The Potential Role of Cardiac CT in Patients with Acute Coronary Syndrome (급성 관상동맥 증후군을 가진 환자에서 심장 CT의 역할)

  • Acute coronary syndrome involves three types of coronary artery disease associated with sudden rupture of coronary artery plaque, and has a clinical presentation ranging from ST-segment elevation myocardial infarction (STEMI) to non-ST-segment elevation myocardial infarction (NSTEMI) or unstable angina. Cardiac CT can help quantify and characterize atherosclerotic plaques. According to a previous study, low-attenuation plaque, napkin ring sign, positive remodeling, spotty calcification, and increased perivascular fat attenuation are associated with plaque ruptures on cardiac CT. Therefore, coronary artery stenosis, as well as acute coronary artery syndrome, can be diagnosed using cardiac CT.

Verification of Reliability and Validity of K-SART to Assess of Situational Awareness of Patients with Acute Coronary Syndrome (급성관상동맥증후군 환자의 상황인식 측정을 위한 K-SART의 신뢰도와 타당도 검증)

  • The purpose of this study was to test the reliability and validity of the SART(Situation Awareness Rating Technique) tool to measure the situational awareness of patients with acute coronary syndrome. The translated SART was tested on 118 hospitalized patients with acute coronary syndrome to verify the reliability and validity of the tool. The analysis was conducted with Cronbach's alpha, exploratory & confirmatory factor analysis. Study findings found that the tool's reliability and model fit index improved when the two problematic items, namely, Situation Awareness 1 (Instability of Situation) and Situation Awareness 7 (Spare Mental Capacity) were removed. According to the results, the revised K-SART is deemed to be an appropriate method for assessing the degree of situational awareness of patients with acute coronary syndrome in decision making about treatment-seeking.

  • The clinical concept of coronary artery disease (CAD) has seen a paradigm shift over the last decade. CAD is mostly a progressive disease, and patients with CAD can develop acute coronary syndromes at any point in disease progression. In this clinical context, a new term, "chronic coronary syndrome," was published in the 2019 European Society of Cardiology guidelines, reflecting the importance of early diagnosis and active management. Recent advances have been made in the evaluation of CAD using coronary CT angiography (CCTA). The clinical usefulness of CCTA in patients with stable angina or chronic coronary syndrome begins with the detection of early asymptomatic CAD. The characterization of atherosclerotic plaque and its role in determining treatment strategies for CAD have been demonstrated for all stages of the disease. This review describes the pathophysiology of stable angina to aid in the understanding of the clinical applications of CCTA.

Surgical Experiences for Non-ST-segment Elevation Acute Coronary Syndrome (ST분절의 상승을 동반하지 않은 급성 관동맥 증후군의 수술 치험)

  • Background: Surgical role for acute coronary syndrome has been reduced in recent years due to development of drug eluting stent. We evaluated the surgical results of acute coronary syndrome in our hospital. Material and Method: Between January 2001 and August 2005, 416 patients underwent coronary artery bypass grafting (CABG) under diagnosis of non-ST-elevation acute coronary syndrome (NSTE ACS). Mean age was $61.8{\pm}9.0$ years and 276 (66.3%) patients were male. 324 (77.9%) patients had triple vessel disease and 92 (22.1%) had left main disease at angiographic study. 236 (56.7%) patients had hypertension and 174 (41.8%) had diabetes mellitus. Conventional on-pump CABG was performed in 194 patients (46.6%) and off-pump CABG in 222 (53.4%). Total arterial revascularization with no touch technique was done in 97 patients (23.3%). The number of total distal anastomosis was 1,306 and the number per patient was $3.21{\pm}1.71$. Result: Surgical mortality rate was 1.0% (4 patients) and postoperative complication rate was 15.6% (65 patients). Graft patency was checked at mean $3.7{\pm}7.6$ months (from 1 to 37 months) postoperatively with multi-directional computed tomography in 152 patients. Left internal mammary artery was patent in 95.3%, right internal mammary artery in 98.1%, radial artery in 92.2% and saphenous vein in 89.0%. Conclusion: The surgical treatment of NSTE ACS showed relatively low mortality rate and good graft patency rate. Further study is needed to compare the long term results with drug eluting stent.

