Robotic Surgery in Head and Neck (두경부 영역에서의 로봇 수술)

  • Organ preservation surgery and minimally invasive surgery have been developed during the past 20 years with major focus on transoral laser surgery, endoscopic surgery, and robotic surgery. Two major robotic surgeries in head and neck area are transoral robotic surgery (TORS) and robotic thyroidectomy. Transoral robotic surgery is a safe and efficacious method of surgical treatment of oropharyngeal. hypopharyngeal and laryngeal neoplasm. Advantages of the technique include adequate ability to visualize and manipulate lesions with two hands. TORS can provide magnified three dimensional views and overcome the limitation resulting from the "line of sight" which hinders transoral laser procedure. The swallowing function following transoral robotic surgery show superior and patients were able to retain or rapidly regain swallowing function in the majority of cases. Recently, robotic thyroidectomy has also been developed to overcome the [imitation of endoscopic thyroidectomy. Robotic thyroidectomy by a gasless unilateral axillo-breast or axillary approach using a da Vinci S Surgical Robot is a feasible and cosmetically excellent procedure. It can be a promising alternative to endoscopic thyroidectomy or conventional open thyroidectomy.

Pulmonary carcinoid tumor presented with recurrent pneumonia in adolescence (반복되는 폐렴으로 내원한 청소년기 폐 칼시노이드 종양)

  • The incidence of endobronchial tumor in the pediatric period is very rare and the symptoms are very close to those of respiratory tract infection. Diagnosis can be delayed because of low clinical suspicion and the many ways in which these tumors can present. We report the first case, in Korea, of a pulmonary carcinoid tumor that presented with recurrent pneumonia at the same location in adolescence. Treatment is surgical and geared towards complete resection while sparing healthy lung parenchyma. Long-term and careful follow up is recommended to detect local and distant metastasis.

  • Epithelioid hemangioendothelioma, originating from the vascular endothelium, is a very rare and low-grade malignancy. World-wide, about 50 cases of pulmonary epithelioid hemangioendothelioma have been reported. This is more common in female and is usually shown as multiple nodules in both lung fields. A 41-year-old male, who had suffered from right pleuritic chest pain for 3 months, was initially diagnosed as adenocarcinoma under bronchofiberscopic biopsy. At that time, the stage of tumor according to the TNM staging was llla. He received bronchoscopic biopsy again during follow-up period and it was diagnosed as hamartoma. After surgery, the final diagnosis was pulmonary epithelioid hemangioendothelioma.

Treatment of Laryngeal Carcinomas by Laser Surgery (후두암의 레이저 수술)

  • Background and Objectives: The treatment of laryngeal carcinoma is not settled to date and surgeons have used lasers for a variety of benign and malignant lesions in the larynx with good success. The aim of this study is to evaluate the potential role of laser surgery for laryngeal carcinoma. Materials and Methods : Medical records from patients who had undergone laser surgery for laryngeal carcinoma at Seoul National University Hospital between January 1988 3nd December 1998 were reviewed retrospectively. Results : A total of 47 patients were seen during that period and their mean follow-up length was 29 months. Their local control rate was 91% (94% for glottic T1, 60% for glottic T2, 50% for supraglottic T1 and 100% for supraglottic T2). The cure rate after laser surgery with or without postoperative radiotherapy was 87% and 6 of 47 patients showed local or regional recurrences. For those 6 patients, neck dissection, partial or total laryngectomy with or without postoperative RT were conducted and they were all followed up without evidence of disease. All 47 patients who had undergone laser surgery for laryngeal squamous cell carcinoma were followed up with NED and their larynx preservation rate was 96%. Conclusion: This study demonstrates the oncologic validity of laser surgery to the treatment of unadvanced laryngeal carcinoma. However, strict case selection is needed to avoid locoregional recurrences and consequent salvage operations.

