernia, which is often associated with a ring or stricture. Recently authors experienced a case of repeated esophageal foreign bodies in 10 days in the patient who had esophageal stricture.

  • Congenital long-segment tracheal stenosis with complex cardiac anomaly has generally been regarded as a fatal disease This report described the successful concomitant repair of unexpected congenital tracheal stenosis and complex cardiac anomaly with the use of edrdiopulmonary bypass. The patient was a 3-month-old girl with coarctation of aorta, V D, and PDA. The presence of tracheal stenosis was not discovered until when difficulty with endotracheal intubation was encountered at operating room. Thus, we decided concomitant repair of both lesions and performed anterior pericardial tracheoplasty combined with one stage repair of coarctation of aorta, VSD, and PDA under the cardiopulmonary bypass. The patient is doing well without any signs of complication at present, 2 years and 1 month after the operation.

  • The complications of endotracheal intubation are inevitable, of which postintubation tracheal stenosis may be required for surgical resection with primary reconstruction. Before surgery, several less invasive therapeutic modalites including bougie dilatation, stenting, and Nd-YAG laser incision are still available in use. Especially, good results were noted in selected patients with lengthy scars of less than 1cm and without tracheomalacia using endoscopic laser incision and dilatation. We report a case of a 54 yr-old woman with postintubation tracheal stenosis who was successfully treated by endoscopic Nd-YAG laser incision and esophageal balloon catheter.

  • Foreign body in esophagus is not uncommon in the field of otolaryngology. There are many reports about it with recent advanced development of esophagobronchology. The authors have recently experienced an interesting case of foreign body retained in second constrictor portion of esophagus, which had been perforated the anterior wall of esophagus and then pierce the trachea.

  • Caustic bums of the upper aerodigestive tract continue to be a significant clinical problem. Wide -field pharyngoesophagectomy is commonly performed as treatment for malignancies of the hypopharynx. A total laryngectomy is often necessary at the time of this procedure because of the anatomical proximity of the cancer or because of the likely compromise of swallowing postoperatively. When preservation of the larynx is attempted, aspirations after surgery frequently require a second-stage laryngectomy. And various flaps are using for reconstruction of esophageal defect. The choice of reconstruction is depended to the patient's status. A 54-year-old women whose symptom was severe dysphagia and X-ray revealed laryngopharyngeal stricuture. She had attempted suicide by swallowing lye liquids 32 years ago. She has entire laryngopharyngeal and esophageal stricutures. Total laryngectomy was performed and reconstruction of theesophagus was carried out with unusual reversed-gastric tube formation. Hence, we report this case with the review of literaturefor proper management in the future.