A Survey on Prehospital Emergency Medical Service for the Improvement of Acute Coronary Syndrome Assessments - Focus on the Jecheon.Danyans Area - (급성관상동맥증후군 환자의 병원 전 119구급의료서비스 실태 및 개선방안 -충북 제천.단양을 중심으로-)

  • From January, 2004 to July, 2007, 97 patients were diagnosed with Acute coronary syndrome(ACS) in Emergency Medical Center in Jecheon City in Korea. Among 97 patients, the prehospital assessment rates were as follows ; blood pressure check(27.80%, pulse rate check(33.0%), respiration check(23.7%). In hospital emergency care, oxygen supply was 52.6%, absolute bed rest was 12.4%, electrocardiogram(EKG) check was 4.1%, and nitroglycerin and intravenous fluid therapy was 0%. Therefore, ACS patients have not received sufficient prehospital cares till now. In order to improve the proficiency of the first aid emergency medical technicians(EMTs), it is necessary to provide the continuing On-the-job training(OJT) programs for prehospital emergency care.

Early-Onset Postcardiac Injury Syndrome after Percutaneous Coronary Intervention Recovered with Steroids (경피적 관상동맥중재술 후 급성으로 발생한 스테로이드 치료로 회복한 심근 손상 후 증후군)

  • Postcardiac injury syndrome (PCIS) is an inflammatory process that usually occurs within 1 to 6 weeks after an injury to the pericardium, epicardium, or myocardium. As more interventions are performed for complicated coronary artery obstructive lesions, there have been some recent reports on PCIS following percutaneous coronary intervention (PCI). The medical management of PCIS depends on nonsteroidal anti-inflammatory drugs (NSAIDs), in addition to colchicine or steroids. An 80-year-old male patient underwent a PCI. Unfortunately, the guidewire piercing failed but he showed no immediate signs of complication. However, 5 hours after the procedure, he complained of chest discomfort. An electrocardiogram showed widespread ST elevation. Chest X-ray and computed tomography showed pulmonary congestion with pleural effusion, while thoracic echocardiography showed a moderate amount of pericardial effusion. NSAIDs were initiated, but there was no improvement of symptoms. We describe an unusual case of atypical earl onset PCIS after PCI, recovered rapidly by steroids.

  • Acute coronary syndrome (ACS) is mainly caused by atherosclerotic coronary artery disease (CAD); however, it can also occur in patients with non-atherosclerotic CAD. Conventional coronary angiography only shows the lumen of arteries, indicating the presence of stenosis or dilatation. Thus, it has limited value in evaluating the coronary artery wall and offers low specificity for diagnosing CAD. Coronary CT angiography provides additional information, including the depiction of the concerned vessel and the aorta, as well as the pulmonary artery, which permits the diagnosis of non-atherosclerotic CAD and the differentiation of various causes of the disease. In this review, we present the pathophysiology and CT imaging features of non-atherosclerotic CAD.

Gender Differences in Cardiac Knowledge and Symptoms Recognition in Patients with Acute Coronary Syndromes (일 대학병원에 내원한 급성 관상동맥증후군 환자의 성별에 따른 질병지식 및 증상인지 비교)

  • Purpose: The purpose of this study was to compare the cardiac knowledge and symptoms recognition between men and women with acute coronary syndrome (ACS). Methods: Cross-sectional survey research design and convenience sampling were used in this study. 64 men and 42 women from a university medical center were participated in this study. Data collection were used with self reported questionnaires and medical records. Results: There was no difference in cardiac knowledge by gender. Women marked the higher score in symptoms recognition than men. Significant gender differences were observed in the reports of several symptoms (headache, nausea, palpitation, hand paresthesia, and leg numbness) and with ACS. There was significant correlation between cardiac knowledge and symptom recognition on both men and women. Conclusion: These findings suggest that health care professionals should pay attention to the differences in clinical symptoms between women and men. Furthermore, tailored information about possible symptoms of coronary artery disease according to the patient's gender is needed.