  • The group of tumors now generally termed Neurilemmoma or Schwanno ma was first described by Verocay, in 1908, and in 1910, he termed them "neurinoma". In 1935 Stout proposed the term neurilemmoma, believing that they arise from nerve sheath or Schwann's cells. Neurilemmoma is an encapsulated, solitary tumor arising in any nerves with Schwann's cell sheath. They may occur at any age and have no preponderance of sex and developing site, however, usually occur in the head and neck area in 25% of cases and have not shown metastasis and are radioresistant. Authors report 5 cases of neurilemmoma surgically surgically removed under the local and general anesthesia at ENT department of SNUH during the last 5 years, with review of the literatures.

  • The are early diagnosis of acoustic neuroma is important for radical removal of it and for early radiologic diagnosis which was considered to be the most reliable method it is required to know the normal dimension of the internal auditory meatus. There are two methods for internal auditory meatal measurement; one is indirect, such as the conventional roentogenography, laminoraphy and cisternography and the other is direct using rubber cast method of Amjad, A.H. et. al. Authors measured the anatomical dimensions of Korean adult internal auditory meatus with regular rubberbase impression material.

Excision of Esophageal Duplication Cyst with VATS - One case report - (흉강 내시경을 이용한 식도 중봉 낭종 수술 치험 -1례-)

  • Esophageal duplication cyst is very rare mediastinal tumor which is congenital lesion of the esophagus. Esophageal duplication cyst could be excised with video assisted thoracoscopic surgery(VATS) if it is relatively small, cystic lesion and not adhered severely to the surrounding tissues such as lung, trachea, bronchus, esophagus and pleura. We report a case of an esophageal duplication cyst which was located in the right thoracic cavity below carina and could be excised completely and repaired by interrupted suture with 3.0 black silk. The patient was discharged at 10 days after operation with good condition and has been in uneventful condition 2 months after operation.

Report of eight cases of thyroglossal duct cyst and fistula (갑상선 설관낭종 및 설관루)

  • The authors have experienced 8 cases of the thyroglossal duct cysts and fistula which were diagnosed and treated surgically in the Dept. of Otolaryngology, Taegu Presbyterian Hospital during the period of last 2 years. 1. Age distribution showed at age 2∼10 years old (4 cases), and 10∼30 years old (3 cases); The youngest age of 2, and the oldest age of 42. 2. The form of disease revealed as cystic form (7 cases) and fistula form (1 case). 3. Operating procedure were excision of thyroglossal duct cyst with partial removal of hyoid bone. 4. Sex distribution 7 in female and 1 in male. 5. Histologic examination revealed as stratified squamous epithelial lining were 7 cases, its cell composed with plasma cells lymphocytes. No epithelial lining is 1 case. 6. Post-operative complications were not developed in all cases.

Quasi-periodic waveform analysis for diplophonia (이중음성에 대한 음성파형분석)

  • Diplophonia is produced by the voice of two separate tones and produced through quasi-periodic variations in the vocal cord vibration. Diplophonia is generally regarded as a symptom of laryngeal pathology. The difference in the vibratory frequency between the vocal cords can be seen in a tension imbalance and a difference in the level of the vocal folds under the special condition such as incomplete glottal closure. So authors have experienced 19 cases of patient with diplophonia for the unilateral vocal cord paralysis, intracordal cysts and other mass lesions. And we analysed the diplophonic voice with peak variability and noise level for the quasi-periodic waveforms and spectrograms pre-and postoperatively.

  • Unilateral vocal cord paralysis is induced by various causes and its effective treatment has been diversely searched out until now. Currently used treatment modalities are intracordal injection of exogenous materials such as Teflon or Silicone, and thyroplasty and so forth. But, with the above mentioned modalities, it has been not satisfactory to obtain a good postoperative results especially in cases when the glottal incompetence is very severe or the level difference between the vocal cords is large. In such cases, vocal cord adduction can be accomplished by anteromedial traction of the muscular process of paralyzed vocal cord via surgical exposure resulting improvement of voice quality. Recently, authors performed arytenoid adduction in 3 cases of unilateral vocal cord paralysis to obtain a better improvement of voice quality, and experienced satisfiable postoperative results.
